Educação profissional sobre demências na atenção primária à saúde: revisão integrativa

RESUMO

Objetivo:

Investigar as abordagens educativas mais comumente utilizadas na capacitação em demência para profissionais da Atenção Primária à Saúde.

Método:

Revisão integrativa de literatura, realizada entre abril e junho de 2018, nas bases de dados: PubMed, LILACS e IBECS. Foram utilizados os descritores Training, Health Personnel, Dementia, Primary Health Care para PubMed e os termos Mesh Training Programs, Health Personnel, Dementia, Primary Health Care, para LILACS e IBECS.

Resultados:

A amostra foi composta por 13 artigos, sendo oito publicados nos últimos 5 anos (62%); sete artigos com abordagem quantitativa (54%); sete artigos produzidos no continente europeu (54%), seguidos de cinco publicados no continente norte-americano (38%). Quanto aos periódicos, todos eram da área da saúde (100%).

Conclusão:

Algumas estratégias educacionais foram combinadas e utilizadas para a capacitação. Melhorias significativas em conhecimento, habilidades e atitudes das equipes no acompanhamento às demências foram evidenciadas.

ABSTRACT

Objective:

To investigate the most commonly used educational approaches in dementia training for primary health care professionals.

Method:

Integrative literature review, conducted between April and June of 2018, in PubMed, LILACS and IBECS databases. The descriptors used were: Training, Health Personnel, Dementia, Primary Health Care for PubMed; and the MeSH terms, Training Programs, Health Personnel, Dementia, and Primary Health Care for LILACS and IBECS.

Results:

The sample consisted of 13 articles; eight were published in the last five years (62%); seven articles with a quantitative approach (54%); seven articles produced on the European continent (54%), followed by five published on the North American continent (38%). All journals were from the health area (100%).

Conclusion:

Educational strategies were combined and used for education. Significant improvements in knowledge, skills, and attitudes of the teams with regard to professional management of dementias were evidenced.

Método:

La muestra constó de 13 artículos, ocho fueron publicados en los últimos cinco años (62%); siete artículos tuvieron un enfoque cuantitativo (54%); se produjeron siete artículos en el continente europeo (54%), seguidos de cinco publicados en el continente norteamericano (38%). Todas las revistas fueron del área de salud (100%).

Conclusión:

Las estrategias educativas se combinaron y se utilizaron para la enseñanza. Se evidenciaron mejoras significativas en el conocimiento, las habilidades y las actitudes de los equipos con respecto a la gestión profesional de las demencias.

