  • Concrescence: can the teeth involved be moved or separated? Orthodontic Insight

    RESUMO A atrofia do ligamento periodontal coloca o dente muito próximo do osso e/ou de outro dente, como ocorre em dentes não irrompidos. O osso interdentário ausente e a falta de estímulo funcional periodontal podem levar à fusão das camadas aposicionais de cemento entre as raízes dos dentes. A concrescência ocorre quase sempre na região dos molares superiores. Assintomática, ela deve sempre ser lembrada quando não se obtém a resposta adequada a uma movimentação ortodôntica, sem uma explicação aparente. Cirurgicamente, quando se está extraindo o dente e ele oferece resistência, insistir não será a melhor estratégia. Movimentar os dentes com concrescência não é conveniente, pois requer forças muito intensas. Depois de separados, esses dentes podem ser considerados normais para a movimentação. É possível separar dois dentes em concrescência, mas depende da extensão da área, do acesso cirúrgico e, em especial, da conveniência clínica. O dente a ser extraído vai reparar-se, com novo cemento depositado na região seccionada. A simples separação, com manutenção da proximidade e da falta de função de um dos dentes, fará com que uma nova concrescência se estabeleça. Transcorrido o período de 1 a 3 meses, os dentes separados poderão, biologicamente, ser movimentados. O detalhe mais importante nessa separação de dentes com concrescência é que o diagnóstico deve ser feito com antecedência, e não no momento da intervenção.

    ABSTRACT The atrophy of the periodontal ligament places the tooth very close to the bone or another tooth, as occurs in unerupted teeth. The absent interdental bone and the lack of functional periodontal stimulus may lead to the fusion of the appositional layers of cement between the roots of the teeth. Concrescence almost always occurs in the region of the maxillary molars. Asymptomatic, it should always be remembered when the proper response to orthodontic movement is not obtained, and there is no apparent explanation. When surgically extracting a tooth and there is resistance, insisting will not be the best strategy. Moving the teeth with concrescence is not convenient, as it requires very intense forces. Once separated, these teeth can be considered normal for movement. It is possible to separate two teeth presenting concrescence, but it depends on the extension of the area, the surgical access and, especially, the clinical convenience. The tooth to be extracted will be repaired with new cement deposited in the sectioned area. The simple separation with the maintenance of the proximity and the lack of function of one of the teeth will cause a new concrescence. After a period of 1 to 3 months, the separated teeth are biologically prepared to be moved. The most important detail in this separation of teeth presenting concrescence is that the diagnosis should be made in advance, and not at the time of the intervention.
  • Evaluation of color changes of white spot lesions treated with three different treatment approaches: an in-vitro study Online Article

    RESUMO Objetivo: avaliar qualitativamente e quantitativamente os efeitos nas mudanças e estabilidade da cor de lesões de mancha branca (LMBs), após tratamento com infiltração de resina, em comparação aos tratamentos com pasta de dentes com nanopartículas de hidroxiapatita (Nano-HA) e com microabrasão. Métodos: As LMBs foram criadas artificialmente em 60 superfícies de esmalte dentário de pré-molares humanos e aleatoriamente divididas em quatro grupos (n=15, cada): pasta de dentes Nano-HA, microabrasão (Opalusture), tratamento com infiltração de resina (Icon) e saliva artificial (grupo controle). A mudança de cor (ΔE) de cada espécime foi aferida com um espectrofotômetro odontológico (Vita Easyshade) em diferentes tempos: início do estudo, após a criação das LMBs, após a aplicação dos tratamentos, um mês, três meses e seis meses após a aplicação dos tratamentos. Resultados: Os valores de ΔE não diferiram significativamente entre os quatro grupos ao início do estudo (p > 0,05). A infiltração com resina Icon melhorou significativamente a coloração das LMBs imediatamente após a sua aplicação, com o menor valor de ΔE (3,00 ± 0,59), quando comparada às outras modalidades de tratamento (p < 0,001). Não houve mudanças significativas nos valores de ΔE (p > 0,05) em qualquer um dos grupos durante os intervalos de acompanhamento (um mês, três meses e seis meses após a aplicação do tratamento). Conclusão: A infiltração de resina é capaz de melhorar a coloração das LMBs e restaurar a aparência natural do esmalte de forma superior à pasta de dentes com Nano-HA e à microabrasão.

