  • Cirurgia ortopédica em idosos: aspectos clínicos Artigos De Atualização

    A atenção a pacientes idosos submetidos à cirurgia ortopédica, particularmente os que necessitam de cirurgia de urgência, deve levar em conta a análise da capacidade física e de riscos específicos dos indivíduos idosos, na tentativa de reduzir riscos que, no entanto, permanecem elevados neste grupo. Apesar dos riscos, procedimentos desenvolvidos com prontidão têm efeito positivo na evolução destes pacientes. A atenção coordenada integrada por equipe clínica/geriátrica, ortopédica, anestésica, intensivistas e demais profissionais de saúde pode ser altamente benéfica neste grupo de pacientes.

    Care for elderly patients undergoing orthopedic surgery, particularly for those requiring emergency surgery, needs to take into account an analysis of physical capacity and risks specific to elderly individuals, in an attempt to reduce the risks. Nevertheless, these remain high in this group. Despite the risks, procedures developed promptly have a positive effect on these patients' evolution. Coordinated care, composed of teams of specialists within clinical medicine, geriatrics, orthopedics, anesthesiology and critical care, along with other healthcare professionals, may be highly beneficial for this group of patients.
  • Lesão muscular: fisiopatologia, diagnóstico, tratamento e apresentação clínica Artigos De Atualização

    O tecido muscular esquelético possui a maior massa do corpo humano, com 45% do peso total. As lesões musculares podem ser causadas por contusões, estiramentos ou lacerações. A atual classificação separa as lesões entre leve, moderada e grave. Os sinais e sintomas das lesões grau I são edema e desconforto; grau II, perda de função, gap e equimose eventual; grau III, rotura completa, dor intensa e hematoma extenso. O diagnóstico pode ser confirmado por: ultrassom - dinâmico, barato, porém examinador-dependente; tomografia ou ressonância magnética - maior definição anatômica, porém estático. A fase inicial do tratamento se resume ao protocolo PRICE. AINH, ultrassom terapêutico, fortalecimento e alongamento após a fase inicial e amplitudes de movimento sem dor são utilizados no tratamento clínico. Já o cirúrgico possui indicações precisas: drenagem do hematoma, reinserção e reforço musculotendíneos.

    Skeletal muscle tissue has the largest mass in the human body, accounting for 45% of the total weight. Muscle injuries can be caused by bruising, stretching or laceration. The current classification divides such injuries into mild, moderate and severe. The signs and symptoms of grade I lesions are edema and discomfort; grade II, loss of function, gaps and possible ecchymosis; and grade III, complete rupture, severe pain and extensive hematoma. The diagnosis can be confirmed by: ultrasound, which is dynamic and cheap, but examiner dependent; and tomography or magnetic resonance, which gives better anatomical definition, but is static. Initial phase of the treatment can be summarized as the "PRICE" protocol. NSAIDs, ultrasound therapy, strengthening and stretching after the initial phase and range of motion without pain are used in clinical treatment. On the other hand, surgery has precise indications: hematoma drainage and muscle-tendon reinsertion and reinforcement.
  • Qual o melhor questionário para avaliar os aspectos físicos de pacientes com osteoartrite no joelho na população brasileira? Artigos Originais

    OBJETIVO: Mensurar a validade e a confiabilidade dos questionários WOMAC, IKDC, Lysholm em pacientes com osteoartrite no joelho e determinar a influência da idade no escore destes. MÉTODO: Cinquenta e sete pacientes com diagnóstico de OA primária de joelho completaram os questionários SF-36, WOMAC, Lysholm e IKDC. A validade foi testada mensurando a correlação (coeficiente de correlação de Pearson, "r") entre os questionários. A consistência interna foi mensurada através do α de Cronbach e a concordância através das representações gráficas de Altman-Bland e concordância- sobrevivência. Para determinar a influência da idade nos resultados correlacionamos esta com os escores dos três questionários de joelho através do coeficiente de determinação de Pearson (r²). RESULTADOS: O IKDC (0,62) e o WOMAC (0,642) apresentaram correlações moderadas para forte em relação ao resumo das capacidades físicas do SF-36, enquanto que o Lysholm apresentou correlações moderadas (0,555). O α de Cronbach apresentou valores de 0,811 para o IKDC, 0,959 para o WOMAC e 0,734 para o Lysholm. Apesar da forte correlação entre WOMAC e IKDC (0,843), WOMAC e Lysholm (0,759) e IKDC e Lysholm (0,858), as representações gráficas de Altman-Bland e concordância-sobrevivência demonstram que a concordância entre os três questionários é baixa. O IKDC, Lysholm e WOMAC apresentaram um coeficiente de determinação de Pearson (r²) de 0,004, 0,010 e 0,043 com a idade, respectivamente. CONCLUSÃO: A idade não demonstrou ser fator limitante à utilização de nenhum dos questionários aplicados neste estudo. Os testes de concordância e das correlações com os componentes físicos do SF-36 sugerem que o WOMAC é mais adequado para avaliar as capacidades funcionais e limitações relacionadas aos aspectos físicos, enquanto que o IKDC parece ser mais adequado para avaliar as limitações funcionais relacionadas à dor.

