  • Simulação em lombalgia: diagnóstico e prevalência Artigo Original

    OBJETIVO: Fazer uma revisão da literatura para diagnóstico de simulação em lombalgia e avaliar sua prevalência nos pacientes que pleiteiam benefício previdenciário. MÉTODOS: Foram avaliados 100 pacientes com lombalgia referida, divididos em dois grupos de 50; no grupo I os pacientes requisitaram o relatório médico a fim de obter benefício, no grupo II os pacientes visavam apenas tratamento. Foram considerados simuladores os pacientes que apresentavam dois sinais positivos, dos três avaliados: dor lombar à compressão axial crânio, dor lombar à rotação em bloco do tronco e diferença em sintomatologia na elevação da perna em posição sentado e supina. RESULTADOS: No grupo I a porcentagem de simuladores foi 72%, no grupo II 18%, não houve diferença significativa com relação a idade ou sexo dos pacientes. DISCUSSÃO: Apesar de haver vários sinais e sintomas para avaliar simulação a maioria dos médicos os desconhece, não fazendo uma avaliação adequada dos pacientes com lombalgia referida, contribuindo assim para o inchaço do sistema previdenciário. CONCLUSÃO: Há uma elevada incidência de simuladores entre os pacientes que requerem relatório médico a fim de conseguir benefício previdenciário. Nivel de Evidência II, prospectivo comparativo.

    OBJECTIVE: To carry out a literature review for the diagnosis of low back pain simulation in patients claiming disability benefit from the social security. METHODS: 100 patients with reported low back pain were evaluated. Thesewere divided in two groups of 50 each; in group I, the patients requested a medical report for the purpose of obtaining disability benefit; in group II the patients were interested only in receiving treatment. Simulators were considered as patients who presented at least two positive signs of the three signs assessed: low back pain on axial cranial compression,low back pain on rotation of the pelvis, and difference in straight leg raising in the supine and sitting positions. RESULTS: In group I the percentage of simulators was 72%, in the group II, 18%. There was no significant difference in relation to age or sex. DISCUSSION: Although there are several signs and symptoms to evaluate simulation, most of the doctors ignored them. However, without them, it is difficult to make a correct evaluation of patients with reported low back pain, thereby contributing to overburdeningthe socialsecurity system. CONCLUSION: There is a high incidence of simulators among patients who request medical reports in order to obtain disability benefits. Level of Evidence: Level II, prospective comparative study.
  • Navegação na artroplastia total do joelho: existe vantagem? Artigo Original

    OBJETIVO: Avaliar se a cirurgia auxiliada por navegação oferece vantagens sobre a técnica convencional, foi desenvolvido um estudo randômico, prospectivo e comparativo analisando o posicionamento do implante, alinhamento do membro, tempo de cirurgia, perda sanguínea e reabilitação funcional. MÉTODOS: Foram estudados 39 pacientes submetidos a 42 Artroplastias Totais de Joelho. No grupo 1 (n=21) a cirurgia foi navegada e no grupo 2 (n=21) convencional. Os pacientes foram avaliados com radiografia panorâmica de membros inferiores, tomografia computadorizada e pelo Knee Society Score (KSS) no pré-operatório e com três meses de pós-operatório. O tempo de cirurgia e a perda sanguínea pós-operatória também foram avaliados. RESULTADOS: O tempo de cirurgia foi maior no grupo 1. Não houve diferença significativa quanto à perda sanguínea e ao KSS. Houve maior proximidade dos três graus de rotação externa e dos cinco graus de inclinação coronal nos pacientes do grupo 2. O eixo mecânico aproximou-se mais de zero grau no grupo 1. CONCLUSÕES: A navegação promove um posicionamento individualizado dos componentes da prótese oferecendo vantagens no realinhamento do membro. O tempo cirúrgico tende a ser maior na técnica navegada, porém, não foi estatisticamente significante. Não há diferenças na perda sanguínea e na melhora do KSS.     OBJECTIVE: Aiming to evaluate whether surgery aided navigation offers any advantages over the conventional technique, a randomized, prospective, comparative study was carried out, analyzing the placement of components, alignment of the limb, surgical time, blood loss and functional rehabilitation. MATERIAL AND METHODS: We studied 39 patients submitted42 Total Knee Arthroplasties. In group 1 (n = 21) the surgery was navigated and in group 2 (n = 21) it was conventional. The patients were evaluated on panoramic radiographs of the lower limbs, CT scan and the Knee Society Score (KSS) preoperatively and three months postoperatively. Surgery time and postoperative blood loss were also evaluated. RESULTS: Surgery time was longer in group 1. There was no significant difference in blood loss and the KSS. There was a greater proximity of the three degrees of external rotation and the five degrees of tilt in the coronal in group 2. The mechanical axis was closer to zero degree in group CONCLUSIONS: Navigation promotes individualizing of the positioning of the components of the prosthesis, and offered advantages in limb realignment, compared with traditional methods. The surgerytime has a tendency to be higher in group 2, but this difference is not statistically significant. There were no differences between the groups in relation to blood loss and improvement in KSS. Level of Evidence: Level I, therapeutic studies.
  • Tratamento das fraturas do processo odontóide Artigo Original

