ABSTRACT Objective: to evaluate the primary outcome of local complications and late recurrence in patients with hidradenitis suppurativa undergoing radical resection and specific reconstruction. Methods: we conducted a retrospective analysis of the medical records of patients attended by the Plastic Surgery Service of the Clinics Hospital, Medical School, USP, between 2010 and 2016. We included patients who underwent radical resection of hidradenitis suppurativa in advanced stage and reconstruction through primary closure, grafts or flaps. Results: we analyzed 34 lesions in 19 patients, of which 64.5% had local complications, though with 73.5% efficient healing after 12 weeks postoperatively. We observed late recurrence in 47%, but in isolation, 22.2% of the reconstructions with locoregional flaps had recurrence after one year. ABSTRACT Objective: to report the characteristics, evolution and outcome of patients with primary umbilical endometriosis. Methods: an observational and descriptive study of patients with primary umbilical endometriosis diagnosed between 2014 and 2017. The clinical variables evaluated were age, clinical picture, lesion characteristics, diagnostic methods, treatment and recurrence. Results: six patients diagnosed with primary umbilical endometriosis aged 28 to 45 years were operated on during the study period. They had lesions ranging from one to 2.5cm in diameter, violet in five patients and erythematous-violaceous in one. The duration of the symptoms until diagnosis ranged from one to three years and in all the cases studied the diagnosis was made through the clinical manifestations and confirmed by histopathological analysis. No case was associated with neoplastic alterations. All patients evaluated had pain and umbilical bleeding in the menstrual period. Conclusion: umbilical endometriosis is an uncommon disease and should be included in the differential diagnosis of women as umbilical nodules. ABSTRACT Objective: to analyze the effects of nicotine patch on pain control, occurrence of nausea and its hemodynamic repercussions in laparoscopic cholecystectomy procedures. Methods: we conducted an analytical, prospective, randomized, triple-blinded, clinical study between January and July 2017. The sample consisted of 17 patients who underwent laparoscopic cholecystectomy for the treatment of cholelithiasis. Nine patients used nicotine patch, and eight, placebo patch. The studied variables were pain, nausea, patient satisfaction, blood pressure, heart rate, oximetry and morphine rescue. Results: taking into account the pain and nausea parameters, there was no statistically significant difference between the groups (p>0.05). Also, the evaluation of rescue medication, both opioids and prokinetics, did not show any significant statistical difference between the groups. Among the hemodynamic parameters, there was only one statistically significant difference in the analysis of oxygen saturation and systolic blood pressure (SBP) six hours after surgery: the mean oxygen saturation was higher in the Test group (97.89 x 95.88) and the mean SBP was higher in the Control group (123.89 x 110.0). Conclusion: although pain levels were lower for nicotine within 24 hours, the action of nicotine and the need for rescue opioids in pain control were not statistically significant between the groups and at the time intervals studied. ABSTRACT Objective: to evaluate the expression of the epithelial growth factor receptor (EGFR) by immunohistochemistry, and to verify its association with prognostic factors and survival of patients operated by cholangiocarcinoma. Methods: we verified the immunohistochemical expression of EGFR in 35 surgical specimens of cholangiocarcinoma (CCA). We obtained survival curves with the Kaplan-Meier method. Results: we found significant EGFR expression in ten (28.6%) of the 35 CCAs, eight with score 3 and two with score 2. Advanced stages (III and IV) presented higher EGFR expression (p=0.07). The clinical characteristics that were most associated with positive EGFR expression were female gender (p=0.06) and absence of comorbidities (p=0.06). Overall survival at 12, 24, 36 and 48 months was 100%, 82.5%, 59% and 44.2%, respectively. The survival of EGFR positive patients at 12, 24, 36 and 48 months was 100%, 75%, 50% and 0%, whereas for negative EGFR patients it was 100%, 87.5%, 65.6% and 65.6%, respectively. ABSTRACT Objective: to describe the first 13 cases of laparoscopic correction of common bile duct cyst in the Pequeno Príncipe Hospital, Curitiba, Paraná, Brazil. Methods: we performed a retrospective analysis of medical records of cases of choledochal cyst operated by laparoscopy between March 2014 and September 2016. Results: of the 13 patients, eight were female and the mean age at surgery was 7.8 years. The most common symptom was abdominal pain. The hepaticoduodenal anastomosis was the most used reconstruction technique, in 84.6% of the cases. There was no conversion to laparotomy or intraoperative complications. Only one patient presented anastomotic fistula and was reoperated by laparotomy. All patients were followed up in an outpatient clinic, were asymptomatic and had no episode of cholangitis