A rare case of giant aneurysm of the P3 segment of the posterior cerebral artery is presented. The aneurysm was acessed through a combined approach proposed by Sano and the aneurysm was treated by proximal clipping. The patient had a good postoperative recovery without any neurological deficit. The anatomy and the operative approaches are discussed and the neurosurgical literature reviewed.
