  • Aspectos eletrencefalográficos das malformações vasculares intracranianas

    Analisando os casos publicados sôbre o assunto, e apresentando um material próprio constituído de 5 casos de aneurismas arteriovenosos, um de aneurisma da artéria silviana e 3 de malformação de Sturge-Weber-Dimitri, estudados clínica, eletrencefalográfica e radiològicamente (craniograma simples e arteriografia cerebral), o autor procura mostrar a importância dos resultados eletrencefalográficos para o diagnóstico exato e para orientação da terapêutica etiológica. Com os elementos assim coligidos conclui: 1) Nos casos de aneurismas arteriovenosos, o EEG deu sinais de diagnóstico focai decisivo em 4, sendo que em 2 levou à arteriografia pacientes que tinham tido uma e duas crises convulsivas, respectivamente, possibilitando, assim, diagnóstico etiológico e terapêutica cirúrgica; em um caso orientou o diagnóstico exato de um paciente de 55 anos, que há 25 anos era tido como epilético; em um caso sòmente não deu sinais de anormalidades, tratando-se de pequeno aneurisma arteriovenoso situado entre circunvoluções parietais. 2) No caso de aneurisma da artéria silviana, situado na base do cérebro, o EEG mostrou somente os distúrbios difusos e bilaterais conseqüentes à hemorragia meníngea. 3) Nos 3 casos de malformação de Sturge-Weber-Dimitri, o EEG demonstrou alterações de tipo depressão de atividade elétrica cerebral correspondente à malformação calcificada; em um dêstes casos mostrou ní- tido foco epileptógeno frontal, explicando perfeitamente sintomatologia de convulsões de tipo bravais-jacksoniano e hemiparesia, independentemente da calcificação tipo angiomatosa, de situação occipital.

    Reviewing the cases published in the literature and studying his own material (3 cases of Surge-Weber-Dimitri's disease, one case of aneurysm of the medial cerebral artery and 5 cases of arteriovenous fistula) by the clinic, the EEG, the roentgenogram and the arteriogram, the author shows the importance of the EEG record in the establishment of correct diagnosis and in the choice of treatment in these cases. The author concludes: 1) In the cases of arteriovenous fistula, the EEG showed focal signs in 4 cases; in 2 of them, the EEG record demonstrated the necessity of arteriograms, for the patients have had one and two seizures, respectively, bringing out the possibility of etiological diagnosis and surgical treatment. In another case, the EEG gave orientation for the diagnosis; the 55 year-old patient had been considered as an epileptic for 25 years. In the fifth case (the aneurysm was small and located between parietal convolutions) the EEG did not show focal signs. 2) In the case of aneurysm of the medial cerebral artery, located in the base of the brain, the EEG revealed diffuse and bilateral signs, probably due to the previous subarachnoid hemorrhage. 3) In the 3 cases of Sturge-Weber-Dimitri's disease, the EEG showed records with depressed electrical activity, corresponding to the calcified areas. In one case the EEG also revealed the presence of a frontal focus which explained very well the Bravais-Jacksonian seizures and the hemiparesis, independently of the occipital angiomatous calcification.

  • Estudo comparativo dos efeitos hematológicos e neurológicos do extrato hepático, ácido fólico e vitamina B12 no tratamento da anemia perniciosa de Addison-Biermer

    Estudo comparativo dos efeitos hematológicos e neurológicos com o emprêgo do extrato hepático, do ácido fólico e da vitamina B12 em 15 casos de anemia perniciosa com sinais de lesão neurológica, não submetidos anteriormente a tratamento, ou apresentando recidiva hêmato-neurológica após o abandono de tratamentos prévios. Foi estudada a evolução da hematimetria, da taxa de hemoglobina, da sensibilidade vibratória e da sintomatologia neurológica em geral. Os resultados foram submetidos a tratamento estatístico. Os autores chegam às seguintes conclusões: 1. Os resultados hematológicos, embora nítidos em todos os casos, evidenciam superioridade do extrato hepático e ácido fólico em relação à vitamina B12; aquêles mostraram-se eqüivalentes. 2. Houve nítida correlação entre a evolução da hematimetria e da taxa de hemoglobina. 3. Os resultados neurológicos, embora favoráveis nos três grupos, denotaram diferenças que, apesar de estatìsticamente não significantes, evidenciaram a superioridade da vitamina B12 sôbre o ácido fólico e o extrato hepático. 4. Não houve correlação estatística entre as evoluções da palestesia e da hematimetria, evidenciando-se acentuada discordância no comportamento das síndromes hematológica e neurológica. 5. O ácido fólico, em média, revelou-se inferior à vitamina B12 no tocante aos efeitos neurológicos, mas ligeiramente superior ao extrato hepático. 6. As melhoras neurológicas foram mais nítidas em relação aos sintomas do que aos sinais. 7. A vitamina B12 e o ácido fólico determinaram acentuada melhora da síndrome piramidal, ao passo que o extrato hepático quase não atuou; em relação à síndrome periférica, a vitamina B12 e o extrato hepático determinaram melhora nítida, enquanto o ácido fólico pouco efeito denotou; os três medicamentos revelaram efeitos semelhantes no que tange à síndrome funicular dorsal. 8. Dentre as manifestações neurológicas, melhoras mais evidentes foram observadas em relação à síndrome psíquica, ao déficit piramidai. à hipotonia e às desordens da artrestesia. Os sinais piramidais de libertação, a depressão dos reflexos tendíneos e particularmente os distúrbios da sensibilidade táctil mostraram-se mais rebeldes ao tratamento.

