  • Fluoride Release from Restorative Materials

    O objetivo deste estudo foi avaliar, in vitro, a liberação de flúor (F) de 4 materiais restauradores (3M-ESPE): KME cimento de ionômero de vidro convencional (CIV) (Ketak Molar Easymix); RX2- GIC modificado por resina (RMGIC) (Rely-X luting 2); VIT-RMGIC (Vitremer); -resina composta Z250 (controle negativo) (Filtek Z250). As amostras foram confeccionadas de acordo com as instruções do fabricante e colocadas em 10 mL de água destilada 37°C até as leituras, que foram feitas utilizando um eletrodo de íon seletivo de fluoreto de membrana líquida (Orion 710). A liberação de flúor foi avaliada a cada 6 h no primeiro dia e, diariamente durante 28 dias (d). Os resultados foram analisados estatisticamente usando ANOVA a dois critérios e teste de Tukey. A média e desvio padrão de liberação de F, valores (em ppm) foram: KME: 6 h- 0,34 ± 0,04; 24 h- 1,22 ± 0,30; 7 d- 0,29 ± 0,09; 14 d- 0,20 ± 0,04; 28 d- 0,16 ± 0,01; RL2: 6 h- 2,46 ± 0,48; 24 h- 12,33 ± 2,93; 7 d- 1,37 ± 0,38; 14 d- 0,80 ± 0,13; 28 d- 0,80 ± 0,21; VIT: 6 h- 0,98 ± 0,35; 24 h- 4,35 ± 1,22; 7 d- 0,66 ± 0,23; 14 d- 0,40 ± 0,07; 28 d- 0,39 ± 0,08; Z250: 6 h- 0,029 ± 0,001; 24 h- 0,024 ± 0,009; 7 d- 0,023 ± 0,004; 14 d- 0,025 ± 0,001; 28 d- 0,028 ± 0,001. Concluiu-se que RL2 liberou mais F do que os outros materiais em todos os períodos. A maior liberação de F ocorreu nas primeiras 24 h.

    The aim of this study was to evaluate in vitro fluoride (F) release from 4 restorative materials (3M ESPE): Ketak Molar Easymix [KME - conventional glass ionomer cement (GIC)]; Rely-X luting 2 [RL2 - resin-modified GIC (RMGIC)]; Vitremer (VIT- RMGIC); and Filtek Z250 [Z250 - negative control]. Disc-shaped specimens were fabricated according to the manufacturer’s instructions and placed into 10 mL of reverse osmosis water at 37°C until the analyses were done using a liquid membrane for selective F ion electrode (Orion 710). F release was evaluated every 6 h in the first day and thereafter daily during 28 days (d). The results were analyzed statistically by two-way ANOVA and Tukey’s test (α=0.05). Mean F release and standard deviation values (in ppm) were: KME: 6 h- 0.34 ± 0.04; 24 h- 1.22 ± 0.30; 7 d- 0.29 ± 0.09; 14 d- 0.20 ± 0.04; 28 d- 0.16 ± 0.01; RL2: 6 h- 2.46 ± 0.48; 24 h-12.33 ± 2.93; 7 d- 1.37 ± 0.38; 14 d- 0.80 ± 0.13; 28 d- 0.80 ± 0.21; VIT: 6 h- 0.98 ± 0.35; 24 h- 4.35 ± 1.22; 7 d- 0.66 ± 0.23; 14 d- 0.40 ± 0.07; 28 d- 0.39 ± 0.08; Z250: 6 h- 0.029 ± 0.001; 24 h- 0.024 ± 0.009; 7 d- 0.023 ± 0.004; 14 d- 0.025 ± 0.001; 28 d- 0.028 ± 0.001. RL2 RMGIC released more F than the other materials in all periods. The greatest release of F occurred in the first 24 h.
  • Effect of cement type and water storage time on the push-out bond strength of a glass fiber post

    Este estudo investigou os efeitos do tipo de cimento e do tempo de armazenamento em água na resistência adesiva ao cisalhamento de pinos de fibra. Pinos de fibra de vidro (Fibrekor, JenericPentron) foram cimentados aos condutos radiculares usando o cimento resinoso autopolimerizável (C&B Cement [CB]), o cimento de ionômero de vidro (Ketac Cem [KC]) ou o cimento de ionômero de vidro modificado por resina (GC FujiCEM [FC]) de acordo com as instruções dos fabricantes. Para cada cimento, os espécimes foram expostos a um dos seguintes tempos de armazenamento em água (n=5): 1 dia (T1), 7 dias (T7), 90 dias (T90) e 180 dias (T180). Os testes de resistência adesiva ao cisalhamento foram realizados após os períodos de armazenamento. Os espécimes controles não foram expostos ao armazenamento em água, mas submetidos ao teste de resistência adesiva ao cisalhamento 10 min após a cimentação do pino. Os dados (MPa) foram analisados usando Kruskal-Wallis e teste de Dunn (α=0,05). O tipo de cimento e o tempo de armazenamento em água afetaram significantemente a resistência adesiva ao cisalhamento (p<0,05). CB apresentou valores de retenção significantemente superiores a KC e FC, independentemente do tempo de armazenamento em água. Esse, por sua vez, aumentou significantemente a resistência adesiva ao cisalhamento em T7 e T90, independentemente do tipo de cimento (p<0,05). Os resultados indicaram que os pinos de fibra cimentados aos condutos radiculares com o cimento resinoso autopolimerizável apresentou a melhor performance adesiva durante todo o período de armazenamento em água de 180 dias. Todos os cimentos apresentaram uma tendência a aumentar a resistência adesiva ao cisalhamento após 7 e 90 dias de armazenamento em água, diminuindo posteriormente.

