Chronic simple glaucoma and central retinal vein occlusion

RESUMO

A relação entre glaucoma crônico simples e oclusão de veia central da retina tem sido demonstrada por alguns autores, embora o mecanismo exato desta associação não esteja ainda completamente esclarecido. No presente estudo, verificamos a existência desta associação em nosso meio, uma vez que de 372 pacientes com glaucoma crônico simples que se consultaram no HCPA entre 1989 e 90, sete apresentaram oclusão de veia central (1,88%). No grupo-controle, de 750 pacientes sem glaucoma, não foi encontrado nenhum caso de oclusão. A diferença foi estatisticamente significativa (p < 0,001). Salientamos a importância do reconhecimento da referida associação e enfatizamos que o achado de oclusão venosa em um olho deve ser seguido de avaliação completa do glaucoma crônico simples em ambos os olhos a fim de prevenir a cegueira por glaucoma no olho adelfo.

SUMMARY

The relationship between Central Retinal Vein Occlusion (CVRO) and Chronic Simple Glaucoma has been reported by some investigators although the exact mechanism underlying this association is not clear yet. ln this study we demonstrate this association out of 372 patients with chronic simple glaucoma that were treated in HCPA between 1989 and 1990, 7 cases of CRVO were diagnosed, an incidence of 1,88%. Whereas, out of 750 patients in one sex and age matched group of controls there were no cases of CRVO. A statistically significant difference was found in a comparison between these two groups (p < 0,001). We emphasize the importance of this association since chronic simple glaucoma is a bilateral disease, therefore, the finding of a CRVO in one eye must be followed by a complete evaluation of the other eye, to prevent blindness by glaucoma in this remaining eye.