    Three groups of 5 cases of pernicious anemia with signs of subacute combined degeneration of the spinal cord were treated with vitamin B12, folic acid and liver extract. Nine patients had not been submitted to other therapies; six patients had some treatment 7 to 48 months before (table 2) but were in hematoneurological relapse. The three drugs were administered twice a week, each treatment lasting for a month, so that 10 injections of each drug were made. In group 1 vitamin B12 was given in the total dosis of 100 y; in group 2 folic acid was administered in the total dosis of 500 mgm; group 3 received liver extract at the total dosis of 10 ml (150-200 USP anti-anemic unities). No associated treatment was instituted; the diet contained 70 to 80 gm of proteins by day. Red cell counts were made by usual methods; hemoglobin contents of the blood were determined by means of an alcaline solution of oxyhemoglobin, with spectrophotometric reading; the hematologic evolution (tables 3 and 4 and graphics 1, 2 and 3) was followed up by examinations made twice a week (in each case an average of 10 examinations was made). Neurologic evolution was studied by examination of the vibration sense every ten days and by complete neurologic examination before and after the treatment (tables 5 and 6). The results were submitted to statistical analysis. The homogeneity (Snedecor) of the initial values was proved. It was calculated the linear regression relating time and the average values of red cell count, hemoglobin content and pallesthesia of each group. The regression coefficients of the evolution of the red cell count and hemoglobin content proved to be statistically significant and were compared; the differences were also significant, showing the superiority of liver extract and folic acid over vitamin B12. The correlation between red cell count and hemoglobin content was positive. The regression coefficients of the evolution of pallesthesia were not significant, as well as the differences between the neurologic scores (table 6) before and after treat- ment in the three groups. However, neurologic results showed the superiority of vitamin B12 over folic acid and liver extract according to the values of the regression coefficients (table 5). No correlation between the evolutions of pallesthesia and of the red cell count was found. Disagreement between hematologic and neurologic results of the drugs was evidenced. Folic acid was less effective than vitamin B12 but it was slightly better than liver extract in relation to the neurologic evolution. Neurologic subjective symptoms improved in higher degree than objective signs. Vitamin B12 and folic acid evidenced better results in relation to the pyramidal syndrome; liver extract and vitamin B12 were more effective in the improvement of the peripheral syndrome; similar effects on dorsal funicular syndrome were evidenced by the three drugs (table 7). Among the neuropsychiatrie manifestations the most impressive results were observed on the psychical syndrome, muscular weakness, hypotonus and position sense disorders; signs of pyramidal release, depression of deep reflexes and particularly superficial sense disorders were the most resistant signs (table 8), irrespective of the kind of treatment.

  • Resultados preliminares do emprêgo do ACTH nas degenerações combinadas da medula Notas Preliminares

    Os autores apresentam os resultados imediatos do emprêgo do ACTH em 4 casos de neuropatía desmiel in izante aquílica. Justificam o uso dêsse hormônio em vista da possível patogenia alérgica dos processos desmielinizantes primarios. O ACTH foi administrado por via intravenosa, sem qualquer outra medicação associada que pudesse interferir nos resultados. Êstes foram avaliados do ponto de vista quantitativo e evidenciaram discreta ação benéfica do hormônio, avaliada em 28,8%, em média. A decisão quanto ao efeito do ACTH nas mieloses funiculares só será possível após maior experiência, baseada em mais volumosa casuística e no segui- mento dos pacientes, bem como no estudo comparativo dêsses resultados com os obtidos com o emprêgo dos demais medicamentos de uso corrente.