INTRODUÇÃO

As demências são uma das principais causas de incapacidade na velhice. Em 2010, estimava-se que haveria um total de 35,6 milhões de pessoas vivendo com essa síndrome em todo o mundo. Projeções indicam que esse número duplica a cada 20 anos, podendo alcançar 65,7 milhões em 2030 e 115,4 milhões em 2050. A cada ano surgem cerca de 7 milhões de casos novos de demência em todo o mundo, ou um novo caso a cada 4 segundos, assim distribuídos: 3,6 milhões (46%) na Ásia, 2,3 (31%) na Europa; 1,2 (16%) nas Américas e meio milhão (7%) na África(11 Burlá C, Camarano AA, Kanso S, Fernandes D, Nunes R. No Brasil, estudo de metanálise que objetivou calcular a taxa de prevalência e projetar o crescimento do número de idosos com demências em um futuro próximo identificou taxa geral de prevalência em torno de 7,6%, maior para a doença de Alzheimer, em torno de 54% dos casos, crescente com a idade e mais elevada entre as mulheres. Em 2020, quando estimada juntamente com o envelhecimento populacional, esta taxa será de 7,9%. No contexto brasileiro, a tarefa de cuidar de um idoso dependente em geral cabe aos familiares mais próximos, em geral do sexo feminino, como companheiras e filhas. Tal prática reduz os custos da assistência hospitalar e institucional aos idosos incapacitados, e a permanência do idoso em sua casa e sob os cuidados de suas famílias visa ainda seu conforto e sua dignidade, não obstante represente um ônus para a maioria das famílias(33 Silva CF, Passos VMA, Barreto SM. No cuidado às demências, os familiares são alçados à condição de cuidadores inesperadamente e deparam-se com tarefas múltiplas e desafiadoras, que abrangem desde aceitar o diagnóstico e até reprogramar a rotina para incluir atividades relacionadas ao cuidado, além de prover suporte financeiro e legal ao portador da demência. Na maioria das vezes, cuidar desses pacientes é um ato solitário, sem garantia de retribuição, reconhecimento ou afeto, o que pode originar sentimentos ambíguos por parte do cuidador em relação ao idoso, tais como raiva, culpa, angústia e medo. Por outro lado, pode ocasionar sentimentos de carinho, amor e gratidão, como forma de retribuir os cuidados recebidos em outro momento da vida. Para os profissionais de saúde, saber reconhecer os sintomas da doença e orientar os cuidadores de forma efetiva pode colaborar para diminuir a tensão entre o paciente e os responsáveis pelo cuidado. Em geral, os cuidadores beneficiam-se do suporte educacional e emocional proveniente do profissional de saúde(33 Silva CF, Passos VMA, Barreto SM. Recentemente, a comunidade global reconheceu a necessidade de ações para colocar a demência na agenda da saúde pública. A prevalência elevada das demências, seu impacto econômico nos cuidadores, nas famílias e na coletividade, assim como o estigma e a exclusão social associados à doença constituem grandes desafios, sobretudo em países que estão envelhecendo rapidamente, como o Brasil(22 World Health Organization-WHO. Na maioria dos países ocidentais, os serviços de cuidados primários estão em melhor posição para fornecer assistência integral à saúde de pacientes com demência(1010 Tuero GC, Garre-Olmo J, Lòpez-Pousa S, Vilalta J, Limon E, Caja C. ) que no Brasil. O Ministério da Saúde tem elaborado propostas de educação permanente aos profissionais da saúde para que sejam capazes de promover o envelhecimento ativo e saudável. De fato, é papel dos trabalhadores da saúde colaborar para que mais pessoas atinjam idades avançadas com a melhor condição de saúde possível. Se considerada de forma ampliada, a saúde requer mudanças substanciais em direção à construção de um ambiente social e cultural mais favorável aos idosos. No trabalho das equipes da Estratégia Saúde da Família (ESF), as atividades realizadas em grupo, as ações coletivas na comunidade e a comunicação nas redes sociais de usuários são alguns dos recursos necessários para atuação nas dimensões cultural e social(1111 Ministério da Saúde (BR). Dada a aceleração do processo de envelhecimento nas últimas décadas, a Secretaria Municipal da Saúde de São Paulo propôs à rede de serviços de saúde na AB a adoção do instrumento Avaliação Multidimensional da Pessoa Idosa na Atenção Básica (AMPI-AB), e realizou a capacitação de alguns profissionais para a aplicação e atendimento a essa parcela da população. Tal instrumento possui o objetivo de auxiliar os profissionais da AB a atender as demandas das pessoas idosas e realizar a gestão do cuidado em saúde dos mesmos(1212 Secretaria de Saúde do Estado de São Paulo. A qualificação permanente da força de trabalho em saúde é essencial para melhorar o conhecimento e a conscientização sobre os benefícios de uma resposta coordenada às necessidades de saúde das pessoas, especialmente dos mais idosos, haja vista que projeções indicam que seremos o sexto país em número de idosos em 2025(22 World Health Organization-WHO. OBJETIVO

Investigar as abordagens educativas mais comumente utilizadas na capacitação em demência para profissionais da Atenção Primária à Saúde (APS).

MÉTODOS

Realizou-se um estudo de revisão de literatura do tipo integrativa, cujo intuito é reunir e sintetizar o conhecimento pré-existente sobre uma determinada temática. Proporciona compreensão global do fenômeno analisado por meio da síntese do conhecimento produzido, pois inclui estudos experimentais e não-experimentais. Envolve seis etapas: identificação do tema e seleção da questão de pesquisa, estabelecimento de critérios para inclusão e exclusão de estudos, avaliação dos estudos incluídos na revisão integrativa, definição das informações a serem extraídas dos estudos selecionados, interpretação dos resultados, apresentação e divulgação dos resultados(1313 Souza MT, Silva MD, Carvalho R. A temática desta produção teve como base a seguinte questão norteadora: “Quais abordagens educativas descritas na produção científica são adotadas na capacitação em demência de profissionais da APS?”

