Foi feita uma revisão não sistemática da literatura. Encontramos numerosas publicações sobre o tema nas quais se verifica controvérsia quanto à capacidade das infecções por geohelmintos de inibir a reatividade ao teste alérgico cutâneo e os sintomas das doenças alérgicas. Por outro lado, estudos em pacientes infectados por Schistosoma spp., apesar de escassos, sugerem que estes helmintos são capazes de inibir a reatividade ao teste alérgico cutâneo e os sintomas de asma. Nos estudos in vitro, as evidências sugerem que as helmintíases inibem as respostas imunes mediadas por linfócitos T auxiliares do tipo 1 e 2, o que abre novas possibilidades terapêuticas para o tratamento de doenças do sistema imunológico.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[To describe the principal evidence in the literature regarding the interrelationship among helminth infections, atopy, and asthma, a nonsystematic review of the literature was conducted. Among the publications on the subject, we found a number in which there was controversy regarding the capacity of geohelminth infections to inhibit responsiveness to skin allergy tests and to minimize the symptoms of allergic diseases. However, although small in number, studies of patients infected with Schistosoma spp. suggest that these helminths can inhibit the responsiveness to skin allergy testing and minimize asthma symptoms. Evidence provided by in vitro studies suggests that helminthiases inhibit T helper 1- and T helper 2-type immune responses. This opens new therapeutic possibilities for the treatment of immune system diseases.]]></p></abstract>

Nos estudos <i>in vitro</i>, as evid&ecirc;ncias    sugerem que as helmint&iacute;ases inibem as respostas imunes mediadas por linf&oacute;citos    T auxiliares do tipo 1 e 2, o que abre novas possibilidades terap&ecirc;uticas    para o tratamento de doen&ccedil;as do sistema imunol&oacute;gico.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Descritores: </b>Asma; Helmintos; Hipersensibilidade; Epidemiologia.</font></p>  <hr size="1" noshade>     <p>&nbsp;</p>      <p>&nbsp;</p>    <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Introdu&ccedil;&atilde;o</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Apesar do grande avan&ccedil;o cient&iacute;fico na &aacute;rea da imunologia e das novas op&ccedil;&otilde;es terap&ecirc;uticas dispon&iacute;veis, a preval&ecirc;ncia das doen&ccedil;as al&eacute;rgicas, tais como a asma e a rinite, tem aumentado nos pa&iacute;ses desenvolvidos.<sup>(1,2)</sup> Por outro lado, a preval&ecirc;ncia da asma &eacute; baixa nos pa&iacute;ses pobres.<sup>(3)</sup> &Eacute; importante encontrar explica&ccedil;&otilde;es para este fato, que possivelmente est&aacute; relacionado com a influ&ecirc;ncia do meio ambiente no sistema imunol&oacute;gico.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Nos pa&iacute;ses ricos, a exposi&ccedil;&atilde;o reduzida a agentes infecciosos est&aacute; entre os fatores ambientais que podem modificar o sistema imunol&oacute;gico dos seres humanos e contribuir para o aumento da preval&ecirc;ncia de doen&ccedil;as al&eacute;rgicas. As infec&ccedil;&otilde;es por helmintos, por exemplo, s&atilde;o raras nos pa&iacute;ses desenvolvidos. Diante disto, diversos estudos foram conduzidos nos &uacute;ltimos anos na tentativa de esclarecer se a aus&ecirc;ncia de exposi&ccedil;&atilde;o &agrave; infec&ccedil;&atilde;o por helmintos pode contribuir para o surgimento de doen&ccedil;as al&eacute;rgicas. Al&eacute;m disto, existe a necessidade de desvendar os mecanismos imunopatol&oacute;gicos que poderiam determinar a inibi&ccedil;&atilde;o da alergia nos indiv&iacute;duos infectados por helmintos.