TECNOLOGIAS EDUCACIONAIS ELABORADAS PARA ENSINAR SUPORTE BÁSICO DE VIDA AO ADOLESCENTE: REVISÃO DE ESCOPO

RESUMO

Objetivo:

mapear as tecnologias educacionais elaboradas para ensinar suporte básico de vida ao adolescente.

Método:

revisão de escopo, conduzida conforme metodologia Joanna Briggs Institute, realizada entre maio e novembro de 2022, em sete bases de dados e no Google Scholar para responder à pergunta: “Quais as tecnologias educacionais elaboradas/utilizadas sobre suporte básico de vida para adolescentes?”. Dois revisores independentes selecionaram, mediante critérios de inclusão/exclusão, os artigos usando os programas EndNote® e Rayyan®. Utilizou-se formulários adaptados da Joanna Briggs para extração e análise metodológica. Os dados foram reportados pela extensão do checklist PRISMA-ScR e o protocolo deste estudo está publicado em: https://doi.org/10.17605/OSF.IO/P87SV.

Resultados:

selecionou-se 34 artigos, os quais foram organizados em seis categorias de tecnologias educacionais: 1) Audiovisuais; 2) Computacionais; 3) Kits de aprendizagem; 4) Dispositivos móveis; 5) Material Impresso; e 6) Manufaturadas. Este tema é publicado desde 1975, sobretudo por revistas médicas, cujos autores são principalmente europeus e norte-americanos. Foram incluídos 16 ensaios experimentais e 18 quase-experimentais, de qualidade metodológica variável, realizados, predominantemente, com estudantes de nível médio.

Conclusão:

as tecnologias educacionais mapeadas foram: vídeos, filmes, Digital Versatile Disc, imagens, músicas, narrativas audiovisuais, jogos, realidade virtual/avatar, web cursos, software computacional, aplicativos para computador e smartphone, cartões de tarefa e manequins (incluído os manufaturados). Embora haja inúmeras tecnologias educacionais sobre Suporte Básico de Vida para adolescentes, estas não têm proporcionado o alcance de bons níveis de habilidade práticas, principalmente quando se propõem a ensinar ventilação para leigos.

ABSTRACT

Objective:

to map the educational technologies devised to teach Basic Life Support to adolescents.

Method:

a scoping review conducted according to the Joanna Briggs Institute methodology and carried out between May and November 2022, in seven databases and Google Scholar, to answer the following question: “Which are the educational technologies devised/used about Basic Life Support for adolescents?”. By means of inclusion/exclusion criteria, two independent reviewers selected the articles resorting to the EndNote® and Rayyan® software programs. Forms adapted from the Joanna Briggs Institute were used for data extraction and methodological analysis. The data were reported by means of the PRISMA-ScR checklist extension and the protocol of this study is published in: https://doi.org/10.17605/OSF.IO/P87SV.

Results:

a total of 34 articles were selected, which were organized into six categories of educational technologies: 1) Audiovisual; 2) Computational; 3) Learning kits; 4) Mobile devices; 5) Printed material; and 6) Manufactured. This topic has appeared in publications since 1975, especially in medical journals, mainly by European and North American authors. The materials included were 16 experimental and 18 quasi-experimental trials, of varied methodological quality levels and predominantly conducted with High School students.

Conclusion:

the educational technologies mapped were as follows: videos, films, Digital Versatile Disc, images, music, audiovisual narratives, games, virtual reality/avatar, web courses, computational software, computer and smartphone apps, task cards and manikins (including manufactured ones). Although there are countless educational technologies on Basic Life Support for adolescents, they have not allowed achieving good levels of practical skills, mainly when they propose to teach ventilation to lay people.

Este tema ha sido objeto de publicaciones desde 1975, especialmente en revistas especializadas en Medicina, y principalmente con autores europeos y norteamericanos. Se incluyeron 16 ensayos experimentales y 18 cuasiexperimentales, de calidad metodológica variable y mayoritariamente realizados con estudiantes de nivel medio.