    Report on the immediate results of the use of ACTH in 4 cases of achylic demyelinating neuropathy. This treatment is justified by the possible allergic pathogenesis of the primary demyelinating processes. Dayly dosis of 12.5 mgm of corticotropin were given by continuous drip method, without any other associated drug which could interfere in the results. These were estimated on a semi-quantitative basis and evidenced slight beneficial effects of the hormone, which averaged 28.8 per cent. The accurate evaluation of the effects of corticotropin in subacute combined degeneration of the cord demands larger experience, based on greater number of cases and follow up of the patients, as well as the comparative study of such effects and the results obtained with classic treatment (liver extract, folic acid and vitamin B12).
  • Exposição do cavo de Meckel por via extradural no tratamento da neuralcia essencial do trigêmeo Notas Preliminares

    Report of a new technique for the treatment of major trigeminal neuralgia, consisting of exposure (with no opening) of the cavum Meckelli following an extradural approach. Four patients were operated on, with the same encouraging results obtained by others (Taarnhöj, Love, Norlen) employing different procedures. Although the author does not intend to give any explanation on pathogenesis of trigeminalgia he feels that the compression of the trigeminal roots inside the cavum Meckelli invoked by Taarnhõj as the mechanism of trigeminal neuralgia has lost its support.
  • Efeitos da ablação e da excitação do hipocampo no homem Notas Preliminares

    A atividade do cíngulo anterior pode ser modificada pela ablação ou excitação do hipocampo; partindo dêste princípio, que se baseia em dados experimentais e anatômicos, o autor pensa ser possível influenciar a evolução de certas enfermidades nervosas e mentais pela ablação do hipocampo. O autor realizou 10 intervenções em doentes mentais, sendo 5 ablações bilaterais do hipocampo e 3 unilaterais; nos 2 outros restantes foi feito o acesso simples ao hipocampo sem ablação, o que é o mesmo que fazer o "undercutting" do giro fusiforme. Nos casos de "undercutting" foram implantados, no hipocampo, eletrodos e agulhas por meio dos quais foram feitas excitações elétrica e química (injeção de acetilcolina com prostigmina e de estricnina) depois de cessado o efeito anestésico. O estímulo elétrico com 5 e 10 ciclos por segundo produziu aumento da amplitude e freqüência respiratórias e aceleração do pulso em mais de 10%. A pressão arterial não foi influenciada. A injeção da acetilcolina com prostigmina produziu apenas elevação da pressão mínima. Os efeitos produzidos pela injeção de estricnina foram semelhantes aos da excitação elétrica, embora menos nítidos. O pouco tempo de observação e o pequeno número de casos não permitem conclusões sôbre a influência da ablação do hipocampo na evolução dos distúrbios psíquicos e neurológicos. Parece, entretanto, que certos distúrbios de conduta na criança podem ser beneficiados pela ablação bilateral do hipocampo.

    Anterior cingular activity may be modified after excision or stimulation of hippocampus; taking this as a principle which is based on experimental and anatomical data, the author thinks that the evolution of some mental and nervous diseases may be modified by hippocampal ablation. Ten patients were operated upon: 5 were submitted to a bilateral ablation of hippocampus; 3 to unilateral ablation; 2 to undercutting of gyrus fusiformis and approach to hippocampus without excision. In patients who were submitted to undercutting of fusiform gyrus, electrical and chemical stimulations of hippocampus were performed after cessation of anesthesia. Electrical stimulation with pulsatile current (5 and 10 cycles per second) produced increase in respiratory rate and amplitude; pulse rate was increased in more than 10 per cent; no change was verified in blood pressure. Acetylcholine plus prostigmine injection produced only increase of diastolic blood pressure. Effects of strychnine injection were like those verified by electrical excitation but less marked. Time and number of cases are still insufficient for a definite conclusion about influence of hippocampal ablation on evolution of some neurological and mental disorders. It seems, however, that behavior disorders in children may be beneficially influenced after bilateral excision.

    The author reports a case of a woman with postencephalitic Parkinson, who affirmed to be dead as all her visceral system had been destroyed. The author thinks that these disbodied disturbances are determined by interruption of the sensitive visceral pathways caused by the encephalitic process, and considers the case proper to bring out the argument favouring the assumption of the diencephalic localization of psychic functions in vital depression.