    OBJECTIVE: To measure the validity and reliability of the WOMAC, IKDC and Lysholm questionnaires among patients with knee osteoarthritis and determine the influence of age on their scores. METHOD: Fifty-seven patients diagnosed with primary osteoarthritis of the knee answered the SF-36, WOMAC, IKDC and Lysholm questionnaires. Validity was tested by measuring the correlation (Pearson's correlation coefficient, "r") between the questionnaires. Internal consistency was measured using Cronbach's α and concordance was measured through the Altman-Bland graphical representations and survival concordance. To determine the influence of age on the results, we correlated this variable with the scores from the three knee questionnaires using Pearson's coefficient of determination ("r2"). RESULTS: IKDC (0.62) and WOMAC (0642) showed moderate to strong correlations in relation to the summarized physical capacity score of the SF-36, while Lysholm showed moderate correlations (0.555). The Cronbach α values for IKDC, WOMAC and Lysholm were 0.811, 0.959 and 0.734, respectively. Despite the strong correlations between IKDC and WOMAC (0.843), Lysholm and WOMAC (0.759) and IKDC and Lysholm (0.858), the Bland-Altman graphical representations and survival concordance showed that the concordance between the three questionnaires was low. IKDC, Lysholm and WOMAC showed coefficients of determination (r2) with age of 0.004, 0.010 and 0.043, respectively. CONCLUSION: Age was not found to be a limiting factor for the use of any of the questionnaires applied in this study. The concordance tests and correlations with the physical components of SF-36 suggest that WOMAC is more appropriate for assessing physical capacities and limitations relating to physical traits, while IKDC seems more appropriate for assessing the functional limitations relating to pain.
  • Reconstrução anatômica do LCA com duplo feixe: primeiros 40 casos Artigos Originais

    OBJETIVO: Discutir a técnica de reconstrução anatômica do ligamento cruzado anterior (LCA) com duplo feixe, a curva de aprendizado e os resultados preliminares. MÉTODOS: Quarenta pacientes com lesão do LCA foram submetidos à reconstrução anatômica com duplo feixe, utilizando-se do tendão semitendinoso para refazer a banda anteromedial (AM) e gracilis para refazer a banda posterolateral (PL) do joelho. RESULTADOS: Tivemos dois casos de limitação de extensão, sendo que em um deles foi necessária a realização de artrólise artroscópica, e um caso de trombose venosa profunda. CONCLUSÃO: A reconstrução do LCA com duplo feixe se mostrou factível, apesar de apresentar uma maior curva de aprendizado; e as vantagens da técnica proposta ainda deverão ser comprovadas se comparada com a técnica de feixe único.

    OBJECTIVE: To discuss the technique of anatomical reconstruction of the anterior cruciate ligament (ACL) with double beam, the learning curve and preliminary results. METHODS: Forty patients with ACL injury underwent reconstruction with anatomical double-bundle, using the semitendinosus tendon to remake the band anterior medial (AM) and gracile to remake the band posterior lateral (PL) of the knee. RESULTS: We had two cases of limitation of extension, and in one of them were necessary to perform arthroscopic artrolise, and one case of deep vein thrombosis. CONCLUSION: ACL reconstruction with double bundle proved feasible, despite having a higher learning curve, and the advantages of the proposed technique still must be proven compared to the single-beam technique.
  • Avaliação da reprodutibilidade das classificações de Outerbridge e da SFA para lesões condrais do joelho Artigos Originais

    OBJETIVO: Avaliar a reprodutibilidade da classificação de Outerbridge e da Sociedade Francesa de Artroscopia entre diferentes observadores e estabelecer uma comparação entre elas. MÉTODO: Foram utilizados 30 vídeos de artroscopia de joelho selecionados aleatoriamente demonstrando lesões condrais que foram classificadas por seis observadores, dois residentes em ortopedia do terceiro ano e quatro ortopedistas, entre os quais dois especialistas em cirurgia de joelho. A avaliação da reprodutibilidade intra e interobservador foi feita através do índice estatístico de Kappa. RESULTADOS: Como resultado da avaliação completa da classificação de Outerbridge com a totalidade dos observadores, encontramos um índice Kappa de 0,434411. Quanto à classificação proposta pela Sociedade Francesa de Artroscopia, encontramos um índice Kappa de 0,45166. CONCLUSÃO: A classificação de Outerbridge e da Sociedade Francesa de Artroscopia para lesões condrais é moderadamente reprodutível entre observadores. Comparando as duas classificações, a proposta pela Sociedade Francesa de Artroscopia se mostrou mais reprodutível, e os autores sugerem o uso dessa classificação como de escolha para a prática clínica da avaliação das lesões condrais do joelho.

