Qualidade de vida de sobreviventes de um período de internação na unidade de terapia intensiva: uma revisão sistemática

RESUMO

Objetivo:

Avaliar a qualidade de vida relacionada com a saúde, em longo prazo, de sobreviventes de um período de internação na unidade de terapia intensiva por revisão sistemática.

Métodos:

Busca, seleção e análise de estudos observacionais que avaliaram a qualidade de vida relacionada com a saúde de sobreviventes de internação na unidade de terapia intensiva nas bases de dados eletrônicas LILACS e MEDLINE® (acessada pelo PubMed), encontrados por meio dos termos MESH indexados "quality of life [MeSH Terms]" AND "critically ill [MeSH Terms]". Foram incluídos estudos publicados nos últimos 5 anos no idioma inglês realizados em pacientes adultos sem doenças prévias específicas. As citações foram selecionadas independentemente por três revisores. Os dados foram extraídos de forma padronizada e independente por dois revisores, e a qualidade dos estudos foi avaliada utilizando a escala Newcastle-Ottawa.

Resultados:

Foram incluídos 19 coortes observacionais e 2 estudos caso-controle de 57.712 doentes críticos. O tempo de seguimento dos estudos variou de 6 meses a 6 anos, sendo a maioria dos estudos com 6 meses ou 1 ano de seguimento. A qualidade de vida relacionada com a saúde foi avaliada utilizando duas ferramentas genéricas, o EuroQol e o Short Form Health Survey. A qualidade geral dos estudos foi baixa.

Conclusões:

A qualidade de vida relacionada com a saúde, em longo prazo, está comprometida em sobreviventes de internação na unidade de terapia intensiva comparada à da população geral correspondente. Porém, esta não é significativamente afetada pela presença de sepse, delírio e lesão renal aguda durante a internação na unidade de terapia intensiva quando comparada com grupos controle de pacientes críticos. São necessários estudos de alta qualidade para quantificar a qualidade de vida relacionada com a saúde em sobreviventes de internação em unidade de terapia intensiva.

ABSTRACT

Objective:

To assess the long-term, health-related quality of life of intensive care unit survivors by systematic review.

Methods:

The search for, and selection and analysis of, observational studies that assessed the health-related quality of life of intensive care unit survivors in the electronic databases LILACS and MEDLINE® (accessed through PubMed) was performed using the indexed MESH terms "quality of life [MeSH Terms]" AND "critically illness [MeSH Terms]". Studies on adult patients without specific prior diseases published in English in the last 5 years were included in this systematic review. The citations were independently selected by three reviewers. Data were standardly and independently retrieved by two reviewers, and the quality of the studies was assessed using the Newcastle-Ottawa scale.

Results:

In total, 19 observational cohort and 2 case-control studies of 57,712 critically ill patients were included. The follow-up time of the studies ranged from 6 months to 6 years, and most studies had a 6-month or 1-year follow up. The health-related quality of life was assessed using two generic tools, the EuroQol and the Short Form Health Survey. The overall quality of the studies was low.

Conclusions:

Long-term, health-related quality of life is compromised among intensive care unit survivors compared with the corresponding general population. However, it is not significantly affected by the occurrence of sepsis, delirium, and acute kidney injury during intensive care unit admission when compared with that of critically ill patient control groups. High-quality studies are necessary to quantify the health-related quality of life among intensive care unit survivors.