Conclusión:

las tecnologías educativas que se mapearon fueron las siguientes: videos, películas, Digital Versatile Disc, imágenes, música, narrativas audiovisuales, juegos, realidad virtual/avatar, cursos web, software de computación, apps para computadoras y smartphones, fichas de tareas y maniquíes (incluyendo los fabricados). Aunque se dispone de innumerables tecnologías educativas sobre Suporte Vital Básico para adolescentes, no ha permitido lograr buenos niveles de habilidades prácticas, principalmente cuando se proponen enseñar ventilación a personas comunes.

INTRODUÇÃO

. O relatório da American Heart Association (AHA) apontou que somente 10,4% dos pacientes com Parada Cardíaca Extra Hospitalar (PCREH) sobrevivem à hospitalização inicial. Neste contexto, só 39,2% dos adultos recebem Reanimação Cardiopulmonar (RCP) por leigos e apenas 11,9% tiveram um Desfibrilador Externo Automático (DEA) aplicado22. . A solução deste problema perpassa pelo ensino e treinamento de leigos em Suporte Básico de Vida (SBV). De acordo com a cadeia de sobrevivência a uma PCREH, SBV é o termo que designa as etapas de ressuscitação cardiopulmonar. As etapas dirigidas a leigos consistem em acionar o Serviço Médico de Emergência (SME), realizar RCP imediata de alta qualidade e aplicar rápida desfibrilação33. Dentre os leigos que precisam ser capacitados em SBV, estão os adolescentes que além de aprender, podem ensinar RCP aos colegas44. Por outro lado, estudo do tipo Ensaio Controlado Randomizado (ECR) evidenciou que, quando os adolescentes recebem treinamento, apresentam habilidades práticas e autoconfiança para ajudar efetivamente vítimas de PCR77. Neste sentido, com vistas a interpor o baixo conhecimento de adolescentes sobre esse tema, a AHA e a Sociedade Europeia de Ressuscitação recomendam o treinamento mediante design instrutivo em combinação com tecnologias educacionais e ensino ministrado por instrutores88. Não obstante, para se fazer uso dessas tecnologias, instrutores precisam de respostas sobre lacunas literárias envolvendo este tema. Por exemplo, ainda não se sabe quais são as tecnologias utilizadas para ensinar SBV ao adolescente, em que contexto podem ser aplicadas e qual a sua efetividade ou limitação. Para melhor uso das tecnologias, é necessário conhecer o seu processo de construção e validação, quais públicos são mais beneficiados e se são permeadas por estudos com qualidade metodológica que possibilita replicação e segurança de suas indicações. feitas em maio de 2022 não encontraram revisões de escopo em curso ou concluídas que respondessem à pergunta deste estudo.

Neste contexto, conhecer a produção científica sobre as tecnologias educacionais é relevante, pois possibilitará aos enfermeiros e demais profissionais da saúde, o ensino e a Prática Baseada em Evidências (PBE). Deste modo, objetivou-se mapear as tecnologias educacionais elaboradas para ensinar suporte básico de vida ao adolescente.

MÉTODO

Esta revisão, realizada entre maio e novembro de 2022, seguiu cinco passos: (1) definição da pergunta do estudo; (2) estabelecimento de critérios de inclusão/exclusão; (3) definição do tipo de fonte e construção da estratégia de pesquisa para cada base de dados; (4) seleção e extração de estudos; (5) análise da evidência e apresentação dos resultados.