    ABSTRACT Objective: To qualitatively and quantitatively assess the color changes effect and the color stability of the resin infiltrant on white spot lesions (WSLs), in comparison with nano-hydroxyapatite (nano-HA) toothpaste and microabrasion. Methods: WSLs were artificially created on sixty human premolars enamel surfaces and randomly assigned to equal four groups (n = 15 each): nano-HA toothpaste, microabrasion (Opalusture), resin infiltrant (Icon) treatment, or artificial saliva (control group). The color change (ΔE) of each specimen was measured by dental spectrophotometer (Vita Easyshade) at different time points: baseline, after WSLs’ creation, after application of treatments, one month, three and six months after treatments application. Results: The ΔE value did not differ significantly for the four groups at baseline measurement before treatment (p> 0.05). Icon resin infiltrant improved the color of WSLs significantly immediately after its application, giving the lowest ΔE value (3.00 ± 0.59), when compared to other treatments (p< 0.001). There were no significant changes in ΔE (p> 0.05) for all groups during the follow up intervals (one month, three and six months after treatments application). Conclusion: Resin infiltrant can improve the color of WSLs and restore the natural appearance of enamel better than nano-HA toothpaste and microabrasion.
  • Effect of Yd:YAG laser irradiation on the shear bond strength of orthodontic metal brackets Original Article

    RESUMO Objetivo: O objetivo do presente estudo foi avaliar o efeito da aplicação do laser Yd:YAG sobre a superfície metálica da base de braquetes ortodônticos, a resistência da colagem ao cisalhamento (RU) e os locais das falhas nas interfaces adesivas. Métodos: Os braquetes foram divididos em dois grupos, de acordo com o tipo de base: OP (One Piece - monobloco, base jateada) e OPL (One Piece - monobloco, base irradiada a laser). Foram selecionados 40 incisivos bovinos hígidos para a colagem de braquetes na superfície do esmalte. Os testes de RU foram realizados por meio de uma máquina de ensaio universal EMIC DL1000. O Índice de Adesivo Remanescente (ARI) e a caracterização das superfícies foram realizados empregando-se estereomicroscópio e microscópio eletrônico de varredura (MEV). O teste t de Student foi aplicado para comparar a RU entre os dois grupos (p< 0,05). O ARI foi avaliado por meio da análise de Frequência e teste do Qui-quadrado. Resultados: O grupo OPL obteve maiores valores de RU (p<0,001) do que o grupo OP (43,95 MPa e 34,81 MPa, respectivamente), e o ARI obteve diferença significativa (p<0,001) entre os grupos OPL (ARI 0 = 100%) e OP (ARI 0 = 15%). A análise proveniente do MEV mostrou alta interação adesiva entre a resina e a superfície metálica da base irradiada pelo feixe de laser. Conclusões: A irradiação com laser Yd:YAG sobre a superfície da base de braquetes metálicos aumentou os valores da RU, demonstrando que as falhas adesivas ocorreram na interface esmalte/adesivo. O uso de braquetes modificados com irradiação a laser pode ser adotado como alternativa aos braquetes com bases convencionais nos casos em que uma maior retentividade for requerida para reduzir as falhas de adesividade.

    ABSTRACT Objective: The purpose of this study was to evaluate the effect of the Yd:YAG laser irradiation on orthodontic bracket base surface. Shear bond strength (SBS) values and sites of the bonding failure interfaces were quantified. Methods: Brackets were divided into two groups: OP (One Piece - integral sandblast base) and OPL (One Piece - laser irradiation). The brackets were randomly bonded on an intact enamel surface of 40 bovine incisors. The SBS tests were carry out using a universal test machine. A stereomicroscopy was used to evaluate the adhesive remnant index (ARI), and surface characterization was performed by scanning electron microscopy (SEM). Student’s t-test was used to compare the SBS between the two groups (p< 0.05). Frequencies and chi-square analysis were applied to evaluate the ARI scores. Results: OPL group showed higher value (p< 0.001) of SBS than OP group (43.95 MPa and 34.81 MPa, respectively). ARI showed significant difference (p< 0.001) between OPL group (ARI 0 = 100%) and OP group (ARI 0 = 15%). SEM showed a higher affinity between the adhesive and the irradiated laser base surface. Conclusions: Yd:YAG laser irradiation on bracket base increased SBS values, showing that bonding failure occurs at the enamel/adhesive interface. Laser-etched bracket base may be used instead of conventional bases in cases where higher adhesion is required, reducing bracket-bonding failure.
  • Orthodontic management of a non-syndromic patient with concomitant bimaxillary hypohyperdontia: a case report Original Article

