Aspectos da função renal em portadores do vírus da imunodeficiência humana (VIH)

Renal functions aspects in HIV-infected patients

ASPECTOS DA FUNÇÃO RENAL EM PORTADORES DO VÍRUS DA IMUNODEFICIÊNCIA HUMANA (VIH)

A síndrome da imunodeficiência humana (SIDA) atinge milhares de pessoas em todo o mundo.

Constatou-se que também o rim poderia ser afetado de forma significativa, quando uma elevação da sobrevida dos infectados com VIH passou a ocorrer.

Em cerca de 10% dos infectados, alterações renais têm sido descritas. Uma glomerulopatia relacionada ao VIH (HIVAN, da literatura inglesa) há muito relatada, caracteriza-se por proteinúria nefrótica, sem hipertensão arterial, curso rápido para deterioração funcional, ocorrência mais freqüente em negros, podendo apresentar aspectos decorrentes de lesões túbulos-intersticiais.

Trabalhos retrospectivos relataram HIVAN em paciente com ARC ou assintomáticos.

No Brasil, diversos autores já referiram amplo espectro de modificações renais em paciente com SIDA.

Nosso trabalho procurou avaliar aspectos da função renal em 47 pacientes infectados pelo vírus da SIDA, assintomáticos. Utilizando, nesta abordagem, exames que expressaram a taxa de filtração glomerular. Também analisamos a excreção de sódio e potássio, tanto em relação à taxa de filtração (excreção fracionada), como diferenciadamente nos seguimentos proximais e distais (reabsorção fracional proximal e distal de sódio) do nefron, através do clearance de lítio. Além disso verificamos a excreção de água (clearance de água livre e clearance osmolar), que se refere a homeostasia hídrica do meio interno e finalmente o pH urinário, refletindo a capacidade de acidificação urinária responsável pela manutenção do equilíbrio ácido-básico.

Todos esses aspectos funcionais revelaram-se dentro da normalidade segundo valores consagrados na literatura, tanto nos infectados quanto nos 23 indivíduos controles, não infectados pelo VIH.

Entre os pacientes que estavam em uso de droga anti-retroviral (21 pessoas) e os que não usavam qualquer medicação (26 pessoas), também não se observaram diferenças para os parâmetros renais avaliados.

Ressaltamos que, em relação aos controles deste trabalho, os infectados com o VIH concentraram mais a urina, reabsorvendo mais água, e apresentaram além disso, pH urinário maior, sem ser possível detectar a causa.

Como no grupo 1 (infectados), a maioria dos indivíduos estava na faixa de até cinco anos de início da doença e o grupo comportou-se de maneira homogênea, não se pode afirmar que o tempo de infecção determinou maior comprometimento renal. Reforçamos a hipótese já existente de que haja uma associação entre doenças oportunistas e disfunção renal.

Cabe também especular se ao invés de uma possível alteração na secreção do hormônio antidiurético (HAD) não estaria modificado nos indivíduos estudados, o limiar para sua secreção, o que explicaria a urina mais concentrada, com o clearance de água livre e osmolar maior nos portadores de VIH. Isso aponta para a necessidade de dosar o HAD e de estimar o limiar osmótico desses indivíduos através de sobrecarga hídrica, de forma a complementar nossa investigação.

RENAL FUNCTIONS ASPECTS IN HIV-INFECTED PATIENTS

AIDS is spread throughout the world and, nowadays, it infects millions of human beings.

Renal involvement has been described in about 10% of the VIH infected population. It was found that the kidneys were affected when those patients happen to live a longer period of life. Attention has been focused on a distinct pattern of structural renal disease that has come to be termed HIV-associated nephropathy (HIVAN).

A clinical picture of the major components of HIVAN began to emerge: nephrotic proteinuria, infrequency of hypertension, rapid advancing renal failure and frequent occurence in black people. It may also present some aspects due to tubular and intersticial lesions.

Early works have reported HIVAN in ARC or asymptomatic HIV-infected patients.

In Brazil, many authors have already referred to a large spectrum of renal abnormalities in patients with HIV-infection.

Our research aimed at assessing those aspects of the renal functions in 47 asymptomatic but infected individuals.

In our methodology, we used some examinations which were able to demonstrate the glomerular filtration rate (GFR). We also analysed sodium and potassium excretion both in relations to filtration rate (Fractional Excretion - FE) and through a differentiated form in the proximal and distal tubules of the nephron (proximal and distal fractional reabsorptions), by the litium clearance. Moreover, it was verified the water release (clearance of free water and solute excretion rate) which is related to a water homeostasis of the internal environment and finally, the urinary pH that reflects the acidification renal capacity to maintain the acid-base balance.

Every studied functional aspect has revealed a normal level either in the infected groups or in the control ones, according to the well known values in the bibliography.

Among the patients in use of antiviral therapy (21) and those who were not taking any medicines (26), differences were not observed against the analysed renal parameters.

It is worth noting that as far as controls are concerned, in the HIV infected individuals the urine was much more concentrate and presented a higher urinary pH.

Also, the infected group showed homogeneous results in relation to the period of infection. These findings may lead to the conclusion that it is not the interval between the diagnosis and the kidney evaluation, the main factor which could determine the renal damage.

It is possible as well, to hypothetize if instead of the altered HAD secretion, the hormone secretion threshold might be altered. This observation could explain the urine higher concentration along with a higher free water and osmolar clearance in the infected HIV patients who participated in this research.

All these investigations may address to the relevance to accurately measure the antidiuretic hormone and to estimating the osmotic threshold in the mentioned individuals through a water overload in order to further complement our studies.