    OBJECTIVE: To assess the reproducibility of the Outerbridge and the French Society of Arthroscopy classifications between different observers, and to establish a comparison between them. METHOD: Thirty videos on randomly selected knee arthroscopy procedures demonstrating chondral lesions were used. These were classified by six observers: two third-year orthopedics residents and four orthopedic surgeons, of whom two were knee surgery specialists. The intraobserver and interobserver reliability was evaluated by means of the kappa index. RESULTS: The result from the complete evaluation on the Outerbridge classification with all the observers gave a kappa index of 0.434411. For the classification proposed by the French Society of Arthroscopy, the kappa index was 0.45166. CONCLUSION: The Outerbridge and French Society of Arthroscopy classifications for chondral lesions are moderately reproducible between observers. Comparing the two classifications, the proposal from the French Society of Arthroscopy was shown to be more reproducible, and the authors suggest that this classification should be used preferentially in clinical practice for evaluations on chondral lesions of the knee.
  • Análise radiológica do posicionamento dos túneis ósseos na cirurgia de reconstrução do ligamento cruzado anterior: comparação entre as técnicas aberta e artroscópica via portal anteromedial Artigos Originais

    OBJETIVO: Avaliar e comparar o posicionamento dos túneis ósseos na cirurgia de reconstrução do ligamento cruzado anterior (LCA) pela técnica artroscópica e pela técnica aberta através da artrotomia. MÉTODO: Foi realizado um estudo retrospectivo comparativo de 70 pacientes acometidos por lesão do LCA. Trinta e cinco pacientes foram submetidos à reconstrução do LCA por via aberta e 35 por via artroscópica utilizando a técnica via portal anteromedial. Todos os pacientes foram submetidos à reconstrução do LCA usando enxerto autólogo do terço central do tendão patelar fixado com parafusos de interferência. As radiografias pós-operatórias foram revisadas e o posicionamento do túnel femoral foi avaliado pelos métodos propostos por Harner e Aglietti, enquanto o túnel tibial foi avaliado pelo método proposto por Staubli e Rauschning. RESULTADOS: Foram observados 54 pacientes do sexo masculino e 16 do sexo feminino. A média de idade na ocasião do procedimento foi de 34 anos e três meses, variando de 17 a 58 anos. A técnica artroscópica mostrou-se mais precisa que a técnica aberta quanto ao posicionamento dos túneis ósseos, tanto femoral quanto tibial. CONCLUSÃO: Através da análise radiológica dos joelhos submetidos à reconstrução do LCA, observou-se que o posicionamento dos túneis ósseos, tanto femoral quanto tibial, apresenta menor variação quando a cirurgia é feita pela via artroscópica.

    OBJECTIVES: To evaluate and compare bone tunnel positioning in anterior cruciate ligament (ACL) reconstruction surgery using the arthroscopic technique and the open technique consisting of arthrotomy. METHOD: A comparative retrospective study on 70 patients with ACL lesions was conducted. Thirty-five patients underwent ACL reconstruction by means of the open technique and 35 by means of the arthroscopic technique using an anteromedial portal. All the patients underwent ACL reconstruction using an autologous graft from the middle third of the patellar tendon, fixed using interference screws. The postoperative radiographs were reviewed and the positioning of the femoral tunnel was evaluated using the methods proposed by Harner and Aglietti, while the tibial tunnel was assessed using the method proposed by Rauschning and Staubli. RESULTS: Fifty-four of the patients were male and 16 were female. Their mean age at the time of the procedure was 34 years and 3 months, with a range from 17 to 58 years. The arthroscopic technique was shown to be more accurate than the open technique for positioning both the femoral and the tibial bone tunnels. CONCLUSIONS: Radiological analysis on the knees subjected to ACL reconstruction showed that the positioning of both the femoral and the tibial bone tunnels presented less variation when the surgery was performed arthroscopically.
  • Avaliação dos resultados do reparo artroscópico de lesões do manguito rotador em pacientes com até 50 anos de idade Artigos Originais

    OBJETIVO: Avaliar o resultado do tratamento cirúrgico das lesões do manguito rotador (LMR) por via artroscópica nos pacientes com até 50 anos de idade. MÉTODOS: Entre agosto de 1998 e dezembro de 2007 foram reavaliados 63 pacientes com LMR submetidos a este tratamento pelo Grupo de Ombro e Cotovelo do Departamento de Ortopedia e Traumatologia da Faculdade de Ciências Médica da Santa Casa de São Paulo - "Pavilhão Fernandinho Simonsen". Foram incluídos no estudo todos os pacientes com LMR com idade até 50 anos e com seguimento mínimo pós-operatório de 24 meses. RESULTADOS: Pelos critérios de avaliação da UCLA, 59 (92%) pacientes tiveram resultados excelentes e bons; cinco (8%), regulares; e nenhum ruim. A amplitude média dos movimentos na avaliação pós-operatória foi de 145˚ na elevação, 47˚ na rotação lateral e de T10 na rotação medial. Os resultados insatisfatórios estiveram associados com o tempo prolongado da lesão, demonstrando uma relação estatisticamente significante. CONCLUSÃO: O reparo artroscópico da LMR nos pacientes jovens traz resultados bons e excelentes na maioria dos pacientes.