    This study investigated the effects of the cement type and the water storage time on the push-out bond strength of a glass fiber post. Glass fiber posts (Fibrekor, Jeneric Pentron) were luted to post spaces using a self-cured resin cement (C&B Cement [CB]), a glass ionomer cement (Ketac Cem [KC]) or a resin-modified glass ionomer cement (GC FujiCEM [FC]) according to the manufacturers’ instructions. For each luting agent, the specimens were exposed to one of the following water storage times (n=5): 1 day (T1), 7 days (T7), 90 days (T90) and 180 days (T180). Push-out tests were performed after the storage times. Control specimens were not exposed to water storage, but subjected to the push-out test 10 min after post cementation. Data (in MPa) were analyzed by Kruskal-Wallis and Dunn`s test (α=0.05). Cement type and water storage time had a significant effect (p<0.05) on the push-out bond strength. CB showed significantly higher values of retention (p<0.05) than KC and FC, irrespective of the water storage time. Water storage increased significantly the push-out bond strength in T7 and T90, regardless of the cement type (p<0.05). The results showed that fiber posts luted to post spaces with the self-cured resin cement exhibited the best bonding performance throughout the 180-day water storage period. All cements exhibited a tendency to increase the bond strength after 7 and 90 days of water storage, decreasing thereafter.
  • Effect of surface sealant on the color stability of composite resin restorations

    Este estudo avaliou o efeito da aplicação de selante na estabilidade de cor de restaurações de compósito. Cavidades foram restauradas em incisivos bovinos com resina composta (Opallis, FGM) e os dentes separados em quatro grupos (n=10). O selante (Fill Glaze; Vigodent) foi aplicado sobre as restaurações em dois grupos. Aferição inicial de cor baseada no sistema CIEL*a*b* foi realizada com espectrofotômetro. Metade do número de espécimes foi imersa em água destilada, e metade imersa 4 h/dia em café. Aferições de cor foram repetidas após 1 h, 24 h, 7 dias e 3 meses. Os dados para cada solução de imersão foram submetidos separadamente à Análise de Variância de 2 vias para medidas repetidas e teste de Tukey (α=0,05). Para o grupo sem selante imerso em água, não foram observadas diferenças entre os períodos (p≥0,138), mas a cor foi diferente comparada à inicial (p<0,001). Para o grupo com selante, os períodos inicial, 1 h e 3 meses apresentaram resultados similares (p≥0,924). Para os grupos imersos em café, quando o selante não foi utilizado, não foram detectadas diferenças entre os períodos inicial, 1 h e 24 h (p≥0,499), mas a cor alterou significativamente após (p≤0,003). No grupo com selante, diferenças significativas foram detectadas para todos os períodos comparados entre si (p<0,001). Em conclusão, a aplicação de selante aumentou drasticamente o manchamento das restaurações expostas ao café.

    This study investigated the effect of sealant application on the color stability of composite resin restorations. Cavities in bovine incisors were restored with composite resin (Opallis; FGM) and the teeth were assigned to 4 groups (n=10). A sealant (Fill Glaze; Vigodent) was applied over the restorations of 2 groups. Baseline color measurements based on the CIEL*a*b* system were carried out using a spectrophotometer. Half the number of specimens was immersed in distilled water, and half was immersed in coffee 4 h/day. Color measurements were repeated after 1 h, 24 h, 7 days and 3 months. Data for each immersion solution were separately subjected to a two-way repeated measures ANOVA and Tukey’s test (α=0.05). For the group without sealant immersed in water, no significant differences were observed among the periods (p≥0.138), but the color was different compared with baseline (p<0.001). For the group with sealant application, the periods baseline, 1 h and 3 months presented similar results (p≥0.924). For groups immersed in coffee, when the sealant was not used, no significant differences were detected between the baseline and the periods 1 h and 24 h (p≥0.499), but the color changed significantly thereafter (p≤0.003). In the group with sealant, significant differences were detected for all periods compared with each other (p<0.001). In conclusion, application of sealant dramatically increased the staining of the restorations exposed to coffee.
  • An endodontic sealer induces a pathological condition when associated with persistent tissue toxicity and presence of myofibroblasts

