Conhecimento científico sobre infecções pelo novo coronavírus no idoso: scoping review

RESUMO

Objetivo:

mapear o conhecimento científico sobre a COVID-19 no idoso.

Método:

revisão da literatura do tipo scoping review. Realizou-se uma busca nas bases de dados PubMed, CINAHL, Web of Science e LILACS. Incluíram-se os artigos originais que responderam à questão norteadora: quais os conhecimentos científicos disponíveis no mundo sobre a COVID-19 no idoso?

Resultados:

foram incluídos 31 artigos, a maioria realizada na China (n=23) e de delineamento retrospectivo (n=13) e estudos de casos/série de casos (n=13). Com base nos achados, emergiram seis categorias temáticas: sinais e sintomas clínicos (n=12); outras informações (n=9); desfechos adversos/complicações decorrentes da COVID-19 (n=8); relação da idade e maior gravidade da COVID-19 (n=8); variáveis relacionadas ao óbito pela COVID-19 (n=8); achados diagnósticos (n=8).

Conclusão:

os achados reforçam a maior vulnerabilidade do idoso ao agravamento da COVID-19, bem como as complicações decorrentes da doença, incluindo a maior ocorrência de óbito.

los hallazgos refuerzan la mayor vulnerabilidad de los ancianos al agravamiento del COVID-19, así como las complicaciones derivadas de la enfermedad, incluida la mayor ocurrencia de muerte.

ABSTRACT

Objective:

to map the scientific knowledge about COVID-19 in older adults.

Method:

this is a scoping review literature review. A search was performed in the PubMed, CINAHL, Web of Science and LILACS databases. Original articles that answered the guiding question were included: what scientific knowledge is available in the world about COVID-19 in older adults?

Results:

thirty-one articles were included, most of them carried out in China (n=23) and with a retrospective design (n=13) and case studies/case series (n=13). Based on the findings, six thematic categories emerged: clinical signs and symptoms (n=12), other information (n=9), adverse outcomes/complications resulting from COVID-19 (n=8), age relationship and greater severity of COVID-19 (n=8), variables related to death by COVID-19 (n=8), and diagnostic findings (n=8).

Conclusion:

the findings reinforce older adults’ greater vulnerability to the worsening of COVID-19, as well as to complications resulting from the disease, including the greater occurrence of death.

INTRODUÇÃO

A COVID-19, do termo em inglês coronavírus disease 2019, é uma doença respiratória aguda, causada pelo novo coronavírus (SARS-CoV-2)(11 Zhang G, Zhang J, Wang B, Zhu X, Wang Q, Qiu S. O quadro clínico da COVID-19, na maioria das ocorrências, tem sido relatado como semelhante a outras infecções respiratórias: febre (≥37,8ºC), coriza, tosse, geralmente seca e cansaço(33 Strabelli TMV, Uip DE. ), sendo a reverse-transcriptase polymerase chain reaction (RT-PCR - reação em cadeia da polimerase com transcrição reversa) por swab nasofaríngeo o padrão de referência para o diagnóstico definitivo de infecção pelo novo coronavírus(33 Strabelli TMV, Uip DE. No entanto, estima-se que 5% dos infectados desenvolvem a forma mais grave da doença, com sintomas de dispneia e/ou sinais de sangramento pulmonar, linfopenia grave e insuficiência renal(33 Strabelli TMV, Uip DE. Devido às alterações fisiológicas que ocorrem com o processo de envelhecimento humano e comprometem o sistema imunológico, e ao maior número de complicações decorrentes das doenças crônicas, a população idosa tem sido a de maior vulnerabilidade às formas graves da COVID-19 e para a evolução a óbito(44 D'Adamo H, Yoshikawa T, Ouslander JG. Em uma pesquisa epidemiológica de coorte realizada na China, a maioria dos casos de COVID-19 e os piores prognósticos desta doença foram entre os idosos(55 Pan A, Liu L, Wang C, Guo H, Hao X, Wang Q, et al. Corroborando esses achados, dados demonstraram que a ocorrência de óbito pela COVID-19 aumentou exponencialmente de 3 a 5% entre pessoas com 65├75 anos, de 4 a 11% entre pessoas com 75├85 anos e de 10 a 27% entre pessoas com 85 anos ou mais de idade. Ademais, as pessoas com 65 anos ou mais de idade representaram mais 45% das internações hospitalares, 53% das ocupações nas Unidades de Terapia Intensiva e 80% dos óbitos(66 Centers for Disease Control and Prevention. ). Isso leva a crer que medidas mais abrangentes sejam necessárias para proteger a população idosa.

