  • Caries risk assessment in chilean adolescents and adults and its association with caries experience Original Articles

    Cariogram is a computer program that uses an algorithm to assess caries risk. Although the use of Cariogram has recently increased, little information is available regarding its effectiveness in adults. This study aimed to determine whether caries risk from Cariogram relates to caries experience in adults. One hundred and eighty Cariogram files were completed from patients aged ten to fifty-six years (mean: 23.28 years). Seven factors from the software were included from patient records to complete the Cariogram: caries experience, diet (content and frequency), stimulated salivary flow, hygiene index, related diseases and fluoride usage. The percentages of "chances of avoiding new lesions" (caries risk) were obtained from Cariogram, and the subjects were classified into five risk groups. Results were compared for each variable with ANOVA, and a correlation between caries and Cariogram variables was calculated by Pearson's correlation coefficient. A multivariate regression model was also used. Only three patients were classified as low risk, and none were classified as very low risk. Thus, only the four upper quintiles were considered for the analysis, and the lower quintile was not considered in the study. Neither DMFT nor the number of lesions were significantly different among the Cariogram's risk categories (p > 0.05). Only diet content was significantly correlated with caries experience (p = 0.006). Caries lesions failed to correlate with any Cariogram variable (p > 0.05). Age, not sex or caries risk scores, showed a strong and positive association with DMFT (p < 0.01). Caries risk from Cariogram appears to be unrelated with caries experience or caries lesions in a high-caries adult population.

    The aim of this study was to evaluate the effect of treating intraradicular dentin with irrigating solutions on the retention of glass-fiber posts luted with self-adhesive resin cement. Bovine incisors were endodontically treated, and 9-mm-deep postholes were prepared. Before inserting the cement, the root canals were irrigated with various solutions: 11.5% polyacrylic acid for 30 s, 17% EDTA for 60 s, or 5% NaOCl for 60 s, respectively. Irrigation with distilled water was used in the control group. After all specimens had been rinsed with distilled water, the excess moisture was removed and the posts were luted using either BisCem (Bisco) or RelyX Unicem clicker (3M ESPE). Seven days after luting, the specimens were sectioned transversally into 1-mm-thick slices, which were submitted to push-out testing on a mechanical testing machine. Bond strength data (n = 6 per group) were analyzed by two-way ANOVA and Student-Newman-Keuls' test (α = 0.05). For Unicem, EDTA showed lower bond strength than the other solutions, which had similar results. For BisCem, EDTA showed higher bond strength than the other treatments, while application of NaOCl yielded higher bond strength than polyacrylic acid whereas the control group had intermediate results. In conclusion, irrigating root canals before insertion of self-adhesive resin cements, especially EDTA, might interfere with retention of the fiber posts.
  • Biofilm biomass disruption by natural substances with potential for endodontic use Original Articles

    This study evaluated the in vitro effects of four natural substances on the biomass of bacterial biofilms to assess their potential use as root canal irrigants. The following substances and their combinations were tested: 0.2% farnesol; 5% xylitol; 20% xylitol; 0.2% farnesol and 5% xylitol; 0.2% farnesol, 5% xylitol, and 0.1% lactoferrin; 5% xylitol and 0.1% lactoferrin; and 20 mM salicylic acid. The crystal violet assay was used to evaluate the effects of these substances on the biomass of biofilms formed by Enterococcus faecalis and Staphylococcus epidermidis. All substances except for 20 mM salicylic acid and 20% xylitol reduced biofilm mass when compared to controls. The combination of farnesol and xylitol was the most effective agent against E. faecalis ATCC 29212 (p < 0.05). Farnesol combined with xylitol and lactoferrin was the most effective against biofilms of the endodontic strain of E. faecalis MB35 (p < 0.05). Similarly, combinations involving farnesol, xylitol, and lactoferrin reduced the biomass of S. epidermidis biofilms. In general, farnesol, xylitol, and lactoferrin or farnesol and xylitol reduced biofilm biomass most effectively. Therefore, it was concluded that combinations of antibiofilm substances have potential use in endodontic treatment to combat biofilms.
  • Micro-CT assessment of two different endodontic preparation systems Original Articles

    The aim of this study was to compare two endodontic preparation systems using micro-CT analysis. Twenty-four one-rooted mandibular premolars were selected and randomly assigned to two groups. The samples (n = 12) of Group 1 were prepared using the ProTaper Universal rotary system, while Group 2 (n = 12) was prepared using the EndoEZE AET system complemented by manual apical preparation with K-type hand files up to #30. A 2.5% sodium hypochlorite solution was used in both groups for irrigating. Both groups were scanned by high-resolution microcomputed tomography before and after preparation (SkyScan 1172, SkyScan, Kontich, Belgium). The root canal volume and surface area was measured before and after preparation, and the differences were calculated and analyzed for statistically significant differences using ANOVA complemented by the Tukey test (p < 0.05). The results showed no statistically significant differences between the mean volumes of dentin removal by the two systems. However, the EndoEZE AET system presented a significantly greater mean surface area compared to the ProTaper system (p < 0.05). The EndoEZE AET system enabled preparation of a greater root canal surface area when compared to the ProTaper Universal system. There seemed to be no difference in dentin volume loss between the two systems used.