</font></p>      ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Uma investiga&ccedil;&atilde;o mais aprofundada deste tema pode identificar alternativas para novos tratamentos para as doen&ccedil;as do sistema imunol&oacute;gico. Uma delas &eacute; o uso de ant&iacute;genos de helmintos para o desenvolvimento de vacinas para doen&ccedil;as al&eacute;rgicas e auto-imunes. O objetivo deste artigo &eacute; descrever as principais evid&ecirc;ncias encontradas na literatura acerca da inter-rela&ccedil;&atilde;o entre infec&ccedil;&otilde;es helm&iacute;nticas e alergia.</font></p>      <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Resposta imune    nas doen&ccedil;as al&eacute;rgicas e nas infec&ccedil;&otilde;es helm&iacute;nticas</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Uma das fun&ccedil;&otilde;es do sistema imunol&oacute;gico &eacute; proteger o indiv&iacute;duo contra agentes infecciosos. Entretanto, o funcionamento inadequado deste sistema pode ser a causa de doen&ccedil;as, como as alergias e as doen&ccedil;as auto-imunes. O sistema imunol&oacute;gico atua por meio de dois padr&otilde;es de resposta imune adquirida: a resposta imune mediada por linf&oacute;citos T auxiliares do tipo 1 (T helper 1, Th1, em ingl&ecirc;s), e a resposta imune Th2. A resposta imune Th1 ocorre na rea&ccedil;&atilde;o &agrave;s infec&ccedil;&otilde;es virais e bacterianas e nas doen&ccedil;as auto-imunes. A resposta imune Th2 ocorre na rea&ccedil;&atilde;o &agrave;s infec&ccedil;&otilde;es helm&iacute;nticas e nas doen&ccedil;as al&eacute;rgicas, como a asma, a rinite e o eczema.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Na resposta imune Th2, os ant&iacute;genos de helmintos ou al&eacute;rgenos estimulam os linf&oacute;citos T a produzir citocinas Th2, como a interleucina (IL) 4 e a IL-5. A IL-4 induz os linf&oacute;citos B a produzir imunoglobulina E (IgE) e a IL-5 atrai e ativa os eosin&oacute;filos. A eosinofilia e o aumento do n&iacute;vel s&eacute;rico de IgE s&atilde;o, portanto, caracter&iacute;sticas da resposta imune Th2. Uma parte da IgE produzida durante a resposta imune Th2 &eacute; ant&iacute;geno-espec&iacute;fica. Quando a IgE espec&iacute;fica se liga a receptores de alta afinidade na superf&iacute;cie de mast&oacute;citos e bas&oacute;filos, ela deixa o sistema imune pronto para rea&ccedil;&otilde;es al&eacute;rgicas a qualquer exposi&ccedil;&atilde;o ao al&eacute;rgeno. A resposta imune Th2 &eacute; refor&ccedil;ada sempre que o ant&iacute;geno se liga &agrave; IgE espec&iacute;fica na superf&iacute;cie dos mast&oacute;citos, que degranulam, liberando mediadores da rea&ccedil;&atilde;o al&eacute;rgica imediata (histamina, prostaglandinas, leucotrienos) e tamb&eacute;m citocinas pr&oacute;-inflamat&oacute;rias (IL-4, IL-13, RANTES).