RESUMO
Objetivo:
estratificar medicamentos prescritos em escala de risco de queda, identificando subgrupos de medicamentos e unidades de internação com maior risco de queda.
Método:
estudo retrospectivo em prescrições de clínica médica, clínica cirúrgica, unidade de terapia intensiva geral. Fatores de risco considerados: 1) hipotensão ortostática; 2) hipotensão arterial; 3) hipertensão arterial; 4) bradicardia; 5) agitação psicomotora; 6) confusão mental; 7) tontura; 8) sonolência/sedação; 9) diminuição da visão; 10) convulsões; 11) atonia/distonia/fraqueza muscular; 12) hipoglicemia; 13) urgência micção e 14) urgência defecação/diarreia. Estabeleceu-se graus de risco: 0: 0 fator; I: 1-2 fatores; II: 3-5 fatores; III: 6-9 fatores e IV: 10-14 fatores.
Resultados:
foram analisados 3893 medicamentos, estratificados como graus: 0 22,7%; I 33,5%; II 28%; III 15,1%; IV 0,7%. Os graus III e IV referiram-se mais frequentemente a fármacos para distúrbios da acidez gástrica, 22,6%, e psicolépticos, 100%.
Conclusão:
conhecer fatores de risco associados aos medicamentos pode contribuir para prevenção e diminuição de quedas, sobretudo quando regimes terapêuticos não podem ser modificados.

Método:
estudio retrospectivo en prescripciones de clínica médica, clínica quirúrgica, unidad de cuidados intensivos. Factores de riesgo considerados: 1) hipotensión postural; 2) hipotensión arterial; 3) hipertensión arterial; 4) bradicardia; 5) agitación psicomotora; 6) confusión mental; 7) mareos; 8) somnolencia/sedación; 9) convulsiones; 10) disminución visión; 11) atonía/distonía/debilidad muscular; 12) hipoglucemia; 13) urgencia orinar; 14) urgencia defecar/diarrea. Grados de riesgo establecidos: 0: 0 factores; I: 1-2 factores; II: 3-5 factores; III: 6-9 factores, IV: 10-14 factores.
Resultados:
analizados 3893 medicamentos estratificados como grados: 0 22,7%; I 33,5%; II 28%; III 15,1%; IV 0,7%. Fueron más frecuentes para los grados III y IV: fármacos para trastornos de la acidez gástrica, 22,6%, y psicolépticos, 100%, respectivamente.
Conclusión:
conocer factores de riesgo asociados con la medicación puede contribuir para prevenir y reducir caídas, sobre todo cuando regímenes terapéuticos no pueden ser cambiados.

ABSTRACT
Objective:
to stratify prescribed medication in a fall risk scale, identifying subgroups of drugs and inpatient units with higher risk of falls.
Method:
retrospective study on prescription order forms given by medical clinic, surgical clinic, and general intensive care unit. Risk factors under consideration: 1) orthostatic hypotension; 2) arterial hypotension; 3) arterial hypertension; 4) bradycardia; 5) psychomotor agitation; 6) mental confusion; 7) dizziness; 8) drowsiness/sedation; 9) reduced eyesight; 10) seizures; 11) atonia/dystonia/muscle weakness; 12) hypoglycemia; 13) urgent urination and 14) urgent defecation/diarrhea. Risk levels adopted: 0: 0 factor; I: 1-2 factors; II: 3-5 factors; III: 6-9 factors; IV: 10-14 factors.
Results:
3893 drugs were analyzed and stratifi ed in levels: 0 22.7%; I 33.5%; II 28%; III 15.1%; IV 0.7%. Levels III and IV more often refer to drugs for stomach acid disorders, 22.6%, and psycholeptics, 100%.
Conclusion:
knowing the risk factors associated with medication may help prevent and reduce falls, especially when therapeutic regimens cannot be modifi ed.

