    RESUMO Devido às particularidades anatômicas da região subtrocantérica, o tratamento das fraturas nessa região permanece desafiador. A incontestável evolução dos implantes não foi acompanhada pela esperada diminuição no índice de complicações. O objetivo do presente estudo é discutir, minuciosamente, pontos críticos como planejamento pré-operatório, táticas de redução e evidências científicas atuais no tratamento das fraturas subtrocantéricas do fêmur.

    ABSTRACT Because of the anatomical peculiarities of the subtrochanteric region, treatment of fractures in this region remains challenging. The undeniable evolution of implants has not been accompanied by the expected decrease in the complication rate. The aim of this study was to discuss critical points in detail, such as preoperative planning, reduction tactics and the current scientific evidence concerning treatment of subtrochanteric fractures of the femur.
  • Estudo comparativo entre três técnicas cirúrgicas para fraturas intra-articulares de calcâneo: redução aberta e fixação interna com placa, fixação externa e minimamente invasiva Original Article

    RESUMO OBJETIVO: Avaliar, comparar e identificar a técnica cirúrgica com melhor resultado para o tratamento de fraturas intra-articulares do calcâneo, levando em consideração evolução pós-operatória, complicações e pontuação no questionário Aofas. MÉTODOS: Estudo retrospectivo de 54 pacientes com fraturas de calcâneo operados entre e 2002 e 2012 com as técnicas 1) redução aberta com incisão lateral alargada em "L" e fixação com placa duplo "H" de 3,5 mm, 2) redução aberta por incisão lateral econômica e fixação percutânea com fios e parafusos e 3) redução aberta por incisão lateral econômica e fixação com fixador externo monoplanar regulável. RESULTADOS: Pacientes tratados pela via de acesso lateral e fixação com placa tiveram média de 76 pontos na escala Aofas, em pacientes tratados pela via de acesso lateral econômica e fixação com fios e parafuso a média foi de 71 e nos pacientes tratados com via de acesso lateral e fixador externo foi de 75 pontos. As três técnicas cirúrgicas demostraram-se efetivas no tratamento da fratura intra-articular do calcâneo, sem evidência de superioridade de uma técnica sobre as demais. CONCLUSÃO: A fratura intra-articular do calcâneo é uma fratura complexa e seu tratamento deve ser individualizado, baseado nas características do paciente, no tipo de fratura e na experiência do cirurgião com a técnica operatória escolhida.

    ABSTRACT OBJECTIVE: To evaluate, compare and identify the surgical technique with best results for treating intra-articular calcaneal fractures, taking into account postoperative outcomes, complications and scoring in the Aofas questionnaire. METHODS: This was a retrospective study on 54 patients with fractures of the calcaneus who underwent surgery between 2002 and 2012 by means of the following techniques: (1) open reduction with extended L-shaped lateral incision and fixation with double-H plate of 3.5 mm; (2) open reduction with minimal incision lateral approach and percutaneous fixation with wires and screws; and (3) open reduction with minimal incision lateral approach and fixation with adjustable monoplanar external fixator. RESULTS: Patients treated using a lateral approach, with fixation using a plate had a mean Aofas score of 76 points; those treated through a minimal incision lateral approach with screw and wire fixation had a mean score of 71 points; and those treated through a minimal incision lateral approach with an external fixator had a mean score of 75 points. The three surgical techniques were shown to be effective for treating intra-articular calcaneal fractures, without any evidence that any of the techniques being superior. CONCLUSION: Intra-articular calcaneal fractures are complex and their treatment should be individualized based on patient characteristics, type of fracture and the surgeon's experience with the surgical technique chosen.
  • Tratamento das fraturas do úmero proximal com placa anatômica bloqueada: correlação dos resultados funcionais e radiográficos Original Article

    RESUMO OBJETIVO: Correlacionar os resultados funcionais e os índices radiográficos das fraturas do úmero proximal tratadas com placa anatômica bloqueada para úmero proximal. MÉTODOS: Examinaram-se 39 pacientes com fraturas do úmero proximal tratados com placa anatômica bloqueada, com seguimento médio de 27 meses. Esses pacientes foram submetidos à análise do escore da Universidade da Califórnia de Los Angeles (UCLA) e à avaliação do arco de movimento pelo método da Academia Americana de Cirurgiões Ortopédicos no ombro operado e a exames radiográficos comparativos de ambos os ombros. Estabeleceu-se a correlação entre as medidas radiográficas e os resultados funcionais. RESULTADOS: Obtivemos 64% de bons e excelentes resultados conforme o escore da UCLA, com médias de 124° de elevação; 44° de rotação lateral; e polegar-T9 de rotação medial. O tipo de fratura, de acordo com a classificação de Neer, e a idade do paciente tiveram significativa correlação com o arco de movimentos; quanto maiores o número de partes das fraturas e a idade dos pacientes, piores os resultados. Encontrou-se associação entre a elevação e o escore da UCLA com o ângulo cervicodiafisário na incidência anteroposterior; as fraturas fixadas com desvios em varo maiores do que 15° apresentaram os piores resultados (p < 0,001). CONCLUSÃO: A variação da medida do ângulo cervicodiafisário na incidência anteroposterior mostrou significativa correlação com o arco de movimento; desvios em varo maiores do que 15° não foram bem tolerados. Esse parâmetro pode ser um dos preditores dos resultados funcionais nas fraturas do úmero proximal tratadas com placa anatômica bloqueada.

