  • Cone beam computed tomography in endodontics

    Tomografia computadorizada de feixe cônico (CBCT, sigla em Inglês para cone beam computed tomography) é um moderno sistema radiológico de imagem idealizado especificamente para uso no esqueleto maxilofacial. Este sistema supera muitas das limitações da radiografia convencional, produzindo imagens não distorcidas e tridimensionais da área examinada. Tais propriedades tornam este método de imagem especialmente apropriado para a Endodontia. O profissional consegue obter uma visão melhor da região anatômica desejada, resultando em melhor detecção de doenças de origem endodôntica e um planejamento mais efetivo do tratamento. Além disso, o sistema utiliza doses de radiação significantemente menores em comparação à tomografia computadorizada convencional. O objetivo deste artigo é revisar a literatura atual relativa às limitações e possíveis aplicações da CBCT na prática endodôntica.

    Cone beam computed tomography (CBCT) is a contemporary, radiological imaging system designed specifically for use on the maxillo-facial skeleton. The system overcomes many of the limitations of conventional radiography by producing undistorted, three-dimensional images of the area under examination. These properties make this form of imaging particularly suitable for use in endodontics. The clinician can obtain an enhanced appreciation of the anatomy being assessed, leading to an improvement in the detection of endodontic disease and resulting in more effective treatment planning. In addition, CBCT operates with a significantly lower effective radiation dose when compared with conventional computed tomography (CT). The purpose of this paper is to review the current literature relating to the limitations and potential applications of CBCT in endodontic practice.
  • In vitro susceptibility of oral Candida albicans strains to different pH levels and calcium hydroxide saturated aqueous solution

    Candida albicans está presente na cavidade oral e em todo trato digestivo de humanos e outros animais, podendo estar relacionada a fracassos do tratamento endodôntico. O presente trabalho determinou a incidência de C. albicans na cavidade oral e a susceptibilidade das amostras isoladas frente a diferentes valores de pH e à solução aquosa saturada de hidróxido de cálcio em pH 12,5. Sessenta e cinco pacientes atendidos na Clínica de Endodontia da Universidade Sagrado Coração participaram da pesquisa. As amostras coletadas foram cultivadas em meios seletivos para C. albicans. As cepas isoladas foram testadas quanto a resistência ao pH alcalino e à solução saturada de hidróxido de cálcio. Frente às variáveis de tempo, a viabilidade das leveduras foi avaliada pelos métodos da cultura sobre ágar Sabouraud e de fluorescência pela técnica do diacetato de fluoresceína e brometo de etídio. Foram isoladas 30 linhagens de C. albicans coletadas da cavidade oral dos pacientes. Quando em contato com a solução de hidróxido de cálcio as leveduras foram totalmente inviabilizadas após 48 h de exposição. Quando expostas ao caldo de cultura alcalinizado as leveduras mantiveram-se viáveis em pH 9,5 e 10,5 por até 7 dias. Em conclusão, C. albicans pode ser completamente inibida pelo contato direto com solução aquosa saturada de hidróxido de cálcio após 48 h de exposição.

    Candida albicans is present in the oral cavity and in the whole digestive tract of humans and other animals, being frequently related to endodontic treatment failure. The present study determined the incidence of C. albicans in the oral cavity and the susceptibility of isolates to different pH values and saturated calcium hydroxide aqueous solution at pH 12.5. Sixty-five patients attending the Endodontic Clinic at the Sagrado Coração University participated in the study. The collected samples were cultivated in selective media for C. albicans and the isolates were tested in terms of resistance to both alkaline pH and saturated aqueous solution of calcium hydroxide. In relation to time variables, yeast viability was assessed by the Sabouraud's agar culture and fluorescein diacetate and ethidium bromide fluorescent staining method. Results from the different pHs and experimental times, including those from different techniques measuring fungal viability, were compared using the chi-square and Fisher's exact tests (α=0.05). The yeasts became completely inviable after 48 h of contact with the calcium hydroxide solution. On the other hand, when exposed to the alkaline culture broth, the yeasts were found to be viable at pHs 9.5 and 10.5 for up to 7 days. In conclusion, C. albicans can only be completely inhibited by direct contact with saturated calcium hydroxide aqueous solution after 48 h of exposure.
  • Ex vivo accuracy of three electronic apex locators using different apical file sizes

