ADESÃO DE GESTANTES À VACINAÇÃO NO CONTEXTO DE PANDEMIAS: REVISÃO INTEGRATIVA

RESUMO

Objetivo:

analisar as publicações nacionais e internacionais com relação à adesão de gestantes à vacinação no contexto de pandemias.

Método:

revisão de literatura, tipo integrativa, realizada em agosto de 2021 nas bases de dados LILACS, MEDLINE, Web of Science e SCOPUS, sem restrição de idioma e de tempo de publicação. Utilizaram-se os descritores indexados no DeCS e MeSH: Immunization, Vaccination, Pregnancy e Pandemics, combinados por meio do operador booleano. Os resultados obtidos foram exportados para o software gerenciador de referências EndNote e, posteriormente, para o aplicativo Rayyan - Intelligent Systematic Review. A amostra foi constituída por 27 estudos. A análise considerou a frequência e as similaridades entre os estudos.

Resultados:

foram evidenciados os fatores que interferem na adesão à vacinação pelas gestantes em tempos pandêmicos: desconfiança com as vacinas; preocupações sobre a segurança da vacinação na gravidez ou para a saúde do feto; falta de informações e desconhecimento de benefícios sobre a vacina. Além disso, as razões para a adesão foram o desejo de proteger o bebê, o conhecimento sobre a pandemia, a preocupação com o risco de infecção e a recomendação e orientação sobre a vacinação durante o pré-natal.

Conclusão:

foram verificados os fatores que podem interferir na adesão à vacinação, principalmente, com relação às novas vacinas no contexto de pandemias. Considera-se que investimentos em estratégias com relação à imunização na gravidez podem proporcionar benefícios para a saúde, prevenindo agravos evitáveis em gestantes e em seus bebês.

ABSTRACT

Objective:

to analyze national and international publications regarding pregnant women’s compliance with vaccination in the context of pandemics.

Method:

this is an integrative literature review, carried out in August 2021 in the LILACS, MEDLINE, Web of Science and Scopus databases, without language and publication time restriction. The descriptors indexed in DeCS and MeSH, Immunization, Vaccination, Pregnancy and Pandemics, combined using the Boolean operator, were used. The results obtained were exported to the EndNote reference manager software and, later, to the Rayyan - Intelligent Systematic Review application. The sample consisted of 27 studies. Analysis considered frequency and similarities between the studies.

Results:

the factors that interfere with compliance with vaccination by pregnant women in pandemic times were highlighted: distrust of vaccines; concerns about vaccination safety in pregnancy or for the fetus’ health; lack of information and lack of knowledge about the benefits of vaccine. Moreover, the reasons for compliance were desire to protect the baby, knowledge about the pandemic, concern about the risk of infection, and recommendation and guidance on vaccination during prenatal care.

Conclusion:

the factors that may interfere with compliance with vaccination were verified, mainly in relation to new vaccines in the context of pandemics. It is considered that investments in strategies related to immunization during pregnancy can provide health benefits, preventing preventable diseases in pregnant women and their babies.

se destacaron los factores que interfieren en la adherencia a la vacunación de las gestantes en tiempos de pandemia: desconfianza en las vacunas; preocupaciones sobre la seguridad de la vacunación en el embarazo o para la salud del feto; falta de información; y falta de conocimiento sobre los beneficios de la vacuna. Además, los motivos de adherencia fueron el deseo de proteger al bebé, el conocimiento sobre la pandemia, la preocupación por el riesgo de infección y la recomendación y orientación sobre la vacunación durante el prenatal.

Conclusión:

se verificaron los factores que pueden interferir en la adherencia a la vacunación, principalmente en relación a las nuevas vacunas en el contexto de pandemias. Se considera que las inversiones en estrategias relacionadas con la inmunización durante el embarazo pueden brindar beneficios para la salud, previniendo enfermedades prevenibles en las gestantes y sus bebés.