    ABSTRACT OBJECTIVE: To correlate the functional outcomes and radiographic indices of proximal humerus fractures treated using an anatomical locking plate for the proximal humerus. METHODS: Thirty-nine patients with fractures of the proximal humerus who had been treated using an anatomical locking plate were assessed after a mean follow-up of 27 months. These patients were assessed using the University of California Los Angeles (UCLA) score and their range of motion was evaluated using the method of the American Academy of Orthopedic Surgeons on the operated shoulder and comparative radiographs on both shoulders. The correlation between radiographic measurements and functional outcomes was established. RESULTS: We found that 64% of the results were good or excellent, according to the UCLA score, with the following means: elevation of 124°; lateral rotation of 44°; and medial rotation of thumb to T9. The type of fracture according to Neer's classification and the patient's age had significant correlations with the range of motion, such that the greater the number of parts in the fracture and the greater the patient's age were, the worse the results also were. Elevation and UCLA score were found to present associations with the anatomical neck-shaft angle in anteroposterior view; fractures fixed with varus deviations greater than 15° showed the worst results (p < 0.001). CONCLUSION: The variation in the neck-shaft angle measurements in anteroposterior view showed a significant correlation with the range of motion; varus deviations greater than 15° were not well tolerated. This parameter may be one of the predictors of functional results from proximal humerus fractures treated using a locking plate.

    RESUMO OBJETIVO: Avaliar os resultados do tratamento cirúrgico de pacientes com lesão de dedo em martelo com o uso de placa-gancho e parafuso. MÉTODOS: Foram analisados 25 pacientes entre 20 e 35 anos, 19 do sexo masculino e seis do feminino, de maio de 2008 a dezembro de 2012. Os pacientes foram submetidos à avaliação de acordo com os critérios de Crawford e o acompanhamento médio foi de 18 meses. RESULTADOS: Os resultados obtidos foram excelentes em 10 pacientes (40%) e bons em 15 (60%); 21 pacientes (84%) não referiram dor no 18° mês de pós-operatório. Foi verificada ausência de limitação da amplitude de movimento em 14 casos (56%), limitação da extensão em sete (28%) e limitação da flexão em quatro (16%). CONCLUSÃO: O tratamento cirúrgico com redução aberta e fixação interna com placa-gancho e parafuso demonstrou ser uma ótima opção de tratamento nas fraturas em martelo e é considerado um método seguro e eficaz.

    ABSTRACT OBJECTIVES: To evaluate the results from surgical treatment of patients with mallet finger injury using a hook plate and screw. METHODS: Twenty-five patients (19 males and six females) between the ages of 20 and 35 years were analyzed between May 2008 and December 2012. They were evaluated in accordance with Crawford's criteria and the mean follow-up was 18 months. RESULTS: The results from 10 patients (40%) were excellent and from 15 (60%), good. Twenty-one patients (84%) reported no pain, 18 months after the operation. There was no limitation to range of motion in 14 cases (56%), limitation of extension in seven (28%) and limitation of flexion in four (16%). CONCLUSION: Surgical treatment by means of open reduction and internal fixation using a hook plate and screw proved to be an excellent option for treating mallet finger fractures and was considered to be a safe and effective method.
  • Técnica transtibial versus duas incisões na reconstrução do ligamento cruzado anterior: posicionamento dos túneis, isometricidade e avaliação funcional Original Article

    RESUMO OBJETIVO: Comparar as técnicas transtibial e de duas incisões na reconstrução do ligamento cruzado anterior (LCA) com banda única. MÉTODOS: Foi feito um estudo prospectivo e randomizado em bloco. Os pacientes foram submetidos a reconstrução do LCA por meio de duas técnicas: transtibial (grupo 1: 20 pacientes) ou de duas incisões (grupo 2: 20 pacientes). Foram avaliados o posicionamento radiográfico dos túneis, a inclinação do enxerto, a isometricidade do enxerto e os resultados funcionais (IKDC e Lysholm). RESULTADOS: O posicionamento do túnel femoral na radiografia em AP expresso em porcentagem em relação à borda medial do planalto tibial no grupo 1 foi em média de 54,6% e no grupo 2 foi de 60,8% (p < 0,05). O posicionamento do túnel femoral na radiografia em P expresso em porcentagem em relação à borda anterior da linha de Blumensaat no grupo 1 foi em média de 68,4% e no grupo 2 foi de 58% (p < 0,05). A inclinação do enxerto no grupo 1 foi em média de 19 graus e no grupo 2 foi de 27,2 graus (p < 0,05). A isometricidade do enxerto no grupo 1 foi em média de 0,96 mm e no grupo 2 foi de 1,33 mm (p > 0,05). O grupo 2 apresentou melhores resultados pela manobra de Pivot-Shift (p < 0,05). CONCLUSÃO: A técnica de duas incisões permitiu um posicionamento do túnel femoral mais lateralizado e anteriorizado e que o enxerto ficasse mais inclinado e demonstrou clinicamente um melhor resultado pela manobra de Pivot-Shift. Não houve diferença na isometricidade e no resultado funcional final no curto tempo de seguimento avaliado.

