    OBJECTIVES: The neuroanatomical basis and the neurochemical abnormalities that underlay juvenile myoclonic epilepsy (JME) are not fully defined. While the thalamus plays a central role in synchronization of widespread regions of the cerebral cortex during a seizure, emerging evidence suggests that all cortical neurons may not be homogeneously involved. The purpose of this study was to investigate the cerebral metabolic differences between patients with JME and normal controls. METHODS: All patients had a JME diagnosis based on seizure history and semiology, EEG recording, normal magnetic resonance neuroimaging (MRI) and video-EEG. Forty JME patients (JME-P) were submitted to 1.5 T MRI proton spectroscopy (1H-MRS), multi-voxel with PRESS sequence (TR/TE = 1500/30 ms) over the following locations: prefrontal cortex (PC), frontal cortex (FC), thalamus, basal nuclei, posterior cingulate gyrus (PCG), insular, parietal and occipital cortices. We determined ratios for integral values of N-acetyl aspartate (NAA) and glutamine-glutamate (GLX) over creatine-phosphocreatine (Cr). The control group (CTL) consisted of 20 age and sex-matched healthy volunteers. RESULTS: Group analysis demonstrated a tendency for lower NAA/Cr ratio of JME-P compared to CTL predominantly on FC, PC, thalamus and occipital cortex. When compared to CTL, JME-P had a statistically significant difference in GLX/Cr on FC, PC, insula, basal nuclei, PCG and on thalamus. When evaluating the relationship among the various components of this epileptic network among JME-P, the strongest correlation occurred between thalamus and PC. Also, we found a significant negative correlation between NAA/Cr and duration of epilepsy. CONCLUSION: Reductions in NAA may represent loss or injury of neurons and/or axons, as well as metabolic dysfunction while glutamate is considered to be an excitatory neurotransmitter in the brain which is involved in the pathogenesis of epileptogenic seizures.
  • Estudos genéticos e moleculares em um grande grupo de pacientes com malformações do córtex cerebral

    OBJETIVOS: As malformações do córtex cerebral (MCC) são uma causa importante de epilepsia. Nossas metas foram: triagem de mutações em genes associados às MCC (FLN1, LIS1, DCX e EMX2), investigar funcionalmente as mutações e mapear o locus para polimicrogiria perisylviana familiar. MÉTODOS: A triagem de mutações foi realizada por PCR, DHPLC e sequênciamento. Estudo funcional foi realizado por RT-PCR, PCR em tempo real e HUMARA. O estudo de ligação foi realizado por PCR e análise com programas Fragment Profiler® e MLINK®. RESULTADOS: Mutações deletérias foram identificadas em 3/108 pacientes. Uma mutação de splicing (G987C) em FLN1 foi identificada em duas pacientes aparentadas com heterotopia nodular periventricular. Mudança no padrão de inativação do cromossomo X é responsável pelas diferenças clínicas entre as pacientes. Uma substituição A1385C (H277P) foi identificada em LIS1 em um indivíduo com lissencefalia. Alterações neutras foram identificadas em DCX e EMX2. A análise de ligação identificou um locus em Xq27.2-Xq27.3 para polimicrogiria familiar. CONCLUSÃO: Mosaicismo, mutações em regiões não codificantes, deleções, rearranjos e casos atípicos podem estar contribuindo para a baixa freqüência de mutações identificadas. Esquizencefalia e polimicrogiria parecem não ter base genética relacionada com o gene EMX2. Um novo locus candidato em Xq27.2-Xq27.3 foi identificado para polimicrogiria perisylviana familiar.

    OBJECTIVES: Malformations of cerebral cortex (MCC) are an important cause of epilepsy. Our main goals were: to search for mutations in genes responsible for MCC (FLN1, LIS1, DCX and EMX2), to map the locus for familial perisylvian polymicrogyria and to investigate the molecular mechanisms of the mutations identified. Methods: Mutation screening was performed by PCR, DHPLC and sequencing. HUMARA and Real Time PCR were performed to study the molecular mechanisms of mutations. Linkage analysis was carried out by PCR, Fragment profiler® and MLINK® software. RESULTS: Deleterious mutations were identified in 3/108 patients. We found a G987C splicing mutation in the FLN1 in two related patients with periventricular nodular heterotopia. Skewed X-chromosome inactivation was detected as the possible mechanism responsible for clinical differences observed in the two patients. An A1385C transversion (H277P) in LIS1 was identified in one patient with lisencephaly. Only neutral variants were identified in DCX and EMX2. Linkage analysis has detected a locus in Xq27.2-Xq27.3 for familial polymicrogyria. CONCLUSION: We believe that the low frequency of mutations identified may be due to mosaicism, mutations in non-coding regions, deletions and patients with atypical neuroimaging findings. Deleterious mutations in EMX2 were not found in patients with schizencephaly and polymicrogyria. We found a locus for familial perisylvian polymicrogyria in Xq27.2-Xq27.3.
  • Epilepsia e neuroproteção: o papel do agonista adenosinérgico A1(RPia) na modulação da crise induzida por pilocarpina

