Achados epidemiológicos e alterações oftalmológicas em diabéticos atendidos em hospital geral secundário

Objetivo:

Avaliar a associação entre alterações no exame oftalmológico, características epidemiológicas e controle metabólico em pacientes diabéticos.

Métodos:

Estudo transversal. Foram selecionados consecutivamante os diabéticos atendidos durante 2011 em um hospital secundário. Todos os pacientes responderam questionário e foram submetidos a exame oftalmológico.

Resultados:

Foram estudados 103 pacientes, dos quais 72 (69,9%) eram do sexo feminino e 66 (64%) da cor branca. A média de idade foi de 59 (+/- 9,21) anos. Sessenta e quatro por cento dos participantes referiram renda aproximada de até 1 salário mínimo, 58,2% tinham ensino fundamental incompleto, 75,7% com história de diabetes familiar, 45,6% informaram realizar controle metabólico regular, 54,3% não observavam cuidados nutricionais, 28% usavam insulina, 99% eram diabéticos do tipo-2. Ao exame, 72,8% apresentaram acuidade visual corrigida de 20/40. Foram estatisticamente significativas as relações entre complicações retinianas e o uso de insulina (OR=8,3; p=0,003) e da baixa acuidade visual com o uso de insulina (OR=5,48, p=0,021) e a idade (OR=11,8; p=0,003). Também foi observada relação entre a baixa de visão com escolaridade, idade e baixa renda

Conclusão:

Na população analisada, predominantemente de baixa renda e escolaridade, a condução inadequada da doença foi expressiva, o que se associou com a presença de complicações retinianas, reforçando a necessidade de adoção de medidas mais amplas para melhorar as estratégias de controle e prevenção do diabete mellitus.

Objective:

To evaluate the association between epidemiological and ophtalmological findings in diabetic patients.

Methods:

Cross-sectional study. We selected consecutively diabetic patients examined during 2011 which responded to a questionnaire and examination.

Results:

The sample comprised 103 patients, of whom 72 (69.9%) were female, 66 (64%) were Caucasian, average age 59 (+/- 9,21) years, 64% reported minimum wages, 58.2% did not finish elementary school, 75.7% reported family history of diabetes, 45.6% reported regularly perform metabolic control, 54.3% did not receive special nutritional care. On examination, 72.8% had visual acuity of 20/40. There was a significant association between retinal complications and insulin usage (OR=8,3; p=0,003), and between low visual acuity and age (OR=11,8; p=0,003) and insulin (OR=5,48, p=0,021), as well with lower education and income.

Conclusion:

In this low-income and low-education population, glycemic control was poor, and related to the development of diabetic retinopathy and the consequent low vision. These findings emphasize the need to adopt broader strategies to improve control and prevention of diabetes mellitus.

INTRODUÇÃO

O diabetes mellitus (DM) acomete, atualmente, aproximadamente 171 milhões de pessoas no mundo, de acordo com dados da Organização Mundial da Saúde(1World Health Organization. Media centre. Diabetes. ). Destes, provavelmente, 10 a 20% tem alguma forma de retinopatia e cerca de 1,78 milhões são cegos. Até 2030 projetase que 7,7% da população entre 20-70 anos (439 milhões) terá a doença, e destes, 70% ou mais estarão concentrados nos países em desenvolvimento(2Shaw JE, Sicree RA, Zimmet PZ. Esta diferença baseia-se em fatores como o crescimento populacional, envelhecimento, dietas inadequadas, obesidade e sedentarismo, além da redução nos valores da glicemia de jejum atualmente adotados para o diagnóstico de DM(1World Health Organization. ), a prevalência de DM em indivíduos com idade entre 30 e 69 anos é de 7,6%, sendo São Paulo, Porto Alegre e João Pessoa as capitais com as maiores prevalências.

O presente estudo tem por objetivo investigar o estado oftalmológico dos pacientes diabéticos provenientes de uma população do Sistema Único de Saúde (SUS) na cidade de Viamão, um município limítrofe com Porto Alegre, Rio Grande do Sul.

MÉTODOS

Trata-se de um estudo observacional com delineamento transversal. O projeto foi aprovado pelo Comitê de Ética e Pesquisa do Instituto de Cardiologia-Hospital de Viamão. Os critérios de inclusão foram: pacientes diabéticos atendidos em 2011 no ambulatório de oftalmologia do Instituto de Cardiologia-Hospital de Viamão, oriundos do SUS.

Todos os pacientes diabéticos atendidos foram consecutivamente convidados a participar do estudo, e, após esclarecidos sobre o mesmo, assinaram termo de consentimento, responderam questionário e realizaram exame oftalmológico completo.