    OBJETIVO: Este artigo faz a avaliação retrospectiva clínica e radiológica das fraturas do odontóide de 20 pacientes atendidos no IOT-HCFMUSP, durante o período de 2004-2010. MÉTODOS: Foi realizado a estratificação destas fraturas segundo sua classificação (AO/Anderson e D'Alonzo), perfil epidemiológico, tipo de tratamento, tempo de consolidação e complicações. RESULTADOS: Observou-se maior número de casos da fratura do odontóide em pacientes do sexo masculino (4:1), com idade entre 3ª e 4ª década de vida (60%), vítimas de quedas de altura (60%) e acidentes automobilísticos (25%) como principais mecanismos de trauma, e 15% dos casos apresentaram-se com déficit neurológico. A fraturas do odontóide de maior prevalência foram as do tipo II (55%), seguidas pelas fraturas do tipo III (40%). Os tratamentos mais empregados nas fraturas do tipo II e III foram, respectivamente, o cirúrgico (73%) e conservador (87,5%). A consolidação ocorreu em até 16 semanas para 87,5% dos casos de tratamento cirúrgico e para 54,5% dos tratados conservadoramente. Não houve casos de pseudoartrose. CONCLUSÃO: O tratamento cirúrgico das fraturas do odontóide do tipo II apresentou resultados satisfatórios em relação ao tempo consolidação e baixa incidência de complicações, bem como o tratamento conservador destinado às fraturas do tipo III.Nivel de Evidência IV, série de casos.

    OBJECTIVE: This article describes a clinical and radiologic retrospective analysis of odontoid fractures in20 patients accompanied byThe IOT-HCFMUSP,from 2004 to 2010. METHODS: These fractures were stratified according to their classification (AO/Anderson andD'Alonzo), epidemiologic profile, type of treatment, time to consolidation of the fracture, and complications. RESULTS: It was observed that there was a higher number of odontoid fractures in males (4:1), between the third and fourth decades of life (60%), and that the main causes of the trauma were falling from heights (60%) and car accidents (25%). Also, 15% of the cases presented neurological deficits. The most prevalent type of odontoid fracture was Type II (55%) followed by Type III (40%). The most prevalent type of treatment used for Type II and III fractures was surgical (73%) and non-surgical (87.5%), respectively. Consolidation of the fracture took place within 16 weeks in 87.5% of surgically treated cases, and in 54.5% of those treated non-surgically. No cases of pseudoarthrosis were found. CONCLUSION: The surgical treatment of Type II odontoid fractures showed satisfactory results in relation totime to consolidation of the fracture and low incidence of complications, as did the non-surgical treatment used for the Type III fractures.     OBJETIVO: Verificar a correlação do desempenho dos músculos do joelho e os domínios dor, rigidez e funcionalidade do Questionário Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) de idosas com osteoartrite de joelho (OA). MÉTODOS: Estudo transversal observacional com uma amostra de 80 idosas (71,2 ± 5,3 anos) com diagnóstico clínico de OA de joelho. Força, resistência e equilíbrio musculares foram avaliados por meio do dinamômetro isocinético Biodex System 3 Pro, nas velocidades angulares de 60º/s e 180º/s; a funcionalidade, presença de dor e rigidez foram avaliadas pelo questionário WOMAC. A correlação entre as variáveis foi analisada pelo teste de Spearman. RESULTADOS: Houve correlação inversa significativa da força e resistência musculares do quadríceps (QUA) e isquiossurais (IQS) nas velocidades de 60º/s e 180°/s, respectivamente, e da relação de equilíbrio muscular IQS/QUA a 180°/s com todos os domínios do WOMAC (p<0.05). CONCLUSÕES: A redução da força e da resistência e a presença de desequilíbrio dos músculos do joelho correlacionaram-se inversamente com todos os domínios do WOMAC, apontando para a necessidade de intervenções que envolvam um trabalho de fortalecimento, resistência e equilíbrio muscular de extensores e flexores de joelho, visando a redução do impacto da OA na dor, rigidez e funcionalidade do idoso.     OBJECTIVE: To determinethe correlation between performance of the knee muscles and pain, stiffness, and functionality, through theWestern Ontario and McMaster Universities Osteoarthritis Index (WOMAC) Questionnaire applied to an elderly population with osteoarthritis of the knee (OA). METHODS: This study uses an observational, cross-sectional approach applied to a convenience sample of 80 elderly individuals (71.2 ± 5.3 years of age) with a clinical diagnosis of OA of the knee. Muscle strength, resistance, and balance of the knee were evaluated using the Biodex System 3 Pro isokinetic dynamometer at angularspeedsof 60º/s and 180º/s. The self-reported functionality, presence of pain, and stiffness were evaluated by the WOMAC questionnaire. The correlation between the variables was analyzed bySpearman's coefficient of correlation (α = 0.05). RESULTS: A significant inverse correlation was observed between muscle strength and resistance of the quadriceps muscle (Q) and the hamstring muscle (H) at speeds of 60º/s and 180°/s, respectively, as well as in the relation between H/Q muscle balance at 180°/s and all domains of the WOMAC (p?0.05). CONCLUSIONS: The reduction in strength, resistance, and presence of imbalance in the knee muscles are inversely correlated with all the domains of the WOMAC in elderly individuals with OA. These results indicate a need for intervention that involves strengthening, resistance, and balance of the knee extensor and flexor muscles, aimed at reducing the impact of OA in relation to pain, stiffness, and functionality in elderly individuals.   • A eletrocauterização isolada como adjuvante no tratamento dos tumores ósseos benignos Artigo Original

