Hipotonia ocular secundária à ciclodiálise: descrição de 4 casos e revisão de literatura

Ocular hypotony secondary to cyclodialysis: Report of 4 cases and literature review

RESUMO

A hipotonia ocular secundária à ciclodiálise provoca alterações funcionais e anatômicas que podem comprometer definitivamente a acuidade visual. Quatro casos consecutivos, secundários a trauma e com hipotonia, são descritos. Três casos foram tratados com fotocoagulação (laser de argônio) da esclera, corpo ciliar e raiz da íris. Os parâmetros de laser utilizados foram mira de 100 μm, tempo de exposição de 0,2s e intensidade variando entre 1 e 1,2 W, em até 2 sessões. Um dos casos foi submetido a ciclopexia direta, com fixação do corpo-ciliar a esclera. Todos os casos evoluíram com fechamento da ciclodiálise e pressão intra-ocular entre 13 e 18 mmHg. Os autores concluem que a hipotonia ocular secundária à ciclodiálise pode ser revertida satisfatoriamente com o uso de laser ou, em casos refratários, através de ciclopexia direta, associada ou não ao laser.

SUMMARY

Ocular hypotony secondary to cyclodialysis leads to functional and anatomic changes that may result in irreversible visual acuity loss. Four consecutive cases with hypotony, following trauma, are described. Three cases were treated with argon laser photocoagulation to the sclera, ciliary body and iris root. The parameters used for the laser treatment were 100 μm spot, 0.2s of exposure time and power varying from 1.0 to 1.2 W in up to two sessions. One of the four cases underwent direct cyclopexy, the ciliary body being sutured to sclera. They all showed closure of the cleft and intraocular pressures between 13 and 18 mmHg. The authors conclude that ocular hypotony secondary to cyclodialysis may be successfully treated with argon laser photocoagulation or, in refractory cases, by performing direct cyclopexy associated or not to laser treatment.