<sup>(4,5)</sup></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Nem toda resposta    imune Th2 &eacute; igual. Na resposta imune &agrave; infec&ccedil;&atilde;o    por helmintos, como o <i>Schistosoma mansoni</i>,<sup>(6,7)</sup> o <i>S. haematobium</i><sup>(8)</sup>    e o <i>Onchocerca volvulus</i>,<sup>(9)</sup> al&eacute;m da produ&ccedil;&atilde;o    aumentada de IL-4 e IL-5, tamb&eacute;m ocorre produ&ccedil;&atilde;o aumentada    de IL-10. A IL-10 &eacute; uma citocina com a&ccedil;&atilde;o imunossupressora    que parece ser importante no estabelecimento da toler&acirc;ncia imunol&oacute;gica    do hospedeiro a estes helmintos, que, em alguns casos, sobrevivem por at&eacute;    30 anos.<sup>(10)</sup> Por outro lado, em outras infec&ccedil;&otilde;es helm&iacute;nticas,    como no caso do <i>Ascaris lumbricoides</i>, n&atilde;o existe produ&ccedil;&atilde;o    aumentada de IL-10.<sup>(11-13)</sup> Na resposta imune Th2 observada nas doen&ccedil;as    al&eacute;rgicas, a produ&ccedil;&atilde;o de IL10 &eacute; reduzida<sup>(14,15)</sup>    e o hapl&oacute;tipo que determina a produ&ccedil;&atilde;o elevada desta citocina    &eacute; menos freq&uuml;ente em pacientes asm&aacute;ticos do que em pacientes    n&atilde;o asm&aacute;ticos.<sup>(16)</sup> A produ&ccedil;&atilde;o reduzida    de IL-10 nas doen&ccedil;as al&eacute;rgicas &eacute; considerada plaus&iacute;vel,    pois &eacute; poss&iacute;vel que a a&ccedil;&atilde;o antiinflamat&oacute;ria    desta citocina impe&ccedil;a a progress&atilde;o da inflama&ccedil;&atilde;o    al&eacute;rgica.</font></p>      <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Epidemiologia    das doen&ccedil;as al&eacute;rgicas e hip&oacute;tese da higiene</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O estudo chamado The International Study of Asthma and Allergies in Childhood realizou uma investiga&ccedil;&atilde;o extensa da preval&ecirc;ncia de asma no mundo. Neste estudo, a preval&ecirc;ncia de sibilos no &uacute;ltimo ano em crian&ccedil;as de 13 e 14 anos foi bastante vari&aacute;vel, indo de 2,1% na Indon&eacute;sia at&eacute; 32,2% na Inglaterra. No Brasil, a preval&ecirc;ncia foi de 23,3%. Pa&iacute;ses industrializados, de cultura ocidental e de l&iacute;ngua inglesa, e alguns pa&iacute;ses da Am&eacute;rica Latina, t&ecirc;m maior preval&ecirc;ncia de asma do que pa&iacute;ses em desenvolvimento.<sup>(3)</sup> Apesar de fatores gen&eacute;ticos serem importantes para explicar a variabilidade da preval&ecirc;ncia de asma no mundo, a gen&eacute;tica n&atilde;o explica o acentuado crescimento recente da preval&ecirc;ncia de asma, rinite al&eacute;rgica, eczema e reatividade ao teste al&eacute;rgico cut&acirc;neo verificado em v&aacute;rios pa&iacute;ses,<sup>(1,2)</sup> o que provavelmente est&aacute; relacionado com mudan&ccedil;as ambientais ocorridas nas &uacute;ltimas d&eacute;cadas.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Entre os fatores ambientais que podem influenciar o surgimento de doen&ccedil;as al&eacute;rgicas est&atilde;o as infec&ccedil;&otilde;es na inf&acirc;ncia. Em rec&eacute;m nascidos, a popula&ccedil;&atilde;o de linf&oacute;citos T do cord&atilde;o umbilical tem atividade imunol&oacute;gica predominantemente Th2, semelhante ao que ocorre em indiv&iacute;duos al&eacute;rgicos.