As bases de dados on-line utilizadas para a composição da amostra deste estudo foram: PubMed, LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde) e IBECS (Índice Bibliográfico Español en Ciencias de la Salud). Os termos Mesh (Medical Subject Heading) utilizados na PubMed foram: Training Programs AND Health Personnel AND Dementia AND Primary Health Care. Na LILACS e IBECS, foram utilizados os descritores em Ciências da Saúde (DECs) Training AND Health Personnel AND Dementia AND Primary Health Care. Incluíram-se artigos primários e secundários publicados na íntegra, que descreveram estratégias educativas ou programas de educação em demência direcionados aos profissionais da APS, publicados nos últimos quinze anos (de 2004 a 2018), nos idiomas inglês e espanhol.

A coleta de dados ocorreu entre abril e junho de 2018, em duas etapas. A primeira consistiu na busca dos artigos nas bases de dados: PubMed - 330; LILACS - 2; IBECS - 7. Na segunda foi realizada a leitura dos títulos (PubMed - 92; LILACS - 0; IBECS - 0), dos resumos (PubMed - 35) e, por fim, dos artigos na íntegra para a identificação das estratégias educativas utilizadas na capacitação dos profissionais da APS, no que se refere às demências (PubMed - 13). Dentre os motivos para exclusão dos artigos obtidos na primeira busca, destacam-se os estudos que descreveram capacitação para profissionais da saúde da APS na temática “transtornos cognitivos no geral”, aqueles que descreveram capacitação em demência para os cuidadores informais e os que descreveram capacitação em demência fora do contexto da APS. A Figura 1 ilustra o processo de seleção dos artigos desta revisão.

Os treze artigos selecionados foram analisados utilizando-se um formulário elaborado pelas próprias autoras, com itens relativos a: a) dados de identificação do estudo (título do artigo, ano de publicação, periódico e tipo de publicação); b) objetivo do estudo; c) delineamento do estudo (experimental, quase experimental, não experimental, e os diferentes modelos dentro desta última categoria, como correlacional, prospectivo, retrospectivo, survey e estudos documentais ou bibliográficos); d) estratégias utilizadas na capacitação em demência de profissionais da APS; e) categorias profissionais que receberam a capacitação. Foram lidos em profundidade e analisados de forma crítica para garantir a validade da revisão(1414 Mendes KDS, Silveira RCCP, Galvão CM. ). Os dados identificados foram transcritos, organizados e armazenados em planilha do Microsoft Office Excel®. A síntese resultante é apresentada a seguir.

RESULTADOS

No tocante à caracterização do corpus, os treze estudos selecionados são em língua inglesa, publicados em sua maioria nos últimos cinco anos (n=8), com abordagem quantitativa predominante (quase experimentais n=4 e ensaios clínicos n=3), produzidos principalmente no continente europeu, que publicou sete artigos (Inglaterra n=3, Holanda n=3 e Alemanha n=1), seguidos de cinco publicados no continente norte-americano (Estados Unidos da América n=3 e Canadá n=2). Quanto à área dos periódicos, todos eram da área da saúde (Medicina n=11, Educação em Saúde n=1 e Enfermagem n=1).

Nos Quadros 1 e 2 apresenta-se o panorama geral das treze publicações, destacando-se os objetivos dos estudos, as estratégias educacionais adotadas para a capacitação em demência dos profissionais de saúde na APS e os desfechos dos estudos de intervenção educativa.