    RESUMO Introdução: A agenesia é uma das anomalias dentárias mais comuns; porém, a ocorrência concomitante de variações numéricas do tipo hipo-hiperdontia em ambas as arcadas é extremamente rara. Objetivo: Descrever o tratamento ortodôntico bem-sucedido de uma paciente não sindrômica que apresentava agenesia bilateral dos caninos inferiores e dois dentes supranumerários na região de linha média superior. Descrição: Paciente do sexo feminino com 21 anos de idade, apresentando queixa principal de protrusão dos incisivos centrais superiores. Foi diagnosticada com leve Classe II esquelética, relação de molares em Classe III de Angle, sobressaliência aumentada e hipo-hiperdontia. Além disso, a paciente apresentava mordida aberta anterior associada ao hábito de interposição lingual. Dois dispositivos de ancoragem temporária (DATs) foram instalados por vestibular na região dos molares superiores, para controle vertical. A retração dos dentes anteriores foi feita após a extração dos primeiros pré-molares superiores, para reduzir a sobressaliência acentuada. A mecânica adotada envolveu o uso de braquetes linguais na arcada superior e barra transpalatina para controle da ancoragem. Resultados: Após 35 meses de fase ativa desse tratamento complexo, com substituição dos pré-molares, obteve-se oclusão aceitável, com aumento da força oclusal e da área de contatos oclusais, bem como movimentos excursivos funcionais bem estabelecidos e sem interferências. Após 29 meses de contenção, verificou-se estabilidade da oclusão e do perfil facial. Conclusão: Esse relato de caso clínico mostra que o tratamento ortodôntico com substituição de dentes ausentes associados à hipo-hiperdontia é uma opção efetiva para a melhora dos aspectos estéticos e funcionais.

    ABSTRACT Introduction: Tooth agenesis is one of the most common dental anomalies; however, the concomitant occurrence of opposite dental numerical variation of hypohyperdontia is extremely rare. Objective: To report the successful orthodontic management of a patient with non-syndromic concomitant bilateral agenesis of mandibular canines and two midline inverted supernumerary maxillary teeth. Case report: 21-year-old female patient with a chief complaint of protrusive right maxillary central incisor. The patient was diagnosed with a mild Class II skeletal base, Angle Class III molar relationship and increased overjet associated with hypohyperdontia. Anterior open bite accompanied with tongue-thrusting habit were also observed. Two temporary anchorage devices (TADs) were implanted at the buccal side of the maxillary molar region to control vertical height. Anterior teeth retraction was done after extraction of the maxillary first premolars, to improve the excessive overjet. The treatment mechanics involved lingual brackets system for the maxillary arch and transpalatal arch for anchorage control. Results: The total active treatment period was 35 months. Acceptable occlusion with increased bite force and contact area as well as functional excursion were established without interference, following complex orthodontic treatment with premolar substitution. The resultant occlusion and a satisfactory facial profile were maintained after 29 months of retention. Conclusion: The present case report provides implications regarding the orthodontic treatment of hypohyperdontia-associated substitution for missing teeth as an effective option for improving aesthetic and functional aspects.
  • Comparison of changes in the dental transverse and sagittal planes between patients treated with self-ligating and with conventional brackets Original Article