    O objetivo deste estudo foi avaliar a relação entre reação inflamatória induzida por quatro cimentos endodônticos e a presença de fibrose e quantidade de miofibroblastos que apresentam positividade para α-SMA. Tubos de polietileno foram preenchidos com o cimento (I: Endofill; II: AH Plus; III: Acroseal; IV: Epiphany) e inseridos em 4 regiões do dorso de 24 ratos Wistar, enquanto 2 tubos vazios (V - controle) foram inseridos em 6 ratos. Após 7, 21 e 45 dias, oito animais foram sacrificados obtendo 6 indivíduos por grupo e 2 para o grupo controle. Os fragmentos foram submetidos ao processamento histológico e à análise imuno-histoquímica para a proteína anti-α-SMA. Todos os grupos, exceto o controle, demonstraram notável reação inflamatória no 7º dia pós-operatório, que também coincidiu com uma grande quantidade de miofibroblastos. No 21º e 45º dia pós-operatório, a reação inflamatória induzida pelo Endofill, AH Plus e Acroseal diminuiu significativamente, o que coincidiu com reduzida presença de miofibroblastos e deposição de colágeno normal. Em contraste, no grupo Epiphany, infiltrado inflamatório significativo esteve presente em todos os períodos analisados. A persistência do infiltrado inflamatório induzido por cimento endodôntico pode também provocar uma fibrose associada com a presença de miofibroblastos.

    The aims of this study were to evaluate the ratio between inflammatory reactions induced by four endodontic sealers and the occurrence of fibrosis and the number of myofibroblasts with positivity to α-smooth-actin muscle (α-SMA). Polyethylene tubes were filled with a root canal sealer (Endofill, AH Plus, Acroseal and Epiphany) and inserted into 4 site at the dorsal region of 24 Wistar rats; 2 empty tubes (control) were grafted in 6 rats. After 7, 21, and 45 days, 8 animals were euthanized, providing 6 specimens per test group and 2 specimens from the control group. The fragments were subjected to histological processing and immunohistochemical analysis for anti α-SMA protein. All specimens, except those from the control group, presented severe inflammatory reaction on the 7th postoperative day, which also coincided with a large number of myofibroblasts. On the 21st and 45th days post-surgery, the inflammatory reaction induced by Endofill, AH Plus and Acroseal decreased significantly, which coincided with reduced presence of myofibroblasts and usual collagen deposition. In contrast, in the group filled with Epiphany, significant inflammatory cell infiltrate was present in all analyzed periods. The persistence of an inflammatory reaction induced by endodontic sealer may also induce the development of fibrosis in combination with presence of myofibroblasts.
  • Nd:YAG laser irradiation effect on apical intracanal dentin - a microleakage and SEM evaluation

    The purpose of this in vitro study was to evaluate the effect of neodymium:yttrium-aluminum-garnet (Nd:YAG) laser irradiation on intracanal dentin surface by SEM analysis and its interference in the apical seal of filled canals. After endodontic treatment procedures, 34 maxillary human incisors were randomly assigned to 2 groups. In the negative control group (n=17), no additional treatment was performed and teeth were filled with vertically condensed gutta-percha; in the laser-treated group (n=17), the root canals were irradiated with Nd:YAG laser (1.5 W, 100 mJ, 15 Hz) before filling as described for the control group. Two specimens of each group were prepared for SEM analysis to evaluate the presence and extent of morphological changes and removal of debris; the other specimens were immersed in 0.5% methylene blue dye (pH 7.2) for 24 h for evaluation of the linear dye leakage at the apical third. SEM analysis of the laser-treated group showed dentin fusion and resolidification without smear layer or debris. The Student’s t-test showed that the laser-treated group had significantly less leakage in apical third than the control group. Within the limitations of this study, it may be concluded that the morphological changes on the apical intraradicular dentin surface caused by Nd:YAG laser resulted in less linear dye apical leakage.
  • Apical microleakage and SEM analysis of dentin surface after 980 nm diode laser irradiation

    Este estudo avaliou o efeito do laser de diodo 980 nm na microinfiltração apical e na morfologia intrarradicular da dentina. Raízes de 110 incisivos inferiores foram utilizadas no estudo: 92 para o teste de microinfiltração e 18 para microscopia eletrônica de varredura (MEV). As raízes foram divididas em 3 grupos de acordo com a solução irrigante (água, NaOCl e NaOCl/EDTA) e subdivididas em 3 de acordo com a irradiação laser (sem irradiação, irradiados com 1,5 W e irradiados com 3,0 W). Duas amostras de cada subgrupo foram preparadas para MEV. As raízes restantes foram preenchidas com AH Plus e guta percha. A infiltração apical foi avaliada nas raízes por meio da penetração do corante e os dados foram analisados por ANOVA e teste de Tukey-Krammer (α=0,05). Os resultados da MEV mostraram intensificação das mudanças quando se aumentou a potência do laser e variações de acordo com a solução irrigadora. Camada de smear modificada foi observada em espécimes tratados com água e irradiados com laser. Raízes irrigadas com NaOCl/EDTA tiveram níveis menores de infiltração (0,17 ± 0,18 mm) estatisticamente diferente (p<0,05) das raízes irrigadas com água (0,34 ± 0,30 mm), mas semelhante (p>0,05) aos irrigados com NaOCl (0,28 ± 0,29 mm). As raízes não irradiadas tiveram níveis menores de infiltração (0,10 ± 0,14 mm), diferente (p<0,05) de 1,5 W (0,32 ± 0,22 mm) e 3.0 W (0,37 ± 0,32 mm). O laser de diodo 980 nm alterou a morfologia da dentina e aumentou a infiltração marginal apical.