DISCUSSÃO

No Brasil, a Educação Permanente em Saúde (EPS) foi instituída pelo Ministério da Saúde como política pública em 2004 e modificada em 2007. Pautada no conceito de educação problematizadora de Paulo Freire, sua consolidação depende de que os processos educativos voltados aos trabalhadores de saúde objetivem a transformação das práticas profissionais e da própria organização do trabalho. Para que uma estratégia de educação integrada possa ser definida como EPS, ela deve estar inserida no contexto das práticas na vida cotidiana das organizações; ser reflexiva e participativa, voltada à construção conjunta de estratégias que busquem soluções para os problemas evidenciados, já que eles não existem sem os sujeitos ativos que os criam e modificam; ser inesgotável, na qual os vários momentos e modalidades específicas combinam-se em um projeto global de desenvolvimento ao longo do tempo; ser orientada para o desenvolvimento e a mudança institucional das equipes e dos grupos sociais, o que pressupõe orientá-la para a transformação das práticas coletivas. A revisão narrativa que objetivou determinar os efeitos de intervenções educativas sobre a demência dirigidas a profissionais da atenção primária identificou que cinco de seis artigos analisados apresentaram efeitos significativos após as intervenções: uma oficina de pequenos grupos e um sistema de apoio à decisão aumentaram as taxas de detecção de demência; um seminário interativo de duas horas aumentou a suspeita dos médicos clínicos gerais em relação à demência; a adesão às diretrizes de demência só melhorou quando uma intervenção educacional foi combinada com a nomeação de gerentes de cuidado, e essa intervenção combinada também melhorou a qualidade de vida dos pacientes e dos cuidadores. Efeitos sobre o conhecimento e atitudes foram menores. Onze (85%) estudos citaram que, além da criação do programa de educação em demência, é necessário estruturar um programa de gestão em demência de caráter permanente, com modificações dinâmicas nos processos de trabalho como forma de atingir melhores resultados. Verifica-se, portanto, que é necessário o desenvolvimento de um programa global de educação interprofissional em demência. Deve incluir aulas teóricas para os profissionais de saúde -estudantes e já formados -, além de treinamento avançado em programas de educação permanente, abordando os tópicos centrais: diagnóstico precoce, apoio pós-diagnóstico, planejamento de cuidados avançados para os pacientes e seus cuidadores, e cuidados colaborativos eficazes. Diferentes metodologias educativas são necessárias, desde a participação em aulas teóricas presenciais, bem como plataformas virtuais com aulas teóricas à distância, e a possibilidade de esclarecer dúvidas, analisar e discutir estudo de casos com participação interdisciplinar, além de discussões dos casos atendidos pela equipe de forma continuada(1515 Dreier-Wolfgramm A, Michalowsky B, Austrom MG, Van der Marck MA, Iliffe S, Alder C, et al. No Brasil, a EPS é ferramenta essencial para a mudança das práticas na AB, uma vez que sua sistemática “vai da prática à informação, da informação à aquisição de competências e capacidades, da aquisição à programação de soluções práticas”. Na problematização de práticas e estratégias educacionais considera: identificar problemas, com estudo de casos, trabalho de campo, sistematização de dados locais, construção e priorização de problemas; ampliar o conhecimento, por meio de seminário de estudos, estágio in loco, grupos de discussão, teleconferências e redes interativas; desenvolver competências específicas e da equipe, mediante supervisão capacitante, treinamentos focalizados específicos, oficinas de elaboração de projetos de trabalho; buscar soluções, colocá-las em prática e avaliá-las, utilizando grupos operativos de qualidade e oficinas de programação local para avaliação de processos e resultados(2929 Ministério da Saúde (BR). Limitações do estudo

Foram analisados comparativamente estudos descritivos, estudos experimentais e quase-experimentais. Neste sentido, como limitação do estudo indica-se a não ampliação do critério de inclusão tempo de publicação (maior que 15 anos).

Contribuições para a saúde e políticas públicas

Considerando que o Brasil se encontra em um período de transição epidemiológica e a ESF é considerada como forma prioritária para a reorganização da AB no País, indicam-se intervenções educativas junto às equipes de saúde da família na temática das demências. Além disso, é recomendável a estruturação de um programa de gestão em demência que dê suporte às equipes e à operação para que possam atender a mais essa necessidade de saúde, sem deixar de assistir aos outros programas vigentes.

As evidências e informações identificadas no estudo fornecem subsídios para o planejamento de políticas públicas de saúde que atendam às necessidades de saúde dos idosos e seus cuidadores na temática demências, no contexto cultural brasileiro.

CONCLUSÃO

Os resultados dessa revisão integrativa evidenciaram que uma combinação de estratégias educacionais foram adotadas junto às equipes da APS, na temática demências, bem como programas estruturados de gestão para suporte, educação permanente e monitoramento das equipes para uma resposta mais efetiva das ações. Com essas medidas, foi possível identificar melhoras significativas no conhecimento, nas habilidades e nas atitudes das equipes no acompanhamento às demências.