<sup>(17)</sup> &Eacute; poss&iacute;vel supor que haja uma predisposi&ccedil;&atilde;o natural para o desenvolvimento de doen&ccedil;as al&eacute;rgicas na inf&acirc;ncia e que as doen&ccedil;as infecciosas adquiridas nesta faixa et&aacute;ria contribuam para o desenvolvimento de um equil&iacute;brio da atividade imunol&oacute;gica, prevenindo o surgimento das alergias. Esta &eacute; a hip&oacute;tese da higiene,<sup>(18)</sup> que tem o respaldo de publica&ccedil;&otilde;es que demonstraram associa&ccedil;&atilde;o inversa entre alergia e exposi&ccedil;&atilde;o a infec&ccedil;&otilde;es virais, bacterianas e helm&iacute;nticas.<sup>(20-24)</sup> De acordo com a hip&oacute;tese da higiene, as pol&iacute;ticas de vacina&ccedil;&atilde;o e de saneamento b&aacute;sico implantadas nas &uacute;ltimas d&eacute;cadas em pa&iacute;ses desenvolvidos previnem doen&ccedil;as infecciosas na inf&acirc;ncia, o que impede o equil&iacute;brio imunol&oacute;gico, explicando o aumento da preval&ecirc;ncia de doen&ccedil;as al&eacute;rgicas.</font></p>      ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Infec&ccedil;&otilde;es    helm&iacute;ticas e alergia</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Os resultados dos    estudos que avaliaram a inter-rela&ccedil;&atilde;o entre infec&ccedil;&atilde;o    por helmintos, reatividade ao teste al&eacute;rgico cut&acirc;neo e sintomas    de doen&ccedil;as al&eacute;rgicas s&atilde;o controversos. Em estudos transversais    &eacute; poss&iacute;vel observar que indiv&iacute;duos infectados pelos helmintos    <i>S. mansoni</i> e <i>S. haematobium</i> t&ecirc;m menor freq&uuml;&ecirc;ncia    de teste cut&acirc;neo positivo para aeroal&eacute;rgenos.<sup>(8,23)</sup>    Em um estudo prospectivo, alguns autores demonstraram que a freq&uuml;&ecirc;ncia    de sintomas de asma, uma doen&ccedil;a associada &agrave; atopia, &eacute; menor    em pacientes infectados por <i>S. mansoni</i>,<sup>(25)</sup> sugerindo que    este helminto pode inibir a inflama&ccedil;&atilde;o al&eacute;rgica das vias    respirat&oacute;rias. Estudos com base populacional demonstraram que a infec&ccedil;&atilde;o    por geohelmintos (<i>A. lumbricoides</i>, <i>Trichuris trichiura</i> e Ancylostomidae)    tamb&eacute;m inibe a reatividade ao teste al&eacute;rgico cut&acirc;neo<sup>(24)</sup>    e a preval&ecirc;ncia de sibil&acirc;ncia, um indicador do diagn&oacute;stico    de asma.<sup>(26)</sup> Em um ensaio cl&iacute;nico foi poss&iacute;vel observar    que o tratamento com albendazol e praziquantel foi capaz de aumentar a reatividade    cut&acirc;nea a aeroal&eacute;rgenos em uma popula&ccedil;&atilde;o infectada    por geohelmintos,<sup>(27)</sup> sugerindo que &eacute; a infec&ccedil;&atilde;o    que inibe a alergia e n&atilde;o a alergia que protege contra a infec&ccedil;&atilde;o.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Alguns estudos,    entretanto, demonstraram que pacientes infectados com geohelmintos apresentavam    maior freq&uuml;&ecirc;ncia de reatividade ao teste al&eacute;rgico cut&acirc;neo.<sup>(28,29)</sup>    Al&eacute;m disto, um ensaio cl&iacute;nico com pacientes moradores de favelas    demonstrou que o tratamento com albendazol reduziu os sintomas de asma,<sup>(30)</sup>    sugerindo que a infec&ccedil;&atilde;o por geohelmintos poderia piorar a inflama&ccedil;&atilde;o    al&eacute;rgica das vias respirat&oacute;rias destes pacientes. Adicionalmente,    um estudo transversal<sup>(31)</sup> e um ensaio cl&iacute;nico<sup>(32)</sup>    publicados recentemente sugerem que as infec&ccedil;&otilde;es por geohelmintos    n&atilde;o influenciam na reatividade ao teste al&eacute;rgico cut&acirc;neo    