    This study evaluated the effect of 980-nm diode laser on apical microleakage and intraradicular dentin morphology. Roots of 110 mandibular incisors were used in the study: 92 for microleakage test and 18 for scanning electron microscopy (SEM). Roots were randomly assigned to 3 groups according to the irrigating solution (water, NaOCl and NaOCl/EDTA) and were divided into 3 subgroups according to the laser irradiation protocol (without irradiation, irradiated at 1.5 W and irradiated at 3.0 W). Two specimens of each subgroup were prepared for SEM. The remaining roots were filled with AH Plus and gutta-percha. Apical leakage was assessed by ink penetration and data were analyzed statistically by ANOVA and Tukey-Krammer test (α=0.05). SEM analysis showed intensification of changes with increase of laser power as well as variations according to the irrigating solution. Modified smear layer was observed in specimens treated with water and irradiated with laser. Roots irrigated with NaOCl/EDTA had lower levels of infiltration (0.17 ± 0.18 mm) differing significantly (p<0.05) from those of roots irrigated with water (0.34 ± 0.30 mm), but similar (p>0.05) to those irrigated with NaOCl (0.28 ± 0.29 mm). Non-irradiated roots had lower levels of infiltration (0.10 ± 0.14 mm), differing (p<0.05) from those irradiated at 1.5 W (0.32 ± 0.22 mm) and 3.0 W (0.37 ± 0.32 mm). The 980 nm diode laser modified dentin morphology and increased apical microleakage.
  • Temperature of denture base resin under different protocols of microwave irradiation

    Este estudo in vitro avaliou a temperatura de próteses submetidas a diferentes protocolos de irradiação de microondas. Duas próteses totais (uma superior e outra inferior) foram irradiadas separadamente quatro vezes para cada um dos 5 protocolos que se seguem: prótese imersas em água (G1- 6 min, G2- 3 min); prótese a seco (G3-6 min); prótese no vapor (G4- 6 min, G5- 3 min). A temperatura final das próteses foi aferida em uma área fina e uma área espessa de cada prótese com um termômetro de infravermelho. Os resultados mostraram que todos os grupos sofreram aumento de temperatura. As áreas finas da prótese tiveram mais aumento da temperatura que as áreas espessas. Não houve uma diferença estatisticamente significante (p>0,05) entre a média da temperatura final das próteses imersas em água por 6 (G1) e 3 min (G2). Entretanto, a temperatura final média observada no G1 e G2 excederam 71o C e foram significativamente maiores (p<0,001) que a temperatura final média dos outros grupos. Pode-se concluir que as bases de prótese submetidas à irradiação por microondas imersas em água podem estar expostas a temperaturas deletérias.

    This in vitro study evaluated the temperature of dentures after different microwave irradiation protocols. Two complete dentures (one maxillary and one mandibular denture) were irradiated separately 4 times for each of the following 5 protocols: dentures immersed in water (G1- 6 min, G2- 3 min); dentures kept dry (G3- 6 min); dentures placed in the steam sterilizer (G4- 6 min, G5- 3 min). The final temperature of the dentures was gauged in a thin and in a thick area of each denture with an infrared thermometer. All groups presented an increase in the resin base temperature. The thin areas of the dentures underwent greater heating than the thick areas. There was no significant difference (p>0.05) between the final mean temperatures of dentures immersed in water for 6 (G1) and 3 min (G2). However, the final mean temperatures recorded in G1 and G2 exceeded 71°C and were significantly higher (<0.001) than the final mean temperatures recorded in the other groups. It may be concluded that denture base resins subjected to microwave irradiation immersed in water may be exposed to deleterious temperatures.
  • Effect of different cleansers on the surface of removable partial denture

    As próteses parciais removíveis (PPR) exigem higienização específica e a associação da escovação com imersão em soluções químicas tem sido o método mais recomendado para controle do biofilme. Entretanto, os efeitos destas soluções não são amplamente reportados em componentes metálicos. Este estudo avaliou o efeito de diferentes agentes de higienização na superfície dos componentes de uma PPR. Foram confeccionados 5 espécimes (disco metálico de 12 x 3 mm centralizado em uma tira de resina com 38 x 18 x 4 mm) para cada situação experimental: 6 soluções [Periogard (PE), Cepacol (CE), Corega Tabs (CT), Medical Interporous (MI), Polident (PO), hipoclorito de sódio 0,05% (HS) e água destilada (AD) como controle)] e 2 ligas de cobalto-cromo [DeguDent (DD) e Vera PDI (VPDI)] foram utilizadas para cada situação experimental. Foram simuladas imersões de 180 dias. As aferições de rugosidade (Ra, μm) tanto em porção metálica quanto em resina acrílica termopolimerizável foram submetidos ao ANOVA e ao teste de Tukey. As alterações superficiais e manchas foram examinadas por meio de microscopia eletrônica de varredura (MEV). Áreas de interesse foram submetidas à espectrometria por energia dispersiva por raios X (EDS). Visualmente, puderam ser verificadas manchas nas superfícies metálicas quando utilizados HS e MI. A rugosidade dos materiais não foi afetada pelas soluções (p>0,05). As fotomicrografias evidenciaram que HS e MI ocasionaram alterações superficiais. As análises de EDS revelaram a presença de oxigênio nos grupos HS e MI, o que pode sugerir que estas duas soluções causaram oxidação das superfícies, provocando pontos de corrosão. Dentre as limitações do presente estudo, pode-se concluir que estas soluções não são apropriadas para a higienização das PPR.