    OBJETIVO: O objetivo desse estudo foi caracterizar a neuroproteção do RPia em ratos submetidos ao status epilepticus (SE) induzido pela pilocarpina (Pilo). MÉTODOS: Avaliou-se o balanço entre utilização local da glicose cerebral (ULGC) e fluxo sanguíneo cerebral local (FSCL) após 4 horas de SE, e a marcação por Fluoro Jade-B (FJB), 24 horas e 90 dias após SE. Quatro grupos foram avaliados: Salina, Pilo, RPia+Salina e RPia+Pilo. RESULTADOS E CONCLUSÃO: Aumentos significantes na ULGC foram observados na maioria das regiões avaliadas nos grupos Pilo e RPia+Pilo quando comparados ao controle. Entretanto, redução significante na ULGC ocorreu na substância negra pars reticulata e giro denteado do grupo RPia+Pilo versus Pilo. Houve aumento significante do FSCL em todas as áreas estudadas, comparando-se os grupos Pilo e RPia+Pilo com o controle. Foi observado um aumento significante do FSCL durante SE em CA2, CA3, giro denteado, córtex entorrinal, corpo mamilar, núcleos talâmicos, núcleo rubro, zona incerta, núcleo oral da ponte e córtex visual, no grupo pré-tratado com RPia comparado ao tratado somente com Pilo. Grande número de células marcadas com FJB foi observado no grupo Pilo e o pré-tratamento com RPia reduziu essa marcação na formação hipocampal, córtex piriforme, amígdala basolateral e substância negra pars compacta.

    OBJECTIVE: The aim of this study was to characterize the neuroprotection of the RPia in rats subjected to status epilepticus (SE) induced by pilocarpine (Pilo). METHODS: We evaluated the mismatch between local cerebral glucose utilisation (LCGU) and local cerebral blood flow (LCBF) 4 hours after SE induction. Neuronal loss was evaluated by Fluoro Jade-B (FJB) 24 hours and 90 days after SE. Four groups were studied: Saline, Pilo, RPia+Saline and RPia+Pilo. RESULTS AND CONCLUSIONS: Significant increases in the LCGU were observed in the almost all brain regions of Pilo and RPia+Pilo groups compared to control. However, significant reduction in the LCGU occurred in the substantia nigra pars reticulata and hippocampal formation of RPia+Pilo group versus Pilo. There was significant increase of the LCBF in all the studied areas, comparing the Pilo and RPia+Pilo groups with the control. The increases of LCBF was more intense in rats from RPia+Pilo compared to Pilo, and located mainly in CA2, CA3, dentate gyrus, entorhinal cortex, thalamic nuclei, mammillary body, red nucleus, zone incerta, pontine nucleus and visual cortex. A great number FJB stained cells was observed in the Pilo group and RPia pretreatment reduced the staining in the hippocampal formation, piriform cortex, basolateral amygdala and substantia nigra pars compacta.
  • Evolução da epilepsia de lobo temporal mesial familiar Xxxii Brazilian Epilepsy Congress - Awards Works: Expanded Abstracts

