  • NEGLIGÊNCIA DENTÁRIA, UMA MODALIDADE DE ABUSO INFANTIL – REVISÃO NARRATIVA Review

    A negligência dentária é definida como a falta intencional do responsável pela criança na procura e manutenção do tratamento necessário para garantir um nível de saúde oral essencial para a função adequada, livre de dor e infeção. Neste sentido, pretendemos esclarecer, de acordo com a literatura científica atual, os indícios clínicos para a identificação de negligência dentária em crianças. Após pesquisa em bases de dados e motores de busca, com as palavras-chave "child", "dental", "neglect", entre 1996 e 2016, os resultados revelaram que a identificação desta condição requer, não só a obtenção de uma história clínica detalhada e de dados clínicos que evidenciem claramente a falta de cuidados na saúde oral como também a investigação dos determinantes sociais e parentais em que a criança se insere. Assim, inserir o médico dentista nas equipas de análise nos casos de suspeita revelou-se indispensável para o despiste desta e outras situações de abuso.

    Dental neglect is defined as an intentional failure of the caretaker in the pursuit and continuity of necessary treatment to achieve an oral health that allows adequate function, free from pain and infection. We intend to clarify, according to the current scientific literature, the clinical indications for the identification of dental neglect in children. After the search in databases and search engines with the keywords "child", "dental" and "neglect", between 1996 and 2016, the results revealed that identifying this condition requires not only a detailed clinical history and clinical data that clearly show the lack of oral health care but also investigating the social and parental determinants in which the child is involved. Thus, inserting the dentist in teams of analysis in case of suspicion proves to be indispensable for the detection of this and other situations of abuse.
  • ANÁLISE DA VARIABILIDADE DA FREQUÊNCIA CARDÍACA PELO MÉTODO DA DIMENSÃO FRACTAL DE HIGUCHI É SENSÍVEL DURANTE A RECUPERAÇÃO APÓS EXERCÍCIO EM HOMENS FISICAMENTE ATIVOS Original Research

    OBJETIVO: Métodos não-lineares para avaliar a variabilidade da freqüência cardíaca (VFC) têm recebido muita atenção recente. Nosso objetivo é analisar as respostas não-lineares da VFC durante a recuperação do exercício através do novo algoritmo da análise fractal da dimensão de Higuchi. MÉTODO: Foram examinados 35 voluntários saudáveis, com idade entre 18 e 35 anos. Para o treinamento de intensidade de exercício utilizamos 60% de Vmax determinado através de um teste progressivo utilizando o limiar de Conconi. A VFC foi analisada 10 minutos antes e 10 minutos após o exercício. Analisamos os índices geométricos de VFC (SD1, uma medida de VFC em curto prazo e SD2, uma medida de VFC em longo prazo) e aplicamos a Dimensão Fractal de Higuchi aos intervalos RR. RESULTADOS: Durante a recuperação do exercício, os índices de plot de Poincaré SD1 e SD1/SD2 diminuíram, enquanto que SD2 e SD2/SD1 aumentaram (p <0,0001) em relação ao repouso. Todos os parâmetros da análise da Dimensão Fractal de Higuchi foram reduzidos após o exercício em comparação com o controle em repouso (p <0,0001). CONCLUSÃO: O exercício físico diminuiu agudamente o comportamento complexo da VFC, conforme medido pela análise da Dimensão Fractal de Higuchi. Fisiologicamente, isso indica que detectamos alterações na complexidade da VFC imediatamente após o exercício.

    OBJECTIVE: Nonlinear techniques to assess heart rate variability (HRV) have received much recent attention. We aimed to evaluate nonlinear HRV responses during recovery from exercise through the novel algorithm Higuchi Fractal Dimension. METHOD: We examined 35 healthy male volunteers aged between 18 and 35 years old. For the training of exercise intensity we used 60% of Vmax determined through a progressive test using the Conconi threshold. HRV was analyzed 10 minutes before and 10 minutes after exercise. We analyzed the geometric indices of HRV (SD1, a measure of short-term HRV and SD2, a measure of long-term HRV) and applied the Higuchi Fractal Dimension to RR intervals. RESULTS: During recovery from exercise the Poincaré plot indices SD1 and SD1/SD2 decreased while SD2 and SD2/SD1 increased (p<0.0001) compared to control at rest. All parameters of Higuchi Fractal Dimension analysis were lowered after exercise compared to control at rest (p<0.0001). CONCLUSION: Physical exercise acutely decreased the chaotic behavior of HRV as measured through the Higuchi Fractal Dimension analysis. Physiologically, this indicates that we detected complex changes in HRV immediately after exercise.
  • AVALIAÇÃO DA PROTEÍNA FECAL S100Q12 EM PACIENTES COM DOENÇA INFLAMATÓRIA INTESTINAL Original Research

