  • Inquérito preliminar pela reação de Montenegro em população rural no Município de Trajano de Moraes, RJ

    Num inquérito pela reação de Montenegro em distrito do município de Trajano de Moraes, RJ, foram testadas 233 pessoas com 12 resultados positivos e 9 duvidosos. Embora a percentagem global (9% somando os positivos e duvidosos) seja bastante baixa, parece haver uma concentração dos resultados positivos em duas localidades (São Francisco de Paula e o Couto), o que poderia sugerir a existência de microfocos numa região onde a doença assume normalmente um caráter eminentemente esporádico.

    In a survey of the Montenegro reaction, carried out in a district of the Municipality of Trajano de Moraes, State of Rio de Janeiro, 233 persons were tested, yielding 12 positive and 9 doubtful results (5,1 and 3,9% respectively). Although the overall percentage 9% (totalling positive and doubtful results) is rather loic, there appears to be a concentration of positivity in two points of the area: São Francisco de Paula and Couto. In a region where the disease (leishmaniasis) bears a distinctly character, this may be considered suggestive of the existence of microfoci.
  • The vaccination of h uman beings with live avirulent vaccine of Trypanosoma cruzi: A two year follow-up of the first two cases

    O autor apresenta o seguimento de dois casos humanos vacinados com vacina viva avirulenta de Trypanosoma cruzi. Os resultados dos exames ditos específicos e também alguns inespecíficos demonstraram que não houve, no período de dois anos, qualquer sinal de infecção-doença. Em um dos voluntários houve acentuada discrepância nos resultados da reação de Guerreiro & Machado, não só em um mesmo laboratório, com a mesma amostra de soro, mas também entre diferentes laboratórios, na mesma e em diferentes oportunidades. Apesar de toda essa confusão, os resultados POSITIVOS não foram além de 4,99% (em dois laboratórios apenas), os DUVIDOSOS não ultrapassaram 18,6% e finalmente os casos NEGATIVOS atingiram 76% de todas essas reações. Os testes de imunofluorescência para T. cruzi deram 94,3% de resultados NEGATIVOS no Paciente 1 e 100% no Paciente 2. Hemoculturas, xenodiagnósticos e sub-inoculação em camundongos jovens foram sempre negativos em ambos os pacientes. Conclue o autor que a cepa PF parece ser avirulenta para o homem tal como ele e outros (Santos16 e Hungerer6) já demonstraram ser para animais de laboratório.

    A two year follow-up of the first two volunteers vaccinated icith the live PF Trypanosoma cruzi strain demonstrated that the parasitological and clinical test were negative during and after that period. Of the serological tests employed, the CFT presented, in only one case, conflicting results, particulary in one laboratory and among different laboratories. However negative results were greater than all doubtful and positive one combined. The IFT icere negative in both patients. Some comments are made about the sensibility and specificity of these tests. The author concluded that the vaccine, in spite of the very large dose used in these cases, seems to be safe for human beings.
  • Influência da fonte de antigeno e dos fatores alérgicos no teste intradérmico da esquistossomose mansônica

    73 (setenta e três) pacientes esquistosscmóticos e 29 (vinte e neve) indivíduos não-parasitados foram testados peio teste intradérmico para esquistossomose mansônioa utilizando-se antígeno de vermes adultos S. manseni obtidos per filtração sanguínea humana. O teste foi comparado com o antígeno clássico obtido de vermes do camundongo, observando-se a interferência de manifestações alérgicas.

    It has been demonstrated by Smithers & Terry (8) that adult worms are destroyed when they are transferred to another host previously immunized by the first one. As is well known, the schistoroma antigen used in the schistosomiasis intradermal test is usually prepared from worms obtained with experimentally infected mice. This fact raised an important question in our mind: Could the antigen prepared from mice worms bring with it some of the host antigens? If this is so, the host antigens contaminate the S. mansoni antigen and would interfere with the specificity of the test. Shistosoma antigens from worms removed by portal blood perfusion during splenectomy of human patients, were prepared and tested side by side, using the well-known antigen prepared from mice in Brazil by Pellegrino. 73 patients without any other parasitosis were tested using both kinds of antigen at the same time, and 29 individuals without the parasitosis were used for control. From table l, we can see that the antigen gave a large size papule with 100% positive, compared with the smaller human antigen, but showed a percentage of 52%, false positive. In the patients tested with antigen from human S. mansoni worms the positivity was 90% and the false cases dropped to 27%. Looking at table 2, (relationship to the allergic factors) one can see that mize antigen gave more positive results in 39 patients without an allergic background. On the other hand in the control group without schistosomiasis and with allergic manifestation the average tize of the papules was positive for the mice S. mansoni antigen and negative for the human S. mansoni antigen. In conclusion, it has been demonstrated that average size of the papules of both antigen were smaller when the antigen from worms of the human host was used. At same time, the allergic patients condition gave papule bigger than the nonallergic patients. The authors admit that in a region with endemic schistosomiasis where could be a great number of individuals showing a bordeline size of papule, this fact has been demonstrated in the paper, as a result of anti-species antigen-anti body reaction.
  • Histoplasmose cutâneo-mucosa: relato de dois casos em irmãos Apresentação De Caso