    Removable partial dentures (RPD) demand specific hygienic cleaning and the combination of brushing with immersion in chemical solutions has been the most recommended method for control of biofilm. However, the effect of the cleansers on metallic components has not been widely investigated. This study evaluated the effect of different cleansers on the surface of RPD. Five disc specimens (12 mm x 3 mm metallic disc centered in a 38 x 18 x 4 mm mould filled with resin) were obtained for each experimental situation: 6 solutions [Periogard (PE), Cepacol (CE), Corega Tabs (CT), Medical Interporous (MI), Polident (PO), 0.05% sodium hypochlorite (NaOCl), and distilled water (DW) control] and 2 Co-Cr alloys [DeguDent (DD) and VeraPDI (VPDI)] were used for each experimental situation. A 180-day immersion was simulated and the measurements of roughness (Ra, µm) of metal and resin were analyzed using 2-way ANOVA and Tukey’s test. The surface changes and tarnishes were examined with a scanning electronic microscopy (SEM). In addition, energy dispersive x-ray spectrometry (EDS) analysis was carried out at representative areas. Visually, NaOCl and MI specimens presented surface tarnishes. The roughness of materials was not affected by the solutions (p>0.05). SEM images showed that NaOCl and MI provided surface changes. EDS analysis revealed the presence of oxygen for specimens in contact with both MI and NaOCl solutions, which might suggest that the two solutions promoted the oxidation of the surfaces, thus leading to spot corrosion. Within the limitations of this study, it may be concluded that the NaOCl and MI may not be suitable for cleaning of RPD.
  • In Vitro Effect of photodynamic therapy on Aggregatibacter actinomycetemcomitans and Streptococcus sanguinis

    Novos tratamentos são propostos para evitar a progressão da periodontite, sendo a terapia fotodinâmica (PDT) uma notória promessa. Sua aplicação associa o Azul de orto-toluidina a 0,01% (TBO) e uma fonte luminosa a laser de diodo (TwinFlex, Mmoptics), liberando assim, toxinas às bactérias. O objetivo do estudo é avaliar in vitro a eficiência da PDT sobre Aggregatibacter actinomycetemcomitans (A.a.) e Streptococcus sanguinis (S.s.). Preparou-se suspensões distintas de 2 mL contendo A.a. e S.s. na concentração de 1,5x108 UFC/mL, e divisão de cada suspensão em 3 grupos: Controle (sem tratamento); Corante (suspensão e TBO por 5min) e Corante/Laser (suspensão, TBO por 5 min e laser por 3 min). Promoveu-se a diluição, a semeadura em 20 mL de TSA (A.a.) e de Ágar Brucella (S.s.), em placas de Petri (Método Pourplate), e a incubação da A.a. em microaerofilia e da S.s. em aerobiose, por 48 h a 35oC, para posterior contagem visual das UFC. Os grupos Controle mostraram ótimo crescimento bacteriano (1,5 x 108 UFC/mL). Os grupos Corante não apresentaram redução significativa para ambas bactérias. Os grupos Laser apresentaram redução em relação ao controle, 61,53% para A.a. e 84,32% para S.s. A análise estatística (ANOVA, p<0,05) corrobou que a PDT é eficaz na redução destas bactérias in vitro.

    New periodontal disease treatments are needed to prevent infection progression. Photodynamic therapy (PDT) is one of the greatest pledges for this purpose. It involves the use of light of specific wavelength to activate a nontoxic photosensitizing agent in the presence of oxygen for eradication of target cells, and can be used for photokilling of microorganisms. This study evaluated in vitro the photodynamic effect of 0.01% toluidine blue-O (TBO) in combination with an AlGaInP diode laser light source on Aggregatibacter actinomycetemcomitans (A.a.) and Streptococcus sanguinis (S.s.). Suspensions (2 mL) containing A.a. and S.s. at 1.5 x 108 CFU/mL concentration were prepared and divided into 3 groups: Control group (no treatment), Dye group (inoculum and TBO for 5 min) and Dye/Laser group (inoculum, TBO for 5 min and laser for 3 min). Next, a dilution for subsequent subculture in 20 mL of Trypic Soy Agar (A.a) and Brucella Agar (S.s.) in Petri dishes (Pourplate Method) was done. Incubation of A.a. in microaerophilia and S.s. in aerobiosis at 35oC for 48 h was performed for subsequent visual counting of CFU/mL. Data were analyzed by one-way ANOVA and Tukey’s HSD test at 5% significance level. For both strains, the control group showed a significantly higher (p<0.05) bacterial growth (1.5 x 108 CFU/mL), while the Dye group presented no significant reduction (p>0.05) in the CFU counts. The Dye/Laser group presented a significant decrease in the CFU counts (p<0.05) compared with the Control group (61.53% for A.a. and 84.32% for S.s.). It may be concluded that PDT was effective in reducing the numbers of A.a. and S.s. in vitro.
  • Accuracy of linear measurements before and after digitizing periapical and panoramic radiography images