    OBJECTIVE: To assess the results from arthroscopic surgical treatment of rotator cuff injuries among patients under 50 years of age. METHODS: Sixty-three patients with rotator cuff injuries who underwent arthroscopic surgical treatment performed by the Shoulder and Elbow Group of the Department of Orthopedics and Traumatology, in the Fernandinho Simonsen wing of Santa Casa Medical School, São Paulo, between August 1998 and December 2007, were reassessed. The study included all patients with rotator cuff injuries who were under 50 years of age and had been followed up postoperatively for at least 24 months. RESULTS: According to the UCLA evaluation criteria, 59 patients (92%) showed excellent and good results; five (8%) showed fair results; and none showed poor results. The postoperative evaluation showed that the mean range of motion was 145˚ for elevation, 47˚ for lateral rotation and T10 for medial rotation. Unsatisfactory results were associated with prolonged duration of the injury, with a statistically significant relationship. CONCLUSION: Arthroscopic repair of rotator cuff injuries in young patients produces excellent or good results for most patients.
  • Prevalência das lesões associadas na luxação recidivante traumática do ombro Artigos Originais

    OBJETIVO: Avaliar a prevalência das lesões associadas à instabilidade anterior traumática do ombro e a relação entre o número de episódios e o tempo do início dos sintomas com a prevalência destas lesões. MÉTODO: Foram selecionados 57 pacientes com instabilidade anterior traumática do ombro, entre 18 e 40 anos, com mais de um episódio de luxação do ombro e com no mínimo, seis meses da primeira luxação, que necessitaram de cirurgia para tratamento da instabilidade. Foi realizada inspeção artroscópica em todos os pacientes para avaliação das lesões associadas. RESULTADOS: Foi avaliada a prevalência das lesões, sendo a lesão de Bankart a mais prevalente seguida pela lesão de Hill-Sachs e as lesões do manguito rotador as menos prevalentes. Não houve correlação comparando o número de episódios de luxação com a prevalência de lesões associadas. Já em relação ao tempo de sintomas, os pacientes com maior tempo de sintomas tiveram menos lesão de Hill-Sachs. CONCLUSÃO: Não foi possível afirmar que, em pacientes com instabilidade crônica do ombro, as lesões associadas aumentam com o tempo de sintomas ou com o número de episódios de luxação.

    OBJECTIVE: To evaluate the prevalence of lesions associated with traumatic anterior shoulder instability and the relationships between the prevalence of these lesions and the number of episodes and time since symptoms started. METHOD: Fifty-seven patients aged 18 to 40 years, with traumatic anterior shoulder instability, more than one episode of shoulder dislocation and at least six months since the first dislocation, who required surgery to treat the instability, were selected. Arthroscopic inspection was performed on all the patients to assess any associated lesions. RESULTS: The prevalence of lesions was assessed, and Bankert lesions were the most prevalent, followed by Hill-Sachs lesions, while rotator cuff injuries were the least prevalent. There was no correlation from comparison between the number of episodes of dislocation and the prevalence of associated lesions. On the other hand, in relation to the time since symptoms started, the patients who had had symptoms for longer times had fewer Hill-Sachs lesions. CONCLUSION: It was not possible to affirm that, in patients with chronic shoulder instability, the numbers of associated lesions increased with the time since symptoms started, or with the number of episodes of dislocation.
  • Artroplastia de superfície no tratamento da osteoartrose primária e/ou secundária do ombro pelo sistema HemiCAP-Arthrosurface® Artigos Originais

    OBJETIVO: Apresentar a técnica cirúrgica pelo sistema HemiCAP-Arthrosurface® e a avaliação dos resultados com esta técnica no tratamento da osteoartrose primária e/ou secundária do ombro. MÉTODO: Entre junho/2007 e junho/2009, 10 ombros de 10 pacientes, sendo nove com osteoartrose primária e um com necrose avascular da cabeça umeral, foram submetidos à artroplastia de superfície pelo sistema HemiCAP-Arthrosurface para correção do problema. O tempo de seguimento variou de seis a 29 meses, com média de 17 meses. A idade variou de 62 a 73 anos com média de 67,5 anos. Seis pacientes eram do sexo feminino e quatro do sexo masculino. Os pacientes foram acompanhados semanalmente no primeiro mês e a cada três meses após o procedimento cirúrgico. A avaliação clínica foi feita por meio dos critérios da University of Califórnia at Los Angeles (UCLA) e escala analógica de dor. RESULTADOS: Todos os pacientes operados encontravam-se satisfeitos com os resultados do tratamento cirúrgico, com uma média de 30 pontos e dois pontos nas escalas de avaliação da UCLA e analógica de dor, respectivamente. CONCLUSÃO: O sistema HemiCAP-Arthrosurface® utilizado na cirurgia do ombro para um grupo específico de pacientes é uma técnica cirúrgica que preserva o estoque ósseo com bons resultados funcionais e antálgicos.

