    A instabilidade postural na doença de Parkinson (DP) tem sido associada a uma diminuição do limite de estabilidade (LE) na direção ântero-posterior (AP). Entretanto, ainda que possíveis alterações do LE na direção látero-lateral (LL) tenham sido sugeridas, tal direção não tem sido avaliada nos estudos com DP, principalmente quando o teste de limite de estabilidade (TLE) envolve movimentos intencionais que deslocam o centro de massa corporal (CMC). O objetivo do presente estudo foi investigar o LE na postura de pé durante movimentos voluntários que promovem deslocamento do CMC nas direções AP e LL de indivíduos com e sem a DP. Doze indivíduos com DP (Hoehn & Yahr=II, III) e 12 sem a doença realizaram o TLE nos sentidos anterior, posterior, direito e esquerdo. A velocidade de movimento (VM), excursão máxima (EM) e o controle direcional (CD) do CMC foram avaliados em cada sentido. Os indivíduos com DP foram significativamente mais lentos em todos os sentidos de deslocamento do CMC (p<0,05). Não houve diferença significativa na EM e CD no sentido anterior entre os grupos (p>0,05). Por outro lado, a EM e CD do CMC foram menores para o grupo DP no sentido posterior (P) e na direção LL (p<0,05). Indivíduos leves a moderadamente afetados pela DP apresentaram redução do LE no sentido P e na direção LL quando comparados ao grupo controle. Os resultados sugerem que tal direção e sentido devam ser treinados em ortostatismo, com movimentos que deslocam voluntariamente o CMC, desde fases iniciais da DP.

    Postural instability in Parkinson's disease (PD) has been associated with decreases of the limits of stability (LOS) in the anterior-posterior (AP) directions. Although LOS changes in the latero-lateral (LL) directions have been suggested, they have not been evaluated in studies with PD, especially when the LOS tests involve intentional displacements of the body center of mass (COM). The aim of this study was to investigate the LOS in standing posture during voluntary movements that promote movement of the COM in AP and LL directions of individuals with and without PD. Twelve subjects with PD (Hoehn & Yahr stages of II and III) and 12 without the disease performed the LOS tests in the anterior, posterior, right and left directions. The movement velocity (MV), maximum excursion (ME), and directional control (DC) of the COM were evaluated in all investigated directions. Individuals with PD demonstrated smaller COM displacements in all directions (p<0.05). There were no significant differences between the groups regarding the ME and DC in the anterior direction (p>0.05). However, the ME and DC of the COM were smaller for the PD group in the backward, right, and left directions (p<0.05). Individuals with mild to moderate PD demonstrated decreases in the LOS in the posterior and LL directions, when compared to individuals without the disease. These findings suggested that such directions should be trained in the standing position with voluntary movements of the COM from the early stages of PD.
  • Perfil dos desvios posturais da coluna vertebral em adolescentes de escolas públicas do município de Juazeiro do Norte - CE Pesquisa Original

    Postura é definida como sendo a posição adotada pelo ser humano. Dados epidemiológicos apontam para uma alta prevalência de alterações posturais de coluna entre crianças e adolescentes. O objetivo do presente estudo foi investigar os desvios posturais da coluna vertebral em escolares (n=670) de 11 a 19 anos. Utilizou-se como metodologia um simetrógrafo e uma máquina fotográfica para pesquisar a presença de desvios na coluna vertebral. Foram utilizados um nível d'água e uma régua para aferir a gibosidade, um fio de prumo e uma régua para medir os desvios laterais da coluna. Dos resultados obtidos, encontrou-se uma prevalência de 8,8% de desvios laterais e 2,4% de gibosidade, além de ser observada a presença de escoliose em portadores de assimetrias de ombros e ilíacos. Porém, não houve influência do peso, altura e índice de massa corporal (IMC) para a prevalência de escoliose. Dessa maneira, com este estudo, constatou-se elevada prevalência dos desvios posturais em escolares.

    Posture is defined as the position taken by humans. Epidemiological data indicate a high prevalence of backbone postural changes among children and adolescents. The aim of this study was to investigate the postural deviations of the spine in schoolchildren (n=670) from 11 to 19 years. The methodology used was a simetrograf and a camera for the presence of deviations in the spine. We used a water level and a ruler to measure the spinal deformity, a plumb line and a ruler to measure the lateral deviation of the spine. Of the obtained results it was found a prevalence of 8.8% of lateral deviation and 2.4% of gibbosity, besides it was observed the presence of scoliosis in patients with asymmetries in the shoulders and hipbones. However, there was no influence of weight, height and body mass index (BMI) for the prevalence of scoliosis. Thus, through this study, it was stated the high prevalence of postural deviations in schoolchildren.
  • Atividade elétrica dos músculos cervicais e amplitude de movimento da coluna cervical em indivíduos com e sem DTM Pesquisa Original

