    OBJETIVO: investigar a eficácia da fisioterapia respiratória (FR) baseada em evidência clínica e nos parâmetros respiratórios. Estudo prospectivo realizado na Unidade de Transplante de Medula Óssea da Universidade Estadual de Campinas. Dois protocolos diferentes de FR foram previamente estabelecidos e aplicados entre D-1 e D+7 visando à reexpansão pulmonar, a desobstrução brônquica e o fortalecimento muscular. No grupo A aplicaram-se os seguintes exercícios: respiração diafragmática, padrão ventilatório com inspiração fracionada e expiração abreviada, espirômetro de incentivo Respiron®, exercícios com o Shaker®, treinamento da musculatura respiratória com Threshold® IMT, tosse espontânea. No grupo B aplicou-se somente a estimulação inspiratória com o espirômetro de incentivo. Os parâmetros analisados foram: volume corrente (VC), volume minuto (VM), pressão inspiratória máxima (PIM), pressão expiratória máxima (PEM), saturação periférica de oxigênio (SaO²), frequência cardíaca (FC) e frequência respiratória (f). Foram incluídos no estudo 67 casos submetidos ao TCPH; 39 foram avaliados e randomizados em dois grupos: A (estudo) e B (controle). Houve diferença significativa no D+2 e D+7, para VC (p = 0,007) no D+2 e PIM (p = 0,03), PEM (p = 0,03) e VC (p = 0,004) no D+7. O protocolo de FR aplicado obteve uma melhora importante no fortalecimento muscular respiratório e na ventilação dos pacientes submetidos ao TCPH, no grupo A. Este estudo obteve apoio do CNPq (Conselho Nacional de Pesquisa) e FAPESP (Fundação de Amparo à Pesquisa do Estado de São Paulo).

    OBJECTIVE: To investigate the effectiveness of respiratory physiotherapy based on clinical evidence and analyze the improvement in respiratory parameters. METHODS: A prospective study was carried out in the Bone Marrow Transplant Unit of the Universidade Estadual de Campinas (UNICAMP). Two different previously established respiratory physiotherapy protocols were applied from days D-1 to D+7 that aimed to improve airway clearance, pulmonary re-expansion and the strengthening of respiratory muscles. Group A were subjected to diaphragmatic proprioceptive stimulation, breathing exercises, incentive spirometry with Respiron®, inspiratory muscle training with the Threshold® Inspiratory Muscle Training device, bronchial hygienization with Shaker® and cough stimulation. Group B performed a protocol that only used incentive spirometry. The parameters analyzed were: tidal volume, minute volume, maximal inspiratory pressure, maximal expiratory pressure, oxygen saturation, heart rate and respiratory frequency. RESULTS: Sixty-seven patients submitted to myeloablative hematopoietic stem cell transplantation were included in this study. Among these, thirty-nine were evaluated and randomized in the two groups. There were significant differences between the groups for tidal volume at D+2 (p-value = 0.007) and maximal inspiratory pressure (p-value = 0.03), maximal expiratory pressure (p-value = 0.03) and tidal volume (p-value = 0.004) at D+7. CONCLUSION: On comparing Group A with Group B, the authors concluded that the protocol of respiratory physiotherapy applied in this study resulted in an improvement in ventilation and in respiratory muscle strength of patients submitted to hematopoietic stem cell transplantation.
  • Pesquisa de anticorpos antidengue em doadores de sangue Original Articles