    JUSTIFICATIVA E OBJETIVO: O diagnóstico e avaliação da doença inflamatória intestinal é bastante complexo. Um marcador ideal, não invasivo para esta doença é urgentemente necessário. O S100A12 fecal é um membro da família de proteínas S100 e é secretado por neutrófilos ativados. Pretendemos avaliá-lo como biomarcador para pacientes com doença inflamatória intestinal na China. MÉTODOS: a proteína fecal S100A12 foi medida em 18 pacientes com Moléstia de Crohn, 21 pacientes com Colite Ulcerativa e 17 voluntários saudáveis (controles). O valor diagnóstico foi avaliado através da análise da característica de operação do receptor (ROC) em comparação com a proteína C reativa e com a taxa sedimentação eritrocitária. A correlação entre S100A12 fecal e características clínicas também foi avaliada. RESULTADOS: Observamos aumentos significativos (p < 0.01) no valor diagnóstico de S100A12 tanto na Colite Ulcerativa quanto na Doença de Crohn quando comparados aos controles saudáveis. Na colite ulcerativa, a proteína S100A12 fecal correlacionou com sangue oculto fecal (p = 0,02, r = 0,55); Na doença de Crohn, correlacionou com a duração da doença, albumina e níveis de plaquetas (p = 0,01, r = -0,53; p <0,01, r = -0,65; p = 0,04, r = 0,45, respectivamente). Não houve correlação entre S100A12 fecal e outras condições clínicas. CONCLUSÃO: O S100A12 fecal é valioso para distinguir pacientes com doença inflamatória intestinal versus controles saudáveis. No entanto, a sensibilidade e especificidade é limitada quando comparada com a descrita nos países ocidentais. A correlação entre S100A12 e características clínicas é limitada. Mais pesquisas são necessárias para explorar melhor essa interação em pacientes chineses.

    BACKGROUND AND OBJECTIVE: The diagnosis and evaluation of inflammatory bowel disease is quite complex. An ideal, noninvasive marker for this disease is quite urgently needed. Fecal S100A12 is a member of the S100 protein family and is secreted by activated neutrophils. We aim to evaluate it as a biomarker for inflammatory bowel disease patients in China. METHODS: Fecal S100A12 was measured in 18 Crohn's disease, 21 ulcerative colitis, and 17 healthy controls. Diagnostic value was evaluated by receiver operating characteristic (ROC) analysis in comparison with C-reactive protein and erythrocyte sedimentation rate. The correlation between fecal S100A12 and clinical characteristics was also evaluated. RESULTS: We found significant increases (p<0.01) in the diagnostic value of S100A12 in both Ulcerative Colitis and Crohn's Disease when compared to healthy controls. In ulcerative colitis, fecal S100A12 correlated with fecal occult blood (p=0.02, r=0.55); in Crohn's disease, it correlated with disease duration, albumin and platelet levels (p=0.01, r=-0.53; p<0.01, r=-0.65; p=0.04, r=0.45. respectively). No correlation occurred between fecal S100A12 and other clinical conditions. CONCLUSION: Fecal S100A12 is valuable in distinguishing inflammatory bowel disease patients versus healthy controls. However, the sensitivity and specificity are limited when compared with that described in western countries. The correlation between S100A12 and clinical characteristics is limited as well. More research is need to better explore this interaction in Chinese patients.
  • RESPOSTAS VASCULARES E INFLAMATÓRIAS AGUDAS DEPOIS DE UMA SESSÃO DE EXERCÍCIO RESISTIDO EM MULHERES JOVENS COM EXCESSO DE ADIPOSIDADE Original Research