    OBJETIVOS: Avaliar o papel da eletrocauterização como método adjuvante isolado na ressecção intralesional dos tumores ósseos benignos, levando em conta fatores associados à recorrência dessas lesões. MÉTODOS: Foram incluídos 47 pacientes com diagnóstico de tumores ósseos benignos que foram tratados com curetagem associada a eletrocauterização como método adjuvante. Os pacientes foram acompanhados por um tempo médio de 32,7± 19,4 meses e foi avaliada a taxa de recidiva em relação às seguintes variáveis: idade, sexo, localização do tumor, tipo histológico, estadiamento (B1, B2, B3), tamanho, tipo de material utilizado como preenchimento (PMMA ou enxerto ósseo). A taxa e o tipo de complicação pós-operatória também foram analisadas. RESULTADOS: A recidiva global foi de 19,1%, havendo correlação significante do grau do tumor em relação ao tempo de recidiva (p=0,028). Não foram observados outros fatores diretamente relacionados à recidiva local. CONCLUSÃO: A eletrocauterização mostrou-se um método simples, barato e eficaz no tratamento adjuvante dos tumores ósseos benignos. Nivel de Evidência IV, serie de casos.

    OBJECTIVES: To evaluate the efficacy of cauterization as a sole adjuvant method inintralesional resection of benign bone tumors, taking into account associated factors that could influence local recurrence. METHODS: 47 patients diagnosed with different benign bone tumors were included in this study, all of whom had been treated with curettage associated with cauterization as an adjuvant method. The medium follow-up was 32.7 ± 19.4 months. The recurrence rate was then evaluated in relation to age, sex, location of the tumor, histologic type, tumor grade ( B1, B2 or B3 ), and size and type of material used to fill the tumor cavity ( PMMA or autologus bone). The type and percentage of complications were also included in the analysis. RESULTS: Global recurrence was 19.1%. There was statistic significance relating tumor grade to time to recurrence (p=0,028). No other factors were found that directly correlated with global recurrence of the tumors. CONCLUSION: Cauterization proved to be a simple, inexpensive and effective adjuvant method for the treatment of benign bone tumors. Level of Evidence: Level IV, case series.
  • Fluxograma diferencial entre a artrite séptica e sinovite transitória do quadril em crianças Artigo Original