    OBJECTIVE: To present the surgical technique for the HemiCAP-Arthrosurface® system and evaluate our results from this technique for treating primary and/or secondary shoulder osteoarthrosis. METHOD: Between June 2007 and June 2009, 10 shoulders of 10 patients (nine with primary osteoarthrosis and one with avascular necrosis of the humeral head) underwent surface arthroplasty using the HemiCAP-Arthrosurface® system to correct the problem. The follow-up time ranged from six to 29 months (mean of 17 months). The patients' ages ranged from 62 to 73 years (mean of 67.5 years). Six patients were female and four patients were male. The patients were followed up weekly for the first month after the surgical procedure and every three months thereafter. The clinic evaluation was done using the criteria of the University of California at Los Angeles (UCLA) and a visual analogue pain scale. RESULTS: All the patients said that they were satisfied with the results from the surgical treatment, with a mean UCLA score of 30 points and a mean analogue pain score of two points. CONCLUSION: The HemiCAP-Arthrosurface® system for shoulder surgery for a specific group of patients is a technique that preserves the bone stock with good functional and antalgic results.
  • Alterações radiográficas do tálus no pé torto congênito após liberação cirúrgica pela técnica de McKay Artigos Originais

    OBJETIVO: Analisar as alterações morfológicas do tálus após o tratamento cirúrgico do pé torto congênito pela técnica de McKay. MÉTODO: Foram analisadas, retrospectivamente, radiografias em perfil com carga dos pés de 14 pacientes com pé torto congênito unilateral submetidos ao tratamento pela técnica de McKay por dupla incisão. Todos os pacientes foram operados pelo mesmo cirurgião, com média de 6,53 anos entre a cirurgia e a radiografia. Comparamos as características do tálus dos pés operados com os parâmetros radiográficos dos pés contralaterais. Avaliamos a presença de deformidade do dômus e da cabeça do tálus (avaliação da esfericidade); a altura e o comprimento do tálus; a presença e grau de subluxação do navicular; a alteração do ângulo de Gissane; e o padrão do trabeculado ósseo. RESULTADOS: Alterações da cabeça do tálus ocorreram em 92,8% dos casos; do dômus em 92,8%; e do trabeculado em 100%. A relação entre o comprimento do tálus do pé operado sobre o contralateral variou de 0,61 a 0,88 (média de 0,79; DP = 0,09), e da altura de 0,57 a 0,98 (média de 0,82; DP = 0,12). O ângulo de Gissane aumentou em todos os pés operados, e todos apresentaram subluxação do navicular, com índice variando de 6,43 a 59,75% (média de 26,34%; DP = 16,66%). CONCLUSÃO: Alterações talares ocorreram em 100% dos pés tratados pela técnica de McKay. Estabelecer parâmetros radiográficos para descrever e quantificar essas deformidades mostrou-se viável, através de técnicas simples e de fácil execução.

    OBJECTIVE: To analyze morphological abnormalities of the talus in patients with clubfoot after surgical treatment using the McKay technique. METHOD: Lateral standing-position radiographs of the feet of 14 patients with unilateral clubfoot who underwent treatment by means of the double-incision McKay technique were retrospectively analyzed. All the patients were operated by the same surgeon, with an average of 6.53 years between surgery and the radiograph. We compared the radiographic characteristics of the talus between the operated and the contralateral foot. We assessed the presence of deformity of the talar dome and head (sphericity evaluation); the talar length and height; the percentage and degree of navicular subluxation; abnormalities of the Gissane angle; and the trabecular bone pattern. RESULTS: Abnormalities of the talar head occurred in 92.8% of the patients; of the talar dome in 92.8%; and of the trabecular pattern in 100%. The talar length ratio between the operated and the contralateral foot ranged from 0.61 to 0.88 (mean 0.79; SD = 0.09), while the height ratio ranged from 0.57 to 0.98 (mean 0.82; SD = 0.12). The Gissane angle was greater in all of the operated feet, and all of them also showed navicular subluxation, at a rate ranging from 6.43 to 59.75% (mean 26.34%; SD = 16.66%). CONCLUSION: Talar abnormalities occurred in 100% of the feet treated using the McKay technique. It was shown that establishing radiographic parameters to describe and quantify these deformities was feasible, through simple and easy-to-perform techniques.
  • Avaliação do tratamento cirúrgico das fraturas da coluna toracolombar com material de terceira geração tipo fixador interno Artigos Originais

    OBJETIVO: Avaliar o resultado funcional dos pacientes com fratura da coluna toracolombar cirúrgica. MÉTODO: Foi feito um estudo prospectivo incluindo 100 pacientes portadores de fratura da coluna vertebral nos segmentos torácico e lombar. As lesões foram classificadas conforme a sistemática da AO e os pacientes foram tratados com cirurgia. Avaliou-se a presença de cifose inicial e sua evolução após a intervenção cirúrgica, a presença de dor pós-operatória e sua evolução até 24 semanas do ato cirúrgico. Comparando nossos dados com a literatura. RESULTADOS: Analisados 100 pacientes cirúrgicos, sendo 37 do tipo A, 46 do tipo B e 17 do tipo C, observamos que os pacientes que se apresentavam com Frankel A mantiveram o quadro, porém, os pacientes com Frankel B ou mais, evoluíram com alguma melhora do quadro; a média da melhora da dor baseada na escala visual analógica (EVA) foi acima de 4 pontos, e o retorno às atividades de rotina diária constatado em todos os pacientes, sendo que o retorno ao trabalho não foi considerado por nós como critério de avaliação. CONCLUSÃO: Apesar da controvérsia quanto à indicação da cirurgia nas fraturas da coluna, consideramos o método por nós utilizado como satisfatório, com bons resultados e baixo índice de complicações, porém mais estudos prospectivos e randomizados, com um seguimento mais longo, são necessários para uma avaliação deste tipo de fixação.