    Este estudo avaliou a precisão de três localizadores eletrônicos foraminais (Root ZX, Novapex e Justy II) nas determinações de odontometria utilizando limas com diferentes padrões de ajuste, considerando-se a constrição apical (CA) e do forame principal (FA) como referências anatômicas. O diâmetro dos forames apicais de 40 dentes unirradiculados foi previamente determinado por visualização direta e a lima compatível com o forame apical foi estabelecida. Medições eletrônicas foram realizadas utilizando 3 instrumentos diferentes: lima compatível com o forame apical (lima ajustada), lima um tamanho menor (lima intermediária), e lima dois tamanhos menores (lima desajustada). As distâncias da ponta dos instrumentos ao FA e a CA foram determinadas em software específico. A precisão na CA e no FA para os instrumentos desajustado, intermediário e ajustado foram: 80% / 88% / 83% e 78% / 83% / 95% (Root ZX); 80% / 85% / 80% e 68% / 73% / 73% (Novapex), e 78% / 80% / 78% e 65% / 78% / 70% (Justy II). Considerando os erros médios oferecidos pelos aparelhos, foi encontrada diferença estatística apenas nas determinações realizadas com os instrumentos adaptados tendo como referência o FA onde o Root ZX ofereceu os melhores resultados. O teste qui-quadrado apresentou diferenças significantes entre as determinações consideradas aceitáveis para a CA e o FA tanto para o Justy II quanto para o Novapex (± 0,5 mm), independente do ajuste do instrumento. Nas condições do presente estudo todos os dispositivos ofereceram medições aceitáveis independentemente do ajuste dos instrumentos; exceção feita apenas ao Root ZX, que teve seu desempenho melhorado quando uma lima ajustada foi utilizada. Os aparelhos Justy II e Novapex ofereceram determinações mais próximas à CA.

    This study evaluated the accuracy of three electronic apex locators (Root ZX, Novapex, and Justy II) in root canal length determinations using different apical file sizes, considering the apical constriction (AC) and the major foramen (MF) as anatomic references. The diameter of the apical foramina of 40 single-rooted teeth was determined by direct visual measurement and the master apical file was established. Electronic measurements were then performed using 3 instruments: the selected master apical file (adjusted file), one size smaller (intermediate file), and two sizes smaller (misfit file). The distances from the tip of files fixed in the canals to the MF and to the AC were measured digitally. Precision at AC and at MF for the misfit, intermediate and adjusted apical files was as follows: 80%/88%/83% and 78%/83%/95% (Root ZX); 80%/85%/80% and 68%/73%/73% (Novapex); and 78%/80%/78% and 65%/78%/70% (Justy II). Considering the mean discrepancies, statistically significant differences were found only for the adjusted file at MF, with Root ZX presenting the best results at MF. The chi-square test showed significant differences between the acceptable measurements at AC and at MF for the Justy II and Novapex (± 0.5 mm) regardless of file adjustment. Under the conditions of the present study, all devices provided acceptable electronic measurements regardless of file adjustment, except for Root ZX which had its performance improved significantly when the precisely fit apical file was used. Justy II and Novapex provided electronic measurements nearest to the AC.
  • Interfacial adaptation of an epoxy-resin sealer and a self-etch sealer to root canal dentin using the System B or the single cone technique

    O objetivo deste estudo laboratorial foi avaliar comparativamente a adaptação na interface obturação/dentina em canais mesiais de molares inferiores obturados com System B, tanto com guta percha/ThermaSeal Plus como com Resilon/Real Seal SE. Os canais mesiais de 60 molares inferiores foram modelados utilizando o sistema rotatório K3, realizando o batente apical com o instrumento 35,04. Em seguida os canais foram obturados com guta percha/ThermaSeal Plus (n=15) e com Resilon/Real Seal SE (n=15) por meio do System B. Obturações com a técnica do cone único utilizando os dois materiais foram empregadas como controle. Os cimentos foram corados com Rodamina B e os dentes obturados foram seccionados a 2, 4 e 6 mm do ápice. A adaptação da interface marginal foi avaliada utilizando a microscopia confocal. A análise estatística foi realizada empregando o teste Kruskall-Wallis (p<0,05). De modo geral, a análise microscópica mostrou um padrão regular na distribuição de fendas na interface cimento-dentina, principalmente para os dois grupos com cimento Real Seal SE. Sendo que os outros dois grupos (com ThermaSeal Plus), apresentaram uma quantidade significativamente menor de áreas com fendas, independentemente da técnica de obturação (p<0,05). A melhor adaptação marginal foi observada quando se utilizou guta percha e o cimento à base de resina epóxica, tanto com o System B como com cone único.