    The aim of this study was to evaluate the genotoxic potential of methyl methacrylate (MMA) vapor by simulating standard occupational exposure of 8 hours per day and using the micronucleus test. We used 32 adult male Wistar rats divided into three groups: A - 16 rats exposed to MMA for 8 hours a day, B - Eight rats receiving single subcutaneous doses of cyclophosphamide on the first day of the experiment (positive control), C - Eight rats receiving only water and food ad libitum (negative control). Eight rats from group A and all of the rats from groups B and C were sacrificed 24 hours after beginning the experiment (acute exposure in group A). The remaining animals in group A were sacrificed 5 days after the experiment began (repeated exposure assessment in group A, simulating occupational exposure 40 hours/week). Femoral bone marrow was collected from each rat at the time of sacrifice for use in the micronucleus test. Two slides were completed per animal and were stained with Giemsa staining. Two thousand polychromatic erythrocytes were counted per animal. The Kruskal-Wallis test followed by a multiple comparisons test (Dunn test) was used for statistical analysis. The median number of micronuclei was 7.00 in the group exposed to MMA for 1 day, 2.00 in the group exposed to MMA for 5 days, 9.00 in the group exposed to cyclophosphamide (positive control) and 0.756 in the negative control group (p < 0.0001). MMA was genotoxic when measured after 1 day of exposure but was not evidently genotoxic after 5 days.

    The aim of the present study was to investigate the relationship between degenerative bone changes of the head of the mandible and the presence of joint effusion (JE). This study was based on sagittal magnetic resonance imaging (MRI) reports of 148 temporomandibular joints (TMJs) of 74 patients complaining of pain and/or dysfunction in the TMJ area. The mandible heads were surveyed for osteoarthritis characteristics, which were classified as osteophytosis, sclerosis or erosion. The presence of JE was checked whenever high signal intensity was observed in the articular space. The results evidenced the presence of bone changes in 30% of the sample. Osteophytes and erosions were the changes most commonly observed. JE was reported in 10% of TMJs. The results from the statistical tests revealed that bone changes in the head of the mandible are associated with the presence of JE.

    The aim of this study was to analyze the association between infantile bruxism and the terminal relationships of the primary second molars. A total of 937 pre-school children (both genders), aged from 2 to 6 years, from municipal schools in São Paulo were evaluated. In this study, a questionnaire considering the bruxism habit and the presence of headaches and/or restless sleep was answered by the parents/guardians. A clinical exam of occlusion in the anteroposterior direction (vertical plane - VP, mesial step - MS and distal step - DS) was performed by the examiners in the school environment. Student's t test, Fisher's test and a logistic regression test were applied for the statistical analysis at a significance level of 5%. The prevalence of the bruxism habit was 29.3% among the total sample. Because there was no significant difference between the sides evaluated, the left side was taken as the standard. Among those children with bruxism, 25.7% presented a mesial step terminal relationship at the primary second molars, 29.1% had DS, and 30.2% had VP. Regarding the association of the parafunctional habit with the type of terminal relationship, no significant results were found. Children who slept restlessly or suffered from headaches were verified to show a higher chance of expressing the habit (OR = 2.4 and 1.6, respectively). The prevalence of bruxism in the studied sample was 29.3%, and its association with the primary second molars' terminal relationship was not statistically significant.

    The aim of this study was to assess the reliability of a method to measure the following upper airway dimensions: total volume (TV), the nasopharyngeal narrowest areas (NNA), and the oropharyngeal narrowest areas (ONA). The sample consisted of 60 cone-beam computed tomography (CBCT) scans, evaluated by two observers twice, using the Dolphin 3D software (Dolphin Imaging & Management solutions, Chatsworth, California, USA), which afforded image reconstruction, and measurement of the aforementioned dimensions. The data was submitted to reliability tests, by the intraclass correlation coefficient (ICC), and the Bland & Altman agreement tests, with their respective confidence intervals (CI) set at 95%. Excellent intra- and interobserver reliability values were found for all variables assessed (TV, NNA and ONA), with ICC values ranging from 0.88 to 0.99. The data demonstrated an agreement between the two assessments of each observer and between the first evaluations of both observers, thus confirming the reliability of this methodology. The results suggest that this methodology can be used in further studies to investigate upper airway dimensions (TV, NNA, and ONA), thereby contributing to the diagnosis of upper airway obstructions.
  • Response of molars and non-molars to a strict supragingival control in periodontal patients Original Articles