after surgery, with a mean follow-up of 16 months. RESUMO Objetivo: determinar, em pacientes pediátricos portadores de neoplasias malignas, as características de nódulos pulmonares identificados à tomografia computadorizada, capazes de diferenciar nódulos benignos de metástases. Métodos: estudo retrospectivo de pacientes submetidos a ressecções pulmonares de nódulos diagnosticados como metástases em um período de sete anos. Achados de tomografia e da cirurgia, assim como resultados dos exames anatomopatológicos foram comparados. Resultados: nove pacientes, submetidos a 11 intervenções cirúrgicas, foram estudados. Entre as variáveis estudadas, apenas o tamanho do nódulo, maior do que 12,5mm provou ser estatisticamente significante para predizer malignidade. RESUMO Objetivo: determinar a incidência de derrame pericárdico com tamponamento cardíaco em recém-natos prematuros em uma unidade de terapia intensiva pediátrica, com ênfase na relação entre o derrame pericárdico e a inserção de cateter central de inserção periférica, e avaliar o papel da ultrassonografia à beira do leito na abordagem desses casos. Métodos: análise retrospectiva dos pacientes internados em unidade de terapia intensiva pediátrica, entre julho de 2014 e dezembro de 2016, que apresentaram derrame pericárdico com repercussão hemodinâmica, avaliados por ultrassonografia. Resultados: foram estudados 426 pacientes admitidos na unidade neonatal de cinco leitos, com realização 285 ultrassonografias à beira do leito. Foram encontrados seis casos de derrame pericárdico, sendo quatro casos com choque obstrutivo e necessidade de realização de drenagem pericárdica, sem mortalidade relacionada ao procedimento e com melhora hemodinâmica em todos os pacientes após o procedimento. A incidência de derrame pericárdico foi de 2,4 casos por ano. Conclusão: a incidência de derrame pericárdico é baixa em neonatos, porém o diagnóstico precoce é fundamental devido à alta morbimortalidade, especialmente nos casos de instalação abrupta. RESUMO Objetivo: mensurar a eficácia da perda de peso, analisar a evolução de comorbidades, investigar a qualidade de vida e avaliar o protocolo BAROS (Bariatric Analysis and Reporting Outcome System) no pós-operatório de pacientes submetidos à cirurgia bariátrica. Métodos: estudo transversal e quantitativo, com força de verdade de 95% (P=0,05), de 103 pacientes submetidos à Gastrectomia Vertical SLEEVE (40) e à Derivação Gástrica em Y de Roux (63), a partir de quatro meses de pós-operatório. A pesquisa foi realizada no Ambulatório de Cirurgia Geral do Hospital das Clínicas da Universidade Federal de Pernambuco, tendo sido utilizado o protocolo BAROS. Resultados: a maioria dos pacientes era do sexo feminino (89,3%). A média de idade foi de 44,23 anos. A média de perda percentual do excesso de peso foi de 69,35%. A média de tempo de seguimento pós-cirúrgico foi de 41,87 meses (±37,35). As comorbidades com maior percentagem de resolução foram: apneia do sono (90,2%), diabetes (80,7%) e hipertensão (70,8%). As complicações mais frequentes foram queda de cabelo (79,6%), deficiência nutricional (37,9%) e anemia (35%). O protocolo BAROS demonstrou que a qualidade de vida foi avaliada de forma positiva em 93,2% dos casos. O questionário Moorehead-Ardelt demonstrou que a qualidade de vida "melhorou" ou "melhorou muito" para 94,1% dos pacientes. Conclusão: a cirurgia bariátrica demonstrou ser um procedimento eficaz no tratamento da obesidade mórbida e no controle das comorbidades. RESUMO Objetivo: relatar uma série de casos de cirurgia bariátrica robótica no tratamento da obesidade no Brasil. Métodos: foram avaliados pacientes submetidos à cirurgia bariátrica robótica no Instituto Garrido, e realizada comparação com grupo submetido à cirurgia laparoscópica convencional. Resultados: foram analisados 45 pacientes, com média de idade de 39,44 anos, sendo 34 do sexo feminino, com média de IMC inicial de 41,26Kg/m2. Dentre as cirurgias realizadas, 91,11% foram bypass gástrico em Y de Roux, enquanto 8,89% foram do tipo gastrectomia vertical. A média de tempo total de cirurgia foi de 158 (±56,54) minutos, com tempo médio de docking de 7,93 (±3,9) minutos e tempo de console 113,0 (±41,4) minutos. A média de dor apresentada na recuperação pós-anestésica foi de 2,61 (±3,30) pontos, em escala de 0 a 10, com quatro pacientes apresentando sinais leves de náusea, respondendo bem ao tratamento medicamentoso. Somente um paciente necessitou internação em UTI por um período de dois dias após a cirurgia, devido à cardiopatia prévia. Em dois casos ocorreram hérnia incisional em sítio de trocater, tratados cirurgicamente, sem posteriores complicações. Na comparação entre os grupos de cirurgia robótica versus laparoscópica, foram selecionados 45 pacientes para cada grupo. O tempo operatório foi significativamente mais longo na via robótica, sendo a maior parte das outras variáveis equivalentes, inclusive controle pós-operatório de comorbidades. RESUMO Objetivo: identificar o conhecimento dos médicos com relação à hipertensão intra-abdominal e síndrome compartimental abdominal e compará-lo com a conduta estabelecida na literatura. Métodos: estudo quantitativo descritivo, observacional, transversal. Foram