    The aim of this laboratory study was to compare the interfacial adaptation of an epoxy-resin and a self-etch sealer in mesial root canals of mandibular molars filled using the System-B/Elements Obturation Unit. Sixty mesial root canals of mandibular molars were prepared using the K3 rotary system up to 35.04 instrument and then filled with the aid of the System-B/Elements Obturation Unit using either gutta-percha/ThermaSeal Plus (n=15) or Resilon/Real Seal SE (n=15). The single cone technique using both materials was used as control. The sealers were stained with Rhodamine B dye and the teeth were filled and sectioned at 2, 4 and 6 mm from the apex. The interfacial marginal adaptation of sealers was evaluated using confocal microscopy. Statistical analyses were performed by the Kruskall-Wallis test (α=0.05). In general, microscopic analysis showed a quite regular gap distribution pattern at sealer-dentin interfaces, mainly for the two groups filled with Real Seal SE. This means that both nonbonding root-filling groups (ThermaSeal Plus) showed significantly higher amount of gap-free regions regardless of the filling technique (p<0.05). Better marginal adaptation was obtained with gutta-percha and epoxy resin-based sealer using either the system B or the single cone technique.
  • Chitosan: effect of a new chelating agent on the microhardness of root dentin

    Avaliou-se o efeito das soluções de quitosana 0,2%, EDTA 15% e ácido cítrico 10% sobre a microdureza da dentina radicular. Foram utilizados 13 incisivos centrais superiores humanos, os quais tiveram suas coroas seccionadas transversalmente e desprezadas. Dez raízes foram incluídas em resina acrílica de rápida polimerização e o bloco formado raiz/resina adaptado à maquina de corte. Desprezou-se o primeiro corte transversal da porção cervical e dividiu-se o segundo, em 4 quadrantes. Cada quarto foi destinado à confecção do corpo-de-prova obtendo-se 4 espécimes para cada raiz, um para cada solução (n=10): EDTA a 15%, ácido cítrico a 10%, quitosana a 0,2% e água destilada (controle). Os espécimes receberam 50 μ L da solução por 5 min, sendo em seguida, lavados com água destilada. Utilizou-se um microdurômetro (dureza Knoop) com carga de 10 g durante 15 s. Os dados foram avaliados por meio do teste ANOVA e Tukey-Kramer (α=0,05). Três incisivos centrais superiores foram instrumentados e irrigados, ao final da biomecânica, com uma das soluções estudadas. Os espécimes foram levados para MEV e posterior análise qualitativa. Todas as soluções avaliadas reduziram a microdureza da dentina radicular de forma semelhante entre si (p>0,05) e estatisticamente diferente do controle (p<0,01). As fotomicrografias mostraram que as 3 soluções removeram a smear layer do terço médio do canal radicular. Concluiu-se que as soluções de quitosana 0,2%, EDTA 15% e ácido cítrico 10% apresentam efeito semelhante na redução da microdureza dentinária.

    The effect of solutions of 0.2% chitosan, 15% EDTA and 10% citric acid on the microhardness of root dentin was evaluated comparatively in this study. Thirteen sound human maxillary central incisors were selected and decoronated at the cementoenamel junction. Ten roots were set into rapid polymerization acrylic resin and the root/resin block was fitted to the cutting machine to obtain slices from the cervical third. The first slice was discarded and the second slice was divided into four quadrants. Each quadrant was used to construct a sample, so that 4 specimens were obtained from each root slice, being one for each chelating solution to be tested: 15% EDTA, 10% citric acid, 0.2% chitosan and distilled water (control). The specimens were exposed to 50 μL of the solution for 5 min, and then washed in distilled water. A microhardness tester (Knoop hardness) with a 10 g load was used for 15 s. Data were analyzed statistically by one-way ANOVA and Tukey-Kramer test (α=0.05). The other 3 roots had the canals instrumented and irrigated at the end of the biomechanical preparation with the test solutions, and then examined by scanning electron microscopy (SEM) for qualitative analysis. All solutions reduced the microhardness of root dentin in a way that was statistically similar to each other (p>0.05) but significantly different from the control (p>0.05). The SEM micrographs showed that the three solutions removed smear layer from the middle third of the root canal. In conclusion, 0.2% chitosan, 15% EDTA and 10% citric acid showed similar effects in reducing dentin microhardness.
  • Adhesive bonding of resin cements to cast titanium with adhesive primers

