A cromoblastomicose no Estado do Paraná: etiologia, epidemiologia, clínica e terapêutica com itraconazol

Chromoblastomycosis in the State of Paraná: etiology, epidemiology, clinic and treatment with itraconazole

A CROMOBLASTOMICOSE NO ESTADO DO PARANÁ: ETIOLOGIA, EPIDEMIOLOGIA, CLÍNICA E TERAPÊUTICA COM ITRACONAZOL

O estudo de 71 pacientes portadores de cromoblastomicose no Estado do Paraná, entre 1985 e 1996, permitiu reunir conhecimentos relacionados à etiologia, epidemiologia, clínica e terapêutica da doença.

O agente etiológico mais freqüente foi Fonsecaea pedrosoi, isolado em 95,5% das vezes; porém, em três ocasiões, detectou-se a presença de agentes incomuns: Fonsecaea compacta, Exophiala jeanselmei e Exophiala castellanii, que em parasitismo, apresentavam-se como elementos muriformes característicos de cromoblastomicose. O encontro de agentes etiológicos do gênero Exophiala, aliado a alguns aspectos clínicos encontrados, permitiu teorizar-se sobre a relação dinâmica entre o hospedeiro humano e alguns fungos da família Dematiaceae. A separação clínico-patológica entre alguns tipos de lesões de cromoblastomicose e da feohifomicose subcutânea é imprecisa, sugerindo tratarem-se de uma doença espectral.

A investigação da trajetória epidemiológica de casos autóctones do Estado do Paraná demonstrou que a transmissão da doença é de caráter ocupacional, ocorrendo em indíviduos rurículas, habitantes em áreas dos três planaltos paranaenses. A maior concentração de casos foi observada nas regiões Oeste e Sudoeste, situadas no Terceiro Planalto e relaciona-se às atividades essencialmente agrícolas daquelas regiões. O atendimento nos últimos 2 anos, de pacientes cuja infecção fora adquirida no Estado de Rondônia, sugere ser aquele Estado, região ecologicamente propícia à aquisição da enfermidade.

Empregou-se metodologia para se classificar os diferentes tipos de lesões, baseada na classificação clínica proposta por Carrion, em 1950, e devido ao polimorfismo clínico observado, introduziu-se uma modificação na classificação original. As lesões foram também analisadas quanto à gravidade, encontrando-se correlação entre tipos e tempo de evolução. Entre os pacientes da casuística, foi assinalada a ocorrência de algumas peculiaridades infreqüentes, como: associação com hanseníase, transformação neoplásica e disseminação linfática.

Em 30 pacientes, foram avaliadas eficácia e toxicidade da terapêutica com o itraconazol, empregado em doses de 200 a 400 mg diários. Os pacientes foram analisados conforme a gravidade das lesões, através de critérios de cura, baseados em parâmetros clínicos, micológicos e histológicos e acompanhados pelo seguimento pós-terapêutico. Os resultados demonstraram que o itraconazol obtém índices de cura clínica e micológica duradouros, em 89% dos pacientes portadores de formas leves, após 10,9 meses em média de tratamento. Esse critério também foi alcançado, após média 12,4 meses, em 91% dos pacientes com lesões de moderada gravidade. A cura clínica e micológica foi atingida por 44% dos casos graves, após 30 meses em média e o restante deles melhoraram significativamente. No acompanhamento pós-terapêutico foram observados 2 casos de recaída que responderam à reintrodução do medicamento. Não se observaram alterações significativas dos valores dos exames hematológicos e bioquímicos, durante o tratamento. Em 15 pacientes avaliados, os níveis de cortisol e testosterona não foram alterados pelo composto antifúngico.

Outrossim, a droga mostrou uma excelente eficácia na terapêutica da cromoblastomicose, especialmente nas formas leves e moderadas. É segura, quanto à toxicidade clínica e laboratorial, mesmo quando nessa posologia e por tempo prolongado.

CHROMOBLASTOMYCOSIS IN THE STATE OF PARANÁ: ETIOLOGY, EPIDEMIOLOGY, CLINIC AND TREATMENT WITH ITRACONAZOLE

The study of 71 patients with chromoblastomycosis in the State of Paraná, Brazil, between 1985 and 1996, allows to accumulate information relating to etiology, epidemiology, clinical course and treatment of the disease.

Fonsecaea pedrosoi was the main etiologic agent and it was isolated from 94.3% of the cases. However, unusual agents as Fonsecaea compacta, Exophiala jeanselmei and Exophiala castellanii were detected in the lesions of three patients. The findings of agents from the genus Exophiala associated to the clinical aspects, allowed to theorize on the dynamic relationships between the human host and the fungi from the Dematiacea family. The clinico-pathological borders among some chromoblastomycosis and subcutaneous phaeohyphomycosis lesions are not clear, it is suggested that these dematious infections are a continuos spectral disease.

The research of the epidemiological pathways of the autochthon cases revealed that the transmission of the disease in the State of Paraná, is mainly occupational, affecting inhabitants of the State uplands. Most of the cases were observed in the West and Southwest regions which are located in the Terceiro Planalto, are related to the agricultural activities of these regions. In the last two years, the care of patients whose infection was acquired in the State of Rondonia, suggests that this region is ecologically favorable for the transmission of the disease.

The lesions of chromoblastomycosis were grouped according the clinical classification of Carrion, proposed in 1950. Due to the observed clinical polymorphism, an alteration of the original classification was introduced. The lesions were also analyzed according to its severity and a correlation between types of lesions and the duration of the disease was made. The occurrence of some unusual aspects as: the association with hanseniasis, malignization and lymphatic dissemination, were observed.

A non-comparative clinical trial with itraconazole was carried out in 30 patients, to evaluate efficacy and toxicity of the triazolic compound. The patients were classified in terms of severity and received itraconazole at the dosage of 200mg to 400mg per day until established criteria of cure had been reached. Clinical, mycological, histopathological and laboratorial evaluations were performed before, during and after the therapy.

The results showed that the therapy with itraconazole can achieve lasting clinical and mycological cures in 89% of the patients having mild forms of the disease, after the mean time of 10.9 months of treatment. The same criteria was reached by 91% of the patients with moderated forms, after the mean time of 12.9 months, 44% of the severe cases were clinically and mycologically cured after the mean time of 30 months and the remaining patients were significantly improved. Two relapses were observed during the follow up, but then responded again to the re introduction of the treatment. No significative changes in the values of hematological and biochemical performed tests were observed. In 15 tested patients, the cortisol and testosterone plasma levels were not disturbed by the antifungal drug. Now a day, itraconazole showed an excellent profile in the therapy of chromoblastomycosis, specially in its mild to moderated forms. It is clinically and laboratorially safe, even when used at high doses and during a long period of time.