entrevistados 38 médicos por meio de um questionário auto-aplicado composto por perguntas objetivas. O estudo foi realizado em um hospital universitário, de referência terciária, localizado em Curitiba, Paraná. Resultados: a média de idade dos participantes foi de 28 anos, 60,5% era do sexo feminino e o tempo médio de experiência médica foi de 3,5 anos. Em relação ao conhecimento sobre o tema, 57,9% considerou que a síndrome compartimental abdominal se inicia com a hipertensão intra-abdominal grau III, 50% considerou a pressão de perfusão abdominal o método mais fidedigno para determinar hipertensão intra-abdominal, 89,4% considerou a técnica de aferição intravesical como a mais utilizada, 71,1% considerou a oligúria o sinal precoce de síndrome compartimental abdominal. Não mensurou a pressão intra-abdominal 81,6% dos entrevistados, por não haver protocolo definido pelo serviço. Setenta e nove por cento dos entrevistados afirmou não conhecer as definições do consenso do World Society of the Abdominal Compartment Syndrome (WSACS). RESUMO Objetivo: identificar os fatores associados à disfagia em pacientes submetidos à intubação orotraqueal prolongada (IOTp) e as consequências pós-extubação. Métodos: participaram do estudo 150 pacientes submetidos à IOTp, avaliados segundo o nível funcional da deglutição (American Speech Language - Hearing Association National Outcome Measurement System - ASHA NOMS), a determinação da gravidade (The Simplified Acute Physiology Score - SOFA) e a coleta das seguintes variáveis: idade, mortalidade, dias de intubação orotraqueal, número de atendimentos para introdução da alimentação oral e dias para alta hospitalar. Os pacientes foram agrupados de acordo com a classificação do ASHA: 1 (níveis 1 e 2), 2 (níveis 3, 4 e 5) e 3 (níveis 6 e 7). Resultados: as análises indicaram as seguintes variáveis associadas a pior funcionalidade da deglutição: idade (p<0,001), mortalidade (p<0,003); dias de IOT (p=0,001), número de atendimentos para introdução de dieta oral (p<0,001) e dias para alta hospitalar (p=0,018). As comparações múltiplas indicaram diferença significante na comparação dos grupos ASHA1 e ASHA2 em relação ao grupo ASHA3. Os grupos ASHA1 e ASHA2 apresentaram menor score na SOFA quando comparados ao grupo ASHA3 (p=0,004). Somente 20% dos pacientes do grupo ASHA1 e 32% dos pacientes do ASHA2 apresentaram níveis seguros de deglutição antes da alta hospitalar. Conclusão: os fatores associados à disfagia em pacientes submetidos à IOTp foram: idade acima de 55 anos e tempo de intubação orotraqueal (maior nos casos com pior funcionalidade da deglutição). ABSTRACT Trauma is a public health problem and the most common cause of death in people under the age of 45. In blunt abdominal trauma, the spleen is the most commonly injured organ. Splenectomy remains the most common treatment, especially in high-grade lesions, despite increased nonoperative treatment. Removal of the spleen leads to increased susceptibility to infections due to its role in the immune function. Postsplenectomy sepsis is an important complication and presents a high mortality rate. Patients undergoing splenectomy should be immunized for encapsulated germs, as these are the agents most commonly associated with such infections. Splenic autotransplantation is a simple procedure, which can be an alternative to reduce infection rates consequent to total splenectomy, and reduce costs related to hospitalizations. This review aims to provide evidence-based information on splenic autotransplantation and its impact on the prognosis of patients undergoing total splenectomy. We searched the Cochrane Library, Medline/PubMed, SciELO and Embase, from January 2017 to January 2018 and selected articles in English and Portuguese, dated from 1919 to 2017. We found that the adjusted risk of death in splenectomized patients is greater than that of the general population, and when total splenectomy is performed, splenic autotransplantation is the only method capable of preserving splenic function, avoiding infections, especially postsplenectomy sepsis. ABSTRACT Objective: to compare the students’ performance in face-to-face and telemedicine courses for the training and necessary action in disasters, using telemedicine as an effective training tool. Methods: online research conducted after the end of the course of preparation in disasters, carried out in-person, as well as by videoconference. We compared the performance of students in the in-person course and through telemedicine. Results: in the comparison of the results obtained with the pre- and post-test data between the students who attended via telemedicine and in-person, we observed that in the two modalities there was an increase in knowledge (p<0.001). We also observed no statistically significant differences in the posterior evaluation between the in-person and telemedicine courses (p=1.0), however, there was a significant difference at the pre-test evaluative moment (p<0.001). Conclusion: videoconferencing can be effectively used to train health professionals in disaster management, being able to provide adequate knowledge and become an important tool to distance reaching in continuing education.