    O objetivo da pesquisa foi avaliar a atividade elétrica e a dor à palpação dos músculos cervicais, a amplitude de movimento (ADM) da coluna cervical e a relação entre atividade elétrica e ADM cervical em indivíduos com e sem desordem temporomandibular (DTM). Participaram do estudo 53 voluntários, sendo 24 com DTM e 29 sem, de ambos os gêneros, com 18 a 32 anos de idade. A DTM foi diagnosticada pelo critério de diagnóstico em pesquisa para DTM (Research Diagnostic Criteria for Temporomandibular Disorders). A eletromiografia dos músculos esternocleidomastoídeo (ECOM) e trapézio superior foi realizada bilateralmente, na situação de repouso, com frequência de 2 KHz e filtro passa-faixa de 10-1000 Hz. As medidas de ADM de flexão, extensão, lateroflexão e rotação foram realizadas com um flexímetro e a comparação entre os grupos contou com o teste de Mann Whitney. O teste de Spearman foi aplicado para a correlação entre as variáveis, com nível de significância de 5%. Não foi verificada nenhuma diferença na ADM cervical entre os grupos. A atividade elétrica demonstrou-se significativamente maior nos músculos ECOM direito (p=0,0130), trapézio superior direito (p=0,0334) e esquerdo (p=0,0335) no grupo DTM em relação ao grupo controle. Não houve correlação significante entre atividade elétrica e ADM cervical. A dor nos músculos cervicais apresentou-se significativamente maior apenas no músculo ECOM direito (p=0,0055). Conclui-se que os grupos estudados registraram diferença na atividade elétrica dos músculos cervicais, sendo esta maior em indivíduos com DTM.

    The aim of the research was to evaluate the electrical activity and pain on palpation of the cervical muscles, the cervical range of motion (ROM) and the relationship between electrical activity and ROM in individuals with and without temporomandibular disorder (TMD). Fifty three volunteers took part in the study, being 24 with TMD and 29 without, both genders, from 18 to 32 years old. The TMD was diagnosed through Research Diagnostic Criteria for Temporomandibular Disorder (RDC/TMD). The electromyografhy of Sternocleidomastoideus (SCM) and upper trapezius muscles was carried out bilaterally during rest situation, with a frequency of 2 KHz and band-pass filter of 10-1000Hz. The measures of flexion, extension, lateral tilt and rotation ROM were acquired through a fleximeter and the comparison between groups was analyzed by Mann-Whitney test. The Spearman test verified the correlation between the variables, with 5% of significance level. There were no differences in the cervical ROM between the groups. The electrical activity was significantly higher in the right SCM (p=0,0130), left (p=0,0335) and right (p=0,0334) trapezius muscles in the TMD group. There were no significant correlations between electrical activity and cervical ROM. Pain in the neck muscles was significantly higher only in the right SCM (p=0,0055). It is concluded that the studied groups had difference in the electrical activity of the cervical muscles, which was larger in the individuals with TMD.
  • Diretrizes para avaliação e tratamento fisioterapêutico da Síndrome de Pusher: estudo de caso Pesquisa Original

    A Síndrome de Pusher (SP) é uma alteração perceptual presente em 10% dos casos de hemiparesia causada por Acidente Vascular Encefálico (AVE). Caracteriza-se por queda para o lado parético, comportamento de empurrar-se para o lado acometido com o hemicorpo não parético e resistência à correção externa. Este estudo visou descrever a avaliação, o tratamento e a evolução de uma paciente com hemiparesia à esquerda e SP, devido a um AVE no hemisfério cerebral direito. Seis meses após a lesão, a paciente realizou testes perceptuais (do desenho da figura humana, comportamental de inatenção, escala de avaliação do sintoma de empurrar, miniexame do estado mental) e de desempenho funcional (escala de avaliação postural para pacientes com AVE, teste de função manual de Jebsen-Taylor, medida de independência funcional e índice de Barthel) e iniciou fisioterapia duas vezes por semana. Cada sessão teve 3 partes de 20 minutos: estimulação sensorial, treino motor e integração sensório-motora. Após o tratamento a paciente foi reavaliada e apresentou melhora em todas as escalas. A de avaliação do sintoma de empurrar e o teste de função manual de Jebsen-Taylor foram os que registraram maiores percentuais de melhora, 79% e 46%, respectivamente. O protocolo utilizado, mesmo tendo sido iniciado seis meses após a lesão, proporcionou melhora perceptual e funcional, o que sugere a importância da fisioterapia na recuperação da SP.