    OBJETIVO: O objetivo desta pesquisa foi realizar uma triagem sorológica para detectar a presença de anticorpos IgG e IgM específicos para os quatro sorotipos da dengue em doadores de sangue atendidos pelo Hemonúcleo de Campo Mourão-PR. MÉTODOS: foram aplicadas fichas epidemiológicas e coletados 4 mL de sangue periférico em tubos sem anticoagulante de 213 doadores de sangue. Posteriormente, foi obtido o soro e realizado o teste imunocromatrográfico (Imuno - Rápido Dengue IgM/IgG). Os indivíduos envolvidos neste estudo responderam um questionário sócioepidemiológico, incluindo informações referentes à caracterização dos participantes, como idade, gênero, diagnóstico prévio de dengue, entre outros. RESULTADOS: Das 213 amostras testadas, três (1,4%) apresentaram resultados positivos na pesquisa de anticorpos IgG contra o vírus da dengue. Em relação à infecção aguda do vírus da dengue, não houve presença de anticorpos IgM nos doadores de sangue, evidenciando assim que as amostras biológicas analisadas não apresentavam riscos à saúde do receptor. CONCLUSÃO: Acredita-se que a introdução de métodos sorológicos e moleculares para determinar a presença de anticorpos antidengue e/ou detecção do vírus da dengue em doadores de sangue em regiões endêmicas deveriam ser implantados a fim de assegurar a qualidade da transfusão sanguínea.

    BACKGROUND: Dengue is an urban arbovirus whose etiologic agent is a virus of the genus Flavorius with four distinct antigen serotypes (DENV-1, DENV-2, DENV-3 and DENV-4) that is transmitted to humans through the bite of the mosquito Aedes aegypti. The Campo Mourão region in Brazil is endemic for dengue fever. OBTECTIVE: The aim of this study was to evaluate the presence of IgG and IgM antibodies specific to the four serotypes of dengue in donors of the blood donor service in the city of Campo Mourão. METHODS: Epidemiological records were evaluated and 4 mL of peripheral blood from 213 blood donors were collected in tubes without anticoagulant. Serum was then obtained and immunochromatographic tests were undertaken (Imuno-Rápido Dengue IgM/IgG TM). Individuals involved in the study answered a social and epidemiological questionnaire on data which included age, gender and diagnosis of dengue. RESULTS: Only three (1.4%) of the 213 blood tests were positive for IgG anti-dengue antibodies. No donors with IgM antibody, which identifies acute infection, were identified. CONCLUSIONS: The results of the current analysis show that the introduction of quantitative or molecular serological methods to determine the presence of anti-dengue antibodies or the detection of the dengue virus in blood donors in endemic regions should be established so that the quality of blood transfusions is guaranteed.

    OBJETIVO: Avaliar a expressão clínica da anemia falciforme (AF) e sua relação com os haplótipos da globina beta e talassemia alfa (Tal) em crianças na primeira e segunda infâncias. MÉTODOS: Foi realizado um estudo descritivo e retrospectivo de série de casos em crianças na primeira e segunda infâncias provenientes da triagem neonatal do Rio de Janeiro. Um total de 96 crianças, 79 com AF e 17 com HbSC, 48 homens e 48 mulheres, média de idade de 29,9 (DP+/-20,9) meses, sendo 62 (16,2+/-8,6) na primeira e 34 (54,9+/-11,3) na segunda infância. Foram avaliados: crises dolorosas (CD), de sequestro esplênico (SE) e hemolítica (CH), síndromes mão-pé (SMP) e torácica aguda (STA) e infecções. RESULTADOS: Os eventos foram mais frequentes na primeira infância; 94% das crianças apresentaram ao menos um episódio. Infecção foi o evento mais frequente, acometendo 88,5% das crianças, SE ocorreu mais na primeira infância, enquanto CD e STA mais na segunda. A Tal alfa 3.7 foi observada em 20,9% dos casos. O haplótipo mais frequente foi o Bantu (73,7%), seguido de Benin (23,0%). Não foi observada associação entre eventos clínicos e haplótipos da globina. Crianças com AF e Tal apresentaram menos eventos infecciosos. Não foi possível associar os polimorfismos com a expressão clínica, porém, a maioria das crianças Bantu/Bantu e sem Tal apresentou mais eventos clínicos.