    The posterior position in the arches is one of the factors that underlies the poor prognosis of molar teeth (M). It is speculated that M do not benefit from the oral hygiene routine as well as non-molars (NM) do. This study evaluated the response of M and NM to supragingival control during a 6-month period in 25 smokers (S) and 25 never-smokers (NS) with moderate-to-severe periodontitis. One calibrated examiner assessed visible plaque (VPI) and gingival bleeding (GBI) indexes, periodontal probing depth (PPD), bleeding on probing (BOP), and clinical attachment loss (CAL) at days 0 (baseline), 30 and 180. At baseline, M showed significantly higher mean values of VPI (p = 0.017) and PPD (p < 0.001) compared with NM; CAL was also greater in M (p < 0.001) and was affected by smoking (p = 0.007). The reductions obtained for periodontal indicators at day 180 showed similar responses between M and NM. For CAL, M (NS 0.57 ± 0.50; S 0.67 ± 0.64) and NM (NS 0.38 ± 0.23; S 0.50 ± 0.33) reached an almost significant difference (p = 0.05). Smoking did not influence the response to treatment. Multilevel analysis revealed that, only for PDD reductions, the interaction between sites, teeth and patient was significant (p < 0.001). It was concluded that M benefit from an adequate regimen of supragingival biofilm control; therefore, supragingival condition should be considered in the prognosis of molar teeth.
  • Malocclusion and deleterious oral habits among adolescents in a developing area in northeastern Brazil Original Articles

    Although malocclusions represent a serious public health issue, there is insufficient information about this problem in adolescents in Brazil, especially in poorer areas. The purpose of this cross-sectional study was to estimate the prevalence of facial alterations, dental malocclusions, and deleterious oral habits (DOH) among adolescents in a developing area in northeastern Brazil and to test the hypothesis that the occurrence of DOH in infancy is associated with DOH during adolescence. The study included a probabilistic population-based sample of 2,060 Brazilian students aged 12-15 years. Facial characteristics (type of facial profile, facial symmetry, and passive lip sealing) and malocclusions (Angle and Dental Aesthetic Index, DAI) were evaluated. DOH in infancy and adolescence were evaluated by interviews with the parents and adolescents. Most adolescents presented with normal facial characteristics. The malocclusion prevalence (Angle) was 83%. The DAI ranged from 13 to 69 (mean ± SD: 25.9 ± 7.7). Orthodontic treatment was necessary in 45.1% of the sample. The most prevalent DOH in adolescents were nail biting, object biting, cheek/lip biting, and bruxism, which were associated with finger sucking during infancy (P < 0.05). We conclude that malocclusions and DOH are common among Brazilian adolescents and that finger sucking during infancy may be a good predictor of DOH occurrence during adolescence.
  • Chemical cleaning agents and bonding to glass-fiber posts Short Communication

    The influence of chemical cleaning agents on the bond strength between resin cement and glass-fiber posts was investigated. The treatments included 10% hydrofluoric acid, 35% phosphoric acid, 50% hydrogen peroxide, acetone, dichloromethane, ethanol, isopropanol, and tetrahydrofuran. Flat glass-fiber epoxy substrates were exposed to the cleaners for 60 s. Resin cement cylinders were formed on the surfaces and tested in shear. All treatments provided increased bond strength compared to untreated control specimens. All failures were interfacial. Although all agents improved the bond strength, dichloromethane and isopropanol were particularly effective.
  • Dentin deproteinization effect on bond strength of self-adhesive resin cements Short Communication

    This study examined the effect of deproteinization on the bond strength between self-adhesive resin cements and dentin surfaces that were untreated (control), acid-etched, or acid-etched and subjected to a post-etch deproteinization treatment. Cylinders of RelyX Unicem or BisCem (n = 6) cement were build-up on the dentin surfaces and tested to determine shear strength. The results were analyzed using two-way ANOVA and Tukey's test (5%). While neither dentin pretreatment improved the bond strength for RelyX Unicem, deproteinization treatments resulted in greater bond strength in BisCem specimens while acid etching alone did not improve the performance of the material.

    This study evaluated whether periodontal ligament (PL) thickness varied with root size and examined the possible influence of this variation on orthodontic mechanics. Measurements were taken of the maxillary left first molar in 54 male Wistar rats. Mean mesial and distal PL thicknesses were compared between the intermediate buccal and mesiobuccal roots using paired Student's t-tests with a 5% significance level. Mean values differed significantly between roots (p < 2.2 × 10-16). PL thickness in rats is directly proportional to root dimensions.