    The Pusher´s Syndrome (PS) is a perceptual disorder that occurs in 10% of the cases of hemiparesis caused by stroke. It is characterized by falling to the paretic side, pushing to the paretic side with the non-paretic side and resisting to external correction. The present study aimed to describe the assessment, treatment and clinical evolution of a patient with left paresis and PS, caused by a stroke on the right hemisphere. Six months after the lesion, the patient was submitted to perceptual (human figure drawing test, behavioral inattention test, scale for contraversive pushing, minimental state examination) and functional performance tests (postural assessment stroke scale, Jebsen-Taylor hand function test, functional independence measure and Barthel index) and started physical therapy twice a week. Each session consisted of 3 parts of 20 minutes: sensory stimulation, motor training and sensory-motor integration. After six months, the patient showed improvement in all scales. The scale for contraversive pushing and the Jebsen-Taylor hand function test showed the highest percentages of improvement, 79% and 46%, respectively. Besides having been started six months after the lesion, the protocol of the present study contributed to the improvement of perception and functional performance. These findings suggest the importance of the physical therapy treatment in the recovery of individuals with PS.
  • Efeitos da Reeducação Postural Global em escolares com escoliose Pesquisa Original

    O objetivo deste estudo foi analisar o efeito do método da Reeducação Postural Global (RPG) em escolares com diagnóstico de escoliose torácica não estrutural (ETNE). Os escolares com indicativo de ETNE ao exame postural e teste de Adams negativo foram encaminhados ao exame radiográfico para comprovação diagnóstica. Foram selecionados 20 participantes (11 meninos e 9 meninas, com 10±3 anos), divididos randomicamente em dois grupos homogêneos: o que realizou o RPG (GRPG) durante 12 semanas com duração de 25 a 30 minutos cada sessão, de acordo com o que aguentou permanecer na postura; e o grupo controle (GC), sem intervenção. Após três meses, os dois grupos repetiram a avaliação postural e o exame radiográfico. Para avaliação das estatísticas, foi utilizada análise de variância (ANOVA) univariada, com medidas repetidas, seguida do Post Hoc de Tukey para identificar as possíveis diferenças intra e intergrupos. O valor de α foi de 0,05. O GRPG apresentou redução significativa no ângulo de Cobb na comparação intragrupo (Δ%=-35,100; p=0,009), mas o GC não (Δ%=9,520; p=0,789). Pode-se concluir que escolares submetidos ao método da RPG apresentaram melhora do quadro de escoliose torácica não estrutural.

    The aim of this study was to analyze the effect of Global Reeducation Postural (RPG) method in scholars diagnosed with thoracic nonstructural scoliosis (ETNE). Scholars with indicative of ETNE to postural exam and Adams test negative were directed to radiographic exam to diagnostic. Twenty participants were selected (11 boys and 9 girls, 10±3 years old) and randomly divided in two homogeneous groups: group treated by RPG (GRPG) and control group (CG). The GRPG was submitted a RPG treatment during 12 weeks in sessions from 25 to 30 minutes in agreement with each student tolerance at posture. The CG didn't suffer intervention. After three months, both groups repeated the postural and radiographic exam. For statistics, it was used ANOVA univariated with repeated measures, followed by Tukey Post Hoc Test to identify the possible differences intra and intergroups. The α value used was 0.05. The GRPG present significant reduction at Cobb angle between pre and posttest (Δ%=-35.100; p=0.009), but the CG didn´t show (Δ%=9.520; p=0.789). It was concluded that scholars submitted to RPG method presented improvement in thoracic nonstructural scoliosis.
  • Análise da eficácia do laser de baixa potência no tratamento da dor tendínea em ratos imunossuprimidos Pesquisa Original

    O comprometimento do sistema imune, que pode ser apresentado por indivíduos com doenças crônicas, leva à baixa resposta imunológica. Um dos tratamentos utilizados para lesões agudas em tendões é o laser de baixa potência, contudo há uma lacuna em relação ao seu uso em imunodepressão. O objetivo do presente estudo foi analisar se o laser de baixa potência é eficaz para o tratamento da dor em ratos imunodeprimidos submetidos a trauma tendíneo. Foram utilizados 23 ratos, machos, da linhagem Wistar, divididos aleatoriamente em três grupos: grupo controle, placebo e laser. Os animais foram imunodeprimidos (por administração de Ciclosporina A) e submetidos à lesão no tendão calcâneo direito, com impacto de cerca de 0,40 J. Para o tratamento, utilizou-se laser de baixa potência, 670 nm, 30 mW e dose de 2 J/cm², durante 3 dias. A avaliação da dor foi realizada pelo teste de incapacidade funcional e por filamento de Von Frey digital. Os resultados apresentaram valores significativos para o grupo laser, com diminuição de dor funcional e da dor à pressão na superfície plantar e no local lesionado (tendão calcâneo direito). Portanto, concluiu-se que o laser de baixa potência é eficaz para reduzir a dor em ratos imunodeprimidos submetidos a trauma tendíneo.