    OBJECTIVE: To describe clinical events of sickle cell disease and the correlation with β-globin haplotypes and α-thalassemia in under 6-year-old children. METHODS: A retrospective study was conducted of under 6-year-old children from the neonatal screening program in Rio de Janeiro. Forty-eight male and 48 female children were enrolled in this study, 79 with sickle cell anemia and 17 with hemoglobin SC. The mean age was 29.9 (standard deviation = 20.9) months, 62 (16.2 ± 8.6) were aged between 0-3 years old and 34 (54.9 ± 11.3) were from 3-6 years old. Painful events, acute splenic sequestration, hemolytic crises, hand-foot and acute chest syndromes and infections were evaluated. RESULTS: The events were more frequent in under 3-year-old children, 94% of children had at least one episode. Infection was the most common event affecting 88.5% of children. Acute splenic sequestration took place earlier, while painful crises and acute chest syndromes in under 6-year-old children. Thal-α 3.7 was observed in 20.9% of cases. Bantu was the most frequent haplotype found, followed by Benin. No correlation was observed between clinical events and β-globin haplotypes. Children with sickle cell anemia and α-thalassemia have less infectious events. No correlation was found among these polymorphisms and clinical events, however, the majority of children with Bantu/Bantu and without α-thalassemia had more clinical events.
  • Frequência dos antígenos plaquetários humanos (HPA) em pacientes trombocitopênicos e predisposição à incompatibilidade transfusional Original Articles

    OBJETIVO: Esse estudo tem o objetivo de avaliar a frequência dos HPAs em pacientes onco-hematológicos trombocitopênicos e analisar a probabilidade de incompatibilidade associada à transfusão de plaquetas. MÉTODOS: A genotipagem plaquetária foi realizada para os alelos HPA-1a, HPA-1b; HPA-2a, HPA-2b; HPA-3a, HPA-3b; HPA-4a, HPA-4b; HPA-5a, HPA-5b; HPA-15a, HPA-15b por PCR-SSP em 150 pacientes atendidos pelo Serviço de Hematologia do Hospital das Clínicas da FMUSP. RESULTADOS: As frequências alélicas encontradas foram: HPA-1a: 0,837; HPA-1b: 0,163; HPA-2a: 0,830; HPA-2b: 0,170; HPA-3a: 0,700; HPA-3b: 0,300; HPA-4a: 1; HPA-4b: 0; HPA-5a: 0,887; HPA-5b: 0,113; HPA-15a: 0,457; HPA-15b: 0,543. CONCLUSÃO: A análise dos HPAs neste estudo demonstrou que os alelos A são mais frequentes na população do que os alelos B, com exceção do HPA-15, sugerindo que pacientes homozigotos para os alelos B apresentam maior predisposição à aloimunização e refratariedade às transfusões de plaquetas por causa imune. A genotipagem plaquetária pode ser de grande valia para o diagnóstico de trombocitopenia aloimune e para prover concentrados de plaquetas compatíveis a esses pacientes.

    OBJECTIVE: The objective of this study was to evaluate the frequencies of human platelet antigens in oncohematological patients with thrombocytopenia and to analyze the probability of their incompatibility with platelet transfusions. METHODS: Platelet antigen genotyping was performed by sequence-specific primer polymerase chain reaction (SSP-PCR) for the HPA-1a, HPA-1b, HPA-2a, HPA-2b, HPA-3a, HPA-3b, HPA-4a, HPA-4b, HPA-5a, HPA-5b; HPA-15a, HPA-15b alleles in 150 patients of the Hematology Service of the Hospital das Clínicas (FMUSP). RESULTS: The allele frequencies found were: HPA-1a: 0.837; HPA-1b: 0.163; HPA-2a: 0.830; HPA-2b: 0.170; HPA-3a: 0.700; HPA-3b: 0.300; HPA-4a: 1; HPA-4b: 0; HPA-5a: 0.887; HPA-5b: 0.113; HPA-15a: 0.457 and HPA-15b: 0.543. CONCLUSIONS: Data from the present study showed that the A allele is more common in the population than the B allele, except for HPA-15. This suggests that patients homozygous for the B allele are more predisposed to present alloimmunization and refractoriness to platelet transfusions by immune causes. Platelet genotyping could be of great value in the diagnosis of alloimmune thrombocytopenia and to provide compatible platelet concentrates for these patients.

