OBJETIVO:
Determinar a prevalência e os fatores de risco para sibilância ocasional (SO) e recorrente - síndrome do lactente sibilante (SLS).
MÉTODOS:
Pais de lactentes (12-15 meses de idade) responderam o questionário escrito Estudio Internacional de Sibilancias en Lactantes.
RESULTADOS:
Foram incluídos 1.269 lactentes residentes na cidade de Blumenau (SC). Desses, 715 (56,34%) apresentavam sibilância, sendo essa mais frequente nos meninos. As prevalências de SO e SLS foram de 27,03% (n = 343) e 29,31% (n = 372), respectivamente. O primeiro episódio de sibilância ocorreu aos 5,55 ± 2,87 meses de idade em média. Em 479/715 (66,99%) lactentes com sibilância, o primeiro episódio ocorreu nos primeiros seis meses de vida, enquanto 372/715 (52,03%) apresentaram três ou mais episódios. Os fatores associados com sibilância foram pneumonia; uso de corticoide oral; resfriado; frequência a creches; pais com asma e/ou alergia; mãe com emprego; gênero masculino; sem aleitamento materno; e mofo. Os fatores associados com SLS foram resfriado; diagnóstico médico de asma; visitas a serviço de emergência; uso de corticoide oral/inalatório; pneumonia, bronquite; dispneia; frequência a creches; uso de broncodilatador; pais com asma; sem aleitamento materno; mãe com emprego; e presença de cachorro na residência.
CONCLUSÕES:
A prevalência de sibilância na população estudada foi elevada, sendo multifatorial e com fatores de risco intrínsecos e extrínsecos (infecções do trato respiratório, alergia nos pais, frequência a creches e idade precoce de chiado). A elevada prevalência e os fatores de risco intrínsecos encontrados indicam a necessidade e a oportunidade para estudos epidemiológicos e genéticos nessa população. Além disso, estratégias para que as mães aumentem o período de amamentação ao seio e evitem que seus filhos frequentem creches antes dos seis primeiros meses de vida devem ser estimuladas.

OBJECTIVE:
To determine, in a sample of infants, the prevalence of and risk factors for occasional wheezing (OW) and recurrent wheezing-wheezy baby syndrome (WBS).
METHODS:
Parents of infants (12-15 months of age) completed the International Study of Wheezing in Infants questionnaire.
RESULTS:
We included 1,269 infants residing in the city of Blumenau, Brazil. Of those, 715 (56.34%) had a history of wheezing, which was more common among boys. The prevalences of OW and WBS were 27.03% (n = 343) and 29.31% (n = 372), respectively. On average, the first wheezing episode occurred at 5.55 ± 2.87 months of age. Among the 715 infants with a history of wheezing, the first episode occurred within the first six months of life in 479 (66.99%), and 372 (52.03%) had had three or more episodes. Factors associated with wheezing in general were pneumonia; oral corticosteroid use; a cold; attending daycare; having a parent with asthma or allergies; mother working outside the home; male gender; no breastfeeding; and mold. Factors associated with WBS were a cold; physician-diagnosed asthma; ER visits; corticosteroid use; pneumonia; bronchitis; dyspnea; attending daycare; bronchodilator use; having a parent with asthma; no breastfeeding; mother working outside the home; and a dog in the household.
CONCLUSIONS:
The prevalence of wheezing in the studied population was high (56.34%). The etiology was multifactorial, and the risk factors were intrinsic and extrinsic (respiratory tract infections, allergies, attending daycare, and early wheezing). The high prevalence and the intrinsic risk factors indicate the need and the opportunity for epidemiological and genetic studies in this population. In addition, mothers should be encouraged to prolong breastfeeding and to keep infants under six months of age out of daycare.

Introdução
A sibilância no primeiro ano de vida pode ocorrer de modo isolado e ocasional -
sibilância ocasional (SO) - ou de forma recorrente, caracterizando a síndrome do
lactente sibilante (SLS). Ambas são condições clínicas frequentes, heterogêneas,
causadas por numerosas doenças e situações de agravos às vias aéreas e se manifestam
clinica e laboratorialmente como vários fenótipos. (