    OBJECTIVE: To evaluate the functional results from patients with surgical fractures in the thoracolumbar spine. METHOD: A prospective study including 100 patients with spinal fractures in the thoracic and lumbar segments was conducted. The lesions were classified in accordance with the AO system, and the patients were treated surgically. The presence of early kyphosis and its evolution after the surgical intervention, and the presence of postoperative pain and its evolution up to the 24th week after the surgery, were evaluated. We compared our data with the literature. RESULTS: One hundred surgical patients were analyzed, of which 37 were type A, 46 were type B and 17 were Type C. Patients who presented Frankel A kept their clinical status, but patients with Frankel B or higher evolved with some improvement. The average improvement in pain based on a visual analog scale was more than four points. All the patients were able to return to their daily routine activities, although we did not take the return to work to be an assessment criterion. CONCLUSION: Despite controversy regarding the indications for surgery in cases of fractured spine, we believe that the method that we used was satisfactory because of the good results and low complication rate. However, more randomized prospective studies with longer follow-up are needed in order to evaluate this type of fixation.
  • Efeito tardio e imediato da sutura em tendão extrassinovial íntegro: estudo biomecânico em ratos Artigos Originais

    OBJETIVO: O objetivo do estudo foi avaliar o efeito, nas propriedades mecânicas, do arranjo de reparo com material de sutura, no tendão calcâneo de rato, na ausência de processo de cicatrização. MÉTODO: Foram utilizados 12 ratos machos da linhagem Wistar. Os animais foram submetidos à colocação de ponto de sutura tipo Kessler modificado no tendão calcâneo. Os sacrifícios foram realizados nos períodos imediato e com seis semanas de pós-operatório. As propriedades mecânicas estudadas foram: carga máxima, tensão na carga máxima e módulo de elasticidade. O tendão contralateral foi usado como controle. RESULTADOS: A análise estatística indicou que, em relação aos períodos estudados, os valores das propriedades mecânicas não apresentaram diferenças significativas. Em relação ao controle, isto é, tendão contralateral sem sutura, os resultados demonstram que, com seis semanas de pós-operatório, houve queda dos valores do módulo de elasticidade, enquanto a carga máxima e a tensão na carga máxima não apresentaram variações significativas. CONCLUSÃO: A colocação de material de sutura em tendão extrassinovial não lesado provocou queda do módulo de elasticidade, porém não interferiu na carga máxima e na tensão na carga máxima após seis semanas de pós-operatório.

    OBJECTIVE: The aim of this study was to evaluate the effects on the mechanical properties of rats' calcaneus tendons, of repair arrangements using suture material in the absence of any healing process. METHOD: Twelve male Wistar rats were used. They were subjected to placement of a modified Kessler suture stitch in the calcaneus tendon. The sacrifices were performed immediately after and six weeks after the operation. The mechanical properties studied were maximum load, tension in the maximum load and module of elasticity. The contralateral tendon was used as a control. RESULTS: The statistical analysis showed that for the times studied, the values for mechanical properties did not present any significant differences. In relation to the control, i.e. the contralateral tendon without a suture, the results demonstrated that, six weeks after the operation, the values for the modulus of elasticity were lower, whereas there were no significant variations in maximum load or tension at maximum load. CONCLUSION: Placement of suture material on an extrasynovial tendon without lesions decreased the modulus of elasticity, but it did not interfere with the maximum load or tension at maximum load, six weeks after the operation.
  • Estudo anatômico comparativo entre os lados direito e esquerdo do nervo axilar em relação à via deltopeitoral e ao acrômio Artigos Originais

    OBJETIVOS: Estabelecer parâmetros anatômicos para o nervo axilar pelas medidas das distâncias ao acrômio e à via de acesso deltopeitoral, e verificar se há diferenças nas medidas comparativas entre os lados direito e esquerdo. MÉTODO: Realizou-se o estudo anatômico do trajeto do nervo axilar pela dissecção de 30 ombros em 20 cadáveres adultos frescos. Em 10 cadáveres foi realizada dissecção bilateral para estudo comparativo. Utilizou-se paquímetro digital com precisão de 0,05cm, mediram-se as distâncias entre a extremidade lateral do acrômio e os ramos anterior e posterior do nervo axilar e a distância entre o espaço deltopeitoral e o ramo anterior do nervo axilar. RESULTADOS: A menor distância entre o acrômio e o nervo axilar foi de 5,47cm e a maior, de 7,06cm. A menor distância entre o sulco deltopeitoral e o nervo axilar foi de 3,94cm. Houve diferença com significância estatística pelo teste de Wilcoxon nas medidas comparativas entre os lados direito e esquerdo, para as distâncias entre o ramo posterior do nervo axilar e o ponto médio da borda lateral do acrômio (A-E) e entre o ramo anterior do nervo axilar e a extremidade anterior do acrômio (B-C), ambas maiores no lado direito. CONCLUSÕES: O nervo axilar está situado entre 5,47 e 7,06cm distal ao acrômio e 3,94cm lateral ao espaço deltopeitoral. Houve diferença com significância estatística no estudo comparativo entre os lados direito e esquerdo, ambas maiores no lado direito.

