FREQUÊNCIA DAS REFEIÇÕES REALIZADAS POR ADOLESCENTES BRASILEIROS E HÁBITOS ASSOCIADOS: REVISÃO SISTEMÁTICA

RESUMO

Objetivo:

Analisar os estudos que identificaram a frequência de refeições ingeridas por adolescentes brasileiros e hábitos associados.

Fonte de dados:

Efetuou-se uma busca sistemática nos bancos e nas bases de dados eletrônicos MEDLINE/PubMed, The Latin American and Caribbean Center of Information in Health Sciences (BIREME), Scopus, Web of Science e Embase, com artigos publicados entre janeiro de 2007 e dezembro de 2017, que abordaram a avaliação da frequência das refeições realizadas por adolescentes, considerando ou não as associações com padrões alimentares e substituição de refeições.

Síntese dos dados:

Foram obtidos 6.608 estudos por meio da busca e incluídos nove nesta revisão, todos de delineamento transversal. Oito deles utilizaram inquéritos escolares e apenas um tratava-se de inquérito populacional. Sete estudos avaliaram a frequência das principais refeições diárias que oscilaram de 47,0 a 79,0% no café da manhã, de 65,0 a 98,4% no almoço e de 51,0 a 94,0% no jantar. Cinco estudos identificaram as frequências de consumo de lanches intermediários, encontrando valores mais elevados para o lanche da tarde (variação de 42,0 a 78,0%). Em relação à substituição de refeições por lanches, em três dos quatros estudos selecionados, observou-se que essa prática ocorreu principalmente no jantar (24,6 a 42,0%).

Conclusões:

O café da manhã foi a refeição mais omitida pelos adolescentes, e o jantar, a mais substituída por lanches. Entre os lanches intermediários, o da tarde foi o mais consumido.

ABSTRACT

Objective:

To analyze the studies that identified the frequency of meals ingested by Brazilian adolescents and associated habits.

Data sources:

A systematic search was made in the databases and electronic databases: MEDLINE/PubMed, The Latin American and Caribbean Center of Information in Health Sciences (BIREME), Scopus, Web of Science and Embase, with articles published between January/2007 until December/2017, which addressed the evaluation of the frequency of meals performed by adolescents, considering or not associations with eating patterns and meal replacement.

Data synthesis:

6,608 studies were obtained through the search and nine were included in this review, all of them with a cross-sectional design. Eight studies used school surveys and only one was a population survey. Seven studies evaluated the frequency of the main daily meals that ranged from 47.0 to 79.0% at breakfast, from 65.0 to 98.4% at lunch, and from 51.0 to 94.0% at dinner. Five studies identified the frequencies of consumption of snacks between meals, finding higher values for afternoon snack (variation from 42.0 to 78.0%). Regarding the substitution of meals for snacks, in three of the four selected studies; it was observed that this practice occurred mainly in substitution of dinner (24.6 to 42.0%).

Conclusions:

Breakfast was the most omitted meal for adolescents, and dinner was replaced with snacks. Among the between meal snacks, the afternoon snack was the most consumed.

INTRODUÇÃO

Nas últimas décadas, no Brasil e em outros países, ocorreram transformações relevantes nos padrões de saúde e alimentação das populações.11. Diante desse cenário, a identificação e o monitoramento do consumo alimentar de indivíduos ou grupos passaram a ser tarefas essenciais para o diagnóstico da situação de saúde, para o planejamento e para a avaliação de programas e políticas nacionais de saúde e nutrição.33. No Brasil, os hábitos alimentares dos adolescentes têm sido caracterizados pelo consumo insuficiente de frutas e hortaliças e pela elevada ingestão de produtos ultraprocessados, que são ricos em gorduras sólidas, açúcares e/ou sódio.55. Dessa forma, o objetivo desta revisão sistemática foi analisar os estudos que identificaram a frequência de refeições realizadas por adolescentes brasileiros e hábitos associados.

MÉTODO

Este estudo foi realizado utilizando-se as recomendações do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA).1212. As buscas de estudos foram conduzidas com o auxílio de uma bibliotecária e dois revisores, em dezembro de 2017.