A questão de pesquisa foi elaborada com base no acrônimo PCC em que P (população) - adolescentes, C (conceito) - tecnologias educacionais e C (contexto) - suporte básico de vida. Deste modo, perguntou-se: “Quais as tecnologias educacionais são elaboradas/utilizadas sobre suporte básico de vida para adolescentes? Definiu-se os critérios de inclusão/exclusão para cada letra do acrônimo PCC. Deste modo, foram incluídos estudos cuja população era constituída por adolescentes com idade entre 10 e 19 anos1313. Excluiu-se artigos que não se encontravam publicados na íntegra ou que apresentassem somente estratégias educativas sem o uso de tecnologias, haja vista que elas foram bem delimitadas pela literatura atual1414. . Esta revisão avaliou pesquisas de todos os delineamentos metodológicos e não estabeleceu limite temporal ou restrições de idioma.

As buscas foram realizadas nas bases Medical Literature and Retrieval System online (MEDLINE) via National Center for Biotechnology Information (NCBI/PubMed); Cumulative Index to Nursing & Allied Health Literature (CINAHL) via coleção Thomson Reuters; Web of Science (WOS) via Clarivate Analytics e Embase via Elsevier, acessadas pelo Portal de Periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES); Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Índice Bibliográfico Español en Ciencias de la Salud (IBECS) e Base de Dados em Enfermagem (BDENF) via Biblioteca Virtual em Saúde (BVS). Também foram realizadas buscas no Google Scholar.

. Na primeira, realizou-se pesquisa nas bases MEDLINE (NCBI/PubMed) e CINAHL para identificar os termos contidos em títulos, resumos e descritores de indexação. Na etapa dois, uma segunda pesquisa foi feita usando os termos da primeira etapa com os descritores identificados na segunda. Na terceira, os pesquisadores buscaram nas listas de referências estudos que porventura não tivessem sido recuperados nas duas primeiras etapas.

Para seleção dos estudos, contou-se com dois revisores independentes, ambos com expertise em SBV e tecnologias educacionais (PKL/RSM). As discordâncias foram gerenciadas por uma terceira revisora (MOM), que emitiu parecer para inclusão e/ou exclusão dos estudos em conflito. Realizou-se pré-teste com os revisores para avaliar a “calibragem” consensual perante a seleção dos estudos. O pré-teste obteve índice de concordância de 94%.

As publicações foram incluídas ou excluídas primeiramente mediante leitura do título e resumo. Em seguida, foram lidas na íntegra e avaliadas de acordo com os critérios de inclusão/exclusão. Nesse sentido, os textos oriundos da literatura cinzenta tiveram a mesma dinâmica de avaliação (peer review).

, que foi transcrito para Microsoft Office Excel 365/2022, na forma de quadro sinóptico, cujas variáveis foram: autores, periódico, ano, país, local de estudo, idioma, objetivo, delineamento, nível de evidência, população, tecnologia, resultados, conclusões, referencial teóricos e processo de construção e/ou validação da tecnologia.

Ainda que não seja obrigatória a avaliação da qualidade metodológica dos estudos, optou-se por fazer esta avaliação em função dos dois delineamentos metodológicos identificados. Deste modo, cada estudo incluído teve sua qualidade metodológica avaliada através de dois instrumentos da JBI’s Critical Appraisal Tools1818. Tufanaru C, Munn Z, Aromataris E, Campbell JHL. Além desta avaliação, os artigos também tiveram seu Nível de Evidência (NE) classificado de acordo com as recomendações de Melnik1919. Melnyk BM, F-OE. O processo de triagem e os resultados estão apresentados em figuras e diagramas que acompanham um resumo descritivo das variáveis inquiridas. As tecnologias encontradas foram sintetizadas, a partir da maneira de aplicação, em seis categorias analíticas. RESULTADOS

Identificou-se 3.280 publicações, sendo 1.567 na MEDLINE/PubMed, 1.038 na EMBASE, 232 na CINAHL, 40 na LILACS/BVS, 17 na BDENF/BVS, 7 na Web Of Science, 29 na IBECS e 350 no Google Scholar. Após exclusão dos duplicados e aplicação de critérios de inclusão/exclusão, 34 artigos compuseram a amostra conforme fluxograma apresentado na Figura 1.

