Manifestações oculares na artrite reumatóide juvenil
Ocular manifestations of juvenile rheumatoid arthritis

Artrite reumatóide juvenil (ARJ) é uma doença artrítica crônica que se associa a um quadro de uveíte anterior crônica, praticamente assintomática, mas potencialmente debilitante. Em estudo prospectivo de 77 pacientes com atendidos na Disciplina de Reumatologia da EPM, São Paulo, observou-se associação com uveíte em 10,4% dos casos, e considerando apenas o grupo pauciarticular, a associação foi de 62,5%. Fator reumatóide em casos de uveíte associada à ARJ foi negativo em 87,5% e o fator antinuclear teve positividade maior (25%) dentre os pacientes com acometimento ocular comparando-seos aos com exame normal (5,8%). O tipo de acometimento ocular observado foi em 100% dos casos uma uveíte anterior não granulomatosa crônica associada ou não às complicações (presentes em 62,5% dos casos com comprometimento ocular). A importância do exame oftalmológico de rotina em crianças com é demonstrada para introdução precoce do tratamento diminuindo a ocorrência de complicações secundárias.

Juvenile rheumatoid arthritis (JRA) is a chronic arthritic disease eventually associated with anterior chronic uveitis, usually assimptomatic but potentially debilitating. In a follow-up study of 77 children with JRA attended in the Department of Rheumatology of Escola Paulista de Medicina, São Paulo, uveitis was associated with JRA in 10,4% of the cases; and considering only the pauciarticular onset JRA, the association was 62,5%. Rheumatoid factor was negative in 87,5% of the cases of uveitis associated with JRA and antinuclear antibodies were present in great number of patients with normal ocular examination (5,8%). The ocular manifestations were in 100% of the cases a chronic anterior non granulomatous uveitis with or without its complications (present in 62,5% of the patients with ocular involvement). The importance of the ophthalmological examination on routine is demonstrated, in order to introduce treatment earlier and diminish the occurrence of secondary complications