    The commitment of immune system, which may be presented by individuals with chronic diseases, leading to a low immune response. One of the treatments used for acute injuries in tendons is the low-power laser, however there is a gap in relation to its use in immunosuppression. The objective of this study was to analyze if low-level laser therapy is effective for the treatment of pain in immunosuppressed rats subjected to trauma tendon. We used 23 male rats of Wistar strain, divided randomly in three groups: control group, placebo and laser. The animals were immunosuppressed (by administration of Cyclosporin A) and underwent right Achilles tendon injury, with impact of about 0.40 J. For treatment, we used low-level laser, 670 nm, 30 mW and 2 J/cm², during 3 days. Pain assessment was performed by the functional incapacitation test and von Frey filament digital. The results showed a reduction of functional pain and pressure pain on the plantar surface and the injured site (right Achilles tendon) for the laser group. Therefore, we concluded that the low-power laser is effective for reducing pain in immunosuppressed rats subjected to trauma tendon.
  • Avaliação postural em pacientes com doença pulmonar obstrutiva crônica Pesquisa Original

    O objetivo deste estudo foi avaliar alterações posturais em pacientes com Doença Pulmonar Obstrutiva Crônica (DPOC), por meio do Software para Avaliação Postural (SAPO). Trinta indivíduos constituíram 2 grupos: 15 pacientes com DPOC e 15 idosos saudáveis (controle). Os grupos realizaram espirometria e foram fotografados para avaliação postural. As imagens obtidas foram digitalizadas e, posteriormente, avaliou-se dez alterações posturais com o SAPO [inclinação lateral da cabeça (ILC), desnivelamento dos ombros (DO), desnivelamento pélvico anterior (DPA), inclinação lateral do tronco (ILT), desnivelamento das escápulas (DE), desnivelamento pélvico posterior (DPP), protusão da cabeça (PC), protusão de ombro (PO), báscula anterior da pelve (BAP) e cifose torácica (CT)]. Essas alterações, obtidas no grupo controle, foram conferidas com o padrão de normalidade para adultos jovens, proposto em estudo prévio. Para a comparação das alterações posturais entre grupo controle e DPOC, foi utilizado o teste de Mann-Whitney em grupo controle, e em adultos jovens, o teste t de Student não pareado, ambos com nível de significância estatística de 5%. Dentre as dez alterações posturais, o grupo controle apresentou sete (ILC, DPA, DE, DPP, PC, PO, BAP) com valores angulares significativamente maiores em relação aos adultos jovens. Quando comparados a idosos saudáveis, indivíduos com DPOC apresentaram um aumento significante na angulação de BAP, DPP e CT. Pacientes com DPOC apresentam três alterações posturais que provavelmente estão relacionadas à doença.

    The aim of this study was to evaluate postural changes in patients with Chronic Obstructive Pulmonary Disease (COPD) by means of the Software for Postural Assessment (SPA). Thirty individuals were divided into 2 groups: 15 patients with COPD and 15 healthy elderly people (control group). The groups underwent spirometry and photographed for postural assessment. The images were scanned and subsequently evaluated ten postural changes with SPA [lateral head tilting (LHT), shoulder asymmetry (SA1), anterior pelvic asymmetry (APA), lateral trunk tilting (LTT), scapular asymmetry (SA2), posterior pelvic asymmetry (PPA), head protrusion (HP), shoulder protrusion (SP), anterior pelvic tilting (ABT), and thoracic kyphosis (TK)]. These postural changes, obtained in the control group, were compared with the normal range for young adults, proposed in a previous study. For the comparison of postural changes between the control and COPD groups was used Mann-Whitney test in the control group, and young adults, the unpaired Students's t-test. The level of statistical significance was 5%. Among the ten postural changes, the control group had seven (LHT, APA, SA2, PPA, HP, SP, ABT) outside the normal range for young adults, with significantly higher angular values. When compared to healthy elderly, patients with COPD showed a significant increase in the angle of three postural changes (ABT, PPA, and TK). Patients with COPD have three postural changes that are probably related to the disease.
  • Efeitos do repouso e do exercício no solo e na água em hipertensos e normotensos Pesquisa Original