    OBJECTIVE: To establish anatomical parameters for the axillary nerve by measuring the distances to the acromion and the deltopectoral access, and to ascertain whether there are any differences in comparative measurements between the left and right sides. METHOD: An anatomical study on the path of the axillary nerve was conducted by dissecting 30 shoulders of 20 fresh adult cadavers. For comparative study, bilateral dissection was performed on 10 cadavers. Digital caliper gauges, accurate to the nearest 0.05 cm, were used to measure the distances between the lateral extremity of the acromion and the anterior and posterior branches of the axillary nerve, and between the deltopectoral space and the anterior branch of the axillary nerve. RESULTS: The shortest distance between the acromion and the axillary nerve was 5.47 cm, and the greatest distance was 7.06 cm. The shortest distance between the deltopectoral groove and the axillary nerve was 3.94 cm. A statistically significant difference was found using Wilcoxon's test in comparative measurements between the left and right sides for the distances between the posterior branch of the axillary nerve and the midpoint of the lateral border of the acromion (A-E), and between the anterior branch of the axillary nerve and the anterior extremity of the acromion (B-C), both of which were larger on the right side. CONCLUSIONS: The axillary nerve was situated between 5.47 and 7.06 cm distally to the acromion, and 3.94 cm laterally to the deltopectoral space. There was a statistically significant difference in the comparison between the left and right sides, and both measurements were larger on the right side.
  • Fratura patológica de vértebra lombar em criança com déficit neurológico agudo: relato de caso Relato De Caso

    O trabalho relata um caso de linfoma em paciente de 13 anos que veio à consulta com dor lombar. O paciente sofreu um traumatismo de baixa intensidade na região lombar que resultou em dor persistente e de caráter progressivo. Ao ser avaliado em uma emergência, foi detectada no exame radiográfico uma fratura de coluna. Nesse momento, o paciente foi encaminhado ao ambulatório de especialidade do mesmo hospital. Após exames iniciais, foi firmado o diagnóstico de fratura patológica envolvido por volumoso tecido, indicando ser uma lesão tumoral. Na sequência do atendimento, o paciente evoluiu com parestesias em membros inferiores e retenção urinária sem possuir diagnóstico patológico da lesão. O paciente foi submetido à cirurgia de emergência com estabilização e descompressão neurológica sendo enviado material da lesão para exame anatomopatológico. O resultado do exame anatomopatológico sugeriu tumor de células pequenas deixando uma dúvida após a imuno-histoquímica, definindo o diagnóstico de linfoma. O paciente foi encaminhado para tratamento oncológico. O objetivo deste trabalho é relatar um caso raro de linfoma em criança com diagnóstico inicial de fratura patológica em coluna vertebral lombar. É importante investigar fratura associada a trauma leve em crianças. A precisão diagnóstica resulta em um atendimento efetivo com resultados melhores para os pacientes. O paciente em foco submeteu-se a tratamento quimioterápico obtendo uma boa resposta com reflexos positivos no seu prognóstico.

    This study reports on a case of lymphoma in a 13-year-old patient who came to a consultation with lumbar pain. The patient had suffered low-intensity trauma in the lumbar region that resulted in persistent pain of progressive nature. In an emergency evaluation, radiographic examination showed a spinal fracture. The patient was then sent to the specialist outpatient clinic of the same hospital. The initial examinations confirmed the diagnosis of a pathological fracture surrounded by a tissue mass, thus indicating the presence of a tumor. Subsequently, the patient evolved with lower-limb paresthesia and urine retention, without any pathological diagnosis for the lesion. The patient then underwent emergency surgery to achieve stabilization and neurological decompression, and material from the lesion was sent for anatomopathological examination. The result from the anatomopathological examination suggested that the lesion was a small-cell tumor, although leaving some doubt. Immunohistochemistry defined the diagnosis of lymphoma. The patient was then sent for oncological treatment. The aim of this study was to report on a rare case of lymphoma in a child with an initial diagnosis of a pathological fracture in the lumbar spine. It is important to investigate fractures associated with mild trauma in children. Precise diagnosis results in effective attendance with better results for these patients. This patient underwent chemotherapy and achieved a good response, with positive repercussions for his prognosis.
  • Luxação glenoumeral anterior bilateral: caso clínico Relato De Caso