    O objetivo deste estudo foi avaliar o efeito da aplicação de primers na resistência de união de cimentos resinosos ao titânio. Quatro primers - Metaltite, Metal Primer II, Alloy Primer and Ceramic Primer - e seus respectivos cimentos resinosos - Bistite II DC, Link Max, Panavia F 2.0, RelyX Unicem and RelyX ARC - foram testados. Placas de titânio foram preparadas a partir da fundição de lingotes (n=6 espécimes/cimento) e as superfícies dessas placas foram jateadas com partículas de Al2O3 (50 μ m). Três cilindros de cimento resinoso foram construídos em cada placa de titânio, utilizando moldes transparentes com formato de cilindro e foram submetidos ao teste de microcisalhamento. Os dados foram analisados pela ANOVA (dois fatores) e teste de Tukey (α=0,05). A aplicação do Metal Primer II e do Ceramic Primer resultou no aumento significativo da resistência de união para os cimentos resinosos Link Max e RelyX Unicem, respectivamente. Os cimentos Panavia F 2.0 e o RelyX ARC mostraram altos valores de resistência de união com ou sem o uso dos primers. A aplicação dos primers pode aumentar a resistência de união dos cimentos resinosos ao titânio, dependendo do tipo de cimento resinoso utilizado.

    The purpose of this study was to evaluate the effects of adhesive primer applications on the bond strength of resin cements to cast titanium. Four adhesive primers - Metaltite, Metal Primer II, Alloy Primer and Ceramic Primer - and their respective resin cements - Bistite II DC, Link Max, Panavia F 2.0, RelyX Unicem and RelyX ARC - were tested. Cast plates were prepared from titanium ingots (n=6 specimens/cement) and had their surfaces airborne-particle abraded with Al2O3 (50 μ m). Three resin cement cylinders were built on each bonded titanium surface, using a cylindrical translucent tubing mold and were subjected to micro-shear testing. Data were analyzed statistically by two-way ANOVA and Tukey's post-hoc test (α=0.05). The application of Metal Primer II and Ceramic Primer resulted in significant higher bond strength for Link Max and RelyX Unicem resin cements, respectively, than nonuse of adhesive primers. Panavia F 2.0 and RelyX ARC yielded high bond strength means with or without adhesive primers. The use of adhesive primers might increase the bond strength to cast titanium depending on the resin cement used.
  • Effect of bonding material, etching time and silane on the bond strength of metallic orthodontic brackets to ceramic

    O objetivo neste estudo foi avaliar a resistência de união de bráquetes a cerâmica feldspática com diferentes tempos de condicionamento ácido, materiais de união, com ou sem aplicação do silano. Cilindros de cerâmica feldspática foram condicionados com ácido fluorídrico 10% por 20 ou 60 s. Para cada tempo de condicionamento, metade dos cilindros recebeu duas camadas de silano. Bráquetes metálicos foram fixados aos cilindros utilizando Transbond XT (3M Unitek) ou Fuji Ortho LC (GC). A fotoativação foi realizada com tempo de exposição total de 40 s utilizando LED UltraLume 5. O ensaio de resistência ao cisalhamento foi realizado após 24 h de armazenagem. Os dados foram submetidos à Análise de Variância a três fatores e teste de Tukey (α=0,05). O Índice de Remanescente Adesivo (IRA) foi utilizado para avaliar a quantidade de adesivo remanescente na superfície cerâmica com magnificação de 8×. Os espécimes condicionados por 60 s apresentaram maior resistência de união comparado com 20 s. A aplicação do silano foi efetiva no aumento da resistência de união ao cisalhamento de bráquetes com a cerâmica para ambos os materiais. O Transbond XT mostrou resistência de união significativamente maior que Fuji Orth LC. O IRA mostrou predominância de escore 0 (ausência de resina na superfície cerâmica) para todos os grupos, com aumento de escores 1, 2 e 3 (aumento da presença de resina na superfície cerâmica) para o tempo de condicionamento de 60 s. Em conclusão, o tempo de condicionamento de 60 s, aplicação de silano e resina Transbond XT melhoraram significativamente a resistência de união ao cisalhamento de bráquetes a cerâmica feldspática.

