    Alcohol and cannabis misuse is currently the most frequent co-morbidity disorder of schizophrenia. The following four issues will be dealt with: 1) the neurobiological basis of the psychosis-inducing, pathogenic effects of THC, the agent contained in cannabis products. 2) Can cannabis use - and for comparison alcohol abuse - prematurely trigger or even cause schizophrenia? 3) Are persons genetically liable to schizophrenia, psychosis-prone individuals or young persons before completion of brain development at an increased risk? 4) What consequences does cannabis use have on the symptomatology and further course of schizophrenia? Results from recent literature and the ABC Schizophrenia Study show that the risk for cannabis use in schizophrenia is about twice the size in healthy controls. In most cases cannabis use starts before first admission, in a third of cases before schizophrenia onset. There is an increased affinity to misuse already at the prodromal stage. Cannabis can prematurely trigger schizophrenia onset - on average eight years earlier than in non-use - and cause the illness partly in interaction with predisposing factors. Cannabis use in the course of schizophrenia increases positive symptoms and reduces affective flattening, thus leading to dysfunctional coping in some cases.
  • A eficácia de treinamento de pais em grupo para pacientes com transtorno desafiador de oposição: um estudo piloto Original Article

    INTRODUÇÃO: O transtorno desafiador de oposição (TDO) é considerado uma condição de difícil tratamento. O objetivo deste estudo foi avaliar a eficácia de um programa em grupo de treinamento de pais (TP) na redução dos sintomas de TDO e transtorno de conduta (TC) em crianças brasileiras com TDO. MÉTODO: conduziu-se um estudo naturalístico em que se analisou dados dos pais de cinco pacientes com TDO que participaram de um grupo de TP. Os sintomas de TDO e TC foram avaliados antes e pelo menos um mês depois de iniciarem a participação no grupo. As medidas utilizadas foram escalas baseadas nos critérios da DSM-IV para TDO e TC. RESULTADOS: A maioria dos pacientes continuou preenchendo critérios para TDO, mas a gravidade dos seus sintomas de TDO diminuiu 48,75%. A diferença entre as médias na escala de gravidade de sintomas desafiadores-opositivo foi estatisticamente significativa (p = 0,031). O número de critérios de TC preenchido pelos pacientes também foi largamente reduzido. CONCLUSÃO: TP mostrou-se eficaz na melhoria de sintomas de TDO e TC em pacientes com TDO. TP pode ser uma opção terapêutica útil para o tratamento de TDO.

    BACKGROUND: Oppositional-defiant disorder (ODD) is considered a hard to treat condition. The aim of this study was to assess the effect of parent management training (PMT) on the symptoms of ODD and conduct disorder (CD) in Brazilian children with ODD. METHODS: We conducted a clinical evaluation in which data was analyzed from parents of five patients with ODD who participated in a PMT group. The ODD and CD symptoms were assessed before and at least a month after they started participating in the group. The outcome measures were rating scales based on the DSM-IV criteria for ODD and CD. RESULTS: Most patients continued to fulfill criteria for ODD, but the severity of their ODD symptoms was reduced 48,75%. The difference between the means on the severity scale of ODD symptoms was statistically significant (p= 0,031) The fulfillment of criteria for CD was largely diminished. CONCLUSIONS: PMT was effective for the reduction of ODD and CD symptoms in patients with ODD. PMT may represent a valuable therapeutic option for patients with ODD in different cultures.
  • Instrumentos de avaliação do transtorno de ansiedade social Revisão De Literatura

    O transtorno de ansiedade social (TAS) é um distúrbio de difícil diagnóstico pelos clínicos, o que estimula o estudo e desenvolvimento de instrumentos que favoreçam seu reconhecimento precoce e sistemático. Objetiva-se identificar na literatura indexada entre janeiro de 1999 e julho de 2004, artigos relativos a estudos psicométricos sobre instrumentos para avaliação do TAS. Procedeu-se à busca sistemática nos indexadores PsycoInfo, Lilacs e Medline, utilizando-se as palavras-chaves: social phobia scale, social phobia validity, social phobia reliability e social anxiety, identificando-se 26 artigos. Para a análise, os estudos foram agrupados em dois conjuntos: a) onze relativos à Liebowitz Social Anxiety Scale (LSAS), e b) quinze relativos a outras escalas já validadas ou em validação. Dos estudos com a LSAS, seis trabalharam com a versão hetero-aplicada e quatro com a auto-aplicada, tendo identificado qualidades psicométricas satisfatórias quanto à validade no idioma original (inglês), e outros idiomas: francês, hebraico, turco e espanhol. Do mesmo modo, bons índices de fidedignidade foram encontrados e quanto à análise fatorial, o modelo de quatro fatores foi considerado o mais adequado. Dos estudos com outras escalas, sete trabalharam com instrumentos já validados e oito com novas escalas, identificando valores satisfatórios quanto à validade e fidedignidade. As principais limitações verificadas nos estudos relacionaram-se à diversidade de padrão-ouro adotada, composição amostral com ausência de claros critérios de inclusão e exclusão, e pequeno número de estudos com amostras não-clínicas, dificultando o uso dos instrumentos para rastreamento na população geral Estudos futuros que visem aprimorar a validação de critério com base na análise fatorial e de cluster, parecem necessários e oportunos.