    O objetivo deste estudo foi avaliar a acurácia de medidas lineares realizadas em radiografias panorâmicas e periapicais convencionais e digitalizadas de hemimandíbulas humanas secas. Imagens da região posterior de 22 hemimandíbulas humanas secas foram obtidas pelas técnicas panorâmica e periapical convencionais. Com o paquímetro eletrônico digital, foram realizadas três medições verticais diretamente sobre as hemimandíbulas (medidas referenciais), bem como sobre o decalque das imagens convencionais, em papel de acetato: Distância 1: Distância entre o limite superior do rebordo alveolar e o limite inferior da base da mandíbula; Distância 2: Distância entre o limite superior do rebordo alveolar e o limite superior do canal mandibular e Distância 3: Distância entre o limite inferior do canal mandibular e o limite inferior da base da mandíbula. Posteriormente, as radiografias foram digitalizadas e as três medidas foram realizadas nas imagens digitais utilizando o programa UTHSCSA Image Tool. Os dados foram analisados estatisticamente pelo teste ANOVA a um critério (α=0,05). Não houve diferenças estatisticamente significantes (p>0,05) entre as radiografias panorâmicas e periapicais, assim como para as medidas obtidas com o paquímetro e pelo programa UTHSCSA Image Tool, quando comparados aos espécimes mandibulares, avaliando as distâncias 1 (p=0,783), 2 (p=0,986) e 3 (p=0,129). Em conclusão, as técnicas radiográficas avaliadas neste estudo mostraram-se confiáveis para mensurações ósseas verticais na área selecionada e o programa de digitalização de imagens UTHSCSA Image Tool é um método adequado de aferição.

    The aim of this study was to evaluate the accuracy of linear measurements made on conventional and digitized periapical and panoramic radiographic images of dry human hemi-mandibles. Images from the posterior region of 22 dry human hemi-mandibles were obtained by conventional panoramic and periapical radiography technique. Using a digital caliper, 3 vertical measurements were marked directly on the dry hemi-mandibles (reference measurements) as well as on the tracing from the conventional radiographic images of the specimens made onto acetate paper sheet: Distance 1: between the upper limit of the alveolar ridge and the lower limit at the mandible base; Distance 2: between the upper limit of the alveolar ridge and the upper limit of the mandibular canal; Distance 3: between the lower limit of the mandibular canal and the lower limit of the mandible base. Next, the radiographs were digitized and the three measurements were made on the digital images using UTHSCSA Image Tool software. Data were analyzed statistically by one-way ANOVA (α=0.05). There was no statistically significant differences (p>0.05) between periapical and panoramic radiographs or between the measurements recorded using the digital caliper and UTHSCSA software compared with dry mandible specimens for Distances 1 (p=0.783), 2 (p=0.986) and 3 (p=0.129). In conclusion, the radiographic techniques evaluated in this study are reliable for vertical bone measurements on selected areas and the UTHSCA Image Tool software is an appropriate measurement method.
  • Relationship between squamous cell carcinoma of the anterior two thirds of the tongue and removable denture use: a pioneer study in a Portuguese population

    O objetivo deste estudo foi determinar se existe alguma relação entre o uso de prótese removível e o carcinoma escamoso dos dois terços anteriores na população portuguesa estudada. Um estudo retrospectivo foi conduzido em paciente com diagnóstico anatomopatologico de carcinoma escamoso dos dois terços anteriores, diagnosticados no Instituto Português de Oncologia, Francisco Gentil, Lisboa desde 1 de Janeiro de 2001 até 31 de Dezembro de 2003. Diferentes fatores foram analisados: o gênero, uso de prótese removível, idade, localização, álcool e tabaco. Dos 151 casos iniciais de carcinoma, foram selecionados 106 casos mostrando numa proporção de 3:1 (masculino/feminino). Os bordos laterais foram a localização mais comum e a prevalência em ambos os sexos sitou-se entre a sexta e a sétima década. A presença de álcool e tabaco foi significativamente mais comum nos homens não se observando relação entre o uso de prótese e a presença do carcinoma na língua. Não se observou relação estatística entre o uso de prótese removível e o carcinoma escomoso dos dois terços anteriores da língua. Face aos resultados, o uso de prótese no sexo feminino desempenha um papel importante ainda que não possa ser considerado um fator etiológico significativo.

    The aim of the study was to determine whether there is any relationship between the presence of removable dentures and squamous cell carcinoma of the anterior two thirds of the tongue in a Portuguese population. A retrospective cross-sectional study was conducted on patients with a biopsy-proven diagnosis of squamous cell carcinoma of the tongue, who were seen and treated at the Department of Head and Neck Surgery of the Portuguese Institute of Oncology 'Francisco Gentil', Lisbon, Portugal, during a 3-year period. Several factors were examined: gender, use of removable dentures, age, location of the lesion, and alcohol and tobacco consumption. One hundred and six cases were selected from the initial 151 cases, with a male:female ratio of 3:1 and the lateral borders being the most commonly affected site. The prevalence in both genders was between the sixth and seventh decade of life. Men were more likely to consume alcohol and tobacco than women, and no relationship was observed between denture use and presence of carcinoma of the tongue. In light of the data obtained, it may be considered that female use of a denture plays an important role, but it cannot be identified as a remarkable etiologic factor.
  • Effectiveness of cognitive-behavioral therapy and amitriptyline in patients with chronic temporomandibular disorders: a pilot study

