Neurological complications in dengue infection

Complicações neurológicas na infecção por dengue

Sir, the recent report on neurological complications in dengue infection is very interesting 1. Puccioni-Sohler M, Rosadas C, Cabral-Castro MJ. Neurological complications in dengue infection: a review for clinical practice. Arq Neuropsiquiatr 2013;71:667-671. . Puccioni-Sohler et al. noted that “neurological manifestations related to dengue infections are increasingly been observed and appears as a challenge for medical practice.” 1. Puccioni-Sohler M, Rosadas C, Cabral-Castro MJ. Neurological complications in dengue infection: a review for clinical practice. Arq Neuropsiquiatr 2013;71:667-671. We would like to share the experience on this topic. First, whether the increasing of dengue neurological manifestation is the fact or not should be discussed. In fact, this observation might be due to the increased concern of the practitioner on this problem. According to the previous report from Thailand, the neurological adverse outcome is common in severe dengue infection and the neurological pathology is a common finding in fatal dengue cases 2. Wiwanitkit V. Magnitude and pattern of neurological pathology in fatal dengue hemorrhagic fever: a summary of Thai cases. Neuropathology 2005;25:398. . Sometimes, the neurological manifestation can be the first clinical feature and this is very difficult for final diagnosis of dengue 3. Samanta M, Kundu CK, Guha G, Chatterjee S. Unique neurological manifestations of dengue virus in pediatric population: a case series. J Trop Pediatr 2012;58:398-401. . Samanta et al. noted that clinicians should “aware of such association and consider dengue infection in cases of febrile encephalitis and myelitis in endemic areas.” 3. Samanta M, Kundu CK, Guha G, Chatterjee S. Unique neurological manifestations of dengue virus in pediatric population: a case series. J Trop Pediatr 2012;58:398-401. Finally, not only the neurological but also the psychological manifestation of the dengue should be kept in mind of all practitioners 4. Wiwanitkit S, Wiwanitkit V. Psychological manifestation in dengue: did it really exist? Indian J Psychol Med 2013;35:222. , 5. Joob B, Wiwanitkit V. Dengue and psychiatry disorder. Arq Neuropsiquiatr 2012;70:560. .
