The authors report a case of massive pulmonary thromboembolism in a 63 year old patient with a severe impairment of hemodynamic condition. The clinical diagnostic was confirmed by 2D echo and the patient underwent successful pulmonary embolectomy, 52 hours after admission and was discharged on the 34rd postoperative day.
