RE: Preoperative serum albumin as a prognostic factor in patients with upper urinary tract urothelial carcinoma

The publication on “Preoperative serum albumin and upper urinary tract urothelial carcinoma” is very interesting (11. Ku JH, Kim M, Choi WS, Kwak C, Kim HH. Preoperative serum albumin as a prognostic factor in patients with upper urinary tract urothelial carcinoma. Int Braz J Urol. 2014;40:753-62.). Ku et al. reported that “hypoalbuminemia is an independent marker of poor prognosis in patients with upper urinary tract urothelial carcinoma (11. Ku JH, Kim M, Choi WS, Kwak C, Kim HH. Preoperative serum albumin as a prognostic factor in patients with upper urinary tract urothelial carcinoma. Int Braz J Urol. 2014;40:753-62.).” Indeed, there are many recent reports confirming the importance of pretherapeutic serum albumin level (11. Ku JH, Kim M, Choi WS, Kwak C, Kim HH. Preoperative serum albumin as a prognostic factor in patients with upper urinary tract urothelial carcinoma. Int Braz J Urol. 2014;40:753-62., 22. Wu N, Chen G, Hu H, Pang L, Chen Z. Low pretherapeutic serum albumin as a risk factor for poor outcome in esophageal squamous cell carcinomas. Nutr Cancer. 2015;67:481-5.). This parameter can represent that nutritional status of the patient which is an important basic determinant for ability of the body to fight with illness (22. Wu N, Chen G, Hu H, Pang L, Chen Z. Low pretherapeutic serum albumin as a risk factor for poor outcome in esophageal squamous cell carcinomas. Nutr Cancer. 2015;67:481-5.). Nevertheless, there are many factors that can affect the serum albumin level.

The good examples are the laboratory interferences and artifacts (such as fibrinogen and heparin) that can alter the serum albumin result (33. Hattingh J, Coetzee N, Ross F P. Erroneous densitometric albumin estimation as a result of prestaining with certain dyes. Anal Biochem. 1977;80:635-8., 44. Hallbach J, Hoffmann GE, Guder WG. Overestimation of albumin in heparinized plasma. Clin Chem. 1991;37:566-8.). In addition, to assess the nutritional status preoperatively, it is suggested not only serum albumin but also other parameters such as hemoglobin has to be concurrently determined and interpreted (55. Melchior JC. [How to assess preoperative nutritional status?]. Ann Fr Anesth Reanim. 1995;14:19-26.). As noted by Delmore, “the Prognostic Nutritional Index (PNI), including serum parameters, immune competence testing and anthropometrics (66. Delmore G. Assessment of nutritional status in cancer patients: widely neglected? Support Care Cancer. 1997;5:376-80.)” is recommended for “predicting postoperative morbidity/mortality and of selecting cancer patients (66. Delmore G. Assessment of nutritional status in cancer patients: widely neglected? Support Care Cancer. 1997;5:376-80.)” Finally, there are also other confounding factors that can alter the final outcome results such as the superimposed nosocomial infection during admission, concurrent anemia, etc.
