Incontinência pigmentar (IP) é uma genodermatose rara, com herança dominante ligada ao cromossomo X, que ocorre em 1:50 mil recém-nascidos.(11. Um recém-nascido a termo, sexo feminino, apresentou múltiplas lesões vesiculares nas primeiras 12 horas de vida. Dada a suspeita de herpes neonatal, iniciou-se aciclovir. A detecção do vírus do herpes simples por cultura e a reação em cadeia da polimerase foram negativas (sangue e lesões cutâneas). Em 36 horas, apareceram lesões cutâneas vesiculares e hiperpigmentadas, distribuídas ao longo das linhas de Blaschko, que levaram à suspeita de IP. Nessa ocasião, a mãe revelou que ela e sua filha mais velha também apresentaram algum tipo de “bolhas d’água” ao nascimento, e esta informação corroborou o diagnóstico.
É necessário um alto grau de suspeita para fazer o diagnóstico de IP. O diagnóstico diferencial de IP varia conforme o estágio da doença.(11. Ehrenreich M, Tarlow MM, Godlewska-Janusz E, Schwartz RA. O prognóstico de IP é geralmente bom, mas uma avaliação periódica por uma equipe multidisciplinar deve ser feita para excluir deficiência visual, motora ou intelectual.(11.