Os fatores de risco para queda podem ser categorizados como fatores intrínsecos, aqueles
diretamente relacionados com o indivíduo, e como fatores extrínsecos, os relacionados a
fatores ambientais. Os fatores intrínsecos englobam idade, sexo, efeitos de certos
fármacos, condições clínicas, tais como cardiopatias, doença osteoarticular, status
neurológico e mental, distúrbios da marcha, sedentarismo, deficiência nutricional. Com
relação aos fatores extrínsecos podem ser citados como exemplos iluminação inadequada,
obstáculos, piso irregular ou escorregadio, grades de proteção não elevadas, falta de
corrimãos(

Este trabalho teve como objetivo verificar a frequência dos medicamentos prescritos em clínica médica, clínica cirúrgica, unidade de terapia intensiva geral, estratificados em uma escala de risco de queda de pacientes.
MÉTODO
Realizou-se um estudo retrospectivo em prescrições médicas de pacientes internados em um hospital universitário, do interior do Paraná, Brasil, enviadas ao Serviço de Farmácia Hospitalar entre 01 e 15 de junho de 2013, totalizando 309 prescrições.
Para a coleta dos dados foi utilizado um formulário estruturado, dividido em duas partes: 1) unidades de internação e 2) graus de risco para queda de pacientes dos medicamentos prescritos.
Critérios de inclusão
Foram analisados todos os medicamentos padronizados na Instituição, prescritos para pacientes adultos, de ambos os sexos, internados no período da coleta de dados, nas unidades assistenciais: clínica médica, clínica cirúrgica e unidade de terapia intensiva geral (UTI).
Considerou-se os medicamentos constantes da padronização vigente, conforme aprovação da Comissão de Farmácia e Terapêutica institucional.
Critérios de exclusão
Foram excluídas as prescrições médicas de pacientes internados em unidades assistenciais não incluídas no estudo e medicamentos não padronizados prescritos.
Classificação de medicamentos
A avaliação dos medicamentos foi embasada no Sistema de Classificação Anatômico Terapêutico Químico, o Anatomical Therapeutic Chemical (ATC) Classification System do WHO Collaborating Centre for Drug Statistics Methodology - World Health Organization - Drug Utilization Research Group (WHO-DURG). O ATC divide e codifica os medicamentos em 14 grupos anatômicos principais (1º nível de classificação), de acordo com o órgão ou sistema sobre os quais possuem ação. Os medicamentos, dentro de cada grupo anatômico, são arranjados em subgrupos terapêuticos (2º nível) e sequencialmente em subgrupos farmacológicos (3º nível) e químicos (4º nível), sendo o 5º nível o próprio fármaco. Baseado no ATC, o fármaco N05BA01, por exemplo, corresponde a: N sistema nervoso, 05 psicolépticos, B ansiolíticos, A derivados benzodiazepínicos e 01 diazepam.
Nesta investigação adotou-se o 2º nível de classificação: subgrupos terapêuticos.
Fatores de risco
A determinação dos fatores de risco para queda de pacientes foi realizada mediante revisão em literatura pertinente.

Verificação dos efeitos de medicamentos definidos como fatores de risco para queda de pacientes
Quanto à verificação dos efeitos de medicamentos, definidos como fatores de risco para queda de pacientes após revisão, utilizou-se a base de dados na internet Micromedex® 2.0 Truven Health Analytics Inc. Foram considerados somente os efeitos que ocorrem em doses recomendadas e descritos como comuns ou com ocorrência maior ou igual a 5%. Foram excluídos os efeitos descritos como raros, muito raros, ocasionais, isolados ou não comprovados.
Graus de risco de queda de pacientes
Foram estabelecidos como graus de risco de queda de pacientes: Grau 0: 0 fator; Grau I: 1-2 fatores; Grau II: 3-5 fatores; Grau III: 6-9 fatores e Grau IV: 10-14 fatores.
Análise dos dados
Os dados foram digitados, revisados, processados e analisados, utilizando-se o programa Microsoft Office Excel 2010®, e submetidos a análise descritiva.
O presente estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Estadual de Ponta Grossa, sob Parecer N.º 347625/2013.
RESULTADOS
Foram incluídas neste estudo 309 prescrições; destas, 138 (44,7%) foram provenientes da clínica médica, 77 (24,9%) da clínica cirúrgica e 94 (30,4%) da unidade de terapia intensiva geral.
Durante o período de coleta de dados foram prescritos 3893 medicamentos, sendo 46,2% (1797/3893) na clínica médica, 10,4% (406/3893) na clínica cirúrgica e 43,4% (1690/3893) na unidade de terapia intensiva geral.
Na Tabela 1 verifica-se que do total de 3893 medicamentos prescritos, 22,7% (883) foram estratificados como Grau 0, 33,5% (1304) como Grau I, 28,0% (1090) como Grau II, 15,1% (588) como Grau III e apenas 0,7% (28) como Grau IV.