    OBJETIVO: Vários aloanticorpos irregulares antieritrocitários, produzidos por sensibilizações a antígenos estranhos durante gestações ou transfusões, possuem importância clínica por provocarem hemólise no feto ou recém-nascido e/ou no receptor de sangue. Diante disso, fizemos uma análise prospectiva exclusiva de pacientes atendidos nos serviços de emergência clínica e cirúrgica, correlacionando as aloimunizações aos dados clínicos e epidemiológicos. MÉTODOS: Amostras sanguíneas de 143 pacientes, com pesquisa inicial de anticorpos negativa, foram coletadas até 15 meses após transfusões de concentrados de hemácias (CH), submetidas a Pesquisas de Anticorpos Irregulares (PAI) e, quando o resultado fosse positivo, à identificação e titulação seriada do(s) aloanticorpo(s). Para comparar proporções, foi empregado Teste Exato de Fisher e Odds Ratio. RESULTADOS: Dos pacientes estudados, 15 (10,49%) produziram aloanticorpos em um período de seis meses, com o título decaindo e negativando até 15 meses em 60% dos aloimunizados. Os aloanticorpos anti-K e os dirigidos contra antígenos do sistema Rh foram os mais frequente, o maior título foi de 1:32 (anti-K) e houve evidente correlação com o número de transfusões. CONCLUSÃO: Diante da alta ocorrência de aloanticorpos antieritrocitários de importância clínica em indivíduos transfundidos em serviço de emergência clínica e cirúrgica, propomos estender a fenotipagem e compatibilização pré-transfusional para os antígenos C, c, E, e (sistema Rh) e K (sistema Kell) para todos os pacientes com cirurgias programadas e/ou com eventos clínicos agudos que não necessitem de transfusões emergenciais.

    BACKGROUND: Several irregular red blood cell alloantibodies, produced by alloimmunization of antigens in transfusions or pregnancies, have clinical importance because they cause hemolysis in the fetus and newborn and in transfused patients. OBJECTIVE: a prospective analysis of patients treated by the surgical and clinical emergency services of Hospital de Clínicas of the Universidade Federal do Triângulo Mineiro (HC/UFTM), Brazil was performed to correlate alloimmunization to clinical and epidemiological data. METHODS: Blood samples of 143 patients with initial negative antibody screening were collected at intervals for up to 15 months after the transfusion of packed red blood cells. Samples were submitted to irregular antibody testing and, when positive, to the identification and serial titration of alloantibodies. The Fisher Exact test and Odds Ratio were employed to compare proportions. RESULTS: Fifteen (10.49%) patients produced antibodies within six months of transfusion. However, for 60% of these individuals, the titers decreased and disappeared by 15 months after transfusion. Anti-K antibodies and alloantibodies against antigens of the Rh system were the most common; the highest titer was 1:32 (anti-K). There was an evident correlation with the number of transfusions. CONCLUSIONS: Given the high incidence of clinically important red blood cell alloantibodies in patients transfused in surgical and clinical emergency services, we suggest that phenotyping and pre-transfusion compatibilization for C, c, E, e (Rh system) and K (Kell system) antigens should be extended to all patients with programmed surgeries or acute clinical events that do not need emergency transfusions.