    OBJETIVOS: Analisar a evolução de famílias com epilepsia de lobo temporal mesial familiar (ELTMF). METODOLOGIA: Seguimento prospectivo de 64 pacientes com ELTMF e 37 membros assintomáticos pertencente a 28 famílias. RESULTADOS: A média de seguimento dos pacientes com ELTMF foi de 93,4 ± 15,8 meses. Na avaliação inicial os pacientes foram divididos em benignos (n = 29), remissão (n = 28) e refratários (n = 7). Na última visita disponível, 41,4% dos pacientes com ELTMF benigna permaneceram classificados como benignos, 20,7% tornaram-se refratários e 37,9% entraram em remissão. No grupo em remissão, 75% permaneceram livres de crise, 21,4% foram classificados como benignos e um faleceu (3,6%) de causa não relacionada à epilepsia. Todos pacientes refratários permaneceram refratários. Em relação aos assintomáticos 10,8% evoluíram com crises. A média de seguimento dos assintomáticos foi de 76,0 ± 21,2 meses. CONCLUSÃO: O seguimento prospectivo de mais de 7 anos de pacientes com ELTMF revelou que é improvável ocorrer controle de crises no grupo refratário. No grupo benigno é muito provável que estes indivíduos entrem em remissão ou permaneçam com evolução benigna. A maioria dos pacientes do grupo em remissão permaneceu em remissão e nenhum se tornou refratário. Em relação aos assintomáticos a probabilidade de apresentar uma crise no decorrer de aproximadamente 6 anos foi maior que o observado na população geral.

    OBJECTIVE: To analyze seizure outcome in individuals with familial mesial temporal lobe epilepsy (FMTLE). METHOD: We followed prospectively 64 individuals with FMTLE and 37 asymptomatic individuals belonging to 28 families. RESULTS: Patients with FMTLE had a mean follow up was 93.4 ± 15.8 months. At baseline they were divided in benign (n = 29), remission (n = 28) and refractory (n = 7). At last follow up visit 41.4% patients with benign FMTLE remained classified as benign, 20.7% became refractory and 37.9% were in remission. In the subgroup of FMTLE in remission 21 75% remained without seizures; 21.4% were classified as benign FMTLE, and one died (3.6%) from cause unrelated to epilepsy. All refractory patients remained refractory. From the asymptomatic group, 10.8% became symptomatic (FMTLE). The mean follow up was 76.0 ± 21.2 months. CONCLUSION: Prospective follow up of more than 7 years in patients with FMTLE revealed that it is unlikely to achieve seizure control in those with refractory seizures. Patients with diagnose of more benign forms of FMTLE for more than one year are likely to either remit or remain under well controlled seizures. The majority of patients who had achieved seizure remission remained seizure-free and none became refractory. Asymptomatic individuals had a greater probability to have seizures compared to the general population in a 6 year period of follow up.
  • Percepção do estigma na criança com epilepsia refratária: estudo comparativo entre doenças crônicas na infância Artigos Originais

    A epilepsia refratária gera impacto na integração social, tendo repercussões emocionais e neuropsicológicas, possibilitando a vivência do estigma pelo paciente e seus familiares. OBJETIVO: Avaliar a percepção do estigma em crianças com epilepsia refratária e asma grave e em seus respectivos cuidadores, correlacionando os resultados entre estas duas doenças crônicas. Verificar as possíveis diferenças na percepção do estigma entre estes dois grupos de crianças. MÉTODOS: Estudo hipotético-dedutivo-observacional-transversal. A amostra foi constituída de crianças com epilepsia refratária (e seus cuidadores) e do grupo controle constituído por crianças com asma grave (e seus respectivos cuidadores). As avaliações foram realizadas a partir de Escalas de Estigma (Child stigma scale e Parent stigma scale) e Escalas Weschsler de inteligência (WISC III - versão para crianças e WAIS III - versão para adultos) (em critérios de exclusão). RESULTADOS: A análise de variância mostrou que a percepção do estigma é semelhante em ambas doenças, com médias de 22,35 nas crianças com epilepsia e 20,84 naquelas com asma. A maioria das crianças apresentou pontuação na escala de estigma, entre valores médios e altos, sugerindo que a percepção do estigma é significativa. Enquanto que os cuidadores de crianças com epilepsia mostraram média 15,35 e aqueles que cuidam de crianças com asma apresentaram média 15,16, indicando igual vivência de estigma entre os cuidadores.