Ao analisar os medicamentos prescritos, estratificados como Grau 0 de risco de queda (0 fator), 84,4% (745/883) pertenciam ao subgrupo terapêutico B05 Substitutos do sangue e soluções de perfusão. Dos 1304 medicamentos prescritos, estratificados como Grau I de risco de queda (1-2 fatores), pertenciam aos subgrupos terapêuticos: B05 Substitutos do sangue e soluções de perfusão, 22,7% (296), B01 Antitrombóticos 20,4% (266) e N02 Analgésicos, 16,7% (218), conforme Tabela 2. Dos 1090 medicamentos prescritos estratificados como Grau II de risco de queda (3-5 fatores), 18,6% (203) faziam parte do subgrupo A10 Fármacos usados no diabetes e 16,2% (177) C03 Diuréticos. Em relação aos medicamentos prescritos estratificados como Grau III de risco de queda (6-9 fatores), percebeu-se que os subgrupos terapêuticos mais frequentemente observados foram A02 Fármacos para distúrbios da acidez gástrica, 22,6% (133/588), A03 Fármacos para distúrbios funcionais gastrintestinais, 22,4% (132/588) e N05 Psicolépticos, 19,7% (116/ 588). Os 28 medicamentos prescritos estratificados como de maior risco de queda, Grau IV (10-14 fatores), 100% (28) pertenciam ao subgrupo terapêutico N05 Psicolépticos. (Tabela 2)

Na clínica médica, 34,6% (622/1797) dos medicamentos foram estratificados como Grau I (1-2 fatores) e 29% (521/1797) como Grau II (3-5 fatores). Na clínica cirúrgica, 32,0% (130/406) dos medicamentos prescritos foram estratificados como Grau I (1-2 fatores). E em relação aos medicamentos prescritos na unidade de terapia intensiva geral, 32,6% (552/1690) foram estratificados como Grau I (1-2 fatores) e 29,5% (498/1690) como Grau II (3-5 fatores). (Tabela 3).

DISCUSSÃO
O maior número de medicamentos prescritos foi nas unidades de internação clínica médica
(46,2%) e UTI (43,4%). O uso de medicamentos é um fator intrínseco de forte relação com
as quedas de pacientes(

quedas em uma população com idade igual ou acima de 55 anos e encontraram uma frequência
percentual de quedas maior, 60% versus 25% respectivamente, em indivíduos que recebiam
seis ou mais medicamentos do que aqueles que recebiam apenas um. Em outro

analisados 81 medicamentos com risco associado à queda de pacientes. Durante o período
de estudo 151 pacientes experimentaram uma queda, destes, 144 (95,4%) estavam tomando ao
menos 1 medicamento classificado pelos autores como de alto risco.
A frequência de prescrição dos medicamentos estratificados como Grau II (3-5 fatores), na clínica cirúrgica (17,5%) foi menor quando comparada à clínica médica (29,0%) e à UTI (29,5%). Entretanto, os medicamentos estratificados como Grau III (6-9 fatores), portanto de risco mais elevado, foram mais frequentemente prescritos (26,1%) na clínica cirúrgica, em comparação aos prescritos na clínica médica (12,9%) e UTI (14,8%).
Em relação aos medicamentos prescritos estratificados como Grau III, verificou-se que os subgrupos terapêuticos mais frequentemente observados foram A02 Fármacos para distúrbios da acidez gástrica (22,6%) e A03 Fármacos para distúrbios funcionais gastrintestinais (22,4%). Contudo, na literatura consultada não foram citados, apesar de fármacos destes subgrupos, como ranitidina e metoclopramida, apresentarem muitos dos fatores de risco incluídos neste estudo.
Vários estudos têm demonstrado que medicamentos com ação sobre o sistema nervoso central
têm maior risco de queda de pacientes, principalmente benzodiazepínicos e
antipsicóticos(