    RESUMO Introdução: Já foram enumeradas diversas vantagens quanto à eficiência dos braquetes autoligáveis (BAL). Apesar das controvérsias na literatura, resultados clínicos confirmam que o “desenvolvimento da arcada” demanda menos extrações. Objetivo: O objetivo deste estudo foi comparar as mudanças nos planos transversal e sagital em pacientes tratados com braquetes convencionais (BC) e pacientes tratados com BAL e arcos superdimensionados. Métodos: A amostra foi selecionada de um grupo de 300 pacientes tratados consecutivamente por um mesmo ortodontista: 51 pacientes com BAL e arcos superdimensionados, e 55 com BC. Esses dois grupos foram comparados a um grupo controle (GC) de 20 pacientes. Todos os modelos de gesso foram escaneados e pontos de referência dentários foram mensurados, para se identificar as mudanças entre o início (T0) e o fim do tratamento (T1). Radiografias cefalométricas laterais foram analisadas para identificar mudanças nos incisivos inferiores (IMPA) e na angulação distal dos primeiros molares inferiores (MAng). A confiabilidade intraexaminador foi testada por meio de uma análise de regressão linear. Para garantir que os grupos fossem comparáveis em T0, realizou-se o teste ANOVA com intervalo de confiança (IC) de 95% para todos os valores. Para avaliar as mudanças entre T0 e T1 em todos os grupos, foi utilizado o teste t de Student com IC de 95%. Por fim, os resultados dos três grupos foram comparados utilizando-se o teste ANOVA (IC 95%) e um teste post-hoc. Resultados: Foram registrados aumentos em todas as variáveis transversais nos dois grupos tratados (BC e BAL), com exceção da distância intercaninos inferiores no grupo BAL. As maiores diferenças de T0 para T1 ocorreram no IMPA do grupo BC e na angulação distal de molares (MAng) do grupo BAL. Conclusões: Ambos os grupos, BAL com arcos superdimensionados e BC, apresentaram aumento em todas as variáveis no plano transversal, com exceção da distância intercaninos inferiores no grupo BAL. Em comparação ao grupo BC, menores diferenças nos valores de IMPA foram observadas no grupo BAL, no qual aconteceu angulação distal de molares.

    ABSTRACT Introduction: Several advantages have been established regarding the efficiency of self-ligating brackets (SL). In spite of some controversy surrounding this question in the literature, clinical results confirm that “arch development” requires fewer extractions. Objective: The objective of this study was to compare changes in the transverse and sagittal planes in patients treated with conventional ligating brackets (CL)as well as in patients treated with SL brackets and oversized arches. Methods: A sample was selected from a pool of 300 consecutive cases treated by a single clinician: 51 patients with SL brackets and oversized wires, and 55 with CL brackets. These two groups were compared with a control group of 20 patients. All plaster models were scanned and dental landmarks were measured to identify changes from commencement (T0) to conclusion (T1) of treatment. Lateral cephalograms were analyzed for changes in the lower incisor (IMPA) and the first lower molar distal angulation (MAng). Intraoperator reliability was tested with linear regression analysis. To assure all groups were comparable at T0, an ANOVA test with a 95%confidence interval (CI) was performed for all values. To assess changes from T0 to T1 in all groups, a Student’s t-test with 95% CI was used. Finally, results from the three groups were compared using an ANOVA-test (95% CI) and a post-hoc test. Results: Increases in all the transverse variables were recorded in the two groups treated (SL and CL), except for the lower intercanine distance in the SL group. IMPA difference from T0 to T1 was higher in the CL group, and molar distal angulation (MAng) took place in the SL group. Conclusions: Self-ligating brackets with oversized arches and conventional ligating brackets showed increases in all variables in the transverse plane, except for the SL group at the mandibular intercanine distance. In comparison with the CL group, fewer different IMPA values were observed in the SL group, in which distal molar angulation occurred.
  • Does the presence of unilateral gingival recession on maxillary canines influence smile esthetics? Original Article