    O objetivo deste estudo piloto foi avaliar a eficácia do uso de uma terapia cognitivo comportamental (TCC) e da amitriptilina, um antidepressivo tricíclico, no tratamento de pacientes portadores de disfunções temporomandibulares (DTM) crônicas. Quarenta e sete mulheres (com 35,4 anos em média) diagnosticadas com DTM crônica foram selecionadas e divididas em 4 grupos: amitriptilina; amitriptilina + TCC; placebo + TCC; e placebo (controle) e controle. As pacientes foram submetidas às terapias por 7 semanas consecutivas. As reavaliações aconteceram 1, 7 e 11 semanas após o início da terapia (11 semanas de avaliações). Foram avaliados a presença e severidade de dor, o nível de depressão, a qualidade de vida e do sono. Os dados foram avaliados através de ANOVA, Qui-quadrado e teste de Cochran, considerando um nível de significância de 5%. Foram observadas melhoras significantes em todos os fatores avaliados para todos os grupos, porém não foram observadas diferenças entre os grupos. Após o final da terapia, os resultados positivos persistiram apenas para o grupo amitriptilina + TCC. Os resultados sugerem que o uso associado de amitriptilina e TCC no tratamento de pacientes portadores de DTM crônicas pode ser efetivo na melhora da dor, nível de depressão, qualidade de vida e do sono.

    The aim of this pilot study was to evaluate the effectiveness of cognitive-behavioral therapy (CBT) and use of amitriptyline, a tricyclic antidepressant, in patients with chronic temporomandibular disorders (TMD). Forty-seven women (mean age = 35.4 years old) with chronic TMD were enrolled in the study and divided into 4 groups: amitriptyline; amitriptyline and CBT; placebo and CBT; and placebo only (control). Patients were managed for 7 consecutive weeks. Follow-up evaluations were done at the 1st, 7th and 11th weeks of treatment. The presence and severity of pain, levels of depression, and quality of life and sleep were measured. Data were analyzed using ANOVA, Chi-square and Cochran tests, considering a significance level of 5%. Improvements were found for all factors considered in the intragroup analysis, although no significant differences were detected among groups. However, at the end of the treatment (11 weeks of follow-up), these positive outcomes persisted only for the women treated with amitriptyline and CBT. The obtained results suggest that the combination of amitriptyline and CBT may be effective in reducing pain and depression levels as well as in improving the quality of life and sleep in patients with chronic TMD.
  • Hypomethylation of tumor suppressor genes in odontogenic myxoma

    O mixoma odontogênico (MO) é um tumor odontogênico benigno de origem mesenquimal caracterizado pela presença de células fusiformes ou estreladas dispostas em abundante matriz extracelular mucóide. A metilação do DNA é caracterizada pela adição de grupos metil em citosinas constituintes de ilhas CpG na região promotora do gene. A metilação pode diminuir a expressão de genes supressores de tumor e contribuir para o desenvolvimento de lesões neoplásicas. O objetivo deste trabalho foi avaliar o padrão de metilação nos genes P16 (CDKN2A), P21 (CDKN1A), P27 (CDKN1B), P53 (TP53), RB1 nos MO e na polpa dental. A metilação foi avaliada pela reação em cadeia da polimerase específica para a metilação. A transcrição dos genes foi estudada em alguns casos pela reação da transcriptase reversa (PCR quantitativa). Uma maior frequência de amostras não metiladas para os genes P27, P53 e RB1 foi observada nos MO quando comparados à polpa dental. Os MO expressaram RNAm de todos os genes avaliados. Considerando todas as amostras juntas, a expressão de Rb foi maior em amostras não metiladas comparadas as amostras parcialmente metiladas. Esta investigação mostrou a hipometilação dos genes P27, P53 e RB1 nos MO. Adicionalmente, a metilação nos genes supressores de tumor é um evento frequente em polpa dental normal.

    Odontogenic myxoma (OM) is an ectomesenchymal benign odontogenic tumor characterized by spindle or stellate-shaped cells embedded in an abundant myxoid or mucoid extracellular matrix. DNA methylation is characterized by the addition of methyl groups in cytosines within CpG islands in the promoter gene. DNA methylation can decrease the expression of tumor suppressor genes and contribute to the development of neoplastic lesions. The aim of study was to evaluate the methylation pattern of the tumor suppressor genes P16 (CDKN2A), P21 (CDKN1A), P27 (CDKN1B), P53 (TP53) and RB1 in OM and dental pulp. Methylation was evaluated using methylation-specific polymerase chain reaction (PCR). The transcription was studied in some cases by using reverse transcription quantitative PCR. A higher frequency of unmethylated P27, P53, and RB1 samples was observed in the OM when compared with the dental pulp. OM expressed mRNA of all the genes evaluated. Considering all the samples together, the expression of Rb was higher in the unmethylated samples compared with the partially methylated samples. This investigation revealed hypomethylation of the genes P27, P53, and RB1 in OM. In addition, methylation of tumor suppressor genes was found to be an usual event in normal dental pulp.
  • 3D mandibular positioning after rapid maxillary expansion in Class II malocclusion