Os estudos foram obtidos em portais de pesquisas e bases de dados, a saber: MEDLINE/PubMed, The Latin American and Caribbean Center of Information in Health Sciences (BIREME), Scopus, Web of Science e Embase. Os termos utilizados foram selecionados a partir das palavras-chave e também dos Descritores em Ciências da Saúde (DeCS) e Medical Subject Headings (MeSH), sendo estes: “Adolescent”, “Food consumption”, “Meal skipping”, “Meal frequency”, “Meal habits”, “Food habits”, “Food behavior”, “Feeding behavior”, “Dietary habits”, “Dietary patterns” e “Food intake”. Utilizaram-se também os operadores booleanos “AND” e “OR”. As palavras-chave e os operadores booleanos foram organizados de forma padrão, mas respeitando-se os critérios existentes em cada local de busca.

Foram considerados como critérios de inclusão: estudos que verificaram a frequência de realização das refeições, independentemente de avaliações com padrões alimentares (conjunto de alimentos frequentemente ingeridos) e substituição de refeições; adolescentes saudáveis e com idades entre 10 e 19 anos; amostras de base populacional e/ou de escolares em âmbitos municipal, estadual e nacional; estudos de coorte, caso-controle e transversais; que tenham sido publicados em português, espanhol ou inglês, sendo a busca limitada ao período de janeiro de 2007 até dezembro de 2017. Foram adotados os seguintes critérios de exclusão: estudos de revisão e metanálise, teses e dissertações.

Os estudos selecionados nas bases de dados, conforme os critérios de inclusão e exclusão, foram arquivados em pastas, e as disparidades de classificação foram resolvidas pelo consenso de dois revisores. As informações extraídas dos estudos para a sua caracterização foram: autor e ano de publicação, faixa etária, local, métodos, delineamento, análises e principais resultados. Nos casos dos estudos que incluíram diversas faixas etárias, foram selecionados somente os resultados relativos aos adolescentes.

foram utilizadas como critério de avaliação da qualidade metodológica dos estudos incluídos. Cada item avaliado pelo STROBE recebe uma pontuação de zero a um, gerando um escore que varia de zero a 22 pontos. Os escores atribuídos aos estudos foram classificados em três categorias de qualidade metodológica: A - quando o estudo atendia mais de 80% dos critérios estabelecidos pelo STROBE; B - quando de 50 a 80% dos critérios eram atendidos; C - quando menos de 50% dos critérios eram preenchidos.

RESULTADOS

Foram identificados, inicialmente, 6.608 estudos nas buscas efetuadas nos bancos e nas bases de dados eletrônicos. Em seguida, foi realizado o refinamento da pesquisa por meio da aplicação de filtros, destinados à exclusão de estudos de revisão de literatura, estudos de casos, pesquisas realizadas com animais e que não incluíam a faixa etária de interesse, resultando em um total de 3.292 estudos. Destes, foram excluídos os duplicados (n=168), totalizando 3.124 para a realização de leitura de título e resumo. Aqueles que não atenderam aos critérios de inclusão foram excluídos (n=3.067), restando 57, que foram identificados e classificados como potencialmente elegíveis, procedendo-se, então, a apreciação na íntegra. Depois de efetuada a leitura completa dos textos, 48 estudos foram excluídos por não apresentarem os dados das frequências das refeições e por não serem dados brasileiros; por fim, 9 estudos foram selecionados.99. Com base nos resultados obtidos, pode-se concluir que todos os estudos encontrados foram realizados com escolares, de abrangências municipal e estadual. Nas avaliações de frequência das refeições, o café da manhã foi a refeição mais omitida; entre os lanches intermediários, o da tarde foi o mais consumido; e o jantar foi a refeição mais substituída por lanches. De modo geral, as avalições dos estudos identificaram um padrão alimentar insatisfatório entre os adolescentes. Nesse contexto, esta revisão ressalta a importância da realização de estudos de base populacional para avaliar também as demais refeições e não apenas o café da manhã, visto que o consumo regular das refeições está associado a uma alimentação equilibrada, além de prevenir o desenvolvimento de doenças como o sobrepeso e a obesidade.