Algumas crianças têm SLS precoce transitória, enquanto, em outras, os sintomas
respiratórios, que ocorrem no início da vida, podem ser a primeira manifestação de
asma. Nas últimas décadas, fatores de risco para a SLS e sua relação com o
desenvolvimento de asma têm sido investigados, o que é fundamental para a prevenção

fatores de riscos envolvidos nessas síndromes, no primeiro ano de vida, em diferentes
regiões do nosso país.
Dois aspectos chamam a atenção para a SO e a SLS no primeiro ano de vida: (i) aqueles
expostos a fatores de riscos para SO ou SLS têm maior probabilidade para desenvolver
asma; (ii) a maioria dos pacientes com SLS tem exacerbações por vírus, sem atopia, e
não desenvolve asma.(

fatores de riscos associados, tais como antecedentes familiares para asma;
determinados hábitos alimentares e ocupacionais durante a gestação; tabagismo
passivo; ausência de amamentação ao seio materno; gênero masculino; hábito de
frequentar creche; determinados poluentes ambientais (fumaça de cigarro,
sensibilização a aeroalérgenos de ácaros, baratas e epitélios de animais); ocorrência
de pneumonia ou infecções respiratórias virais por vírus sincicial respiratório ou
rinovírus; e uso de antibióticos ou paracetamol.(

cálculo do tamanho da amostra, foi considerado um poder amostral de 95% e α de 0,01,
obtendo-se o valor de 1.100 crianças no primeiro ano de vida a serem incluídas no
estudo.
O QE-EISL contém 50 perguntas sobre sibilância, fatores de risco, e características
demográficas e ambientais, tendo sido traduzido para o português e validado para
nossa população. (

O QE-EISL foi aplicado pelo pesquisador principal e por funcionários dos Centros de
Saúde, previamente treinados, e respondidos pelos cuidadores dos lactentes (faixa
etária de 12-15 meses), conforme o método preconizado e utilizado no estudo

) durante consultas para a imunização de rotina ou consultas de
puericultura nos centros de saúde num período de 18 meses.
Os responsáveis que aceitaram e assinaram o termo de consentimento livre e esclarecido para participar do estudo tiveram o lactente incluído. O estudo foi aprovado pelo Comitê de Ética da Fundação Universidade Regional de Blumenau, sob o número 039/08.
Os indivíduos com diagnóstico pré-existente de doença genética, neuropatia, miopatia, doença cardíaca, desnutrição primária ou secundária, fibrose cística e malformações somáticas, assim como aqueles com expectativa de vida limitada, foram excluídos do estudo. Os lactentes foram divididos em três grupos: SLS (apresentaram três ou mais episódios de sibilância); SO (apresentaram menos de três episódios); e não sibilantes (nunca apresentaram sibilância).
Os dados obtidos pelo QE-EISL foram codificados e transferidos para um banco de dados elaborados no Microsoft Excel(r) 2007 e analisados estatisticamente com a utilização do programa Statistical Package for the Social Sciences for Windows, versão 18.0 (SPSS Inc., Chicago, IL, EUA). A análise estatística para os fatores de risco entre os lactentes com e sem sibilância e entre os grupos SO e SLS foi realizada pelo teste de regressão logística bivariada e multivariada. Os resultados estão apresentados em OR e IC95%. Foram descritas as variáveis que apresentaram valores significantes para a associação estudada. A significância estatística nos testes foi considerada para um valor de α < 0,05.
Resultados
Foram entrevistados os cuidadores de 1.269 lactentes com idade entre 12 e 15 meses de idade, sendo que 1.211 (95,43%) eram caucasoides. Dos 1.269 pacientes incluídos, episódios de sibilância foram verificados em 715 (56,34%), dos quais 343 (27,03%) e 372 (29,31%) apresentavam, respectivamente, SO e SLS.
O questionário foi respondido, respectivamente, pelas mães, pais e outros - 1.073 (84,55%), 106 (8,35%) e 90 (7,09%). Em relação à idade em meses, a distribuição foi a seguinte: 12 meses, em 549 lactentes (43,26%); 13 meses,em 295 (23,25%); 14, em 331 (26,08%); e 15, em 94 (7,41%).
Os fatores de risco identificados e que mostraram ORs positivas estão descritos nas figuras e nas tabelas. A Figura 1A e a Tabela 1 mostram a análise bivariada para a presença de sibilância na população estudada. A Figura 1B e a Tabela 1 mostram a análise multivariada para a presença de sibilância na população estudada. Nas Figuras 1A/B e na Tabela 1 foram incluídos todos os indivíduos do estudo, sendo a comparação realizada entre os lactentes com sibilância (grupos SO e SLS combinados) e sem sibilância (grupo NS). A Tabela 1 mostra a distribuição dos fatores de risco com os respectivos ORs e IC95% para a presença ou não de sibilância entre os lactentes estudados.

A Tabela 2 mostra a distribuição dos fatores de risco com os respectivos ORs e IC95% para a gravidade da recorrência de sibilância entre os lactentes estudados.