    The purpose of this study was to evaluate the bond strength of metallic orthodontic brackets to feldspathic ceramic with different etching times, bonding materials and with or without silane application. Cylinders of feldspathic ceramic were etched with 10% hydrofluoric acid for 20 or 60 s. For each etching time, half of the cylinders received two layers of silane. Metallic brackets were bonded to the cylinders using Transbond XT (3M Unitek) or Fuji Ortho LC (GC). Light-activation was carried out with total exposure time of 40 s using UltraLume 5. Shear bond strength testing was performed after 24 h storage. Data were submitted to three-way ANOVA and Tukey's test (α=0.05). The adhesive remnant index (ARI) was used to evaluate the amount of adhesive remaining on the ceramic surface at ×8 magnification. Specimens etched for 60 s had significantly higher bond strength compared with 20 s. The application of silane was efficient in increasing the shear bond strength between ceramic and both fixed materials. Transbond XT showed significantly higher (p<0.05) bond strength than Fuji Orth LC. There wasa predominance of ARI score 0 (clean ceramic failure surface) for all groups, with an increase in scores 1, 2 and 3 (adhesive material increasingly present on ceramic failure aspect) for the 60-s etching time. In conclusion, 60-s etching time, silane and Transbond XT improved significantly the shear bond strength of brackets to ceramic.
  • Cigarette smoke inhalation influences bone healing of post-extraction tooth socket: a histometric study in rats

    O objetivo do estudo foi avaliar a influência da inalação da fumaça de cigarros (IFC) sobre os períodos iniciais de reparo ósseo alveolar. Quarenta ratos Wistar foram aleatoriamente divididos em teste: animais expostos à IFC, que se iniciou 3 dias antes das extrações mantendo-se até o sacrifício (n=20)e controle (animais que não foram expostos à IFC; n=20). Os animais tiveram seus segundos molares inferiores extraídos bilateralmente (n=5/grupo/período) e foram sacrificados nos dias 3, 7, 10 e 14 dias após a cirurgia. Imagens digitalizadas foram analisadas de acordo com os seguintes parâmetros histométricos: tecido osteóide (OT), área remanescente (RA), tecido mineralizado (MT) e tecido não mineralizado (NMT) em cada alvéolo. Análise intergrupo mostrou que não houve diferença estatisticamente significante aos 3 dias (p>0,05) para todos os parâmetros. Aos 7 dias, a IFC influenciou negativamente (p?0,05) o preenchimento ósseo mostrando diferença estatisticamente significante para os parâmetros tecido não mineralizado e defeito remanescente (MT: 36%, NMT: 53%, RA: 12%; and MT: 39%, NMT: 29%, RA: 32%, para controle e teste, respectivamente). No entanto, não houve diferença estatisticamente significante (p>0,05) aos 10 e 14 dias. Conclui-se que a IFC pode afetar o reparo ósseo alveolar inicial, o que pode ser crítico para a quantidade e qualidade da nova formação óssea em fumantes.

    The aim of this study was to evaluate, histometrically, the bone healing of the molar extraction socket just after cigarette smoke inhalation (CSI). Forty male Wistar rats were randomly assigned to a test group (animals exposed to CSI, starting 3 days before teeth extraction and maintained until sacrifice; n=20) and a control group (animals never exposed to CSI; n=20). Second mandibular molars were bilaterally extracted and the animals (n=5/group/period) were sacrificed at 3, 7, 10 and 14 days after surgery. Digital images were analyzed according to the following histometric parameters: osteoid tissue (OT), remaining area (RA), mineralized tissue (MT) and non-mineralized tissue (NMT) in the molar socket. Intergroup analysis showed no significant differences at day 3 (p>0.05) for all parameters. On the 7th day, CSI affected negatively (p<0.05) bone formation with respect to NMT and RA (MT: 36%, NMT: 53%, RA: 12%; and MT: 39%, NMT: 29%, RA: 32%, for the control and test groups, respectively). In contrast, no statistically significant differences (p>0.05) were found at days 10 and 14. It may be concluded that CSI may affect socket healing from the early events involved in the healing process, which may be critical for the amount and quality of new-bone formation in smokers.
  • Supragingival plaque removal with and without dentifrice: a randomized controlled clinical trial