INTRODUÇÃO

Os avanços tecnológicos nos cuidados intensivos têm proporcionado, cada vez mais, a redução da mortalidade nas unidades de terapia intensiva (UTI).(11 Wunsch H, Guerra C, Barnato AE, Angus DC, Li G, Linde-Zwirble WT. Three-year outcomes for Medicare beneficiaries who survive intensive care. JAMA. 2010;303(9):849-56.) Entretanto, as consequências de uma doença crítica podem perdurar por muito tempo, afetando a saúde física, cognitiva e mental dos sobreviventes de um período de internação na UTI.(22 Needham DM, Davidson J, Cohen H, Hopkins RO, Weinert C, Wunsch H, et al. Crit Care Med. 2012;40(2):502-9.)A multiplicidade destas consequências foi reconhecida como "síndrome pós-terapia intensiva", e pode resultar em um grande impacto negativo na funcionalidade e na qualidade de vida relacionada com a saúde (QVRS).(22 Needham DM, Davidson J, Cohen H, Hopkins RO, Weinert C, Wunsch H, et al. Tão importante quanto avaliar a taxa de mortalidade em longo prazo em sobreviventes de um período de internação na UTI é avaliar os desfechos relacionados a fatores físicos e psicológicos, estado funcional, interação social, bem como a QVRS.(33 Graf J, Koch M, Dujardin R, Kersten A, Janssens U. Idealmente, os pacientes sobreviventes de um período de internação na UTI devem atingir seus escores de QVRS pré-morbidade e/ou internação, ou ainda um escore melhor ou similar ao de um indivíduo de mesma idade, sexo e comorbidade.(55 Black NA, Jenkinson C, Hayes JA, Young D, Vella K, Rowan KM, et al. Embora, cada vez mais, os escores relacionados à QVRS tenham sido incluídos nos estudos e reconhecidos como parâmetros de desfecho importantes nesta população, tais resultados geram imprecisão em sua interpretação por diversas razões. Primeiro, o período em que a recuperação na QVRS deve ocorrer não está definido e, consequentemente, o período ideal de seguimento para avaliação da QVRS está indeterminado. Frequentemente, as manifestações clínicas da síndrome pós-terapia intensiva podem apresentar-se não apenas como eventos transitórios,(66 Davidson JE, Jones C, Bienvenu OJ. Crit Care Med. 2011;39(2):371-9.) estando presentes 5 anos após a alta hospitalar(88 Herridge MS, Tansey CM, Matté A, Tomlinson G, Diaz-Granados N, Cooper A, Guest CB, Mazer CD, Mehta S, Stewart TE, Kudlow P, Cook D, Slutsky AS, Cheung AM; Canadian Critical Care Trials Group. Functional disability 5 years after acute respiratory distress syndrome. N Engl J Med. 2011;364(14):1293-304.) e, para alguns sobreviventes, podem ser permanentes. Segundo, a avaliação da QVRS na linha de base é difícil, dificultando a investigação da sobrecarga da doença crítica. Além disso, os instrumentos de avaliação utilizados nos estudos são diferentes, dificultando a sistematização e a interpretação dos resultados da QVRS. No entanto, faz-se necessário melhor compreender o quanto o tratamento intensivo afeta a QVRS de seus sobreviventes em longo prazo, no intuito de auxiliar os profissionais na tomada de decisão sobre a determinação de esforços futuros, para reduzir a sobrecarga da doença crítica.

O objetivo deste estudo foi revisar sistematicamente a literatura, para avaliar e sintetizar a QVRS, em longo prazo, de sobreviventes de um período de internação na UTI, quando comparados a população geral correspondente e a grupos controle, por meio de uma revisão de estudos observacionais.

MÉTODOS

Esta revisão sistemática foi realizada de acordo com as Preferred Reporting Items for Systematic Reviews and Meta-Analyses Guidelines (PRISMA). (99 Moher D, Liberati A, Tetzlaff J, Altman DG; PRISMA Group. Preferred reporting items for systematic reviews and meta-analyses: the PRISMA statement. PLoS Med. 2009;6(7):e1000097.) Uma estratégia de busca sistemática por meio dos termos MESH indexados "quality of life [MeSH Terms]" AND "critically illness [MeSH Terms]" foi utilizada nas bases de dados eletrônicas Literatura Latino-Americana e do caribe em Ciências da Saúde (LILACS) e Medical Literature Analysis and Retrieval System Online (MEDLINE®) acessada pelo PubMed, a partir de 13 de outubro de 2016, com pesquisas concluídas em 7 de novembro de 2016.

Os títulos e os resumos dos artigos identificados na estratégia de busca foram avaliados em função dos critérios de elegibilidade (Tabela 1) por três revisores independentes. O texto completo dos artigos foi obtido quando o resumo continha informações insuficientes sobre os critérios de inclusão e exclusão. A lista de referências dos artigos selecionados e os arquivos pessoais dos investigadores também foram pesquisados para identificar eventuais trabalhos que pudessem também preencher os critérios de elegibilidade do estudo e, porventura, não tivessem sido encontrados na busca inicial. Quaisquer discrepâncias entre os revisores foram resolvidas por consenso e, em casos de desacordo persistente, um quarto revisor avaliou as publicações.