    O objetivo do presente estudo foi avaliar, por meio de tomografia computadorizada de feixe cônico (TCFC), o posicionamento espacial da mandíbula após expansão rápida da maxila (ERM) na maloclusão de Classe II. Este estudo prospectivo avaliou 17 crianças (idade média inicial de 10,36 anos) com maloclusão de Classe II Divisão 1 e atresia esquelética da maxila, as quais foram submetidas ao protocolo de Haas para ERM. TCFCs foram realizadas antes do tratamento (T1), imediatamente após estabilização do parafuso expansor (T2) e após o período de 6 meses de contenção (T3). Com o software Dolphin Imaging®, 11.0, foi possível a manipulação das imagens e as mensurações das landmarks (condílio direito e esquerdo; gônio direito e esquerdo; e mento) em relação aos planos sagital, coronal e axial, para que fosse possível verificar os efeitos transversais, anteroposteriores e verticais, respectivamente. O teste t de Student pareado foi utilizado para identificar significância estatística (p<0,05) entre os intervalos T2 e T1, T3 e T2, e T3 e T1. Imediatamente após a ERM o gônio direito e esquerdo deslocaram para baixo 1,11 mm e 0,89 mm, respectivamente; e o mento deslocou 1,90 mm para baixo e 1,50 mm para trás. Durante o período de contenção houve apenas deslocamento significativo no sentido anteroposterior; sendo que o gônio direito, esquerdo e o mento deslocaram para frente, 0,97 mm, 1,26 mm e 2,29 mm, respectivamente. A avaliação tridimensional de pacientes com maloclusão de Classe II Divisão 1 mostrou que a mandíbula se posiciona transitoriamente para trás e para baixo logo após a ERM. Os 6 meses seguintes de contenção permitiram que a mandíbula se deslocasse significativamente para frente, exibindo uma posição mais anterior, mesmo permanecendo mais inferior.

    This study investigated, using cone beam computed tomography (CBCT), the spatial mandibular positioning after rapid maxillary expansion (RME) in Class II Division 1 malocclusion. This prospective study evaluated 17 children (mean initial age 10.36 years old) presenting Class II, Division 1 malocclusion and skeletal maxillary constriction that underwent to RME Haas’ protocol. CBCT was performed before treatment (T1), immediately after the stabilization of expander screw (T2) and after the retention period of 6 months (T3). The scans were managed in Dolphin Imaging® 11.0 software, where landmarks (right and left condylion, right and left gonion, and menton) were positioned and measured in relation to sagittal, coronal and axial plane to verify, respectively, transverse, anteroposterior and vertical displacement of the mandible. Paired Student’s t-test was used to identify significant differences (p<0.05) between T1 and T2, T2 and T3, and T1 and T3. After RME, right and left gonion moved downward (1.11 mm and 0.89 mm) and menton displaced downward (1.90 mm) and backward (1.50 mm). During the retention period, only anteroposterior displacement was significant, with the right and left gonion (0.97 mm and 1.26 mm) and the menton (2.29 mm) moving forward. Three-dimensional assessment of the mandible in Class II Division 1 patients subjected to RME showed a transitory backward and downward mandibular positioning, without any lateral displacement. The 6-month retention period allowed the mandible shifting significantly forward, exhibiting a more anterior position compared with the initial condition, even remaining in a more downward direction.
  • Ligature of external carotid artery as an optional technique in a patient with von Willebrand disease

    A doença de von Willebrand (DvW) é uma coagulopatia hereditária. Não há predileção por sexo. Clinicamente caracteriza-se por hemorragias mucocutâneas, sobretudo nasais, menorragias e hemorragias pós-trauma. Este artigo relata um caso clínico de DvW em paciente de 52 anos de idade, leucoderma, do sexo masculino, que apresentou extensa hemorragia em bordo lateral de língua após ferimento lacerante, além de descrever alguns aspectos clínicos, patológicos e terapêuticos da DvW. Após repetidas tentativas de sutura do ferimento e reposição dos fatores de coagulação, optou-se pela ligadura da artéria carótida externa ipsilateral ao ferimento, com resolução favorável do caso, notando-se bom aspecto cicatricial 2 meses após a ligadura. Este caso reforça que é de extrema importância a realização de anamnese criteriosa, buscando-se identificar possíveis distúrbios hemorrágicos prévios ou antecedentes familiares.

    The von Willebrand disease (vWD) is a hereditary coagulopathy. There is no gender predilection. Clinically characterized by mucocutaneous bleeding, especially nose bleeding, menorrhagia and bleeding after trauma. This article reports a case of a 52-year-old Caucasian male patient with vWD, who presented with extensive bleeding in the tongue after a lacerating injury caused by accidental biting, and describes some clinical, pathological and treatment aspects of vWD. After repeated attempts to suture the wound and replace clotting factors, a decision was made to perform the ligature of the external carotid artery ipsilateral to the injury. There was favorable resolution of the case, with a good aspect of the scar 2 months after ligation. This case reinforces that it is extremely important to make a thorough review of medical history of all patients, searching for possible bleeding disorders or previous family history.