    O diagnóstico diferencial entre a artrite séptica e a sinovite transitória do quadril não é fácil de ser realizado, pois não há um exame que seja simples, nem satisfatoriamente sensível e específico para diferenciá-las. Sendo assim, é muitas vezes utilizada uma propedêutica armada de exames que demanda maior custo e requer melhor infra-estrutura da instituição de saúde. Perante isso, torna-se evidente a necessidade de uma análise multifatorial dos dados clínicos e laboratoriais envolvidos para a proposição de um fluxograma em que se possa racionalizar exames visando a correta abordagem e evitar a indicação de procedimentos, muitas vezes, desnecessários como a ressonância magnética ou mais invasivos como a artrocentese e a própria drenagem cirúrgica. Realizamos uma ampla revisão da literatura nas bases de dados do Pubmed e Cochrane até maio de 2009 em que foi analisada a importância do exame clínico, dos testes laboratoriais e de imagem para a diferenciação entre as duas afecções. Mediante o cruzamento dos dados foi elaborado um fluxograma para o diagnóstico e conduta na criança e no adolescente, com idade de seis meses a dezoito anos, com sintoma de dor no quadril, na suspeita de quadro inflamatório.     The distinguishing diagnosis between the septic arthritis and the transitory synovitis of a hip is not easy to be carried through; therefore, there is not a simple, nor sensible and satisfactorily specific examination to differentiate them. Thus, the use of a propedeutic set of examinations becomes necessary to demand greater cost and depend on a bigger infrastructure of the health institution. First of all, the necessity of a multifactorial analysis on a flowchart is evident, so it can rationalize the indication of unnecessary or more invasive procedures as artrocentesis, magnetic resonance and the surgical draining. A revision of literature on the databases of Pubmed and Cochrane until May of 2009 was carried through, and the importance of the clinical examination, laboratorial tests and imaging exams was analyzed. By studies on the data, it was elaborated a flowchart for the diagnosis and the conduction of the management on the patient, between the ages of six months and eighteen years old, with complaint of pain on the hip under suspicion of inflammatory picture.   • Tratamento conservador da fratura toracolombar explosão e Classificação de McComack Artigo Original

    OBJETIVO: Correlacionar a classificação de McCormack e o resultado funcional nos portadores de fratura toracolombar explosão tratados com gesso em hiper-extensão ou ortese toracolombossacra. MÉTODOS: Análise retrospectiva de prontuários, radiografias e de tomografia de 31 pacientes no período de 1996 a 2005. O resultado funcional ao final do tratamento obtido mediante as escalas funcionais de dor e trabalho de Denis. RESULTADOS: Dos 31 pacientes avaliados, cinco apresentavam incapacidade total ou parcial para retorno ao trabalho ao final do período de segmento; 26 (83,9 %) estavam aptos ao trabalho com ou sem mudança da atividade trabalhista. Em relação à dor (r=0,258;p=0,161) e função (r=0,204;p=0,272), não houve correlação entre a pontuação da classificação e a função no final do acompanhamento. CONCLUSÃO: Considerando critérios funcionais centrados nos pacientes, não observamos correlação entre a Classificação de McCormack e os resultados do tratamento conservador.     OBJECTIVE: To correlate the McCormack classification and functional outcomes in patients with thoracolumbar burst fractures submitted to conservative management. METHODS: We performed a retrospective study on a consecutive series of 31 patients with thoracolumbar burst fractures treated with cast or brace immobilization between 1996 and 2005.The functional result at theend of the treatment was obtained through Denis' functional scales of pain and work. RESULTS: Of the 31 patients evaluated, five presented total or partial inability return to work at the end of the follow-up period; 26(83.9%) were able to return to work with or without a change in work activity. In relation to pain (r=0.258;p=0.161) and functional scores (r=0.204;p=0.272) there was no correlation between the classification score and function at the end of the follow-up. CONCLUSION: Considering functional criteria focused on the patients, we did not observe any correlation between McCormack's Classification and the results of the conservative treatment.   • Impacto do posicionamento da placa na osteotomia em cunha de fechamento lateral em cúbito varo Artigo Original