Os dados dos artigos selecionados foram extraídos de forma independente por dois revisores. Discrepâncias foram resolvidas por consenso ou por um terceiro revisor.

A qualidade metodológica dos estudos foi avaliada utilizando a escala Newcastle-Ottawa por dois revisores independentes, previamente treinados e qualificados. A pontuação da qualidade metodológica dos estudos de coorte e caso-controle foi calculada em três componentes: seleção dos grupos (0 - 4 pontos), qualidade de ajuste para confusão (0 - 2 pontos) e avaliação da exposição após desfecho (0 - 3 pontos). A pontuação máxima pode ser de 9 pontos, o que representa alta qualidade metodológica. Discrepâncias entre os revisores foram resolvidas por consenso e, em caso de desacordo persistente, a avaliação foi feita por um terceiro revisor.

RESULTADOS

Seleção dos estudos

As buscas nas bases de dados recuperaram 417 referências e a consulta a outras fontes, 5 referências. Destas, 289 foram excluídas por terem sido publicadas fora do período de 5 anos, estipulado para esta revisão. Do restante, 112 foram excluídas após a leitura do título e do resumo, e eventualmente do texto completo, por não atenderem aos demais critérios de inclusão (Figura 1). Não houve discrepância quanto ao número de artigos selecionados pelos três revisores, e 21 artigos foram incluídos.

Os 21 estudos incluíram um total de 57.712 doentes críticos, e os indivíduos eram, em sua maioria, do sexo masculino, adultos e idosos, com escores variados de gravidade na admissão na UTI e setorizados de acordo com os grupos de diagnóstico primário na UTI, como: cardiovascular, respiratório, renal, gastrintestinal, neurológico, trauma, ortopédico, cirúrgico, sepse, hematológico, ginecológico e metabólico.

Risco de viés nos estudos incluídos

A qualidade metodológica geral dos estudos incluídos nesta revisão foi baixa (Tabela 3). Os escores da escala Newcastle-Ottawa para os estudos variaram de 2 a 5, com classificação inferior a 4, indicando evidência limitada ou baixa qualidade. Consenso foi alcançado em todas as ocasiões, e nenhum estudo foi excluído desta revisão, baseado no risco de viés avaliado. Metanálises não puderam ser conduzidas devido ao desenho metodológico predominante dos estudos incluídos ser coorte observacional.

DISCUSSÃO

Esta revisão sistemática descreve a QVRS em longo prazo para sobreviventes de um período de internação na UTI. Foram identificados 21 estudos para inclusão. A qualidade geral dos estudos foi baixa pela escala Newcastle-Ottawa, tornando evidente a necessidade de estudos de alta qualidade metodológica para determinar a QVRS, em longo prazo, de sobreviventes de um período de internação na UTI. Novos estudos com desenhos metodológicos adequados poderão fornecer dados importantes sobre os principais fatores que resultam em sua alteração, assim como possíveis alternativas terapêuticas.

Doente crítico

A QVRS no doente crítico em longo prazo é divergente entre os estudos analisados, variando de acordo com a população e o tempo de seguimento. Em curto prazo, o componente mental da QVRS em uma população de doentes críticos com escore de gravidade baixo foi comparável ao da população correspondente pareada para idade, em 8 e 26 semanas após a alta hospitalar.(2626 McKinley S, Aitken LM, Alison JA, King M, Leslie G, Burmeister E, et al. Entretanto, o tamanho do efeito na redução da QVRS foi pequeno em todos os domínios do instrumento de avaliação, sugerindo que o efeito da internação na UTI sobre a percepção da QVRS após 5 anos pode não ser clinicamente relevante.

Doente crítico idoso

É compreensível que idosos sobreviventes de uma internação na UTI apresentem prejuízo na QVRS devido ao provável aumento de comorbidades e que esse prejuízo seja mais evidente na função física.