    Social anxiety disorder (SAD) is difficult to diagnose by clinicians, a fact that stimulates the study and development of instruments that will favor its early and systematic recognition. The objective of the present study was to identify in the literature indexed from 1999 to July 2004 articles related to psychometric studies on instruments for the assessment of SAD. A systematic search was performed in the PsychoInfo, Lilacs and Medline indexing services using the following key words: social phobia scale, social phobia validity, social phobia reliability and social anxiety scale, with 26 papers being identified. For analysis, the studies were divided into two groups: a) Eleven regarding the Liebowitz Social Anxiety Scale (LSAS) and b) Fifteen regarding other scales already validated or in the process of validation. Among the studies with the LSAS, six worked with the hetero-applied version and four with the self-applied version, identifying satisfactory psychometric qualities regarding validity in the original language (English) and in other languages such as French, Hebrew, Turkish and Spanish. In addition, good reliability indices were found, and regarding factorial analysis, the four-factor model was considered to be the most adequate. Among the studies using other scales, eight worked with instruments already validated and seven with new scales, identifying satisfactory values regarding validity and reliability. The main limitations pointed out in the studies surveyed were related to the diversity of the gold standard adopted; sample composition with the absence of clear inclusion and exclusion criteria, and the small number of studies on non-clinical samples, impairing the use of the instruments for general population screening. Future studies are clearly needed to improve the validation of the criterion based on factorial and cluster analysis.
  • Alterações cutâneas com ziprasidona Relato De Caso

    Os antipsicóticos podem estar associados a alterações cutâneas adversas. Relatamos o caso de uma paciente do sexo feminino, 47 anos, em acompanhamento ambulatorial com o diagnóstico de esquizofrenia. Estava em uso de ziprasidona há sete semanas. Prescrita inicialmente na dose de 80 mg ao dia, a medicação foi aumentada para 120 mg após seis semanas. Sete dias após o aumento da dose, a paciente apresentou quadro de prurido associado a lesões eritematosas em alvo, acometendo tronco, membros superiores e inferiores. Após a suspensão do antipsicótico, houve remissão do quadro dermatológico. Embora haja menção prévia a efeitos colaterais de natureza cutânea durante o uso desta medicação, não encontrou-se na literatura relatos de associação entre eritema multiforme e ziprasidona.

    There are descriptions of skin reactions secondary to antipsychotic use. We reported a case of erythema multiform associated with ziprasidone use in a 47-year-old female patient diagnosed with schizophrenia. The patient was receiving the medication (80 mg/day) for seven weeks, and the dose was increased to 120 mg/day. Seven days after the dose change, the patient presented pruritus and erythematosus lesions in trunk, upper and lower members. The medication was discontinued and there was a complete improvement of the lesions. We were unable to find previous reports of association between erythema multiform and ziprasidone.
  • Mutilação genital e psicose Relato De Caso

    A automutilação em pacientes psiquiátricos não é incomum, podendo estar relacionada a crenças religiosas, impulsividade, abuso de substâncias e sintomas psicóticos. Relatos de mutilação genital em pacientes esquizofrênicos são encontrados na literatura médica. Encontra-se aqui descrito um caso de automutilação genital em um paciente de 42 anos de idade, portador de esquizofrenia.

    Self-mutilation in psychiatric patients is not uncommon, and may result from religious beliefs, impulsivity, substance abuse and psychotic symptoms. Reports of genital mutilation among patients with schizophrenia can be found in medical literature. We described a case of genital self-mutilation in a 42-year-old schizophrenic patient.