    Este estudo visou avaliar o efeito do repouso e do exercício, realizados no solo e na água, sobre a frequência cardíaca (FC), pressão arterial sistólica (PAS) e diastólica (PAD) e o volume de diurese em indivíduos hipertensos e normotensos. Foram analisados 20 indivíduos divididos em dois grupos, normotensos (GN, n=8) e hipertensos (GH, n=12). Em ambos foram realizados quatro protocolos distintos, dois de exercícios e dois de repouso, no solo e na água. A PAS, PAD e FC foram mensuradas durante repouso e aos 30, 60 e 90 minutos após cada protocolo. O volume de diurese foi corrigido pelo peso corporal e coletado 30 minutos após cada protocolo. No GH, o protocolo de exercício no solo promoveu redução média de 16,5±3,7 mmHg (p=0,01) da PAS aos 90 minutos pós-exercício. No GN, o protocolo de repouso na água promoveu redução média de 14 bpm (p<0,01) da FC e o volume de diurese foi maior quando comparado aos protocolos realizados no solo (p<0,01). Portanto, a imersão desencadeou bradicardia e aumento do volume de diurese no GN. Não houve efeito hipotensor significativo nos protocolos realizados na água em ambos os grupos. Os resultados sugerem que uma sessão de exercício físico no solo com duração de 45 minutos, em intensidade submáxima, provoca redução da PAS em indivíduos hipertensos.

    The aim of this study was to assess the effect of rest and exercise, accomplished on the ground or in water on the heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) and the volume of diuresis in hypertensive and normotensive individuals. Twenty individuals were analyzed and divided into two groups, normotensive (GN, n=8) and hypertensive (GH, n=12). Both groups accomplished four distinct protocols, two protocols of exercise and two protocols of rest on the ground and in the water. The SBP, DBP and HR were measured at rest and at 30, 60 and 90 minutes after each protocol. The volume of diuresis was corrected for the body weight and collected 30 minutes after each protocol. In the GH, the exercise protocol on the ground caused an average reduction of 16.5±3.7 mmHg (p=0.01) in SBP at 90 minutes post-exercise. In the GN, the protocol of rest in the water caused an average reduction of 14 bpm (p<0.01) in HR and the volume of diuresis was increased when compared with the protocols accomplished on the ground (p<0.01). Therefore, the immersion triggered bradycardia and increase in volume of diuresis in the normotensive group. There was no significant hypotensive effect in the protocols performed in water in both groups. The results suggest that an exercise session on the ground during 45 minutes in submaximal intensity causes a reduction in SBP in hypertensive subjects.
  • Influência da Escola de Postura na qualidade de vida, capacidade funcional, intensidade de dor e flexibilidade de trabalhadores administrativos Pesquisa Original

    O objetivo do estudo foi analisar os efeitos de um programa "Escola de Postura" em relação à qualidade de vida, capacidade funcional, intensidade de dor e flexibilidade em trabalhadores com dor lombar inespecífica. Participaram 33 trabalhadores do setor administrativo. O programa foi realizado em sete encontros com quatro grupos (com sete ou oito participantes), uma vez por semana, com duração de uma hora cada. Antes e após a intervenção, os voluntários responderam ao questionário de qualidade de vida Short-Form Health Survey (SF-36) e ao questionário de incapacidade funcional Roland-Morris, realizaram o teste sentar e alcançar com o banco de Wells e assinalaram a intensidade de dor na escala visual analógica. O teste estatístico de Shapiro-Wilk foi usado para analisar a normalidade de distribuição dos dados. Os dados foram analisados pelo teste Wilcoxon e pelo teste t de Student, com nível de significância de 5% (p<0,05). Foi observada melhora da qualidade de vida em sete domínios do SF-36 (p<0,005), da incapacidade funcional (p<0,005), da intensidade de dor (p<0,005) e da flexibilidade (p<0,005). O índice de adesão foi de 58,93%. O programa "Escola de Postura" proposto melhorou significativamente a qualidade de vida, capacidade funcional, flexibilidade e intensidade de dor de adultos trabalhadores de setores administrativos.

    The purpose of this study was to analyze the effects of the "Back School" in the quality of life, functional capacity, pain intensity and flexibility for workers with nonspecific low back pain. Thirty-three administrative sector workers participated. The program was conducted in seven meetings with four groups (seven to eight subjects) once a week, lasting one hour each. Before and after the intervention, the subjects answered a questionnaire on quality of life Short-Form Health Survey (SF-36), the incapacity functional questionnaire, Roland Morris, performed the sit and reach test with the Wells' bank, and noted the intensity pain in visual analog scale. The statistical test of Shapiro-Wilk test was used to analyze the normality of data distribution. The data were analyzed by Wilcoxon and Student's t-test with significance level of 5% (p<0.05). In the improvement of quality of life in seven domains of the SF-36 (p<0.005), functional incapacity (p<0.005), pain intensity (p<0.005) and flexibility (p<0.005). The rate of adherence was 58.93%. The "Back School" program offered significantly improved the quality of life, functional capacity, flexibility and intensity of pain in adult administrative workers.