Doente crítico com sepse

A QVRS em pacientes críticos com sepse não difere significativamente quando comparada à de pacientes críticos não idosos sem sepse,(3030 Contrin LM, Paschoal VD, Beccaria LM, Cesarino CB, Lobo SM. Functional disability 5 years after acute respiratory distress syndrome. N Engl J Med. 2011;364(14):1293-304.) e de grande relevância sobre o tema demonstrou que sobreviventes de um período de internação na UTI que desenvolveram síndrome do desconforto respiratório agudo apresentavam sequelas físicas e psicológicas e redução do componente função física da QVRS 5 anos após a alta da UTI, corroborando achado do estudo incluído nesta revisão.

DOENTE CRÍTICO COM MÁ QUALIDADE DO SONO

Em indivíduos com má qualidade do sono, o único estudo publicado sobre QVRS nesta população demonstrou que as funções física e mental do instrumento de avaliação foram significativamente menores nestes indivíduos 6 meses após a alta da UTI.(2727 McKinley S, Fien M, Elliott R, Elliott D. Evidências apontam que o sono na UTI é de baixa qualidade e que a privação aguda do sono pode relacionar-se negativamente na recuperação dos pacientes críticos,(3333 Pulak LM, Jensen L. Sleep in the intensive care unit: a review. J Intensive Care Med. 2016;31(1):14-23.,3434 Kamdar BB, Needham DM, Collop NA. Sleep deprivation in critical illness: its role in physical and psychological recovery. J Intensive Care Med. 2012;27(2):97-111.) incluindo a recuperação física e psicológica.

Doente crítico com fragilidade

A fragilidade é um estado multidimensional caracterizado pela perda de reserva fisiológica e cognitiva que se acumula em pacientes com idade mais avançada e prediz eventos adversos e desfechos desfavoráveis.(3535 Clegg A, Young J, Iliffe S, Rikkert MO, Rockwood K. Frailty in elderly people. Lancet. 2013;381(9868):752-62. Erratum in: Lancet. 2013;382(9901):1328.) Doentes críticos em estado de fragilidade classificados pela Canadian Study of Health and Ageing Clinical Frailty Scale foram avaliados em uma coorte multicêntrica no Canadá 1 ano após a internação na UTI.(2424 Bagshaw SM, Stelfox HT, Johnson JA, McDermid RC, Rolfson DB, Tsuyuki RT, et al. Long-term association between frailty and health-related quality of life among survivors of critical illness: a prospective multicenter cohort study. Crit Care Med. 2015;43(5):973-82.) Estes indivíduos apresentaram pior QVRS em comparação com indivíduos não frágeis e a população geral saudável.(2424 Bagshaw SM, Stelfox HT, Johnson JA, McDermid RC, Rolfson DB, Tsuyuki RT, et al. Long-term association between frailty and health-related quality of life among survivors of critical illness: a prospective multicenter cohort study. Crit Care Med. 2015;43(5):973-82.) Outro estudo incluído nesta revisão, realizado com idosos com 80 anos ou mais, demonstrou que a fragilidade foi preditor independente mais significativo que idade, gravidade da doença crítica ou comorbidade − que são comumente considerados importantes determinantes dos desfechos em longo prazo.(2323 Heyland DK, Garland A, Bagshaw SM, Cook D, Rockwood K, Stelfox HT, et al. Síndrome pós-terapia intensiva