    A comunicação versa sobre dois casos de histoplasmose cutâneo mucosa ocorrendo em dois irmãos. No primeiro doente as lesões cicatrizaram sob terapêutica específica: recidivando porém três anos após, ao nível do sistema nervoso central. Essas lesões levaram a doente ao óbito. No segundo doente o diagnóstico foi estabelecido mais precisa e precocemente e sob terapêutica específica houve remissão completa das lesões, estando o doente sob observação. Em ambos os casos havia, grande semelhança, do quadro clínico com o da blastomicose sul-americana (paracoccidiodomicose). No primeiro doente o diagnóstico foi estabelecido à necropsia e no segundo em vida, pelo encontro do Histoplasma capsulatum em cortes histológicos e pelo isolamento do fungo por cultura.

    Two cases of muco-cutaneous histoplasmosis in brothers are presented. The first case under specific treatment improved, quickly but had recidive with lecions in the nervous system, three years afterwards, leading to death. In the second cave, the diagnosis was performed earlier and under specific treatment clinical cure ocurred. The pacient is still under clinical control. In both cases, the clinical aspects of muco-cutaneous ristoplasmosis were similar to those of South American blastomycosis (paracoccidicdcmycosis). The firts diagnosis was post-mortem and the second one was by fungus isolation, in culture, and the presence of the Histoplasma capsulatum in the histological sections.
  • Estudo radiológico do pulmão durante o período exantemático do sarampo

    Os AA. procederam ao estudo radiológicc do tórax em duas incidências p.a. e perfil - em 69 casos de sarampo, durante o período exantemático. Verificaram .95,55% de enfísemas, 60,87% de pneumonias, 45,48% de broncopneumonias, 18,84% de atelectasias e 7.25% de empastamento hilar. Os AA. destacam a importância da radiografia em perfil, sobretudo para o diagnostico das pneumonias retrocardiacas, das alterações pulmonares localizadas nos segmentos posteriores das bases, assim como o diagnóstico diferencial entre atelectasia e pneumonia. Os vários tipos de enfisemas, de pneumonias e de atelectasias são comentados. Dentre os 69 pacientes, 30 foram internados com suspeita de complicação pulmonar, e 39, sem suspeita de complicação pulmonar, foram controlados em regime ambulatorial. Não se registrou diferença estatisticamente significativa, com relação à ocorrência de enfisema, pneumonia, atelectasia e de empastamento hílar, nos pacientes internados e não internados; por outro lado, comprovou-se maior incidência de broncopneumonia, em pacientes internados, nos grupos etários de 1/2 a 2 e 2 a 6 anos.

    The AA. proceeded to a radiological study of the chest in two position - postero-anterior and lateral - in 69 cases of measles, during the phase of disease. 98,55% of emphysemas, 60,87% of pneumonias, 45,48% of bronchopneumonias. 18,48% of atelectasia and 7,25% of hilar enlargment were found. It was stressed the importance of the lateral position in the chest X ray, for the diagnosis of retrocardiac pneumonias and other pulmonar alterations localized in posterior segments of the basis; this lateral position is also important in the diferential diagnosis between atelectasis and pneumonia. The various forms of emphysemas, pneumonias and atelectasis are discussed. Among the 69 patients examined, 30 were inpatients with suscepted pulmonary computations. Another 39 patients were seen as out patients, because the pulmonary complications were not suspected on clinical grounds. There was no statistical difference between the groups, regarding the occurence of emphysemas, pneumonias, atelectasis and hilar enlargment. On the ether side, it was observed that there was larger incidence of bronchopneumonias in the hospitalized patients, in the age group of 1/2 to 2 years and 2 to 6 years of age.
  • Tratamento de parasitoses intestinais pela associação piperazina-tiabendazol

    Os autores apresentam os resultados obtidos com o emprego da associação antihelmintica piperazina-tiabendazol no tratamento de infecção por Ascaris lumbricoides e ancilostomídeos. Considerando a cura de 81 % dos casos de ascaridíase e de 80% dos casos de ancilostomíase, bem como a ótima tolerância ao medicamento, acreditam que esta associação medicamentosa possa ser empregada com sucesso para o tratamento das referidas parasitoses.