    A força muscular é a valência física mais importante. É provável que exista correlação entre a força de preensão manual e outros testes para o membro superior afetado por um acidente vascular cerebral (AVC). O propósito deste estudo é analisar o valor preditivo da dinamometria de preensão manual (DPM) para recuperação do membro superior parético por AVC. Foram avaliados 43 pacientes hemiparéticos pós-AVC (60,7 anos±12,1). Vários testes (Escala de movimento da mão (EMM), Estesiometria, Teste de caixa e blocos, 9 buracos e pinos, Escala de Ashworth modificada e Índice de Barthel) foram relacionados com a DPM. Os resultados apontaram que a DPM apresenta boa correlação com EMM e, contrariamente, os testes de destreza manual, a sensibilidade e o índice de independência funcional não apresentaram valores significativos. A DPM é um teste rápido, fácil e acessível, e pode fazer parte dos protocolos de avaliação funcional do membro superior de pacientes hemiparéticos por AVC.

    Muscle strength is the most important physical valence. It is likely that there is a correlation between handgrip strength and other tests for the upper extremity affected by stroke. The purpose of this study is to analyze the predictive value of handgrip dynamometry (HD) for upper limb recovery in stroke. Were studied 43 hemiparetic patients after stroke (60.7 years±12.1). Several tests (Hand moviment scale (HMS), Esthesiometry, Box and blocks test, 9 hole and peg test, Modified Ashworth scale and Barthel index) were related to HD. The results showed that there is good correlation of the HD with HMS and, by contrast, the tests of manual dexterity, the sensitivity and index of functional independence showed no significant values. The HD is a quick, easy and affordable test, and can be part of protocols for functional evaluation of upper extremity of hemiparetic patients by stroke.
  • Estudo morfométrico da terapia LED de baixa potência em tendinite de ratos Pesquisa Original

    A terapia LED de baixa potência possui efeitos analgésico e antiinflamatório. O objetivo desse estudo foi analisar a ação da terapia com LED de baixa potência na reparação tendínea por meio de histologia e histomorfometria. Foram usados 25 ratos Wistar, de 220 a 250 g, divididos em três grupos experimentais avaliados no 7º e 14º dia: A, tendinite induzida sem tratamento; B, tendinite induzida tratada com LED de baixa potência, densidade de energia 4 J/cm² por 120 segundos; e C, sem indução de tendinite. A histomorfometria mostrou-se altamente significativa (p<0,001) na terapia com LED em contrarrelógio de 14 dias, comparada ao grupo saudável (C). Entretanto, não mostrou significância levando em conta o grupo tendinite (A14), em que valor p>0,05. As análises histológica e histomorfométrica demonstraram qualitativo e quantitativo aumento no número de fibroblastos aos sete dias de tratamento, e das fibras colágenas, aos 14 dias, para grupo tratado com LED de baixa potência em relação ao grupo sadio (C). O mesmo não foi percebido quando relacionado ao grupo tendinite (A14).

    The low power LED therapy has analgesic and anti-inflammatory effects. The objective of this study was to analyze the action of therapy with low power LED on the tendon repair, using histology and histomorphometry. Were used 25 Wistar rats, with 220 to 250 g, divided into three experimental groups (7 and 14 days): A7 and A14 induced tendonitis without treatment; B7 and B14, induced tendinitis treated with LED low power density energy 4 J /cm² for 120 seconds; and C, without induction of tendinitis. Histomorphometry was highly significant (p<0.001) with LED therapy against the clock for 14 days, compared with the healthy group (C). But not significant compared with the tendinitis group (A14) (p> 0.05). Histology and histomorphometry analysis demonstrated qualitative and quantitative increase in the number of fibroblasts to seven days of treatment, and collagen fibers at 14 days for the group treated with LED low power compared to the healthy group (C). But it is unclear even when related to the tendinitis group (A14).
  • Vigilância do desenvolvimento neuropsicomotor de crianças de um programa DST/AIDS Pesquisa Original