    OBJETIVO: Este estudo teve como objetivos: 1) implementar a genotipagem para os antígenos S, s e U do sistema de grupo sanguíneo MNS na Fundação Hemominas e 2) avaliar pela primeira vez em uma população miscigenada de Minas Gerais, Brasil a ocorrência de polimorfismos do gene GYPB relacionados aos fenótipos U- e U+var, assim como a deleção de GYPB. Os antígenos S, s e U podem ocasionar reações transfusionais e doença hemolítica perinatal. A genotipagem é uma ferramenta útil na imuno-hematologia, principalmente quando a fenotipagem não pode ser realizada. MÉTODOS: 96 amostras de doadores de sangue e pacientes com doença falciforme previamente fenotipadas para os antígenos S, s e U foram selecionadas. As técnicas AS/PCR e ensaio combinado AS/PCR - RFLP foram empregadas para identificar os alelos GYPB*S, GYPB*s e as variantes GYPB(P2) e GYPB(NY), bem como a deleção do gene GYPB. RESULTADOS: Os resultados da genotipagem alelo-específica GYPB*S e GYPB*s foram 100% concordantes em relação aos fenótipo S+ (n = 56), s+ (n = 60) e s- (n = 35). Entretanto, o alelo GYPB*S, em associação com a variante GYBP(P2), foi detectado em 17,5% das amostras S- (n = 40), o que revela a importância da avaliação desta variante em nossa população. Entre as amostras S-s- avaliadas (n = 10), 60% apresentaram deleção do gene GYPB, e 40% apresentaram GYPB (P2) em homo ou hemizigose. CONCLUSÃO: A genotipagem é uma estratégia eficaz na inferência dos fenótipos S, s e U na população miscigenada de Minas Gerais, podendo contribuir para a segurança transfusional.

    OBJECTIVE: To implement genotyping for S, s and U antigens of the MNS blood group system at the Fundação Hemominas and to evaluate the occurrence of GYPB gene polymorphisms associated with the U- and U+var phenotypes and deletion of the GYPB gene for the first time in an admixed population of Minas Gerais, Brazil. The S, s and U antigens can cause transfusion reactions and perinatal hemolytic disease. Genotyping is a useful tool in immunohematology, especially when phenotyping cannot be performed. METHODS: Ninety-six samples from blood donors and patients with sickle cell disease previously phenotyped for the S, s and U antigens were selected. Allele-specific primer polymerase chain reaction (ASP-PCR) and polymerase chain reaction -restriction fragment length polymorphism (PCR-RFLP) assays were employed to identify the GYPB*S and GYPB*s alleles and the GYPB(P2) and GYPB(NY) variants, as well as deletion of the GYPB gene. RESULTS: The results of allele-specific genotyping (GYPB*S and GYPB*s) were totally in agreement with the phenotyping of S+ (n = 56), s+ (n = 60) and s- (n = 35) samples. However, the GYPB*S allele, in association with the GYPB(P2) variant, was detected in 17.5% of the S- samples (n = 40), which shows the importance of assessing this variant in the Brazilian population. Of the S-s- samples (n = 10), 60% had the deletion of the GYPB gene and 40% were homozygous or hemizygous for the GYPB(P2) variant. CONCLUSION: Genotyping was an effective strategy to infer the S, s, and U phenotypes in the admixed population from Minas Gerais (Brazil) and may contribute to transfusion safety.
  • Modelo logístico na determinação de fatores associados à infecção HIV em doadores de sangue na Fundação HEMOPE Original Articles

