ABSTRACT: Chronic supurative otitis media (CSOM) is a common condition in patients with unrepaired cleft palate and cleft lip palate. Muscular abnormality leads to auditive tube malfunction, which will not equilibrate differential air pressure between middle ear and nasopharynx. It consequently increases middle ear cavity fluid.

Effusive CSOM samples from 40 patients with clef palate and cleft lip palate from the HRAC-USP-Bauru-SP were studied. After culture, the bacteria were isolated and submitted to in vitro standard clinical drug susceptibility tests. Positive cultures were obtained in all studied cases. Pseudomonas aeruginosa (35%), Staphylococcus aureus (15.5%), Enterococcus faecalis (14%), and Proteus mirabilis (12%) were the most frequent bacteria in 57 strains. Gram-negative bacilli (enteric Gram-negative bacilli and non-fermentative bacilli) were observed in 67% of samples. Pseudomonas aeruginosa showed better sensitivity to ciprofloxacin, and enteric Gram-negative bacilli to gentamicin.

S. aureus and E. faecalis showed high sensitivity to imipenem and trimethoprim/sulfamethoxazole, respectively. These data will help in choosing antibiotics for CSOM patients with cleft palate or clef lip palate, where non-surgical management is the main therapy.
