BK Poliomavírus (BKPyV) é um agente causal de nefropatia, estenose ureteral e cistite hemorrágica em receptores de transplante renal, sendo considerado uma importante doença emergente na transplantação. Rastreamento regular para reativação do BKPyV, principalmente nos dois primeiros anos pós-transplante, com subsequente redução preemptiva da imunossupressão é considerada a melhor conduta para evitar a progressão da doença, já que a eliminação ou redução da viremia é alcançada na grande maioria dos pacientes dentro de 6 meses. O uso de drogas com propriedades antivirais para os pacientes com viremia persistente tem sido tentado, embora sem benefícios claros. As manifestações clínicas da nefropatia por BKPyV, as estratégias para o diagnóstico e monitoramento da infecção por BKPyV, o manejo do regime de imunossupressão após a detecção da reativação do BKPyV e o uso de drogas antivirais são discutidas nesta revisão.

BK polyomavirus (BKPyV) is a causal agent of nephropathy, ureteral stenosis and hemorrhagic cystitis in kidney transplant recipients, and is considered an important emerging disease in transplantation. Regular screening for BKPyV reactivation mainly during the first 2 years posttransplant, with subsequent pre-emptive reduction of immunosuppression is considered the best option to avoid disease progression, since successful clearance or reduction of viremia is achieved in the vast majority of patients within 6 months. The use of drugs with antiviral properties for patients with persistent viremia has been attempted despite unclear benefits. Clinical manifestations of BKPyV nephropathy, current strategies for diagnosis and monitoring of BKPyV infection, management of immunosuppressive regimen after detection of BKPyV reactivation and the use of antiviral drugs are discussed in this review.

Introdução
O poliomavírus BK (BKPyV) pertence à família Polyomaviridae

Manifestações clínicas
A replicação do BKPyV ocorre durante estados de imunossupressão. Virúria ocorre em
casos de gravidez, câncer, infecções por HIV, diabetes e transplante. No entanto,
viremia e nefropatia associada ao BKPyV (BKVN) são raras em casos outros que não de

Além do estado
imunológico, outras variáveis como idade avançada, sexo masculino, raça branca,
diabetes, soronegatividade para BKPyV antes do transplante, tratamento com
imunossupressores, lesão isquêmica durante o transplante e mutações virais, são

Infecções por BKPyV em indivíduos imunodeprimidos podem originar entidades
patológicas distintas em diferentes grupos de pacientes: em receptores de transplante

A cistite hemorrágica (CH) é a manifestação mais comum nas infecções gênito-urinárias por BKPyV em receptores de TCTH. A forma viralmente induzida da CH normalmente ocorre após o transplante e, por conseguinte, é chamada de cistite hemorrágica de início tardio. Ela ocorre em de 6% a 29% dos pacientes de TCTH, geralmente dentro dos primeiros dois meses após o transplante. Estenose ureteral ocorre em aproximadamente três por cento (2-6%) dos pacientes
transplantados renais e se desenvolve geralmente vários meses após o

transplante e progride em estágios detectáveis: virúria para viremia e, em seguida,

Os testes baseados em PCR para detecção/quantificação de DNA na urina ou sangue

Iniciativas de triagem tem se concentrado principalmente nos primeiros 6-12 meses.
Contudo, devido à precocidade da viremia do BKPyV vista na maioria dos casos, a
tendência aponta para a concentração da triagem durante os primeiros meses.
As atuais estratégias de triagem se baseiam em dois princípios básicos: 1) virúria seguida de viremia; 2) apenas viremia. Apesar de suas variações metodológicas, as duas estratégias mostraram-se igualmente eficazes na detecção de infecção por BKPyV, permitindo intervenção em tempo hábil.
Estratégia 1: virúria seguida de viremia
Como citado anteriormente, a virúria pode ser avaliada por microscopia eletrônica,
citologia de células decoy e qPCR. No entanto, a execução dessas técnicas ao mesmo

inibidores da mTOR ao regime imunossupressor. Após a redução da imunossupressão, a
função renal deve ser cuidadosamente monitorizada por conta do risco de rejeição.
Embora a redução da imunossupressão seja o tratamento de primeira linha lógico, não
há uma segunda linha bem definida. Apesar da falta de benefícios claros, tratamentos
com imunoglobulina, cidofovir e fluoroquinolonas já foram oferecidos a pacientes que
não conseguiram reduzir a viremia após redução da imunossupressão. Dentre estas
opções, o uso de fluoroquinolonas foi mais amplamente estudado. Análises in
vitro mostram que as fluoroquinolonas podem ter propriedades antivirais

O cidofovir é um análogo de nucleótido da citosina que atua sobre o DNA viral,

Devido à sua nefrotoxicidade, o ciclofovir
deve ser considerado para o tratamento da BKVN somente quando as outras opções
fracassaram.
A administração de imunoglobulina intravenosa para o tratamento de BKVN é uma ideia
atraente, dado que o BKPyV é onipresente na população humana. Assim, espera-se que a
imunoglobulina contenha anticorpos contra este vírus. O uso de imunoglobulina parece
ser especialmente interessante quando o diagnóstico de rejeição do enxerto não pode
ser descartado. Neste caso, o uso de doses maciças de imunoglobulina pode ser útil
tanto em casos de rejeição como de BKVN. No entanto, há uma escassez de estudos sobre

126 pacientes submetidos a retransplante renal após perda do enxerto por BKVN mostrou

Em outro estudo, 11 de 31 pacientes apresentaram viremia por
BKPyV após o transplante e apenas dois evoluíram para BKVN. A eliminação de viremia
após BKVN no transplante inicial foi significativamente associada a menor risco de

O manejo
pós-transplante deve seguir a triagem e o seguimento descritos anteriormente, mas
sempre tendo em mente os tênues limites entre imunossupressão excessiva, com risco de
reativação da viremia do BKPyV, e um regime imunossupressor pouco rigoroso para um
paciente já sensibilizado pelo transplante anterior.
Conclusões
O advento de novos agentes imunossupressores mais potentes pode contribuir para uma incidência aparentemente crescente de BKVN em receptores de transplante renal. O método de triagem ideal e o melhor momento para detectar o BKPyV ainda não foram determinados. Valores de corte, especialmente para testes quantitativos, precisam ser definidos e padronizados. Atualmente, o diagnóstico precoce e a redução da terapia imunossupressora parecem resultar no tratamento mais eficaz para a infecção pot BKPyV.