    RESUMO Objetivo: o objetivo do presente estudo foi determinar a percepção estética do sorriso, por ortodontistas, periodontistas e leigos, em relação à presença de recessão gengival no canino superior esquerdo. Métodos: foram selecionadas duas imagens aproximadas do sorriso (frontal e oblíqua) de uma mulher leucoderma. As imagens foram manipuladas para criar diferentes níveis de recessão gengival no canino superior esquerdo, em incrementos de 0,5 mm. Foram, então, distribuídas aleatoriamente em um álbum, que foi entregue a 135 avaliadores: 45 ortodontistas, 45 periodontistas e 45 leigos. Cada avaliador foi convidado a avaliar a atratividade das imagens em uma escala visual analógica. Os dados foram analisados estatisticamente pela ANOVA e pós-teste de Tukey e teste t de Student. Resultados: para ortodontistas e periodontistas, todos os níveis de recessão foram detectados como antiestético, em ambas as formas de visualização. Para os leigos, na vista frontal, a recessão gengival foi considerada antiestética a partir de 1,5 mm; enquanto na vista oblíqua, a partir de 1,0 mm. Conclusão: os resultados demonstraram que a presença de recessão gengival em caninos superiores pode impactar negativamente a atratividade do sorriso, dependendo do tipo de avaliador e da magnitude da recessão.

    ABSTRACT Objective: The objective of this study was to determine orthodontists’, periodontists’, and laypersons’ perception of smile esthetics, regarding the presence of different levels of gingival recession on the maxillary left canine. Material and Methods: Two close-up smile images (frontal and oblique) of a white female were selected for this study. The images were digitally altered to create different levels of gingival recession on maxillary left canine, in 0.5-mm increments. They were randomly arranged into a photo album that was shown to 135 evaluators: 45 orthodontists, 45 periodontists, and 45 laypersons. Each evaluator was asked to rate the smile attractiveness, using to a visual analog scale. Data were analyzed statistically using ANOVA, Tukey’s post-hoc test, and Student t-test. Results: According to the orthodontists and periodontists, all levels of recession were considered as unesthetic in both types of images. According to the laypersons, gingival recession > 1.5 mm in the frontal image and > 1.0 mm in the oblique image were considered unesthetic. Conclusion: The results showed that the presence of unilateral gingival recession on maxillary canines may negatively influence smile attractiveness, depending on the evaluator type and the level of the recession.
  • Impact of catastrophizing on pain during orthodontic treatment Original Article

    RESUMO Objetivo: O presente estudo propôs-se a investigar a influência da catastrofização e outros fatores relacionados à dor durante o tratamento ortodôntico. Métodos: Foram selecionados 27 pacientes em tratamento com braquetes Straight-wire, slot 0,022” x 0,028”, na fase de alinhamento e nivelamento com fios de níquel-titânio. Usando Escalas Visuais Analógicas, mensurou-se a intensidade da dor em seis momentos após a ativação do aparelho: primeiras 6 horas; após 1, 2, 3, 5 e 7 dias. Por meio de regressão linear múltipla e regressão stepwise, avaliou-se a influência das seguintes variáveis sobre a dor: catastrofização, sexo, idade, tempo de tratamento, turno de atendimento (manhã ou tarde) e diâmetro do fio ortodôntico. Resultados: A maior intensidade de dor foi relatada 24 horas após a consulta de ativação do aparelho. Assim, esses dados foram usados para analisar os fatores associados ao nível de dor. Na análise estatística univariada, idade (r = 0,062, p= 0,7586), sexo (p= 0,28), catastrofização (r = -0,268, p= 0,1765) e diâmetro do fio ortodôntico (r = 0,0245, p= 0,2181) não estavam associados à dor. Apesar disso, a catastrofização foi incluída no modelo de regressão múltipla, por ser uma variável de maior interesse nesse estudo. A duração do tratamento ortodôntico (r = 0,6045, p= 0,0008) e o turno no qual o aparelho foi ativado (p= 0,0106) mostraram associação estatisticamente significativa com a dor e também foram incluídos na regressão multivariada, que mostrou que cerca de 32% da ocorrência de dor no tratamento ortodôntico poderiam ser explicados pelo tempo de tratamento (R2= 0,32, p= 0,0475). Catastrofização (R2= 0,0006, p= 0,8881) e turno de ativação do aparelho (R2= 0,037, p= 0,2710) não tiveram influência significativa sobre a ocorrência de dor. Conclusão: A dor após a ativação do aparelho não está associada à catastrofização, bem como à idade, sexo, diâmetro do fio ortodôntico e turno de atendimento.