    OBJETIVO: O cúbito varo é uma doença muito comum em crianças e adultos, ocasionada por fratura supracondilar. Existem vários procedimentos cirúrgicos e fixações internas para correção do cúbito varo, com diferentes desfechos, embora a fixação interna com placa e parafusos seja o mais comum. Contudo, o impacto do posicionamento da placa sobre a cirurgia raramente foi estudado até agora. MÉTODO: Em nosso estudo, 12 pacientes com cúbito varo foram divididos em dois grupos, operados pelo método de osteotomias em cunha com fechamento lateral e fixações internas com placa e parafusos. Em um grupo, as placas foram colocadas no lado póstero-lateral; no outro, as placas foram colocadas no lado lateral do úmero. RESULTADO: O período de acompanhamento foi 4,5 meses (faixa de 2 a 7 meses). Houve cinco resultados excelentes (83,3%) e um bom (16,7%) em cada grupo. Em todos os casos, a aparência é muito semelhante ao lado oposto; não há diferenças de amplitude de movimento (AM) no cotovelo depois da cirurgia. Um paciente no grupo B teve paralisia nervosa transitória; não houve infecções nem osteomielite. CONCLUSÃO: A posição da placa de fixação interna não tem impacto sobre a osteotomia em cunha de fechamento lateral. Nivel de Evidência II, Prospectivo Comparativo.

    OBJECTIVE: To study the effects of low intensity ultrasound irradiation applied on the spinal cord, in the regeneration of the rat's sciatic nerve after a controlled crush injury, evaluating the functional results of the sciatic functional index as measured on video recorded images of the foot sole. METHODS: Eighteen rats were submitted to a controlled crush injury of the right sciatic nerve, and divided into two groups according to the treatment: Group 1 (n=9), simulated irradiation; Group 2 (n=9), effective irradiation. Low-intensity ultrasound irradiation was started on the 7th postoperative day and applied daily for 6 weeks. Images of the animals´ foot soles were video recorded on a transparenttreadmill belt at weekly intervals until the 6th week of irradiation, and the corresponding sciatic functional index (SFI) was measured usingspecific software. RESULTS: The SFI during the first and last week of treatment was -59.12 and -12.55 in Group 1, -53.31 and -1.32 in Group 2, indicating improvements of 79% and 97%, respectively, but differences between the groups were only significant (p<0.05) during the third week of treatment. CONCLUSION: The authors conclude that low intensity therapeutic ultrasound enhances nerve regeneration, with significance during the 3rd week of treatment.     OBJETIVO: Estudar os efeitos da irradiação ultrassônica de baixa intensidade aplicada sobre a medula espinhal na regeneração do nervo ciático de ratos após lesão por esmagamento controlado, avaliando os resultados pelo índice funcional do ciático (SFI), medido nas imagens vídeo-filmadas das plantas das patas. MÉTODOS: Dezoito ratos foram submetidos a esmagamento controlado (do nervo ciático direito e divididos em dois grupos de acordo com o tratamento: Grupo 1 (n=9), irradiação simulada; Grupo 2 (n=9), irradiação efetiva. Irradiação ultrassônica de baixa intensidade foi iniciada no 7º dia pós-operatório e aplicada diariamente por 6 semanas. Imagens das plantas das patas dos animais foram vídeo-filmadas em uma esteira transparente sob velocidade controlada a intervalos semanais até a 6ª semana de irradiação e o correspondente SFI medido com um programa de computador específico. RESULTADOS: O SFI durante a 1ª e a 6ª semana de tratamento foi de -59,12 e -12,55 no Grupo 1, e -53,31 e -1,32 no Grupo 2, indicando uma melhora de 79% e 97%, respectivamente, mas as diferenças entre os grupos somente foram significantes (p<0,05) durante a 3ª semana de tratamento. CONCLUSÃO: Os autores concluem que o ultrassom terapêutico de baixa intensidade estimula a regeneração nervosa, com significância durante a 3ª semana de tratamento.     Esta revisão da literatura descreve o processo do transplante autólogo de condrócitos em todas as suas etapas, indicações clínicas, técnica operatória, técnica laboratorial, reabilitação e resultados clínicos. Desde 1994, quando a técnica de ACI foi descrita pela primeira vez, este procedimento foi aprimorado e tornou-se uma das mais importantes alternativas cirúrgicas para o tratamento das lesões condrais do joelho. Nivel de Evidência II, Prospectivo Comparativo.

    This literature review article describes the autologous chondrocyte implantation (ACI) process - its stages, clinical indications, surgical technique, laboratory protocol, rehabilitation and clinical outcomes. Since 1994, when the ACI was described for the first time, the procedure has improved to become one of the most important surgical alternatives for the treatment of chondral lesions of the knee.