Neste sentido, fazem-se necessários estudos que subsidiem o atendimento dos profissionais de saúde à população idosa com COVID-19 e que, do mesmo modo, deem o direcionamento de ações setoriais e intersetoriais sob um olhar integrador entre todas as instâncias envolvidas no enfrentamento dessa pandemia. Este embasamento científico pode fundamentar o trabalho das equipes de saúde, a fim de elaborar intervenções que possam favorecer o gerenciamento e a atenção à saúde do idoso.

Diante do exposto, a scoping review é considerada uma ferramenta útil para identificação e síntese dos conhecimentos científicos(77 Peters MDJ, Godfrey C, McInerney P, Munn Z, Tricco AC, Khalil H. ), principalmente na pandemia de COVID-19, possibilitando o planejamento de ações assistenciais ao idoso, alicerces na prática baseada em evidências. Além disso, a avaliação dos estudos poderá direcionar áreas prioritárias que necessitam avançar no conhecimento sobre esse tema emergente.

Mapear o conhecimento científico sobre a COVID-19 no idoso.

MÉTODOS

Para a elaboração da pergunta de pesquisa, utilizou-se a estratégia Population, Concept and Context (PCC)(77 Peters MDJ, Godfrey C, McInerney P, Munn Z, Tricco AC, Khalil H. Como critério de elegibilidade, consideraram-se os artigos originais, os relatos de caso e série de casos, todos sobre a COVID-19 e com dados referentes à população idosa, a partir de dezembro de 2019, momento em que se publicou o primeiro estudo sobre a referida doença(11 Zhang G, Zhang J, Wang B, Zhu X, Wang Q, Qiu S. ). Adotaram-se como critérios de exclusão cartas ao editor, artigos de opinião e reflexão.

Estratégia de busca

Na primeira etapa, a busca foi realizada nas bases de dados: National Library of Medicine National Institutes of Health (PubMed), Cumulative Index to Nursing and Allied Health Literatura (CINAHL), em que se analisaram os títulos, resumos e descritores dos artigos encontrados. Na segunda etapa, todas as palavras-chave e descritores foram utilizados para identificar os artigos nas demais bases de dados: Web of Science e Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). Na terceira etapa, foram analisados, como fonte adicional, os estudos divulgados na plataforma PubCovid-19 (http://pubcovid19.pt/index.php), que abrange os artigos publicados sobre a COVID-19, divididos em diversos temas. Consideraram-se os estudos na temática “idosos”.

Realizou-se a busca de estudos primários no dia 17 de abril de 2020. Para tal, foram delimitados os descritores controlados e não controlados de acordo com as bases de dados, utilizando ferramentas de pesquisa avançada, ou seja, os descritores selecionados foram cruzados e combinados usando os operadores booleanos “AND” e “OR”. Os descritores controlados (MESH) adotados na estratégia de busca dos estudos primários foram: “Coronavirus, Coronavirus Infections”, “Severe Acute Respiratory Syndrome”, “SARS Virus” e “aged”, “aged, 80 and over”.

Considerando que não há descritores específicos disponíveis para o novo coronavírus e que o primeiro caso da doença foi relatado em dezembro de 2019(11 Zhang G, Zhang J, Wang B, Zhu X, Wang Q, Qiu S. ), utilizou-se o recorte temporal para a busca, nas bases de dados, dos estudos primários (dezembro de 2019 até o momento da coleta de dados).

Seleção dos estudos e extração dos dados

Os artigos encontrados foram importados para o programa na Web Endnote - Clarivate Analytics (https://access.clarivate.com/login?app=endnote) para a identificação e exclusão dos duplicados. Posteriormente, para o gerenciamento do processo de seleção da amostragem final, utilizou-se o programa na Web Rayyan QCRI - Qatar Computing Research Institute (https://rayyan.qcri.org/welcome).

Para a extração dos dados, elaborou-se um roteiro contendo informações relacionadas à autoria, ano e local de publicação, tipo de estudo, participantes, período do estudo e resultados. A avaliação dos artigos e a extração dos dados foram conduzidas por dois revisores independentes (NGNO e MADR), com discordâncias resolvidas por um terceiro revisor (AFB).

Análise das evidências

A síntese dos dados foi realizada de maneira descritiva considerando a construção de categorias temáticas, que abordaram os resultados de estudos originais sobre a COVID-19 na população idosa mundial.

RESULTADOS

) foi utilizado para apresentar as etapas de seleção dos artigos (Figura 1).

Na primeira etapa, foram encontrados 440 artigos nas bases de dados e 79 identificados na plataforma PubCovid-19, sendo 103 excluídos por duplicata. Após a leitura do título e resumo, 92 artigos foram incluídos para leitura na íntegra, com o intuito de responder à pergunta de pesquisa. Os estudos que não atenderam aos critérios de inclusão (n=61) foram excluídos pelos motivos: população diferente (n=19), delineamento do estudo (n=37) e outros (n=5). Portanto, 31 artigos compuseram a amostra final da scoping review (Figura 1).