    OBJETIVO: Determinar fatores associados à infecção por HIV em doadores de sangue no Recife, Brasil. MÉTODOS: Desenvolveu-se um estudo transversal com 106.203 doadores de sangue de 1ª doação, aptos na Triagem Clínica (TC) no Hemocentro Recife da Fundação de Hematologia e Hemoterapia de Pernambuco (HEMOPE), considerando-se os critérios rotineiros empregados pelo Hemocentro para seleção de candidatos no período de janeiro de 1998 a novembro de 2003. Os testes Imunofluorescência Indireta e Western Blot foram utilizados para confirmação da infecção HIV, e doadores foram classificados como HIV positivo ou negativo. O teste do qui-quadrado e método stepwise de regressão logística múltipla foram realizados para examinar associação entre a infecção pelo HIV e as variáveis: idade, sexo, área de residência, escolaridade, tipo de doação e testes sorológicos de triagem para Anti-HBc, HBsAg, Anti-HCV, Anti-HTLV 1/2 e VDRL. RESULTADOS: Na amostra observada, foram encontrados 0,204% doadores de sangue HIV positivo. Entre as variáveis estudadas, idade, escolaridade, residência, tipo de doação e status sorológico para Anti-HBc e VDRL foram associados com a infecção pelo HIV. CONCLUSÃO: Doadores com doação espontânea, da área metropolitana de Recife, de menor idade e nível de ensino, positivo para o Anti-HBc e VDRL, apresentam risco aumentado de infecção pelo HIV. Tais dados são necessários para compreender a dinâmica da infecção e para guiar políticas sanitárias.

    OBJECTIVE: To determine factors associated with HIV infection in blood donor candidates in Recife, Brazil. METHODS:A transversal study was performed of 106,203 blood donor candidates found eligible by the routine clinical screening process in the blood bank in Recife of the Fundação de Hematologia e Hemoterapia de Pernambuco (HEMOPE) in the period from January 1998 to November 2003. Additional indirect immune and western blot confirmation tests for HIV infection were performed and the candidates were classified as HIV positive or negative. The Chi-squared test and stepwise multiple logistic regression were conducted to examine any association between HIV infection and age, gender, place of residence, schooling, number of donations and serological tests for core hepatitis B antigen virus (anti-HBc), the hepatitis C antivirus (anti-HCV), human T-type antivirus lymph cells (anti-HTLV 1 and 2), serological tests for syphilis (VDRL) and the hepatitis B virus surface antigen (HBsAg). RESULTS: In the observed sample, 0.204% of blood donor candidates were found to be HIV positive. Among the studied variables, the age, education level, residency, donation type, and serologic status for anti-HBc and VDRL tests were found to be associated with HIV infection. CONCLUSION: The younger, spontaneous donor candidates living in the Recife metropolitan area with a low level of education and positive for anti-HBc and VDRL have an higher risk of HIV infection than the other candidates. Data such as these are useful to understand the dynamics of infection and to guide healthcare policies.

    The aim of this study was to conduct a systematic review on the photoinactivators used in hemotherapy, with action on viral genomes. The SciELO, Science Direct, PubMed and Lilacs databases were searched for articles. The inclusion criterion was that these should be articles on inactivators with action on genetic material that had been published between 2000 and 2010. The key words used in identifying such articles were "hemovigilance", "viral inactivation", "photodynamics", "chemoprevention" and "transfusion safety". Twenty-four articles on viral photoinactivation were found with the main photoinactivators covered being: methylene blue, amotosalen HCl, S-303 frangible anchor linker effector (FRALE), riboflavin and inactin. The results showed that methylene blue has currently been studied least, because it diminishes coagulation factors and fibrinogen. Riboflavin has been studied most because it is a photoinactivator of endogenous origin and has few collateral effects. Amotosalen HCl is effective for platelets and is also used on plasma, but may cause changes both to plasma and to platelets, although these are not significant for hemostasis. S-303 FRALE may lead to neoantigens in erythrocytes and is less indicated for red-cell treatment; in such cases, PEN 110 is recommended. Thus, none of the methods for pathogen reduction is effective for all classes of agents and for all blood components, but despite the high cost, these photoinactivators may diminish the risk of blood-transmitted diseases.