    A terapia anti-retroviral de alta potência (TARV) é uma forma eficaz de prevenção da transmissão do vírus HIV de mãe para filho. No entanto, os estudos ainda investigam os efeitos da exposição intraútero à TARV, dentre eles o atraso no desenvolvimento neuropsicomotor (DNPM). O presente estudo apresenta o relato de um projeto de extensão, cujos objetivos foram verificar o DNPM de crianças de um programa DST/AIDS, orientar as famílias considerando seu contexto socioeconômico e realizar encaminhamentos para serviços de saúde específicos. A vigilância do DNPM foi feita em três etapas: (1) avaliação em ambulatório; (2) avaliação e orientações em domicílio; (3) elaboração de relatórios aos gestores de saúde. Foram utilizados os testes DENVER II e o PEDI, além de um questionário socioeconômico. Participam do programa DST/AIDS 15 crianças, sendo 12 soro-revertidas, 1 soropositiva e 2 indefinidas. Doze crianças foram avaliadas, e os domínios mais comprometidos foram linguagem, pessoal-social e motor fino, respectivamente. Quanto ao nível econômico, 73,3% pertenciam ao nível E, e 58,3% das mães eram analfabetas ou cursaram apenas o primário. Crianças filhas de mães HIV positivo, além de fatores biológicos, geralmente estão expostas a fatores de risco ambientais que contribuem para alterações do DNPM. Desta forma, o acompanhamento por uma equipe de profissionais de saúde, em parceria com a família da criança, torna-se uma importante ferramenta para a identificação e intervenção precoce.

    Highly active antiretroviral therapy (HAART) is an effective way of preventing mother-to-child transmission of HIV. However, further studies investigate the effects of short and long term exposure to HAART in-utero and its consequence on child neuropsychomotor development (NPMD). The paper presents a report and discussion of results of an extension project whose objectives were to verify the NPMD of children participating of the STD/AIDS program, to orientate families according to their socioeconomic context and make referrals to specific health services. The NPMD surveillance was divided into three parts: (1) ambulatory evaluation; (2) home evaluation and orientations; (3) reporting health managers. DENVER II and PEDI tests were used and also a socioeconomic questionnaire. Fifteen children were on the program of which 12 uninfected, 1 HIV+ and 2 indeterminate. Twelve children were evaluated and the most impaired domain were language, personal-social and fine motor, respectively. Regarding to socioeconomic status, 73,3% were E level and 58,3% of mothers were analphabet or had primary school. Children born of infected mothers, besides the biological risks, usually are exposed to environment/social risks that can affect the NPMD. Thus, monitoring by a team of health professionals, in partnership with the child's family, becomes an important tool for identification and early intervention.

    A proposta deste estudo foi analisar e verificar a existência de alterações neurofisiológicas nos indivíduos com síndrome de Down, como a hiporreflexia, estática e dinâmica, que, se presente, pode ocasionar prejuízo no controle sensório-motor dos músculos, e consequentemente a hipotonia, contribuindo para alterações no desenvolvimento motor. O sinal eletromiográfico (EMG), início e término do sinal eletromiográfico, foi coletado de 24 voluntários com diagnóstico de síndrome de Down e 25 voluntários como grupo controle, durante 2 situações distintas: período de latência dinâmico e teste de reflexo monossináptico (estático) patelar e aquileu. Em relação aos aspectos neurofisiológicos, observou-se que, para todas as variáveis analisadas, houve diferença estatisticamente significante (p<0,05) entre os grupos. A existência de alterações neurofisiológicas (retardo do período de latência) comprovadas neste estudo, oriundas de um prejuízo no controle sensório-motor, as quais podem ser responsáveis pela hipotonia muscular, mostrou-se como um dos vários possíveis fatores determinantes pelas alterações no desenvolvimento motor dessas crianças, tornando-se importante devido às alterações neuromusculares que podem interferir nas atividades funcionais.

    The purpose of this study was to analyze and verify the existence of neurophysiological changes in individuals with Down syndrome, such as hyporeflexia, static and dynamic, which, if present, may cause impairment in sensory-motor control of muscles, and consequently hypotonia, contributing to changes in motor development. The electromyographic signal (EMG), start and termination electromyographic signal, was collected from 24 volunteers with a diagnosis of Down syndrome and 25 volunteers as controls for 2 different situations: the dynamic latency period and monosynaptic reflex (static) patellar and Achilles. For neurophysiological aspects, it was observed that for all variables analyzed, statistical difference (p<0.05) between groups. The existence of neurophysiological changes (delay of the latency period) demonstrated in this study originated from an impairment in sensory-motor control, which may account for the decreased muscle tone, presents as one of several possible factors for the changes in motor development of children, which shows important due to neuromuscular disorders that can interfere in functional activities.