O questionário foi elaborado para coletar informações sobre sexo, idade, cor, renda, escolaridade, histórico familiar de diabetes, presença de outras doenças sistêmicas, autoavaliação do controle realizado (boa, quando cumpriam todas as orientações rotineiramente; regular, traduzindo aderência parcial; ruim, quando não cumpriam o determinado), dieta (aderência, supervisão de nutricionista), frequência das revisões clínicas, conhecimento sobre sequelas potenciais do diabetes, periodicidade das glicemias de jejum (incluindo testes com glucômetros pessoais). Nos registros hospitalares foram levantadas informações sobre a hemoglobina glicosilada (último resultado disponível), e registros referentes a outras complicações não oculares associadas ao diabetes.

Todos foram submetidos à medida da acuidade visual (tabela logMAR) com a melhor correção, avaliação das motricidade intrínseca e extrínseca, tonometria de aplanação (Perkins), biomicroscopia anterior e oftalmoscopia binocular indireta sob midríase. Foram excluídos aqueles casos com preenchimento in- completo do consentimento, questionário, ou do exame oftalmológico.

Naqueles indivíduos que estavam em jejum superior a 2 h foi realizado o teste da glicemia capilar (glucômetro Accu-Chek Performa, Roche, Germany).

Todas as informações coletadas foram armazenados em banco de dados no programa Microsoft OfficeExcel 2003 e analisadas através do programa Stata 12.1.

A análise estatística incluiu descrição através de médias e desvios padrão ou proporções, correlações bivariadas de Pearson ou Spearman e análise multivariada (regressão logística). Foi considerado significativo um p alfa <0,05.

RESULTADOS

A amostra foi composta por 103 pacientes atendidos no período de março a dezembro de 2011. A média de idade dos entrevistados foi de 59, 6 (41-85) anos. Observou-se predomínio da cor branca (64%), sendo 26% negros e 10% mistos. Os dados completos encontram-se na tabela 1.

Ao exame oftalmológico, a acuidade igual ou superior a 20/40 foi observada em 73%, entre 20/50 e ausência de percepção luminosa, em 27%. Exame fundoscopico normal em 53%, 32% com RD não proliferativa sem edema macular, 8% RD não proliferativa com edema macular, 6% RD proliferativa de alto risco.

O teste do glucômetro foi obtido em 60% da amostra, tendo os seguintes resultados: 31% até 200 mg/dl; 18%, entre 201 a 300 mg/dl; 9% de 301 a 400 mg/dl; 1% entre 401 a 500 mg/dl e 1% acima de 501 mg/dl (59% estavam com mais de 6h de jejum).

Foram estatisticamente significativas na regressão logística, controlando-se entre si as variáveis, as relações entre complicações retinianas e o uso de insulina (OR=8,3; p=0,003) e da baixa acuidade visual com o uso de insulina (OR=5,48, p=0,021) e a idade (OR=11,8; p=0,003) (tabela 2).

DISCUSSÃO

No presente estudo, o perfil mais prevalente é o de um indivíduo adulto, diabético-2, de baixa renda e escolaridade, usando hipoglicemiante oral, e em quase metade dos casos, com retinopatia. Observa-se que a maioria refere ter acesso ao atendimento médico de forma trimestral ou semestral, mas desconhece as doenças sistêmicas associadas e não segue, de forma adequada, as recomendações recebidas.

Os valores obtidos na testagem da glicemia capilar foram elevados em sua maioria, e considerados fidedignos neste propósito(3434 Bergenstal RM. Evaluating the accuracy of modern glucose meters. Insulin. 2008;3(1):5-14.

Por estes dados verifica-se que o controle metabólico encontrado foi precário, ainda que estes doentes tenham acesso a atendimento especializado.

Relações estatísticas significantes foram detectadas, controlando-se as variáveis entre si, com o uso de insulina e a presença de retinopatia e da baixa de acuidade visual com a idade e a necessidade de insulinoterapia. Comment in Ophthalmology. 1998;105(10):1799-800.), e que, em nosso sistema público de saúde prevalecem aqueles perfis de maior risco, a inclusão de programas educacionais direcionados e de sistemas digitalizados não midriáticos para a aquisição de imagens retinográficas com telemedicina precisam ser adotados visando reduzir as estimativas de cegueira.

CONCLUSÃO

O presente levantamento persiste demonstrando que as condições essenciais no manejo da RD nos pacientes de nosso Sistema de Saúde, como o autoconhecimento da doença, periodicidade dos controles, suporte nutricional e adesão ao tratamento clínico, seguem completamente distantes do necessário. Pacientes com maior tempo de doença ou necessitando de maior quantidade de medicações mostram chances muito elevadas de perda funcional e dano retiniano severo.