    The refractory epilepsy causes an impact in the social integration, emotional and neuropsychological repercussions, which make living with the stigma possible by the patient and the patient's family. OBJECTIVE: To evaluate the stigma perception on children who suffer from refractory epilepsy and the severe asthma and also on their caregivers, correlating the results between these two chronic diseases. To check the possible differences in the perception of the stigma between these two groups of children. METHODS: Hypothetical-deductive-observational-transversal research. The sample was done with children who suffer from refractory epilepsy (and their caregivers) and with the control group of children with severe asthma (and their caregivers). The evaluations were realized based on the Stigma scale (Child stigma scale e Parent stigma scale) and the Weschsler intelligence scale (WISC III - child version and WAIS III - adult version) (in exclusion criteria). RESULTS: The analysis of the variances has shown that the stigma perception is similar on both diseases, with average figures of 22, 35 on children with epilepsy and 20, 84 on those with asthma. Most of the children have shown punctuation in the stigma scale, among average and high numbers, suggesting a significant perception of the stigma. While the caregivers of children with epilepsy have shown average 15, 35 and those who take care of children with asthma have presented an average 15, 16, indicating the same living stigma among caregivers.
  • Transtornos psiquiátricos na epilepsia: uma proposta de classificação elaborada pela comissão de neuropsiquiatria da ILAE

    INTRODUÇÃO: Um dos aspectos mais importantes da epileptologia é o da sua relação com as comorbidades psiquiátricas. Transtornos psiquiátricos (TP) são associados a praticamente todas as síndromes epilépticas e contribuem para uma maior dificuldade no manejo desses pacientes. OBJETIVO: O presente trabalho tem como objetivo fazer uma revisão das classificações dos TP na epilepsia e destacar a proposta elaborada pela Comissão de Neuropsiquiatria da ILAE. CONCLUSÃO: A proposta de classificação elaborada pela ILAE procurou diferenciar os TP que ocorrem em comorbidade daqueles que refletem atividade epileptiforme ou que ocorrem especificamente na epilepsia, sendo baseada em aspectos clínicos e descritivos mais do que em etiologias. Apresenta ainda critérios bastante claros e operacionais, constituindo-se de grande utilidade para futuros estudos dos quadros psiquiátricos na epilepsia.

    INTRODUCTION: The relationship between psychiatric disorders (PD) and epilepsy is among the most frequent and important aspects of epileptology. PD are present in all epileptic syndromes and contribute to increase the difficulties in treating these patients. OBJECTIVE: We reviewed the classifications for the PD in epilepsy and highlighted the proposal by the ILAE Commission on Neuropsychiatry. CONCLUSION: The classification proposal by the ILAE attempted to separate disorders comorbid with epilepsy and those that reflect ongoing epileptiform activity from epilepsy-specific disorders and are based on clinical and descriptive symptoms rather than in etiological classification. It also presents clear and operational criteria that could be of greater utility for future studies of PD in epilepsy.
  • Síndrome de Landau-Kleffner sem crises: atraso na aquisição da fala justifica o tratamento com drogas epilépticas? Review Articles

    INTRODUÇÃO: Síndrome de Landau-Kleffner é uma síndrome epiléptica rara caracterizada pela associação de afasia de recepção, crises epilépticas, distúrbios do comportamento e alterações eletrencefalográficas com descargas nos lobos temporais unilaterais ou bilaterais. Crises convulsivas não são essenciais para o diagnóstico. RELATO DO CASO: J.V.S.S., masculino, três anos de idade, com atraso na aquisição da fala, agitação psicomotora e distúrbio do sono. Ele usava gestos e apontava para os objetos tentando se comunicar. Seu comportamento era muito agitado, impaciente, períodos de agressividade e muita dificuldade em estabelecer contato social com outras crianças da mesma idade. RNM foi normal e o EEG mostrou descargas de ondas agudas nas regiões temporais média e posterior esquerda. Após três meses de tratamento com carbamazepina a criança retornou para reavaliação, apresentando importante melhora da fala, falando palavras simples, melhora do comportamento e do padrão de sono e da interação social. DISCUSSÃO: Alterações na linguagem de recepção e de expressão são relativamente comuns em crianças com diferentes síndromes epilépticas, particularmente na síndrome de Landau-Kleffner. Descargas epileptiformes podem ocorrer em pessoas sem epilepsia sendo denominadas descargas subclínicas. Qual o impacto das descargas subclínicas para estas pessoas é incerto até o momento. Relatamos o caso de um menino com descargas epileptiformes no EEG, sem história de crises convulsivas e excelente evolução clínica após tratamento com droga antiepiléptica.