    ANTECEDENTES: A disfunção endotelial e a inflamação de baixo grau estão positivamente associadas a estados de adiposidade excessiva; entretanto os efeitos agudos do exercício resistido sobre estas variáveis ainda não estão esclarecidos. Avaliamos os efeitos agudos do exercício resistido sobre a reatividade vascular e sobre o perfil inflamatório em mulheres jovens. MÉTODOS: As participantes foram divididas em dois grupos: controles magras (n = 16) e aquelas com sobrepeso (n = 16). A sessão de exercício resistido consistiu de flexões unilaterais de cotovelo em cinco séries de 10 repetições (com 70% de uma repetição máxima). Avaliamos tanto no repouso quanto durante o período pós-exercício agudo a pressão arterial, a frequência cardíaca, o fluxo sanguíneo do antebraço (FBF) e a condutância vascular (CVF), as citocinas, os adipopeptídeos e a endotelina-1. RESULTADOS: O grupo com sobrepeso apresentou maior FBF em repouso (p = 0,03) e pós-exercício (p <0,001), enquanto a CVF foi maior somente após o exercício, aos 20 min (p = 0,02) e aos 40 min (p <0,001) . A vasodilatação endotélio-dependente durante o período pós-exercício foi maior no grupo Overweight em relação aos controles (p = 0,01). No grupo Overweight, a sessão de exercício resistido reduziu a interleucina-6 (p = 0,02) e a leptina (p <0,001) e o aumentou os níveis de endotelina-1 (p = 0,02). CONCLUSÃO: Concluímos que a sessão de exercício resistido provocou um incremento agudo da reatividade vascular basal e um aumento da vasodilatação endotélio-dependente com alterações concomitantes no perfil inflamatório e da endotelina-1 em mulheres com adiposidade excessiva.

    BACKGROUND: Endothelial dysfunction and low-grade inflammation are both positively associated to states of excessive adiposity but reports on the acute effects of resistance exercise on these variables are still lacking. We evaluated these acute effects of resistance exercise on vascular reactivity and on the inflammatory profile in young women. METHODS: Participants were divided into two groups: lean Controls (n=16) and Overweight (n=16). The resistance exercise session consisted of unilateral elbow flexions for five sets of 10 repetitions at 70% of one repetition maximum. Blood pressure, heart rate, forearm blood flow, vascular conductance, cytokines, adipopeptides and endothelin-1 were evaluated at rest and during the acute post-exercise period. RESULTS: The overweight group had higher forearm blood flow at rest (p=0.03) and during post-exercise (p<0.001) while forearm vascular conductance was higher only during post-exercise, at 20 (p=0.02) and 40 min (p<0.001). Endothelial-dependent vasodilation was higher during the post-exercise period in the Overweight group compared to controls (p=0.01). In the Overweight group, the resistance exercise session reduced interleukin-6 (p=0.02) and leptin (p<0.001) but increased endothelin-1 levels (p=0.02). CONCLUSIONS: We conclude that the single resistance exercise session elicited an acute increment of baseline vascular reactivity and an increased endothelial-dependent vasodilation with concomitant changes in inflammatory profile and endothelin-1 in our tested women with excessive adiposity.
  • O USO DO ÍNDICE DE DESVIO DE MARCHA PARA A AVALIAÇÃO DE PACIENTES HEMIPARÉTICOS APÓS ACIDENTE VASCULAR CEREBRAL Original Research

    BASES: O Índice de Desvio da Marcha, inicialmente utilizado para avaliar a marcha de crianças com paralisia cerebral, tem sido utilizado como parâmetro quantitativo de alterações de marcha de indivíduos com outras condições. No entanto, não existem estudos que avaliem alterações no padrão de marcha de indivíduos hemiparéticos crônicos pós-AVC com base nesse índice. OBJETIVOS: Caracterizar as alterações no padrão de marcha de acordo com o Índice de Desvio da Marcha e parâmetros espácio-temporais da marcha de sujeitos hemiparéticos crônicos em relação a indivíduos saudáveis. MÉTODOS: Estudo retrospetivos. Os dados foram obtidos a partir da base de dados do Laboratório de Marcha do Hospital Israelita Albert Einstein. Trinta indivíduos foram incluídos neste estudo, com AVC unilateral, isquêmico ou hemorrágico, crônico (tempo pós-lesão> 6 meses) e habilidade de caminhada classificada como 2, 3, 4 ou 5 segundo a Categoria de Ambulação Funcional. Os dados de 87 indivíduos saudáveis foram incluídos no grupo controle, obtido a partir da base de dados de normalidade. A análise estatística foi aplicada através do teste de Kruskall Wallis, seguido do teste pós-hoc de Mann-Whitney, considerando um valor p crítico <0,05. RESULTADOS: Os valores observados para o Índice de Desvio da Marcha foram menores para os membros paréticos (64,69 ± 16,29) e não-paréticos (64,88 ± 15) em relação ao controle (101,01 ± 10,12; p <0,001). Não foram observadas diferenças nos escores do Índice de Desvio da Marcha entre os membros paréticos e não paréticos (p> 0,99). CONCLUSÃO: Os achados do presente estudo demonstram que o Índice de Desvio da Marcha pode ser um parâmetro sensível para identificar alterações no padrão de marcha de indivíduos hemiparéticos crônicos pós-AVC.