ou nos sintomas de asma. (<a href="/img/revistas/jbpneu/v33n3/14t1.gif">Tabela 1</a>)</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Existem algumas    propostas de explica&ccedil;&atilde;o para estes resultados conflitantes. Como    muitos helmintos t&ecirc;m ciclo pulmonar na fase aguda da infec&ccedil;&atilde;o,    &eacute; poss&iacute;vel que, durante esta fase, haja piora dos sintomas de    asma e que, na infec&ccedil;&atilde;o cr&ocirc;nica, os sintomas melhorem. Outra    explica&ccedil;&atilde;o poderia estar relacionada &agrave; carga parasit&aacute;ria    da popula&ccedil;&atilde;o. &Eacute; poss&iacute;vel que a inibi&ccedil;&atilde;o    das manifesta&ccedil;&otilde;es al&eacute;rgicas ocorra apenas em indiv&iacute;duos    infectados com alta carga parasit&aacute;ria. Alguns estudos indicam que indiv&iacute;duos    com baixa carga parasit&aacute;ria n&atilde;o t&ecirc;m inibi&ccedil;&atilde;o    do teste al&eacute;rgico cut&acirc;neo<sup>(13)</sup> ou t&ecirc;m inibi&ccedil;&atilde;o    fraca,<sup>(31)</sup> refor&ccedil;ando esta hip&oacute;tese. Entretanto, outros    estudos demonstraram que a inibi&ccedil;&atilde;o do teste al&eacute;rgico cut&acirc;neo<sup>(24)</sup>    e dos sintomas de asma<sup>(33)</sup> ocorreu tanto em indiv&iacute;duos com    alta carga parasit&aacute;ria como em indiv&iacute;duos com baixa carga parasit&aacute;ria.    Uma outra possibilidade &eacute; a de que a menor capacidade dos geohelmintos    em estimular a produ&ccedil;&atilde;o de citocinas imunossupressoras como a    IL-10 determine uma menor capacidade de inibir a inflama&ccedil;&atilde;o al&eacute;rgica.<sup>(11-13)</sup>    O <i>S. mansoni</i> e o <i>S. haematobium</i> s&atilde;o helmintos que induzem    produ&ccedil;&atilde;o aumentada de IL-10,<sup>(6-8)</sup> o que possivelmente    os tornaria mais capazes de inibir a inflama&ccedil;&atilde;o al&eacute;rgica.</font></p>      <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Mecanismos de    inibi&ccedil;&atilde;o da alergia pelas infec&ccedil;&otilde;es helm&iacute;nticas</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Apesar da controv&eacute;rsia    existente sobre este tema, algumas hip&oacute;teses foram elaboradas para explicar    o mecanismo da poss&iacute;vel inibi&ccedil;&atilde;o da reatividade ao teste    al&eacute;rgico cut&acirc;neo e dos sintomas de asma nos indiv&iacute;duos infectados    por helmintos. Inicialmente, acreditava- se que a resposta imune Th1 tivesse    a&ccedil;&atilde;o antag&ocirc;nica &agrave; resposta imune Th2, uma inibindo    a outra.<sup>(34)</sup> Desta forma, seria esperado que as infec&ccedil;&otilde;es    virais e bacterianas, que desencadeiam uma resposta imune Th1, inibissem a inflama&ccedil;&atilde;o    al&eacute;rgica (Th2), e que as infec&ccedil;&otilde;es helm&iacute;nticas (Th2)    estimulassem o surgimento da inflama&ccedil;&atilde;o al&eacute;rgica. Entretanto,    esta forma polarizada de classifica&ccedil;&atilde;o da resposta imune &eacute;    muito simplista para os seres humanos, nos quais as citocinas Th1 participam    do processo inflamat&oacute;rio das doen&ccedil;as al&eacute;rgicas (Th2)<sup>(35,36)</sup>    e do processo inflamat&oacute;rio das doen&ccedil;as auto-imunes (Th1).