Uma proporção substancial, mas desconhecida, de sobreviventes de internação na UTI está em risco de desenvolver síndrome pós-terapia intensiva. Há um esforço crescente em busca da utilização do termo "síndrome pós-terapia intensiva" para descrever défices físicos, cognitivos ou mentais novos ou agravados, resultantes de um doença crítica, persistentes após a hospitalização,(22 Needham DM, Davidson J, Cohen H, Hopkins RO, Weinert C, Wunsch H, et al. Improving long-term outcomes after discharge from intensive care unit: report from a stakeholders' conference. Crit Care Med. 2012;40(2):502-9.,3636 Elliott D, Davidson JE, Harvey MA, Bemis-Dougherty A, Hopkins RO, Iwashyna TJ, et al. Exploring the scope of post-intensive care syndrome therapy and care: engagement of non-critical care providers and survivors in a second stakeholders meeting. Crit Care Med. 2014;42(12):2518-26.)com o objetivo de conhecer a epidemiologia desta síndrome e sua sobrecarga na QVRS em longo prazo de sobreviventes de internação na UTI. Cerca de 25 a 50% dos pacientes submetidos à ventilação mecânica evoluem com fraqueza muscular adquirida na UTI,(77 Desai SV, Law TJ, Needham DM. Long-term complications of critical care. Crit Care Med. 2011;39(2):371-9.) e, destes, cerca de 85 a 95% persistem com défices neuromusculares por 2 a 5 anos após a alta hospitalar.(77 Desai SV, Law TJ, Needham DM. Long-term complications of critical care. Crit Care Med. 2011;39(2):371-9.) Cerca de 30 a 80% dos pacientes apresentam comprometimento cognitivo após a estadia na UTI,(3737 Hopkins RO, Weaver LK, Collingridge D, Parkinson RB, Chan KJ, Orme JF Jr. Two-year cognitive, emotional, and quality-of-life outcomes in acute respiratory distress syndrome. Am J Respir Crit Care Med. 2005;171(4):340-7.) e 10 a 50% apresentam novos sintomas de depressão, ansiedade, transtorno por estresse pós-traumático e problemas no sono.(77 Desai SV, Law TJ, Needham DM. Long-term complications of critical care. Crit Care Med. 2011;39(2):371-9.) A prevalência elevada e persistente dessas alterações, relacionadas à síndrome pós-terapia intensiva, parece justificar os prejuízos em longo prazo na QVRS em sobreviventes de internação na UTI, sendo tais consequências mais proeminentes em algumas situações específicas encontradas no ambiente de cuidados intensivos, como evidenciado nesta revisão sistemática.

Um importante fator a ser destacado é que a avaliação da QVRS em longo prazo apresentada pelos estudos considera somente aqueles pacientes que não são descontinuados do estudo ou vão a óbito. Isto fica evidente quando apenas 42% (24.200 doentes) da amostra total (57.712) é avaliada. Acreditamos que este achado pode comprometer a avaliação da QVRS desta população. Isto porque a perda amostral pode estar relacionada à piora do quadro clínico ou ao óbito do paciente.

A estratégia de busca utilizada nas bases eletrônicas falhou em capturar alguns estudos elegíveis. Crit Care Med. 2010;38(12):2386-400.) sobre o tema utilizaram estratégias de busca mais abrangentes como: ("quality of life" OR "health status indicators") AND ("intensive care units" OR "critical care" OR "critical illness" OR "sepsis" OR "adult respiratory distress syndrome") e ("quality of life" OR "long-term outcome") AND ("intensive care" OR "critical care" OR "critically ill patients" OR "ICU patients" OR "critical care patients" OR "ICU stay" OR "ICU"). Dowdy et al.(3838 Dowdy DW, Eid MP, Sedrakyan A, Mendez-Tellez PA, Pronovost PJ, Herridge MS, et al. Quality of life in adult survivors of critical illness: a systematic review of the literature. Intensive Care Med. 2005;31(5):611-20. Erratum in Intensive Care Med. 2005;31(7):1007.) incluíram na estratégia de busca os termos "sepse" e "síndrome do desconforto respiratório agudo", porque um estudo elegível identificado antes de conduzir a pesquisa não foi capturado usando os termos iniciais. No entanto, complementamos a busca nas listas de referências de outras revisões sistemáticas e publicações relevantes sobre o tema.

O escopo desta revisão foi abrangente, portanto, a heterogeneidade dos estudos foi uma limitação, impossibilitando a comparação entre eles. Escolhemos abranger nossa revisão a fim de melhor descrever a potencial sobrecarga da internação na UTI na QVRS em longo prazo. Estudos futuros com alta qualidade em populações específicas são necessários para a elaboração de metanálises para grupos específicos da UTI.

COMENTÁRIOS FINAIS

A qualidade de vida relacionada com a saúde, em longo prazo, está comprometida em sobreviventes de internação na unidade de terapia intensiva comparada a população geral correspondente. Porém, esta não é significativamente afetada pela presença de sepse, delírio e lesão renal aguda durante a internação na unidade de terapia intensiva quando comparada com grupos controle de pacientes críticos. São necessários estudos de alta qualidade para quantificar a qualidade de vida relacionada com a saúde em sobreviventes de internação em unidade de terapia intensiva.