Discussão
O presente estudo foi o primeiro sobre a prevalência e os fatores de risco para
sibilância em lactentes no estado de Santa Catarina, com a maior prevalência de
sibilância em lactentes no primeiro ano de vida entre os estudos realizados no Brasil
utilizando o QE-EISL. As seguintes prevalências foram encontradas para a presença de

Infecções respiratórias virais podem provocar efeitos adversos graves em pacientes
com asma e são responsáveis por quase 80% dos episódios de exacerbação da mesma,
tanto em crianças quanto em adultos. Se as infecções respiratórias determinam a
progressão da SLS ou a sua eventual gravidade da doença ainda é pouco claro na

Em nosso estudo, os lactentes que frequentavam creches tiveram um maior risco para a
SO e SLS do que aqueles que não as frequentavam. Pais e médicos são conhecedores de
que o hábito de frequentar creches nos dois primeiros anos de vida é um fator de
risco para infecções respiratórias de repetição. Esse é um aspecto muito estudado. Em

associada à presença de sibilos recorrentes, o prognóstico desses pacientes, em longo
prazo, é provavelmente bom. Por outro lado, se frequentar creches for um fator de
risco para a sibilância grave, isso implica um risco aumentado de desenvolvimento de
asma. Dessa maneira, estudos em longo prazo e com o isolamento dos vírus envolvidos
são necessários para esclarecer o papel de permanência em creches como um fator de
risco para o desaparecimento ou para a persistência de sibilância.(

Nossos dados apontam que lactentes do gênero masculino apresentam uma maior chance de
apresentar sibilância. Tal fato tem sido verificado em outros estudos, e sabe-se que
os meninos têm as vias aéreas mais estreitas que as meninas, demonstram maior grau de
sensibilização a aeroalérgenos e têm níveis mais elevados de IgE no início da
vida.(26) Entretanto, essa maior prevalência de sibilância recorrente
em meninos diminui à medida que a criança cresce e atinge a adolescência.(

Embora nós não tenhamos avaliado a gravidade dos quadros de sibilância, cada vez mais
estudos mostram a importância do aleitamento materno, principalmente na proteção
contra episódios de sibilância grave em lactentes. Um estudo, que avaliou 12.474
crianças com bronquiolite, das quais 1.588 necessitaram de internação, demonstrou que
lactentes cujas mães não deram início ao aleitamento materno na maternidade
apresentavam um maior risco para internação por bronquiolite viral.(

Um grupo de autores estudou crianças internadas com bronquiolite viral aguda e observou que a duração do aleitamento materno foi inversamente relacionada ao tempo de utilização de oxigênio e à duração da internação, evidenciando que, para cada mês de aleitamento materno, havia uma redução de onze horas no tempo de utilização de oxigênio.(29) Assim, estratégias para que as mães aumentem o período de amamentação de seus filhos e evitem creches antes dos 6 primeiros meses de vida devem ser estimuladas.

) verificaram que a não exposição ao tabaco e o aumento da ingestão de
óleo de peixe durante a gravidez e na primeira infância podem ser eficazes na redução
da incidência de asma aos dois anos de idade. O impacto diferencial em meninos e
meninas indica que a fisiopatologia da asma pode depender do gênero dos filhos.
Um dos fatores de risco para SLS e asma em crianças é a presença de antecedentes de atopia e alergia nos pais e familiares. Em nosso estudo, encontramos que lactentes que tinham pais com asma e alergia apresentaram uma maior chance de ter episódios SO e SLS do que aqueles cujos pais não apresentaram história familiar de asma e alergia, evidenciando que fatores genéticos são importantes para a SO e a SLS.
O presente estudo apresentou como limitação a não inclusão de novos questionamentos sobre possíveis fatores de risco que podem ser específicos na população estudada. Em contrapartida, esse fato pode ser motivo de novos estudos, como a análise de frequência em creches e a presença/ausência de irmãos mais velhos.
Em conclusão, a prevalência de sibilância em lactentes na cidade de Blumenau foi de 56,34%, sendo 27,31% e 29,31% de SO e SLS, respectivamente. Tais resultados elevados são multifatoriais e sujeitos a fatores de risco intrínsecos e extrínsecos. Entre os fatores de maior risco para sibilância encontram-se as infecções do trato respiratório, a presença de alergia nos pais, o hábito de frequentar creches e a idade precoce do surgimento de chiado. A elevada prevalência da SLS, com um alto índice de fatores de risco intrínsecos na população estudada, indica a necessidade e a oportunidade para estudos epidemiológicos e genéticos nessa população.