Dos 31 artigos analisados, todos foram publicados no ano de 2020 e a maioria na China (n=23). Predominaram os estudos do tipo relato de casos (n=13) e retrospectivo (n=13), que objetivaram, principalmente, investigar as características epidemiológicas, clínicas e diagnósticas da COVID-19.

No que se refere aos estudos de casos/série de casos (n=13), grande parte (n=6) ocorreu no período de janeiro de 2020, visando relatar, de maneira geral, as características clínicas, os achados diagnósticos e as perspectivas de tratamento dos primeiros casos confirmados da COVID-19. Outras informações foram destacadas nesses relatos como: a melhora dos parâmetros laboratoriais seriados e de oxigenação com a infusão de plasma no tratamento de dois idosos internados com COVID-19(1010 Ahn JY, Sohn Y, Lee SH, Cho Y, Hyun JH, Baek YJ, et al. A síntese do conhecimento científico foi distribuída em dois grupos de acordo com o delineamento dos estudos, sendo o primeiro os relatos de casos/série de casos (n=13) (Quadro 1); e o segundo, os demais resultados científicos (n=18) sobre a COVID-19 na população idosa (Quadro 2).

Quanto aos outros delineamentos dos estudos (n=18) (Quadro 2), verificou-se que dois foram realizados somente com idosos; nos demais (n=16), os participantes foram constituídos pela população geral, os quais apresentavam resultados sobre a COVID-19, estratificados segundo faixa etária, que incluía os idosos. DISCUSSÃO

O atual estudo é um dos primeiros a mapear o conhecimento científico sobre a infecção pelo novo coronavírus no idoso. O mapeamento da literatura, que trata dos aspectos relativos à COVID-19 nessa população, permitiu delinear os diferentes contextos das investigações com esta temática no que se refere aos desfechos adversos/complicações, à relação da idade e maior gravidade da doença, aos óbitos, sinais clínicos, achados diagnósticos e outras informações.

A maioria dos estudos desta revisão concentrou na China, em razão de o surto ter iniciado neste país(11 Zhang G, Zhang J, Wang B, Zhu X, Wang Q, Qiu S. ), apresentando delineamento descritivo, com amostras não probabilísticas e consecutivas. A opção pelo delineamento possivelmente decorreu do caráter emergencial de construir conhecimento sobre a pandemia de COVID-19. Contudo, o conjunto dos achados denota que ainda são escassos os estudos no mundo que foram realizados especificamente com idosos, evidenciando a necessidade de ampliar o conhecimento científico nesta temática. Assim, cabe qualificar cada vez mais as produções científicas de acordo com a evolução da pandemia e, por conseguinte, avançar no conhecimento sobre a dinâmica da doença e seu impacto para a sociedade, população idosa e seus familiares.

A maior probabilidade de o idoso desenvolver complicações pela infecção do novo coronavírus comparado à população geral foi identificada nesta revisão. A análise das informações dos desfechos adversos relacionados à COVID-19 nos idosos, como a progressão da gravidade da doença e alta hospitalar, pode fornecer subsídios no direcionamento do tratamento mais eficaz da doença(3939 Lithander FE, Neumann S, Tenison E, Lloyd K, Welsh TJ, Rodrigues JCL, et al. ) no gerenciamento do cuidado e na gestão dos recursos dos serviços de saúde.

Favorece, ainda, a organização do trabalho na atenção primária, que, além de manter a continuidade do cuidado às doenças crônicas, também deverá empreender ações de vigilância em saúde. Nesta perspectiva, o enfermeiro, pautado no diagnóstico situacional de saúde, poderá identificar os idosos mais vulneráveis e promover a educação em saúde. A aproximação com a rede de apoio social do idoso propicia a capilaridade de informações efetivas de medidas preventivas e cuidados relacionados à COVID-19, postergando seus efeitos adversos.

Considerando a gravidade da doença e o seu pior prognóstico, reforça-se a necessidade de disseminar, em todos os espaços de atenção, medidas preventivas da COVID-19, visando propiciar os direitos à vida e à saúde do idoso. Nesse sentido, a prevenção da COVID-19 em idosos com idade avançada deve ser preferencial, sendo necessárias intervenções que possam conter a piora da doença em casos positivos, minimizando a ocorrência desses desfechos. Equipes experientes em geriatria e gerontologia precisam ter agilidade na tomada de decisão mais adequada ao contexto do idoso com idade avançada.

Entretanto, não foram encontrados estudos que oferecessem informações específicas sobre o tratamento desta doença, denotando a relevância de investimentos em pesquisas.