    INTRODUCTION: Landau-Kleffner Syndrome is a rare epileptic syndrome characterized by the association of receptive aphasia, epileptic seizures, behavioral disorders and electroencephalographic changes with discharges at the temporal lobe unilaterally or bilaterally. Seizures are not essential for diagnosis. CASE REPORT: J. V. S. S., 3 y/o male, with delay in acquirement of speech, psychomotor agitation and sleep disorder (sleeplessness). Attempt to communicate was established by gestures and pointing at things. His behavior characterized by agitation, restlessness, aggressiveness and difficulty to establish social contact with other children by the same age. MRI was normal and the EEG showed sharp-wave discharges in the left medial and posterior temporal regions. After three months of treatment with carbamazepine the child returned to an evaluation, presenting substantial improvement at speech, speaking simple words and with meaningful improvement on both behavioral and sleep patterns, as well as, social interaction. DISCUSSION: Expression and reception speech disorders are relatively common in children with different epileptic syndromes, particularly in Landau-Kleffner syndrome. Epileptiform discharges can occur in people without epilepsy and use to be called subclinical discharges. What would be the impact of subclinical discharges on this population remains uncertain at the present time. We reviewed these interactions in the light of a representative case, showing favorable outcome following introduction of an antiepileptic drug.

  • Resolução 267: o resgate da cidadania da pessoa com epilepsia

    INTRODUÇÃO: O mundo inteiro passa por expressivas transformações em todos os campos da ciência, seja tecnológica com a produção de veículos cada vez mais aprimorados e com uso de equipamentos de ponta, seja na medicina com o progresso de tratamentos em distúrbios médicos que antes se imaginava não terem muitas opções terapêuticas, como a epilepsia. Um avanço significativo na área da Epidemiologia, relativo à Direção Veicular para pessoas com epilepsia (PCE), trouxe à tona a necessidade de estudos para a imposição de parâmetros relativos à habilitação desses indivíduos. MÉTODOS: Revisão da literatura. RESULTADOS: No Brasil o Grupo de Consenso reunido em 1.999, composto pela Associação Brasileira de Medicina de Tráfego (ABRAMET), Associação Brasileira de Epilepsia (ABE) e Liga Brasileira de Epilepsia (LBE), conduziu a "Diretriz Nacional para Direção de Veículos Automotores para Pessoas com Epilepsia", que foi apresentada à Câmara Temática de Saúde do Conselho Nacional de Trânsito (CONTRAN), e que foi baseada no artigo 147 da Lei nº 9.503 de 23 de setembro de 1997, que instituiu que "para habilitar-se como motorista, o candidato deverá submeter-se ao exame de aptidão física e mental", e com a Resolução nº 80/98 do Conselho Nacional de Trânsito estabeleceu normas regulamentadoras para o procedimento do exame, não fazendo referência específica à epilepsia, mas adotando-a e incluindo-a, do ponto de vista legal, nas condições que restringiam ou contraindicam, a critério médico, a Direção Veicular. Em menos de uma década, tais condições tiveram que ser revisadas, atualizadas e modificadas diante da necessidade de uma nova visão da Lei. CONCLUSÕES: Atualmente são fundamentais, visão mais ampla de Cidadania e um Código de Trânsito mais sensível aos direitos relativos à PCE.

    INTRODUCTION: The world has witnessed significant transformation in many scientific fields, such as in technological area with the manufacturing of advanced vehicles and the use of high tech equipments, as well as, in medicine with the progress of treatments in medical disturbances that did not have many therapeutic options, such as epilepsy. A significant advancement in epidemiology, concerning to vehicle conduction, brought the need of studying the imposition of parameters regarding the driving license of the people with epilepsy (PCE). METHODS: Review of literature. RESULTS: In Brazil the Consent Group in 1999, composed by "Associação Brasileira de Medicina de Tráfego" (ABRAMET), "Associação Brasileira de Epilepsia" (ABE) and "Liga Brasileira de Epilepsia" (LBE), conducted the "The National Policy for Conduction of Automotor Vehicles by PWE", that was presented to "Câmara Temática de Saúde do Conselho Nacional de Trânsito" (CONTRAN), that was based on the article 147 of Law number 9.503 of September, 23, 1997, which instituted that "to be qualified as a driver, the applicant should be submitted to the exam of physical and mental capability", and with the Resolution number 80/98 of "Conselho Nacional de Trânsito", rules were established to regulate the proceeding of the exam, without specific reference to epilepsy, but including and adopting it according to the legal point of view, in conditions that restricted or did not indicate, by medical criteria, the vehicle conduction. After less than a decade, such conditions had to be reviewed, upgraded and modified in face of the needs of new view of the Law. CONCLUSIONS: Nowadays it is necessary a wide view of citizenship as well as a traffic law more sensible to the rights of the PCE.