    A luxação glenoumeral anterior bilateral é uma ocorrência rara. Apresentamos um caso de luxação glenoumeral anterior bilateral com origem após uma queda da própria altura. O interesse desta publicação reside no fato de se tratar de uma raridade clínica com poucos casos descritos na literatura. Paciente do sexo feminino com 89 anos recorre ao serviço de urgência (SU) após queda referindo dor intensa e incapacidade de mobilização de ambos os ombros. Ao exame objetivo apresentava sinais clínicos suspeitos de luxação glenoumeral anterior bilateral confirmados por radiografia. Ambas as luxações foram reduzidas no SU pela técnica de Milch modificada, com sucesso. Quando existe uma força simétrica e síncrona sobre os ombros e estes se apresentarem dolorosos e com limite funcional significativo, a suspeita de luxação glenoumeral bilateral, embora rara, é um diagnóstico diferencial a ter em conta.

    Bilateral anterior glenohumeral dislocation is a rare occurrence. We present a case of bilateral anterior glenohumeral dislocation caused by a fall. The interest in publishing this case is that this is a clinical rarity with few cases reported in the literature. An 89-year-old female patient was brought to the emergency department after a fall, complaining of intense pain in both shoulders and inability to move them. Objective examination showed clinical signs giving the suspicion of bilateral anterior glenohumeral dislocation, which was confirmed by x-ray imaging. Both dislocations were successfully reduced in the emergency department using the modified Milch technique. When a synchronous and symmetrical force has acted on both shoulders and these are painful with significant functional limitation, the suspicion of bilateral glenohumeral dislocation is a differential diagnosis to be considered, even though it is rare.
  • Indicações de artroplastia radiocubital distal: relato de três casos clínicos Relato De Caso

    A artroplastia radiocubital distal é uma solução atrativa no tratamento de diferentes patologias da articulação radiocubital distal, pois permite restaurar a estabilidade, a transmissão de cargas e a função. As principais indicações são: o impingement radiocubital após ressecção parcial ou completa do cúbito distal e a artrite degenerativa, inflamatória ou pós-traumática da articulação radiocubital distal. Os autores apresentam três casos clínicos de patologia radiocubital distal: dois pacientes com sequelas pós-traumáticas e um caso de impingement radiocubital distal após operação de Sauvé-Kapandji. Os três casos foram tratados cirurgicamente com prótese metálica de substituição do cúbito distal tipo First Choice - Ascencion®. Os dois primeiros foram tratados com prótese tipo resurfacing e o último com prótese tipo modular. Em todos os pacientes obteve-se diminuição da dor e aumento da amplitude da RCD ao fim de um ano de seguimento pós-cirúrgico.

    Distal radioulnar arthroplasty is an attractive solution for treating various pathological conditions of the distal radioulnar joint because it allows restoration of stability, load transmission and function. The main indications are: radioulnar impingement after partial or complete resection of the distal ulna; and degenerative, inflammatory or post-traumatic arthritis of the distal radioulnar joint. The authors present three clinical cases of distal radioulnar pathological conditions: two patients with post-traumatic sequelae and one case of distal radioulnar impingement after a Sauvé-Kapandji operation. The three cases were treated surgically with a metallic prosthesis to replace the distal ulna (First Choice - Ascension®). The first two were treated with a resurfacing prosthesis and the last one with a modular prosthesis. All of the patients had achieved pain relief and increased movement of the distal radioulnar joint after one year of postoperative follow-up.

  • Uma forma simplificada de determinar a direção da osteotomia de corte único para a correção de deformidade combinada rotacional e angular dos ossos longos Nota Técnica

    A direção da obliquidade do corte para a execução da osteotomia de corte único, para correção de deformidades angulares e rotacionais combinadas, é de difícil determinação. A obliquidade adequada, se no sentido horário ou anti-horário em relação à transversal ao eixo do osso, é usualmente determinada através de ensaios em modelos ósseos plásticos imitando a deformidade ou em bananas, onde diferentes simulações podem ser feitas. Isto é muito confuso e difícil. Neste trabalho propomos uma tabela, com entradas para as deformidades angular e rotacional e o lado afetado. Na tabela é indicada diretamente a obliquidade correta do corte em relação à perpendicular ao eixo diafisário. Uma revisão passo a passo da osteotomia de corte único é também apresentada, com vistas ao planejamento pré-operatório, com ênfase na contribuição proposta.

    The direction of the obliquity of the cut for performing single-cut osteotomy to correct combined angular and rotational deformities is difficult to determine. The appropriate obliquity, i.e. whether clockwise or anticlockwise in relation to the perpendicular to the bone axis, is usually determined through trials using plastic bone models to imitate the deformity, or on bananas, on which different simulations can be made. This is very confusing and difficult. In this study, we propose a table, with entries for angular and rotational deformities and the affected side. The correct obliquity of the cut in relation to the perpendicular to the diaphyseal axis is directly indicated in the table. A step-by-step revision of the preoperative planning of the single-cut osteotomy is also presented, with emphasis on the proposed contribution.