    BACKGROUND: The Gait Deviation Index, initially conceived to evaluate the gait of children with cerebral palsy, has been used as a quantitative parameter of gait pattern changes of individuals with other conditions. However, there are no studies evaluating changes in the gait pattern of chronic hemiparetic post-stroke subjects based on this index. AIMS: To characterize the changes in gait pattern according to the Gait Deviation Index and gait spatiotemporal parameters of chronic hemiparetic subjects compared to healthy subjects. METHODS: Retrospective study. Data were obtained from the database of the Gait Laboratory of Hospital Israelita Albert Einstein. Thirty subjects were included in this study, with previous unilateral, ischemic or hemorrhagic chronic stroke (time post-lesion > 6 months) and ability of walking classified as 2, 3, 4 or 5 according to Functional Ambulation Category. Data from 87 healthy subjects were included in control group, obtained from a normality database. Statistical analysis was applied through the Kruskall Wallis test, followed by Mann-Whitney post-hoc test, considering a critical p value <0.05. RESULTS: The Gait Deviation Index scores were decreased for both paretic (64.69 ± 16.29) and non-paretic limbs (64.88 ± 15.00) compared to control (101.01 ± 10.12; p < 0.001). No differences were observed in Gait Deviation Index scores between paretic and non-paretic limbs (p > 0.99). CONCLUSION: The findings of the current study demonstrate that the Gait Deviation Index may be a sensitive parameter to identify changes in the gait pattern of chronic hemiparetic post-stroke subjects.
  • EFEITO AGUDO DO ALONGAMENTO ESTÁTICO SOBRE A DESTREZA MANUAL DE JOVENS DESTREINADOS Rapid Communication

    OBJETIVO: Este estudo objetivou avaliar a influência do alongamento sobre a função manual. MÉTODOS: A amostra foi composta por 10 homens destreinados em um delineamento experimental cross-over randomizado, com quatro sessões de testes. Cada sessão foi constituída apenas por um dos protocolos: a) controle ou b) série única de alongamento estático passivo seguidos pelo minnesota Hand Dexterity Test ou pelo Teste de Preensão Manual com analise eletromiográfica do Tempo de Reação Manual. Para as comparações dos dados, o teste T de Student foi usado, com o índice de significância adotado de p ≤ 0,05. RESULTADOS: O alongamento aumentou a destreza manual em ambos Placing e Turning Tests, sem alterar a força de preensão manual ou o tempo de reação. CONCLUSÃO: Os resultados indicam que o alongamento melhorou a destreza manual de jovens destreinados.

    OBJECTIVE: This study aimed to evaluate the influence of acute muscle stretching on manual function. METHODS: The sample consisted of 10 untrained men in a randomized, four test session cross-over experimental design. Each session was composed of only one of two protocols: a) control, or b) single series of passive static stretching; followed by either Minnesota Hand dexterity test or hand grip strength test with eletromyographical recording of reaction time. For data comparison, the Student T-test with significance level of p ≤ 0.05 was used. RESULTS: Manual dexterity increased after stretching for both placing and turning tests, with no changes in hand grip strength or reaction time. CONCLUSION: The results show that a 30 second static stretch of the hand decreases time to complete the Minnesota Hand Dexterity test without affecting handgrip strength or hand reaction time; thus it improves manual dexterity of young untrained men.