    ABSTRACT Objective: This study proposed to investigate the influence of catastrophizing and others factors related to pain during orthodontic treatment. Methods: 27 patients with 0.022 x 0.028-in Straight-wire brackets were evaluated during alignment and leveling phase with nickel-titanium wires. Visual Analog Scales measured the intensity of orthodontic pain at six moments after a clinical appointment: 6 first hours; 1, 2, 3, 5, and 7 days. Multiple linear regression and stepwise approach assessed the influence of the following variables on pain: catastrophizing, sex, age, duration of treatment, clinical appointment time (morning or afternoon), and wire diameter. Results: The highest pain intensity was reported 24 hours after activation. These data were used to analyze factors associated with pain level. Age (r = 0.062, p= 0.7586), sex (p= 0.28), catastrophizing (r = -0.268, p= 0.1765), and orthodontic wire diameter (r = 0.0245, p= 0.2181) were not correlated with orthodontic pain in the univariate statistics. Catastrophizing was included in the multiple regression model because it was of great interest. Duration of orthodontic treatment (r = 0.6045, p= 0.0008) and the time when orthodontic appliance was activated (p= 0.0106) showed statistical significant associations with pain, and were also included in the multivariate regression, which showed that about 32% of orthodontic pain could be explained by the duration of treatment (R2= 0.32, p= 0.0475). Catastrophizing (R2= 0.0006, p= 0.8881) and clinical appointment time were not significantly associated with pain (R2= 0.037, p= 0.2710). Conclusions: Pain after activation of fixed orthodontic appliance is not associated with catastrophizing as well as age, sex, orthodontic wire diameter, and period of activation.

    RESUMO Os problemas transversais devem ser priorizados no tratamento ortodôntico e corrigidos assim que diagnosticados, para restituir a correta relação transversal entre maxila e mandíbula e, consequentemente, restabelecer o crescimento maxilar normal. A correção pode ser realizada em nível esquelético, por meio da abertura da sutura palatina mediana (SPM), ou por expansão dentoalveolar. A opção de tratamento depende de alguns fatores como idade, sexo, grau de hipoplasia da maxila e a maturação da SPM. Assim, o objetivo do presente trabalho foi discutir as diferentes abordagens terapêuticas para correção da hipoplasia maxilar em pacientes com maturação esquelética avançada.

    ABSTRACT Transverse deficiencies should be a priority in orthodontic treatment, and should be corrected as soon as diagnosed, to restore the correct transverse relationship between maxilla and mandible and, consequently, normal maxillary growth. Corrections may be performed at the skeletal level, by opening the midpalatal suture, or by dentoalveolar expansion. The choice of a treatment alternative depends on certain factors, such as age, sex, degree of maxillary hypoplasia and maturation of the midpalatal suture. Thus, the present study discusses different treatment approaches to correct maxillary hypoplasia in patients with advanced skeletal maturation.

    RESUMO Introdução: A análise da estética do sorriso pode ser facilitada pelo uso de um template que agregue diversas referências estéticas dentárias e que auxilie e simplifique o diagnóstico, oferecendo diretrizes para o planejamento estético de tratamentos ortodônticos e integrados. Objetivo: Apresentar uma metodologia que possibilite a avaliação estética do sorriso, de maneira simples e objetiva, através do template digital SmileCurves (TSC), utilizado por meio da sobreposição de imagens de fotografias intrabucais e do sorriso aproximado. Conclusão: O uso do template TSC facilita a análise da estética do sorriso de modo simples e objetivo.

    ABSTRACT Introduction: The aesthetic analysis of a smile may be facilitated by the use of a template that provides several dental aesthetic references and support for the diagnosis, simplifying it and defining guidelines for the aesthetic planning of orthodontic and integrated treatments. Objective: To describe a simple and objective procedure for the evaluation of smile aesthetics using the SmileCurves digital template (SCT), based on the superimposition of intraoral photographic images and close-up views of a smile. Conclusion: SCT is a simple and objective tool for the aesthetic analysis of a smile.