A pandemia de influenza é uma ocorrência cíclica e não previsível que se relaciona à emergência de um novo subtipo viral, resultante de mutações capazes de gerar um novo vírus.44. As gestantes representam um grupo vulnerável a surtos de doenças infecciosas devido às alterações fisiológicas próprias da gestação, que as deixam mais suscetíveis à infecção.66. As manifestações clínicas da COVID-19 impactam, de forma grave, as gestantes e puérperas, acarretando desfechos obstétricos desfavoráveis, como pré-eclâmpsia, sofrimento fetal, aborto espontâneo, desconforto respiratório materno, prematuridade, restrição de crescimento intrauterino, maior necessidade de parto cirúrgico, coagulopatias acompanhadas de disfunção hepática e óbito.66. As vacinas contribuem para a erradicação e para o controle efetivo das doenças imunopreveníveis, na redução da morbimortalidade por diferentes agravos com expressivo impacto na saúde e na qualidade de vida da população.1212. Porém, vislumbra-se a necessidade de conhecer os fatores que levem a adesão à vacinação e de consolidar estratégias de imunização na população de gestantes e puérperas, sobretudo em contextos pandêmicos.2020. Partindo desse pressuposto, a Revisão Integrativa tem como objetivo analisar as publicações nacionais e internacionais com relação à adesão de gestantes no contexto de pandemias.

MÉTODO

Trata-se de uma Revisão Integrativa (RI), a qual consiste em um método que proporciona a síntese de conhecimento, permitindo a inclusão de estudos experimentais e não experimentais, para uma compreensão completa do fenômeno ou do problema analisado com discussões de resultados para a aplicação na Prática Baseada em Evidência.2121. P (População), I (Fenômeno de Interesse) e Co (Contexto), em que a população é representada pelas gestantes; o interesse, pela adesão à vacinação e o contexto, pelas pandemias. A partir da estratégia, foi gerada a seguinte pergunta norteadora: “Quais evidências disponíveis na literatura sobre a adesão de gestantes com relação à vacinação no contexto de pandemias?”.

Para o levantamento dos estudos primários, realizou-se uma busca avançada no mês agosto de 2021, via endereço eletrônico, com acesso pelo portal CAPES, nas seguintes bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) via Biblioteca Virtual de Saúde (BVS); Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed) via National Library of Medicine); Web of Science via Clarinete Analytics, SCOPUS via Elsevier.

Para a busca dos artigos, foram utilizados os descritores exatos Immunization, Vaccination, Pregnancy e Pandemics, localizados nos Descritores em Ciências da Saúde (DeCS)/Medical Subject Headings (MeSH), combinados por meio dos operadores booleanosOR” e “AND” conforme o Quadro 1.

Os critérios de inclusão foram: artigos primários e que abordassem a temática relativa à vacinação de gestantes no contexto de pandemias; artigos publicados sem restrição de idioma e sem recorte temporal. Foram excluídos artigos de revisão de literatura, reflexões, guias, comentários, resumos de anais, teses, dissertações, trabalhos de conclusão de curso, carta do editor, relatórios, documentos oficiais de programas nacionais e internacionais, capítulos de livros e e-books.

A busca nas bases de dados identificou 1007 publicações, sendo 608 na Scopus, 329 na Web of Science, 69 na MEDLINE e um na LILACS (Tabela 1). Os resultados obtidos foram exportados para o software gerenciador de referências EndNote, no qual foram excluídos 282 estudos duplicados e, posteriormente, para o aplicativo Rayyan - Intelligent Systematic Review - em que foram excluídos mais 31 estudos duplicados, restando 694 artigos que foram selecionados para a leitura dos títulos e dos resumos por dois revisores independentes.

Foram lidos os títulos e os resumos dos 694 artigos com o auxílio do aplicativo Rayyan para a verificação de sua adequação aos critérios de elegibilidade. Foram excluídos 650 artigos. A partir da pré-seleção, totalizaram-se 44 artigos e, após a leitura dos textos completos, foram selecionados 27, os quais respondiam à pergunta norteadora da revisão. As etapas da seleção dos artigos que fizeram parte da RI estão demonstradas na Figura 1, seguindo a recomendação Preferred Reporting Items for Systematic reviews and Meta Analyses (PRISMA).2424. Quanto à análise, dois revisores independentes fizeram a avaliação às cegas dos estudos. No caso de discordância, era solicitado o parecer de um terceiro revisor. As reuniões para a avaliação dos artigos que iriam compor a RI aconteceram via plataforma Google Meet. O processo de inclusão dos artigos estava de acordo com os critérios de inclusão e a questão de pesquisa, assim, buscou-se reduzir riscos de vieses de seleção, conferindo-lhe rigor metodológico na inclusão dos estudos até a amostra final.