<sup>(37,38)</sup>    Al&eacute;m disto, como descrito anteriormente, alguns estudos sugerem que as    infec&ccedil;&otilde;es helm&iacute;nticas (Th2) inibem o surgimento das alergias    (Th2), o que &eacute; incompat&iacute;vel com o modelo de polariza&ccedil;&atilde;o    da resposta imune. (<a href="/img/revistas/jbpneu/v33n3/14t2.gif">Tabela 2</a>)</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Atualmente, um dos mecanismos fisiopatol&oacute;gicos mais aceitos para explicar como as infec&ccedil;&otilde;es helm&iacute;nticas inibem a alergia envolve a indu&ccedil;&atilde;o de mecanismos regulat&oacute;rios capazes de limitar as atividades imunes Th1 e Th2 exacerbadas, o que evitaria o surgimento n&atilde;o apenas das doen&ccedil;as al&eacute;rgicas, mas tamb&eacute;m das doen&ccedil;as auto-imunes. Esta regula&ccedil;&atilde;o seria feita por c&eacute;lulas regulat&oacute;rias<sup>(39)</sup> e por citocinas com a&ccedil;&atilde;o imunossupressora, como a IL-10.<sup>(40,41)</sup></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">As c&eacute;lulas regulat&oacute;rias s&atilde;o linf&oacute;citos e podem ser agrupadas em 2 categorias principais: c&eacute;lulas regulat&oacute;rias CD4/CD25<sup>+</sup> e c&eacute;lulas T regulat&oacute;rias ant&iacute;geno-espec&iacute;ficas, ambas importantes no controle da inflama&ccedil;&atilde;o al&eacute;rgica. As c&eacute;lulas regulat&oacute;rias CD4/CD25<sup>+</sup> t&ecirc;m a&ccedil;&atilde;o imunossupressora predominantemente via contato c&eacute;lula-c&eacute;lula e as c&eacute;lulas regulat&oacute;rias ant&iacute;geno-espec&iacute;ficas t&ecirc;m a&ccedil;&atilde;o imunossupressora via secre&ccedil;&atilde;o de IL-10 e fator de crescimento transformador beta.<sup>(42)</sup></font></p>      ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O papel das c&eacute;lulas    regulat&oacute;rias na inibi&ccedil;&atilde;o da resposta imune Th2 foi demonstrado    <i>in vitro</i>: as c&eacute;lulas CD4/CD25<sup>+</sup> foram capazes de inibir    a prolifera&ccedil;&atilde;o das c&eacute;lulas CD4<sup>+</sup>/CD25<sup>-</sup>    e a produ&ccedil;&atilde;o de IL-4 e IL-5 em indiv&iacute;duos at&oacute;picos    e n&atilde;o at&oacute;picos.<sup>(43)</sup> <i>In vivo</i>, a imunoterapia    com p&oacute;len de gram&iacute;neas foi capaz de induzir o surgimento de c&eacute;lulas    CD4/CD25<sup>+</sup>, o que possivelmente contribui para a melhora dos sintomas    de indiv&iacute;duos al&eacute;rgicos submetidos a esta terapia.<sup>(41)</sup>    Nos indiv&iacute;duos infectados por helmintos, as c&eacute;lulas T regulat&oacute;rias    contribuem para um estado de imunossupress&atilde;o<sup>(44)</sup> e, conseq&uuml;entemente,    de inibi&ccedil;&atilde;o da inflama&ccedil;&atilde;o al&eacute;rgica.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Estudos indicam    que a IL-10 tamb&eacute;m tem papel importante na inibi&ccedil;&atilde;o da    inflama&ccedil;&atilde;o al&eacute;rgica nos indiv&iacute;duos com infec&ccedil;&atilde;o    helm&iacute;ntica. Indiv&iacute;duos com doen&ccedil;as al&eacute;rgicas t&ecirc;m    produ&ccedil;&atilde;o reduzida desta citocina<sup>(14,15)</sup> e determinadas    infec&ccedil;&otilde;es helm&iacute;nticas, por sua vez, t&ecirc;m grande capacidade    de induzir a produ&ccedil;&atilde;o de IL-10.<sup>(6-9)</sup> Em indiv&iacute;duos    infectados por <i>S. haematobium</i>, alguns autores demonstraram que a IL-10    inibe a prolifera&ccedil;&atilde;o de c&eacute;lulas mononucleares de sangue    perif&eacute;rico, o que induz um estado de anergia e contribui para o controle    da inflama&ccedil;&atilde;o al&eacute;rgica.