Dos 34 artigos, 7 (20,5%) foram publicados na revista Resuscitation, a qual concentrou o maior número de pesquisas sobre a temática. Outros 27 periódicos publicaram entre um e dois estudos. O texto mais antigo é de 1975 e, entre os anos de 2016 e 2019, houve o maior número de publicações, 18 (52,9%).

O continente Europeu concentrou maior número de publicações, com 24 (70,5%) estudos, com destaque para Espanha e Bélgica com cinco (14,7%) cada, seguido do continente Americano, com cinco (14,7%) pesquisas, sendo quatro (11,7%) dos Estados Unidos da América (EUA) e uma (2,9%) do Brasil. Houve três (8,8%) estudos asiáticos, sendo dois (5,8%) da Índia e um (2,9%) da Coreia do Sul; um (2,9%) euroasiático publicado na Turquia e outro (2,9%) de cooperação conduzido por EUA e Suécia (2,9%).

Quanto ao idioma, 28 (82,3%) estudos foram publicados em inglês, quatro (11,7%) em espanhol, um (2,9%) em Húngaro e um (2,9%) em português. Com relação ao delineamento, 18 (52,9%) eram quase-experimentais, com nível III na pirâmide de evidências e 16 (47%) eram ensaios controlados randomizados, sendo nível II.

A avaliação crítica metodológica, indicada pelo instrumento JBI, revelou que os estudos experimentais apesar de atenderem a maioria dos critérios indicados para a avaliação de qualidade, não atendiam itens importantes relacionados à alocação e ao cegamento dos participantes, do pesquisador e dos responsáveis, por fornecer o tratamento e dos avaliadores de resultados. Constatou-se que 14 estudos quase-experimentais atenderam a maioria dos quesitos da avaliação da qualidade apontados pelo instrumento, sendo considerados de boa qualidade. Os principais pontos não atendidos foram relativos à presença de grupo controle e/ou à realização de múltiplas medições.

Os 34 estudos foram dirigidos, em sua totalidade amostral, à 19.878 adolescentes escolares. Destes artigos, 17 (50%) foram realizados com 8.977 alunos do ensino médio, dois (5,8%) com 313 adolescentes do ensino fundamental, dois (5,8%) envolvendo 547 estudantes do ensino fundamental e médio, e em 13 (38,2%) pesquisas não houve descrição do nível escolar dos 10.041 participantes envolvidos.

Apenas dois (5,8%) artigos fundamentaram a construção/desenvolvimento da tecnologia sobre pressupostos de teorias educacionais, sendo a teoria cognitiva da aprendizagem multimídia a única adotada2121. Há grande diversidade de tecnologias envolvidas no ensino do SBV para adolescentes. Por este motivo e por apresentarem características operacionais diferentes, sintetizou-se para análise seis categorias: audiovisuais; computacionais; kits de aprendizagem, tecnologias para dispositivos móveis; material impresso; e artigos manufaturados (Figura 2). Se por um lado, os Apps identificados nesta revisão não tiveram bom desempenho no processo de ensino do SBV, por outro lado há evidências de que, quando são utilizados como dispositivo de alerta, acionam pessoas que rapidamente prestam assistência, gerando, portanto, melhora nos índices de sobrevivência6060. Acredita-se que, por meio do advento da tecnologia digital, materiais impressos estão sendo substituídos por tecnologias de alto custo e baixa dispersão. As pesquisas que envolveram cartões de tarefa tiveram resultados antagônicos. No primeiro estudo envolvendo este material impresso, ao adotar o princípio da contiguidade espacial, identificou-se melhoras de aprendizagem sobre o volume e taxa de ventilação2121. Pesquisadores apontam que a produção dessas ferramentas abre possibilidades, pois trata de recurso visual com informações claras e atrativas que podem ser facilmente visualizadas e capazes de proporcionar a assimilação do conteúdo6161. O segundo estudo, ao combinar os cartões de tarefa com outras tecnologias, porém sem fundamentação de teorias educacionais, identificou que os resultados das práticas revelaram menor volume de ventilação, profundidade e velocidade de RCP2424. As tecnologias manufaturadas identificadas nesta pesquisa foram construídas com o intuito de constatar se equipamentos de baixo custo são efetivos para ensinar RCP. Estes achados corroboram com os dados encontrados em uma pesquisa sobre programa Kids Save Lifes (KSL). O KSL objetiva disseminar, por meio de matérias de baixo custo, princípios da RCP entre crianças em idade escolar. Do mesmo modo, pesquisa realizada na Espanha evidenciou que as tecnologias de baixo custo foram capazes de aumentar o nível de conhecimento dos alunos sobre RCP porém, após dois meses de treinamento, houve queda do nível de conhecimento6464. . Neste contexto, surge a necessidade de identificar ou até mesmo elaborar novos estudos que possam revelar tecnológicas que gerem conhecimento e habilidade, mas que estejam associados a melhores índices de retenção de aprendizagem.