III). Psicolépticos alteram as funções cognitivas como atenção, memória e orientação,
importantes para o controle postural e a manutenção do equilíbrio. Além disso, causam
sedação, alterações psicomotoras, relaxamento muscular e bloqueio adrenérgico,
aumentando a ocorrência de hipotensão ortostática(

No nosso estudo, somente 0,7% dos medicamentos prescritos foram estratificados como de maior risco de queda, Grau IV (10-14 fatores), entretanto 100% pertenciam ao subgrupo terapêutico N05 Psicolépticos. Em relação às unidades de internação, apenas 1,4% e 0,2% do total de medicamentos prescritos na clínica médica e na UTI, respectivamente, foram estratificados como Grau IV. Este dado é importante, pois pacientes internados em unidade de terapia intensiva, apesar de condições clínicas mais graves, são assistidos em tempo integral e, na maioria das vezes sedados, correndo menor risco de queda. Pacientes internados em unidades de clínica médica podem se sentir autônomos para realizar atividades básicas, como por exemplo, deambular e ir ao banheiro, estando desta forma mais expostos aos fatores de risco extrínsecos.
Constatou-se também que 18,6% dos medicamentos estratificados como Grau III pertenciam
ao subgrupo A10 Fármacos usados no diabetes. O principal efeito deste subgrupo
terapêutico que pode propiciar quedas é a hipoglicemia. No trabalho de Johnston et

). Na nossa investigação, os diuréticos foram estratificados
como Grau II (3-4 fatores) e representaram 16,2% dos medicamentos desta categoria
prescritos. A diurese está associada à hipotensão arterial, atonia, distonia ou fraqueza
muscular, urgência em micção e, da mesma forma que muitos N05 Psicolépticos, à
hipotensão ortostática.
É importante ressaltar que queda é um risco assistencial multifatorial que pode ocorrer em decorrência tanto de condições prévias do paciente, ou seja, presentes na admissão hospitalar, quanto da assistência prestada durante a internação. Na realização deste estudo, não foram considerados importantes fatores causais ou interferentes, tais como idade, condição clínica, patologias e principalmente medicamentos em uso ou anteriormente utilizados pelo paciente. O foco do nosso trabalho foi contribuir para o conhecimento acerca dos medicamentos utilizados no hospital para que, desta forma, possam ser implementadas medidas preventivas específicas e de segurança, de modo a preservar a saúde dos pacientes e a qualidade do atendimento prestado.
CONCLUSÃO
Apesar da nossa investigação ter sido de caráter descritivo e não ter tido como objetivo realizar inferências de causa e efeito entre diversos fatores de risco de queda, foi possível encontrar resultados semelhantes aos encontrados na literatura científica sobre o assunto.
O conhecimento sobre os fatores de risco associados aos medicamentos pode contribuir para a prevenção e diminuição de quedas, sobretudo quando regimes terapêuticos não podem ser modificados.
É importante uma avaliação constante de todos os profissionais de saúde sobre os fatores de risco intrínsecos e extrínsecos para implementar estratégias de prevenção que compreendam não só as ações voltadas aos medicamentos, mas também reabilitação da capacidade funcional, educação para o autocuidado e aumento da supervisão de enfermagem nos períodos e locais de maior incidência de quedas, diminuindo desta forma estes eventos adversos e trilhando o caminho para a excelência do cuidar.
