Um olhar para a função diastólica na sepse grave e no choque séptico

A disfunção miocárdica na sepse é um processo complexo, devido à adaptação dinâmica do sistema cardiovascular ao processo de adoecimento e da resposta do organismo do paciente aos efeitos da ressuscitação. A sua fisiopatologia é multifatorial: foram descritos mecanismos sistêmicos, celulares e extracelulares, inclusive má distribuição do fluxo coronáriano, lesão miocárdica, falha contrátil do miócito mediada por complemento (C5a), ativação neutrofílica induzida por citocinas (fator de necrose tumoral, interleucina 1β, interleucina 6), desregulação do controle do fluxo de cálcio intracelular e hipóxia celular por disfunção mitocondrial.(11 Flynn A, Chokkalingam Mani B, Mather PJ. O acometimento cardiovascular é um componente central da síndrome de disfunção de múltiplos órgãos, uma consequência frequentemente fatal da sepse grave e do choque séptico. Com relação à sepse, existem diversos estudos relacionados à disfunção ventricular sistólica esquerda,(33 Krishnagopalan S, Kumar A, Parrilo JE, Kumar A. Crit Care Med. 2008;36(6):1701-6.) enquanto outras formas de disfunção miocárdica têm sido frequentemente negligenciadas, tais como a disfunção ventricular diastólica(55 Bouhemad B, Nicolas-Robin A, Arbelot C, Arthaud M, Féger F, Rouby JJ. É interessante observar que todos os tipos de disfunção miocárdica podem se apresentar de forma isolada ou combinada, e usualmente são completamente reversíveis com a resolução da doença crítica.(55 Bouhemad B, Nicolas-Robin A, Arbelot C, Arthaud M, Féger F, Rouby JJ. Com o uso do Doppler tecidual (DT), uma técnica que tem conquistado aceitação entre cardiologistas e intensivistas para avaliar a função diastólica, tornou-se evidente que a disfunção diastólica é comum nos pacientes críticos.(1212 Sturgess DJ, Marwick TH, Joyce CJ, Jones M, Venkatesh B. O DT é uma técnica ecocardiográfica que se baseia em mensurações das velocidades miocárdicas,(1313 Isaaz K, Thompson A, Ethevenot G, Cloez JL, Brembilla B, Pernot C. Doppler echocardiographic measurement of low velocity motion of the left ventricular posterior wall. Am J Cardiol. 1989;64(1):66-75.) sinais de baixa frequência e com alta amplitude, filtradas dos sistemas convencionais de imagens por Doppler.(1414 Dokainish H. Trata-se de uma técnica particularmente útil para estimar o relaxamento ventricular e suas pressões de enchimento.(1515 Nagueh SF, Appleton CP, Gillebert TC, Marino PN, Oh JK, Smiseth OA, et al. A importância da disfunção diastólica foi recentemente enfatizada por estudos que demonstraram que o DT pode ter utilidade prognóstica na população geral dos pacientes na unidade de terapia intensiva (UTI).(77 De Geer L, Engvall J, Oscarsson A. Neste número da Revista Brasileira de Terapia Intensiva, um interessante estudo unicêntrico de coorte prospectiva confirmou a importância da disfunção diastólica em pacientes com sepse grave e choque séptico. Os autores estudaram 53 pacientes com o objetivo de avaliar a prevalência da disfunção miocárdica, tanto na admissão na UTI, quanto 1 semana após, e avaliaram também o impacto da disfunção ventricular sistólica e diastólica na mortalidade na UTI.(2323 Rolando G, Valenzuela Espinoza ED, Avid E, Welsh S, Del Pozo J, Vasquez AR, et al. Foi realizado um exame ecocardiográfico detalhado dentro das primeiras 48 horas após a admissão na UTI e 7 a 10 dias depois. Essa avaliação incluiu uma ecocardiografia no modo M, bidimensional e Doppler. Um único cardiologista especializado em ecocardiografia realizou a observação da função sistólica e diastólica, inclusive com uso do DT, assim como uma análise do ventrículo direito com a avaliação da excursão sistólica do plano do anel da valva tricúspide.

Os autores encontraram disfunção diastólica em cerca de 84% dos pacientes sépticos, uma prevalência bastante elevada. Este achado permaneceu inalterado no exame ecocardiográfico de seguimento, realizado 1 semana mais tarde. Este número proeminente pode estar relacionado à faixa etária (74 ± 13 anos) dos pacientes incluídos. No entanto, não podemos excluir a possibilidade de que a prevalência tenha sido superestimada em função de um viés de seleção, já que apenas 25% dos pacientes elegíveis entraram no estudo, e houve um número comparável de pacientes com perda de seguimento. Além disso, não podemos garantir que todos os achados ecocardiográficos sejam atribuíveis à miocardiopatia séptica. A disfunção diastólica poderia também ser uma condição mórbida prévia nesta amostra de pacientes idosos, uma hipótese que é apoiada pela falta de resolução dos achados ecocardiográficos no exame de seguimento.

Outro achado relevante foi a associação da disfunção diastólica com a mortalidade, confirmando(2323 Rolando G, Valenzuela Espinoza ED, Avid E, Welsh S, Del Pozo J, Vasquez AR, et al. Prognostic value of ventricular diastolic dysfunction in patients with severe sepsis and septic shock. Rev Bras Ter Intensiva. 2015;27(4):xxx-xx.) a importância dessa variável, antes negligenciada, do comprometimento miocárdico. A razão E/e', uma das medidas do DT utilizada para definir a disfunção diastólica, foi a única mensuração ecocardiográfica associada de forma independente à mortalidade, mesmo após a análise ser ajustada para fatores como a idade e a gravidade da doença aguda. É interessante notar que, neste estudo, a disfunção sistólica não foi um fator preditor de óbito. Estes achados salientam a necessidade de realizar avaliações ecocardiográficas abrangentes em pacientes com sepse e choque séptico.
