A case of a boy 11 years old with crural paraplegia coincident with osteomyelitis of the mandible is reported. Cerebrospinal fluid examination showed nearly complete block of the spinal cord space and protein-cytologic dissociation. Antibiotics were used with total remission of the osteomyelitis and partially of the spinal cord symptoms; the cerebrospinal fluid was normal after treatment. The differential diagnosis of intraspinal pyogenic infections is discussed.
