Sir, the recent publication on shark island pedicle flap for repairing of basal cell carcinoma is very interesting. André et al. concluded that "This technique allows correction of surgical defects of the ala rebuilding the original anatomy, maintaining cosmetic units, without need for a graft."1 I agree that this technique might be useful but it is still questionable to state that it is simple. It should be noted that this technique require high experience of the surgeon.

Furthermore, the present report lacks for the long term following up. In addition, it will be useful if the cost of the technique can be clarified comparing to other alternative methods.