    O objetivo deste estudo foi comparar a eficácia de remoção mecânica da placa dental através da escovação com e sem dentifrício. Vinte e quatro estudantes com idade de 17 a 28 anos participaram deste ensaio clínico randomizado. Os quadrantes 1-3 ou 2-4 foram randomizados para alocação no grupo teste (escovação sem dentifrício) ou grupo controle (escovação com dentifrício). Após 72 h de cessação de higiene bucal, o índice de placa de Quigley & Hein (Turesky) foi avaliado antes e após a escovação por um examinador cego e calibrado. Comparações intra e intergrupo foram realizadas pelo teste t pareado, a um nível de significância de 5%. Os resultados demonstraram que, após a escovação, ambos os grupos apresentaram redução significativa de placa, porém sem diferenças intergrupos (3,06 ± 0,54 a 1,27 ± 0,26 versus 3,07 ± 0,52 a 1,31 ± 0,23). Uma análise separada das faces vestibulares e linguais também não revelou diferenças entre os grupos. Conclui-se que a utilização de dentifrícios associado ao controle mecânico parece não contribuir para a remoção da placa dental.

    The aim of this study was to compare the efficacy of dental plaque removal by brushing with and without conventional dentifrice. Twenty-four students aged 17 to 28 years participated in this randomized controlled clinical trial. Quadrants 1-3 or 2-4 were randomly allocated to the test group (brushing without dentifrice) or control group (brushing with dentifrice). After 72 h of cessation of oral hygiene, Quigley & Hein (Turesky) plaque index was assessed before and after brushing by a calibrated and blind examiner. Overtime and intergroup comparisons were performed by Student's paired sample t-test at 5% significance level. The results showed that both groups after toothbrushing presented statistically significant reductions in plaque, with no differences between them (from 3.06 ± 0.54 to 1.27 ± 0.26 versus from 3.07 ± 0.52 to 1.31 ± 0.23). A separate analysis of the buccal and lingual aspects also showed no significant differences between groups. It may be concluded that the use of a conventional dentifrice during toothbrushing does not seem to enhance plaque removal capacity.
  • Reorganization of secondary and tertiary health care levels: impact on the outcomes of oral cancer screening in the São Paulo State, Brazil

    Este estudo teve como objetivo apresentar as estratégias de prevenção e detecção precoce de câncer bucal, através de triagem (rastreamento) na população idosa do Estado de São Paulo. Esta pesquisa foi um estudo longitudinal retrospectivo baseado na análise de dados secundários. As variáveis -número de cidades participantes, a cobertura da triagem e o número de casos suspeitos e confirmados de câncer bucal - foram divididos em 2 períodos (2001-2004 e 2005-2008). As variáveis foram testadas através do teste do qui-quadrado. A implantação da política nacional de saúde bucal, em 2004, foi incluída como variável capaz de interferir nos resultados alcançados. De 2001 a 2008, 2.229.273 exames foram realizados. Foi observado um aumento de 205 cidades participantes até o final do período estudado (p<0,0001). A extensão do rastreamento para o câncer de boca aumentaram de 4,1% para 16% (p<0,0001). Houve uma diminuição no número de lesões suspeitas (de 9% em 2005 para 5% em 2008) (p<0,0001) e na taxa de casos confirmados de câncer bucal por 100.000 (20,89 em 2001 e 10,40 em 2008) (p=0,0003). Após 8 anos de triagem, observamos uma diminuição no número de lesões suspeitas e casos confirmados de câncer bucal entre os voluntários. É possível que a reorganização dos níveis secundário e terciário de cuidados bucais ajudou na modificação desses números.

    This study presents the strategies for prevention and early detection of oral cancer by means of screening in the elderly population of São Paulo, the richest and the most populous state of Brazil. This research was a retrospective longitudinal study based on the analysis of secondary data. The variables - number of participating cities, coverage of screening, and number of suspicious and confirmed cases of oral cancer - were divided into two periods: 2001-2004 and 2005-2008. Data were analyzed statistically by the chi-square test at 5% significance level. The implementation of a nationwide public oral health policy in 2004 and the reorganization of the secondary and tertiary health care were evaluated as mediator factors able to interfere in the achieved outcomes. From 2001 to 2008, 2,229,273 oral examinations were performed. There was an addition of 205 participating cities by the end of the studied period (p<0.0001). The coverage of oral cancer screening increased from 4.1% to 16% (p<0.0001). There was a decrease in the number of suspicious lesions (from 9% in 2005 to 5% in 2008) (p<0.0001) and in the rate of confirmed oral cancer cases per 100,000 examinations (from 20.89 in 2001 to 10.40 in 2008) (p<0.0001). After 8 years of screening, there was a decrease in the number of suspicious lesions and confirmed cases of oral cancer in the population. The reorganization of secondary and tertiary health care levels of oral care seems to have contributed to modify these numbers, having a positive impact on the outcomes of oral cancer screening in the São Paulo State.
  • Behavioral problems and emotional stress in children with bruxism