    The authors tried piperazine plus thiabendazole for the treatment of Ascaris lumbricoides and hookworn infections. The drug was given for 2 consecutive days in a daily dose of 750 mg of piperazine hydrate and 375 mg of thiabendazole for 33 kg of body weight. Cure was obtained in 81% of those infected with A. lumbricoides and in 80% of hookworm infections. Tolerance to the drug was considered very good.
  • Inquérito clinico-nutricional e antropométrico, em áreas endêmicas de esquistossomose mansônica no nordeste do Brasil

    Os autores apresentam, os resultados de um inquérito clínico-nutricional e antropométrico realizado no período 1965 a 1967, em 11.283 indivíduos residentes nas zonas rural e urbana de três Estados do nordeste brasileiro (Alagcas, Rio Grande do Norte e Pernambuco), com o objetivo de estudar o estado nutricional de populações residentes em áreas endêmicas de Esquistossomose mansônica. Chamam a atenção para a freqüência relativamente baixa de manifestações clínicas de doenças carenciais, apesar de tratar-se de região cujos baixos padrões sociais e econômicos são bastante conhecidos, sugerindo que a maioria dos indivíduos deve apresentar deficiência nutricional apenas a nível bioquímico. Dentre os sinais de doença, carencial pesquisados, foram registrados com maior freqüência: estomatite angular, cicatrizes das comissuras labiais, atrofia das papilas linguais, gengivite, dermatite seborreica naso-labial e dermatite pelagrosa. De um modo geral, as manifestações clínicas de doença carencial pareciam incidir com maior freqüência entre os indivíduos parasitados pelo S. mansoni. embora não tenham sido encontrados resultados estatisticamente significativos. Os níveis de hemoglobina, pesquisados apenas em população rural do Estado de Pernambuco (Município de São Lourenço), foram muito baixos, não havendo, contudo, diferença estatisticamente significante entre crianças exibindo manifestações clínicas de carência duplamente parasitadas, e aquelas apenas portadoras de esquistossomose. Estudo antropométrico sumário, realizado no grupo etárío de 1 a 12 anos. e baseado na tomada de pese e altura, não revelou diferença estatisticamente significativa, entre crianças exibindo ou não doença carencial, e/ou ccm e sem Esquistossomose, contrariando achados de outros autores, e para cuja explicação pode ser invocado o pequeno número de formas graves (hepato-esplênicas) encontradas no campo, nesse grupo de idade. Os resultados do presente trabalho, por tratar-se de estudo preliminar, não permitem inferir conclusões definitivas, perém reforçam a suposição do provável papel desempenhado pela desnutrição na evolução da Esquistossomose mansônica, sugerindo a necessidade de continuação desses estudos, para uma melhor compreensão dos aspectos nutricionais das relações hospedeiro-parasito, nessa helmintose.

    Results are presented of a nutrition survey including clinical mid anthropometric measurements done from 1965 to 1967 which included 11.283 people from both urban and rural areas of three states in northeastern Brazil (Alagoas, Rio Grande do Norte and Pernambuco), since it was intended to study the nutritional status of people living in endemic areas of schistosomiasis (Schistosomiasis mansoni). A relatively low frequency of clinical signs of the deficiency diseases were noticed even though the low social and economic standards of these population is quite well known. It is therefore sugested that most of these people possibly have nutritional deficiencies at the metabolic level. The most frequent clinical signs noted, were: angular stomatitis (angular lesions), angular scars, filiform papillary atrophy of the tongue. marginal swelling of the gums, nasolabial seborrhea and pellagrous dermatitis. In general, the clinical signs seemed to be most common among people with S. mansoni, although no statistical significance was found. The hemoglobin levels, as determined on just the rural population (State of Pernambuco), showed very low values, particularly among the people showing clinical signs and infected with both S. mansoni and hookworm, as compared to those with only S. mansoni. Weight and height measurements for the age group 1 to 12 year old showed no estatistically significant results among children with and without deficiency diseases, and children with ami withcut schistosomiasis, which is in disagreement with other reported papers. However, the small percentage of severe (hepato-splenic) clinical forms of Schistosomiasis found in the field, in this age group, may account for the difrefence in findings. From the results presented in this preliminary study, no definitive conclusons can be drawn. However, the probable influence of undernutrition on the evolution of Schistosomiasis mansoni indicates the continuation of this line of research in order to obtain a better understanding of the nutritional aspects of the host-parasite relationships in this helminthosis.