Nesta perspectiva, além do enfoque clínico individual, torna-se necessário, nessa fase da pandemia, avançar nas reflexões e pesquisas sobre a abordagem geriátrica e gerontológica ampla no cuidado aos idosos, a saber: quais os impactos do isolamento social para o idoso? Como lidar com as diversas situações que desfavorecem medidas de mitigação social no idoso, como aqueles que vivem em contexto de alta vulnerabilidade social e residem sozinhos? Qual a relação da fragilidade e das síndromes geriátricas com a ocorrência da COVID-19?

Tais reflexões e investigações são prementes, pois poderão subsidiar a atuação dos profissionais de saúde no cuidado ao idoso e sua família diante desse contexto pandêmico. Os especialistas da Sociedade Americana de Geriatria ressaltaram a relevância de expandir os conhecimentos da área de gerontologia e geriatria aos profissionais de saúde para compreenderem as peculiaridades da saúde do idoso e o seu impacto no enfrentamento da COVID-19(4242 Trucil DE, Medina-Walpole A. Ainda, a Sociedade Brasileira de Geriatria e Gerontologia (SBGG), ciente da gravidade da doença em idosos com comorbidades, recomenda a adoção de medidas de restrição de contato social, evitando aglomerações ou viagens durante a pandemia de COVID-19. Além disso, incentiva que as pessoas idosas devam ser preferencialmente atendidas em domicílio, evitando-se a exposição coletiva em serviços de saúde e que, em caso de serem assistidas por cuidadores ou profissionais de saúde, essas sejam triadas para síndromes gripais. Limitações do estudo

Os tipos de estudos disponíveis na literatura científica, no momento inicial da pandemia, eram, em sua maioria, descritivos e poucos realizaram investigações experimentais, com testagem das principais intervenções. Por outro lado, os estudos incluídos na revisão não foram analisados quanto às limitações metodológicas ou risco de viés, conforme proposto para scoping review. Esses dois aspectos podem ter reunido investigações com baixo nível de evidências, contudo atenderam ao objetivo proposto de apresentar uma visão geral sobre o conhecimento científico disponível, no mundo, sobre a COVID-19 no idoso.

Contribuições para a área da enfermagem

Nesta scoping review, não foram identificados estudos que abordassem os cuidados de enfermagem direcionados ao idoso acometido com COVID-19. Vale mencionar que o ano de 2020 foi escolhido para homenagear o bicentenário de Florence Nightingale, precursora da enfermagem moderna. ), e, a partir do seu legado, os enfermeiros têm desempenhado papel essencial na prevenção e controle de infecções assim como nas medidas de mitigação social(4646 Smith GD, Ng F, Li HCW. ). Tais bases teóricas precisam ser resgatadas para subsidiar a atuação dos profissionais de enfermagem no enfrentamento da COVID-19. O momento pandêmico da COVID-19 reforça a necessidade de o enfermeiro prestar um cuidado gerontológico qualificado e seguro, mediante fundamental capacitação profissional, sendo essencial desenvolver ações de atenção ao idoso, considerando, tanto no presente quanto nos cenários futuros, as particularidades e especificidades dessa população.

CONCLUSÃO

A maioria das pesquisas foi realizada na China com delineamento retrospectivo e do tipo estudo de casos ou série de casos. No que se refere aos relatos de casos ou série de casos, a maior parte objetivou relatar as características clínicas, achados diagnósticos e perspectivas de tratamento dos primeiros casos confirmados da doença. Quanto aos demais estudos, os participantes variaram de 17 a 13.909 idosos, com critérios heterogêneos de seleção. Foram identificadas seis categorias temáticas: desfechos adversos/complicações decorrentes da COVID-19; relação da idade e maior gravidade da COVID-19; variáveis relacionadas ao óbito pela COVID-19; sinais e sintomas clínicos; achados diagnósticos; outras informações.

De maneira geral, o conhecimento científico sobre a COVID-19 no idoso ainda é incipiente. Entretanto, já sinalizam elevada vulnerabilidade do idoso ao agravamento da COVID-19 e às complicações decorrentes da doença, que inclui maior proporção de internações na Unidade de Terapia Intensiva e de pneumonia severa e menor proporção de alta hospitalar comparada aos mais jovens. Demonstram, ainda, que a idade avançada foi relacionada à ocorrência de óbito. Observou-se também maior episódio de infecção da COVID-19 no idoso, elevado custo decorrente das mortes pela doença e alta ocorrência de comorbidades neste grupo populacional quando infectados. Neste contexto, sugere-se a realização de inquéritos nacionais, com amostras representativas da população idosa nos diversos estados brasileiros a fim de contribuir com melhoria da atenção à saúde do idoso frente à pandemia de COVID-19.