    A Síndrome Pós-Pólio (SPP) caracteriza-se por um novo episódio de fraqueza muscular e/ou fadiga muscular anormal em indivíduos que apresentaram poliomielite anterior aguda (PAA) anos antes. Ainda são poucos os relatos na literatura sobre o gerenciamento da dor na SSP. O objetivo deste trabalho é analisar o controle da dor na síndrome pós-pólio por meio de um relato de caso. Um homem de 57 anos com SPP foi submetido a 24 sessões de fisioterapia para controle do quadro álgico na síndrome, baseada em recursos eletrotermofototerápicos e cinesioterapia. Para análise da dor pré e pós-intervenção fisioterapêutica foram aplicados dois instrumentos: a Escala Analógica Visual (EVA) e a Intensidade de Dor Presente (ID). O quadro álgico no joelho direito sofreu redução de grau 8 para 4 pela EVA e grau 5 para 2 pela ID, enquanto a dor no tornozelo direito reduziu de grau 2 pela EVA e grau 1 pela ID para zero em ambas as escalas. A dor é apontada como um sintoma predominante por muitos pacientes com SPP. O sujeito deste relato, após o programa de tratamento, apresentou melhora expressiva da dor. Entretanto, devido à escassez da literatura sobre dados substanciais referentes ao seu gerenciamento, especialmente na fisioterapia, novos estudos devem ser propostos para gerar maior fundamentação teórica à abordagem fisioterapêutica na SPP.

    The Post-polio Syndrome (PPS) is characterized by a new episode of muscular weakness and/or abnormal muscular fatigue in subjects that presented acute previous polio (APP) years ago. There still are few studies about the pain management at PPS. The aim of this study is to analyze pain´s management in PPS by a case report. A men with PPS, 57 years, was submitted to 24 sessions of physical therapy for pain´s control in PPS, based at electro-thermo physical therapy features and therapeutics exercises. For pain´s analysis, pre and post physical therapy interventions were applied two instruments: the Analogic Visual Scale (AVS) and Present Pain Intensity (PPI). The pain in right knee was reduced from grade 8 to 4 on AVS, grade 5 to 2 on PPI, while the pain in his right ankle decreased from grade 2 on AVS and 1 on PPI to zero in both scales. The pain is the predominant symptom for many patients with PPS. This relate, the patient after program of treatment presented an improvement of pain. However, because of there are few studies in the literature about the pain management, especially in physical therapy, new studies must be proposed to generate more theorical at Physical therapy intervention in PSS.
  • Complicações respiratórias pós-operatórias em cirurgia bariátrica: revisão da literatura Revisão De Literatura

    Obesidade é um problema de saúde pública mundial com altos índices de morbi-mortalidade. A cirurgia bariátrica tem sido o tratamento mais usado e desperta atenção para seu desenvolvimento e resultados. Porém, até o momento, não há revisão sobre a incidência de complicações respiratórias nesta população. O objetivo deste estudo foi revisar a literatura sobre as complicações respiratórias após cirurgias bariátricas convencionais com a utilização das bases de dados PubMed, Cochrane e Scielo buscando os termos: complicações, pulmonar, pós-operatório e cirurgia bariátrica. Os limites foram os últimos dez anos, adultos, inglês e espanhol. Encontramos 69 artigos e utilizamos 21. As complicações respiratórias mais observadas em cirurgia bariátrica são: embolia pulmonar, atelectasias e pneumonia, estando relacionadas à idade e à hipoventilação. A obesidade mórbida está associada a disfunções respiratórias, incluindo diminuição da resistência cardiorrespiratória e dispneia, sendo as alterações mais comuns: diminuição da ventilação e da complacência torácica, taquipneia e aumento do trabalho muscular respiratório, com altos índices de hipoxemia e fadiga respiratória. Nossos resultados mostram que embolia pulmonar, atelectasias e pneumonias são as complicações respiratórias mais incidentes em cirurgias bariátricas convencionais, sendo os idosos ou portadores de síndrome da hipoventilação e apneia obstrutiva do sono os que apresentam maior risco de desenvolvê-las.

    Obesity is a public health problem worldwide because of the high rates of morbimortality. Bariatric surgery has been the most often treatment and attracts attention on its development and results. However, there is no review about the incidence of respiratory complications in this population. The aim of this study was to review the literature on respiratory complications in conventional bariatric surgery utilizing the databases PubMed, Scielo and Cochrane. The terms searched were complications, pulmonary, postoperative care and bariatric surgery, and the limits, the last ten years, adults, English and Spanish. We found 69 articles, and used 21, showing that the most common respiratory complications in bariatric surgery are pulmonary embolism, atelectasis and pneumonia, being related to age and the presence of hypoventilation. Morbid obesity is associated with respiratory dysfunction, including decreased cardiorespiratory endurance and dyspnea, being the most common changes: the decrease in ventilation and chest wall compliance, and tachypnea and respiratory muscle workload, with high rates of hypoxemia and respiratory fatigue. Our results suggest that pulmonary embolism, atelectasis and pneumonia are the pulmonary complications with the highest incidences in conventional bariatric surgery, and elderly and patients with hypoventilation or syndrome and obstructive sleep apnea have higher risk of developing postoperative pulmonary complications.