<sup>(45)</sup> Em outros estudos    foi poss&iacute;vel observar que, em indiv&iacute;duos infectados por <i>S.    haematobium</i>, a produ&ccedil;&atilde;o aumentada de IL-10 est&aacute; associada    a uma menor reatividade ao teste al&eacute;rgico cut&acirc;neo<sup>(8)</sup>    e que, em indiv&iacute;duos infectados por <i>S. mansoni</i>, a IL-10 inibe    a produ&ccedil;&atilde;o de citocinas Th2 por c&eacute;lulas mononucleares de    sangue perif&eacute;rico.<sup>(7)</sup> Al&eacute;m disto, o tratamento de indiv&iacute;duos    infectados por helmintos promove a redu&ccedil;&atilde;o da produ&ccedil;&atilde;o    de IL-10<sup>(7)</sup> e o aumento da reatividade cut&acirc;nea a aeroal&eacute;rgenos,<sup>(27)</sup>    refor&ccedil;ando as evid&ecirc;ncias de que a IL-10 &eacute; importante na    inibi&ccedil;&atilde;o da alergia na infec&ccedil;&atilde;o helm&iacute;ntica.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Um outro mecanismo de inibi&ccedil;&atilde;o da alergia nos pacientes com infec&ccedil;&atilde;o helm&iacute;ntica est&aacute; relacionado com a produ&ccedil;&atilde;o aumentada de IgE inespec&iacute;fica nos pacientes infectados, o que satura os receptores de IgE na superf&iacute;cie dos mast&oacute;citos, impedindo a liga&ccedil;&atilde;o da IgE espec&iacute;fica para os al&eacute;rgenos. Conseq&uuml;entemente, os al&eacute;rgenos n&atilde;o degranulariam os mast&oacute;citos, impedindo o surgimento da inflama&ccedil;&atilde;o al&eacute;rgica. Um estudo demonstrou que a inibi&ccedil;&atilde;o da reatividade cut&acirc;nea para aeroal&eacute;rgenos em indiv&iacute;duos com infec&ccedil;&atilde;o helm&iacute;ntica esteve associada &agrave; produ&ccedil;&atilde;o aumentada de IgE total, refor&ccedil;ando esta hip&oacute;tese.<sup>(24)</sup> Entretanto, existem argumentos contra este modelo. Estudos epidemiol&oacute;gicos indicam que a inibi&ccedil;&atilde;o da reatividade cut&acirc;nea a aeroal&eacute;rgenos em indiv&iacute;duos infectados por geohelmintos n&atilde;o est&aacute; associada ao n&iacute;vel s&eacute;rico elevado de IgE total.<sup>(8,33)</sup> Al&eacute;m disto, existem evid&ecirc;ncias de que a concentra&ccedil;&atilde;o de IgE inespec&iacute;fica necess&aacute;ria para saturar os receptores na superf&iacute;cie dos mast&oacute;citos &eacute; muito elevada, n&atilde;o sendo encontrada na maioria dos pacientes com infec&ccedil;&atilde;o helm&iacute;ntica.<sup>(46)</sup> Isto ocorre porque existe uma regula&ccedil;&atilde;o da concentra&ccedil;&atilde;o dos receptores para IgE na superf&iacute;cie dos mast&oacute;citos, onde o maior n&iacute;vel s&eacute;rico de IgE induz o aumento da concentra&ccedil;&atilde;o dos receptores, sempre existindo a possibilidade de a IgE espec&iacute;fica encontrar receptores dispon&iacute;veis para se ligar.