    A etiologia do bruxismo é multifatorial, e fatores psicossociais têm sido apontados como notáveis potencializadores do risco para a ocorrência dessa parafunção. O presente estudo teve como objetivo avaliar o perfil comportamental de um grupo de crianças com bruxismo. Oitenta crianças de ambos os sexos, com idade entre 7 e 11 anos (idade media de 8,8 anos) foram recrutadas como possíveis participantes. Vinte e nove crianças, 18 do sexo masculino e 11 do feminino, cujos pais ou responsáveis relataram que as mesmas apresentavam rangimento dental freqüente (3 noites por semana) ou apertamento/rangimento dental em vigília, nos últimos 3 meses, foram selecionadas para o estudo. O diagnóstico de bruxismo foi estabelecido com base no relato dos pais ou responsáveis sobre os hábitos cotidianos das crianças e possíveis desordens do sistema estomatognático aliados à presença de sinais e sintomas tais como dor à palpação da musculatura mastigatória, hipertrofia do músculo masseter, facetas de desgaste, fratura de restaurações, impressões dentais na bochecha e lingual. Como parte da avaliação psicológica, a Escala Comportamental Infantil A2 de Rutter foi aplicada aos pais ou responsáveis (um por criança) e a Escala de Stress Infantil foi aplicada às crianças. Os dados foram tabelados e analisados descritivamente com base na freqüência de cada variável estudada. Vinte e quatro (82,76%) crianças necessitavam de intervenção psicológica ou psiquiátrica, sendo que 17 crianças apresentavam desordens neuróticas e 7 apresentavam desordens anti-sociais. Seis (20,70%) crianças apresentaram manifestações físicas e psicológicas de stress. Os resultados do presente trabalho sugerem que problemas comportamentais e potencialidades para o desenvolvimento de desequilíbrios emocionais podem ser fatores de risco para o bruxismo em crianças.

    Bruxism has a multifactorial etiology, and psychosocial factors have been considered to increase the risk of occurrence of this parafunction. The aim of this study was to evaluate the behavior profile of a group of children diagnosed with bruxism. Eighty 7-11-year-old children of both genders (mean age 8.8 years) first recruited as eligible participants. Twenty-nine children (18 males and 11 females) whose parents/guardians reported to present frequent episodes of tooth grinding/clenching while awake or during sleep (at least 3 nights a week) in the previous 3 months were enrolled in the study. The diagnosis of bruxism was established based on the parents/guardians' report about the children's behavior, habits and possible discomforts in the components of the stomatognathic system allied to the presence of signs and symptoms such as pain on the masticatory muscles, masseter muscle hypertrophy, wear facets, fractures of restorations, dental impressions on the cheek mucosa and tongue. As part of the psychological evaluation, the Rutter's Child Behavior Scale-A2 was applied to the parents/caregivers (one for each child) and the Child Stress Scale was applied to the children. Data were analyzed descriptively based on the frequency of each studied variable. Twenty-four (82.76%) children needed psychological or psychiatric intervention; 17 of them presented neurotic disorders and 7 children presented antisocial disorders. Six (20.70%) children presented significant physical and psychological manifestations of stress. The findings of the present study suggest that behavioral problems and potential emotional problems can be risk factors to bruxism in children.
  • Coronoid process hyperplasia: an unusual cause of mandibular hypomobility

    Um grande número de distúrbios pode provocar limitação no grau de abertura bucal. As condições mais comuns relacionadas com esse problema são aquelas que envolvem a articulação temporomandibular (ATM) e músculos mastigatórios, em que, além da limitação, a dor facial é um achado comum. Distúrbios congênitos ou de desenvolvimento da mandíbula também são possíveis causas para limitação de abertura bucal, embora possuam uma baixa prevalência. A hiperplasia do processo coronóide (HPC) é um exemplo dessas causas, caracterizada por um desenvolvimento excessivo do processo coronóide, em que o movimento mandibular torna-se limitado pela impacção dessa mesma estrutura na parte posterior do osso zigomático. Esta condição é rara, indolor, geralmente bilateral e progressiva, afetando principalmente homens. O diagnóstico de HPC é feito por meio dos sinais clínicos de abertura bucal limitada associado com exames de imagem, especialmente a radiografia panorâmica e tomografia computadorizada (TC). O tratamento é exclusivamente cirúrgico. O objetivo desse artigo é apresentar um caso de paciente do sexo masculino com hiperplasia bilateral do processo coronóide, inicialmente diagnosticado com deslocamento de disco sem redução bilateral, e tratado com sucesso com coronoidectomia intraoral. Assim, é enfatizada a importância do diagnóstico diferencial para um correto diagnóstico e, consequentemente, estratégias efetivas de tratamento.