Outro dado que chama atenção diz respeito ao processo de construção e validação das tecnologias utilizadas. A maioria dos estudos não esclarece como e se foi realizada essa etapa. No mesmo sentido, apesar de terem a pretensão comum de aferir os benefícios da tecnologia sobre o conhecimento teórico e prático, inclusive sobre o mesmo tema, a maioria dos estudos não utiliza instrumentos comuns que permitam comparação válida e reprodutível entre as pesquisas. Assim, o que é considerado um bom conhecimento e/ou prática em RCP em determinado estudo, pode não ser para outro.

Por motivo semelhante, uma pesquisa do tipo revisão sistemática encontrou dificuldades para estabelecer qual o momento ideal para realização de intervenção em emergência. Dentre os motivos apontados, cita-se a baixa qualidade metodológica dos estudos que não adotavam variáveis generalizáveis passíveis de comparação interestudo6565. . Ao considerar que qualquer teste precisa ser reprodutível, sugere-se o desenvolvimento de instrumentos padronizados para aferição de conhecimento e prática em SBV.

CONCLUSÃO

Essa revisão de escopo possibilitou mapear as tecnologias educacionais sobre suporte básico de vida para adolescentes. Os estudos revelaram que este tema é publicado desde 1975, sobretudo por revistas médicas, cujo autores são, principalmente, europeus e norte-americanos. As pesquisas foram feitas por meio de ensaios experimentais e quase-experimentais, com qualidade metodológica variável, dirigidas predominantemente para estudantes de nível médio.

Dentre as tecnologias mapeadas, citam-se: vídeos, filmes, DVDs, imagens, músicas, narrativas audiovisuais, jogos, realidade virtual/avatar, web cursos, software computacional, aplicativos para computador e smartphone, cartões de tarefa e manequins (incluído os manufaturados). Embora haja inúmeras tecnologias educacionais sobre SBV para adolescentes, estas não têm proporcionado o alcance de bons níveis de habilidade práticas, principalmente quando se propõem a ensinar ventilação para leigos.

As lacunas dos estudos são o não detalhamento do processo de construção e validação das tecnologias e dos instrumentos que aferiram conhecimento e prática, a não adoção de embasamento teórico no processo de desenvolvimento, o baixo número de pesquisas na América Latina, a ausência de publicações desta temática no continente africano e a limitada quantidade de estudos que avaliaram tecnologias de baixo custo.

Sugere-se, portanto, que sejam elaborados novos estudos envolvendo tecnologias educacionais feitas com base em processo reprodutível de construção, validação e aferição da efetividade. Propõe-se também que, ao aplicar estas tecnologias, seja oportunizado o treinamento para o aumento de habilidades práticas para que este conhecimento seja traduzido de forma consistente em cuidados clínicos com pacientes reais.