<sup>(47)</sup></font></p>      <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>A exposi&ccedil;&atilde;o    a infec&ccedil;&otilde;es tamb&eacute;m protege os indiv&iacute;duos contra    doen&ccedil;as auto-imunes</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Tendo em vista que os mecanismos regulat&oacute;rios induzidos por agentes infecciosos tamb&eacute;m inibem a resposta imune Th1 exacerbada,<sup>(48-50)</sup> &eacute; esperado que as infec&ccedil;&otilde;es tenham capacidade de prevenir o surgimento de doen&ccedil;as auto-imunes. Assim como ocorre com as doen&ccedil;as al&eacute;rgicas, a preval&ecirc;ncia das doen&ccedil;as auto-imunes aumentou nos pa&iacute;ses desenvolvidos,<sup>(51&#150;55)</sup> reflexo da influ&ecirc;ncia de fatores ambientais. Existem evid&ecirc;ncias de que indiv&iacute;duos que vivem em ambientes com maior risco de infec&ccedil;&otilde;es t&ecirc;m menos chance de desenvolver doen&ccedil;as auto-imunes.<sup>(56,57)</sup> Al&eacute;m disso, um ensaio cl&iacute;nico demonstrou que a exposi&ccedil;&atilde;o a helmintos inibiu as manifesta&ccedil;&otilde;es cl&iacute;nicas da doen&ccedil;a de Crohn e da retocolite ulcerativa, ambas doen&ccedil;as auto-imunes.<sup>(58)</sup></font></p>      <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>O potencial    terap&ecirc;utico da IL-10 e de ant&iacute;genos de helmintos no tratamento    de doen&ccedil;as do sistema imunol&oacute;gico</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A partir dos conhecimentos obtidos com o estudo da influ&ecirc;ncia das infec&ccedil;&otilde;es nas manifesta&ccedil;&otilde;es al&eacute;rgicas e auto-imunes, foi poss&iacute;vel propor novas op&ccedil;&otilde;es para o tratamento das doen&ccedil;as do sistema imunol&oacute;gico. Entre as perspectivas para o futuro est&aacute; o uso da IL-10 e de ant&iacute;genos de helmintos como formas de tratamento.</font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ensaios cl&iacute;nicos    j&aacute; foram conduzidos para avaliar a seguran&ccedil;a e a efic&aacute;cia    da IL-10 no tratamento de doen&ccedil;as auto-imunes. Em um estudo com 46&nbsp;pacientes    com doen&ccedil;a de Crohn refrat&aacute;ria ao tratamento com cortic&oacute;ide,    a administra&ccedil;&atilde;o de IL10 via intravenosa foi segura e clinicamente    eficaz quando comparada ao placebo.<sup>(59)</sup> Em um estudo com 10 pacientes    com psor&iacute;ase, a IL-10 se mostrou segura e possivelmente eficaz no controle    dos sintomas.<sup>(60)</sup> Em outro estudo, a administra&ccedil;&atilde;o    de ovos de <i>Trichuris suis</i> em pacientes com doen&ccedil;a de Crohn e retocolite    ulcerativa levou a remiss&atilde;o de 86% dos casos.<sup>(58)</sup></font></p>      ]]></body>
<body><![CDATA[<p>&nbsp;</p>    <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Conclus&otilde;es</b></font></p>      <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Apesar das numerosas    publica&ccedil;&otilde;es sobre o tema, ainda existem controv&eacute;rsias quanto    &agrave; capacidade das infec&ccedil;&otilde;es por geohelmintos de inibir a    reatividade ao teste al&eacute;rgico cut&acirc;neo e os sintomas das doen&ccedil;as    al&eacute;rgicas, como a asma. Apesar de escassos, os estudos em pacientes infectados    por helmintos que induzem a produ&ccedil;&atilde;o de IL-10, como os <i>Schistosoma    spp.</i>, sugerem que estes helmintos podem inibir a reatividade ao teste al&eacute;rgico    cut&acirc;neo e os sintomas de asma. Nos estudos <i>in vitro</i>, as evid&ecirc;ncias    sugerem que as helmint&iacute;ases inibem as respostas imunes Th1 e Th2 por    meio da produ&ccedil;&atilde;o aumentada de IL-10, o que abre novas possibilidades    terap&ecirc;uticas para o tratamento de doen&ccedil;as do sistema imunol&oacute;gico.