    Adult stem/progenitor cells are found in different human tissues. An in vitro cell culture is needed for their isolation or for their expansion when they are not available in a sufficient quantity to regenerate damaged organs and tissues. The level of complexity of these new technologies requires adequate facilities, qualified personnel with experience in cell culture techniques, assessment of quality and clear protocols for cell production. The rules for the implementation of cell therapy centers involve national and international standards of good manufacturing practices. However, such standards are not uniform, reflecting the diversity of technical and scientific development. Here standards from the United States, the European Union and Brazil are analyzed. Moreover, practical solutions encountered for the implementation of a cell therapy center appropriate for the preparation and supply of cultured cells for clinical studies are described. Development stages involved the planning and preparation of the project, the construction of the facility, standardization of laboratory procedures and development of systems to prevent cross contamination. Combining the theoretical knowledge of research centers involved in the study of cells with the practical experience of blood therapy services that manage structures for cell transplantation is presented as the best potential for synergy to meet the demands to implement cell therapy centers.
  • Diagnóstico laboratorial de leucemia mielomonocítica crônica agudizada em associação com leucemia linfocítica crônica: aspectos morfológicos e imunofenotípicos Case Report

    A Leucemia Mielomonocítica Crônica (LMMC) é uma desordem clonal de células-tronco hematopoiéticas caracterizada principalmente por monocitose absoluta no sangue periférico. Esta doença pode apresentar características de síndromes mielodisplásicas e de doenças mieloproliferativas. De acordo com a classificação estabelecida pela OMS, a LMMC está inserida no grupo de neoplasias mieloproliferativas/mielodisplásicas e seu diagnóstico requer a presença de monocitose persistente no sangue periférico e sinais de mielodisplasia envolvendo uma ou mais linhagens mieloides. Além disso, deve apresentar ausência de cromossomo Philadelphia ou fusão BCR/ABL e menos de 20% de blastos no sangue e na medula óssea. Fenotipicamente, as células presentes na LMMC podem apresentar antígenos de membrana como o CD33 e o CD13, forte expressão de CD56 e CD2 e expressão variável de HLA-DR, CD36, CD14, CD15, CD36, CD68 e CD64, sendo que um aumento da expressão de CD34 pode estar associada a uma transformação para leucemia aguda. Alterações citogenéticas são frequentes na LMMC e mutações moleculares como NRAS já foram identificadas. O presente artigo trata-se de um estudo de caso de uma LMMC agudizada, diagnosticada através de achados morfológicos e fenotípicos que, embora sugiram uma Leucemia Monocítica Aguda, pôde ser diferenciada através de uma minuciosa análise da morfologia celular. Além disso, foram encontradas células típicas de Leucemia Linfocítica Crônica (LLC), tornando este caso um achado raro.

    Chronic myelomonocytic leukemia is a clonal stem cell disorder that is characterized mainly by absolute peripheral monocytosis. This disease can present myeloproliferative and myelodysplastic characteristics. According to the classification established by the World Health Organization, chronic myelomonocytic leukemia is inserted in a group of myeloproliferative/myelodysplastic disorders; its diagnosis requires the presence of persistent monocytosis and dysplasia involving one or more myeloid cell lineages. Furthermore, there should be an absence of the Philadelphia chromosome and the BCR/ABL fusion gene and less than 20% blasts in the blood or bone marrow. Phenotypically, the cells in chronic myelomonocytic leukemia can present myelomonocytic antigens, such as CD33 and CD13, overexpressions of CD56 and CD2 and variable expressions of HLA-DR, CD36, CD14, CD15, CD68 and CD64. The increase in the CD34 expression may be associated with a transformation into acute leukemia. Cytogenetic alterations are frequent in chronic myelomonocytic leukemia, and molecular mutations such as NRAS have been identified. The present article reports on a case of chronic myelomonocytic leukemia, diagnosed by morphologic and phenotypical findings that, despite having been suggestive of acute monocytic leukemia, were differentiated through a detailed analysis of cell morphology. Furthermore, typical cells of chronic lymphocytic leukemia were found, making this a rare finding.