, considerando as seguintes variáveis: a identificação do artigo (título do periódico, autores, país, ano de publicação e idioma); instituição sede do estudo; características metodológicas (tipo de estudo, seleção da amostra); intervenções realizadas; resultados e implicações, nível de evidência e avaliação do rigor metodológico.

Na Figura 1, apresentam-se a triagem e o processo de seleção dos estudos avaliados até a amostra final, seguindo os critérios do PRISMA.2424. No Quadro 2, encontram-se os resultados apresentados de forma descritiva com o objetivo de sintetizar e de fomentar a discussão. Com relação à interpretação dos resultados, esses foram analisados qualitativamente, a partir da análise da frequência e de similaridades entre os estudos relacionados, tendo como ponto central a análise de fatores com relação à adesão de gestantes à vacinação.

RESULTADOS

Dos artigos selecionados para compor a RI, 22 abordavam sobre a vacinação na pandemia H1N1 e cinco sobre a pandemia da COVID-19, sendo 26 estudos no idioma inglês e publicados em 19 periódicos diferentes. Quanto à origem dos estudos, os países comumente encontrados foram Estados Unidos, Canadá, Turquia, China, Coreia do Sul, Austrália e Suíça. Identificaram-se, com menos frequência, estudos realizados no Catar, Irã, Marrocos, Costa do Marfim, Brasil, Reino Unido e Índia. Os artigos foram, em sua totalidade, classificados como nível seis de evidência. Quanto ao rigor metodológico, 24 artigos foram classificados como nível A, boa qualidade metodológica e viés reduzido (Quadro 2).

A hesitação vacinal é definida como o atraso na aceitação ou na recusa das vacinas apesar da disponibilidade dos serviços de vacinação. É um fenômeno comportamental que varia ao longo do tempo, do local e dos tipos de vacinas e inclui fatores como complacência, conveniência e confiança. A complacência resulta da baixa percepção de risco de contrair a doença prevenível por vacinação. A conveniência considera a disponibilidade física, financeira, a acessibilidade geográfica, a capacidade de compreensão e o acesso à informação em saúde. Na divulgação da vacina contra a COVID-19, deve-se reforçar a disseminação de informações corretas de forma transparente, de modo a evitar dar informações erradas ou preocupantes, como recurso para aumentar a aceitação das vacinas, especialmente para populações vulneráveis5353. Considera-se como principal limitação desta RI a possibilidade de perdas de estudos relevantes indexados em outras bases de dados. Além disso, a análise realizada das publicações está relacionada aos critérios de seleção de busca propostos neste estudo, contudo, foi possível inferir os fatores relacionados à adesão de gestantes à vacinação no contexto de pandemias. Salienta-se que vieses de publicação podem estar relacionados às particularidades de cada instituição sede onde foram realizados os estudos incluídos na revisão.

CONCLUSÃO

Os principais fatores na decisão vacinal das gestantes em situações de pandemia estão relacionados às preocupações com a segurança da vacina, ao medo de complicações e de seus efeitos colaterais, à falta de informações sobre a vacina e à ausência de recomendação pelos profissionais de saúde. Além disso, o desejo de proteger o bebê, o conhecimento sobre a pandemia e a recomendação e orientação sobre a vacinação são fatores para a adesão à vacinação no período gestacional.

Diante disso, pode-se considerar que investimentos em estratégias com relação à imunização na gravidez proporcionam benefícios para a saúde, prevenindo agravos evitáveis em gestantes e em seus bebês. Fortalecer programas de imunização, como uma prática avançada e tendo a assistência pré-natal como uma janela de oportunidades para adesão às vacinas, é considerado relevante na aceitação da vacinação entre gestantes e puérperas, sobretudo no contexto de pandemias.