    A large number of disorders affecting the masticatory system can cause restriction of mouth opening. The most common conditions related to this problem are those involving the temporomandibular joint (TMJ) and the masticatory muscles, when facial pain also is an usual finding. Congenital or developmental mandibular disorders are also possible causes for mouth opening limitation, although in a very small prevalence. Coronoid process hyperplasia (CPH) is an example of these cases, characterized by an excessive coronoid process growing, where mandibular movements become limited by the impaction of this structure on the posterior portion of the zygomatic bone. This condition is rare, painless, usually bilateral and progressive, affecting mainly men. Diagnosis of CPH is made based on clinical signs of mouth opening limitation together with imaging exams, especially panoramic radiography and computerized tomography (CT). Treatment is exclusively surgical. This paper presents a case of a male patient with bilateral coronoid process hyperplasia, initially diagnosed with bilateral disk displacement without reduction, and successfully treated with intraoral coronoidectomy. It is emphasized the importance of differential diagnosis for a correct diagnosis and, consequently, effective management strategy.
  • The challenges of treating a fused tooth

    Este caso descreve o tratamento multidisciplinar de um incisivo lateral superior permanente fusionado a um dente supranumerário, ambos apresentando necrose pulpar e lesão periapical. Paciente compareceu ao consultório se queixando de dor, edema e mobilidade do incisivo lateral superior. O preparo endodôntico foi realizado no sentido coroa-ápice e foi colocada pasta de hidróxido de cálcio como medicação intra-canal por 15 dias. A seguir, a obturação foi realizada pela termoplastificação da guta-percha. Após 18 meses, foram realizados exames clínicos e radiográficos indicando ausência de dor e edema. Dois anos após o tratamento endodôntico, o paciente retornou para o tratamento periodontal e estético. Nove meses depois, foi realizada tomografia computadorizada e observou-se presença de defeito periodontal e lesão periapical. Foi então indicada e planejada a cirurgia paraendodôntica. O dente supranumerário foi removido e a área de comunicação com o canal radicular do incisivo lateral foi preenchida; a loja cirúrgica foi preenchida com vidro bioativo e osso orgânico bovino desmineralizado. O tecido patológico da lesão periapical foi submetido à análise histopatológica sendo diagnosticado como cisto periapical. Um ano após a cirurgia parendodôntica, uma nova tomografia computadorizada mostrou neoformação óssea na região periapical do dente em questão. Doi anos após a cirurgia, a restauração foi trocada devido à motivos estéticos e uma radiografia periapical mostrou sucesso do tratamento (5 anos após o tratamento inicial). Um correto diagnóstico e plano de tratamento multidisciplinar é essencial para o sucesso do tratamento de dentes fusionados à dentes supranumerários.

    This paper describes and discusses the multidisciplinary treatment involving a permanent maxillary lateral incisor fused to a supernumerary tooth, both presenting pulp necrosis and periapical lesion. A 15-year-old male patient sought treatment complaining of pain, swelling and mobility on the maxillary right lateral incisor. After clinical and radiographic examination, root canal preparation was performed according to the crown-down technique and a calcium hydroxide dressing was placed for 15 days. The patient returned and the definitive endodontic filling was done with thermomechanical compaction of gutta-percha and sealer. After 18 months, clinical and radiographic examinations were carried out and no pain or swelling was reported. Two years after endodontic treatment, the patient returned for periodontal and cosmetic treatments. Nine months later, a cone-beam computed tomography (CBCT) revealed that the previously detected periodontal defect and periapical lesion were persistent. Apical endodontic surgery was indicated. The supernumerary tooth was removed, the communicating distal surface was filled and the surgical site received bioactive glass and demineralized bovine organic bone. The pathological tissue was submitted to histopathological examination and the diagnosis was periapical cyst. One year after the apical endodontic surgery, CBCT showed bone formation at maxillary lateral incisor apical area. Two years after the surgery, the restoration was replaced due to aesthetic reasons and periapical radiograph showed success after 5 years of treatment. A correct diagnosis and establishment of an adequate treatment plan resulted in a successful management of the case.