    ABSTRACT OBJECTIVE: To compare the transtibial and two-incision techniques for anterior cruciate ligament (ACL) reconstruction using a single band. METHODS: A prospective and randomized study was conducted in blocks. Patients underwent ACL reconstruction by means of two techniques: transtibial (group 1: 20 patients) or two incisions (group 2: 20 patients). The radiographic positioning of the tunnel, inclination of the graft, graft isometricity and functional results (IKDC and Lysholm) were evaluated. RESULTS: The positioning of the femoral tunnel on the anteroposterior radiograph, expressed as a mean percentage relative to the medial border of the tibial plateau, was 54.6% in group 1 and 60.8% in group 2 (p < 0.05). The positioning of the femoral tunnel on the lateral radiograph, expressed as a mean percentage relative to the anterior border of Blumensaat's line, was 68.4% in group 1 and 58% in group 2 (p < 0.05). The mean inclination of the graft was 19° in group 1 and 27.2° in group 2 (p < 0.05). The mean graft isometricity was 0.96 mm in group 1 and 1.33 mm in group 2 (p > 0.05). Group 2 had better results from the pivot-shift maneuver (p < 0.05). CONCLUSION: The technique of two incisions allowed positioning of the femoral tunnel that was more lateralized and anteriorized, such that the graft was more inclined and there was a clinically better result from the pivot-shift maneuver. There was no difference in isometricity and no final functional result over the short follow-up time evaluated.

    RESUMO OBJETIVO: Definir o perfil antropométrico do joelho em população brasileira portadora de gonartrose, com o uso da mensuração intraoperatória, e avaliar a compatibilidade de três implantes disponíveis para artroplastia total do joelho. MÉTODOS: Foram coletados, de forma prospectiva, os dados morfométricos de 117 pacientes portadores de gonartrose. Documentaram-se seis dimensões no fêmur distal e duas na tíbia proximal em 118 joelhos, durante a artroplastia total. Esses dados foram comparados com as dimensões dos três implantes disponíveis para artroplastia total do joelho. RESULTADOS: A análise estatística revelou que mais de um quarto dos pacientes apresentou relação inadequada entre os joelhos e as próteses. CONCLUSÃO: Os implantes avaliados necessitam de ajustes para melhor atender aos pacientes brasileiros.

    ABSTRACT OBJECTIVE: To define the anthropometric profile of the knee in a Brazilian population with gonarthrosis using intraoperative measurements; and to evaluate the compatibility of three implants available for total knee arthroplasty. METHODS: Morphometric data were collected prospectively from 117 subjects with gonarthrosis. Six dimensions in the distal femur and two in the proximal tibia were documented in 118 knees while performing total arthroplasty. These data were compared with the dimensions of three implants available for total knee arthroplasty. RESULTS: The statistical analysis showed that more than a quarter of the patients presented an unsatisfactory relationship between the knee and prosthesis. CONCLUSION: The implants evaluated need to be adjusted to better fit Brazilian patients.

    RESUMO OBJETIVO: Avaliar, em médio prazo, a reconstrução do ligamento patelofemoral medial (LPFM) com duplo-feixe com enxerto do tendão semitendíneo e fixação com âncoras metálicas. MÉTODOS: Estudo prospectivo de corte transversal. De maio de 2010 a janeiro de 2015, após aprovação do comitê de ética em pesquisa, foram analisados 31 pacientes com instabilidade patelofemoral, submetidos à cirurgia de reconstrução do ligamento patelofemoral medial (LPFM) com a técnica anatômica do duplo-feixe com fixação com âncoras metálicas. Para avaliar a eficácia da cirurgia de reconstrução do LPFM, foram utilizadas a escala de Kujala e o escore de Tegner-Lysholm, antes do procedimento e após um ano. Foram avaliados os dados clínicos como o arco de movimento, presença do Sinal do J e nível de dor. Os dados foram tabulados no programa Excel(r) e analisados com o programa SPSS Statistics(r) versão 21. A análise estatística foi feita com o teste T de Wilcoxon e o teste de McNemar. RESULTADOS: A média dos resultados obtidos no pré-operatório com o teste de Kujala foi de 45,64 ± 1,24 e no pós-operatório de 94,03 ± 0,79 (p < 0,001). O escore do joelho de Tegner-Lysholm alcançado foi de 40,51 ± 1,61 no pré-operatório, para 91,64 ± 0,79 (p < 0,001) no pós-operatório. O arco de movimento obteve média de 125,96 ± 2,11 no pré-operatório e 138,38 ± 1,49 no pós-operatório (p < 0,05). CONCLUSÃO: A reconstrução do LPFM com duplo-feixe é uma técnica de fácil reprodução, sem episódios de recidiva, e com resultados adequados para a restauração da estabilidade.

    ABSTRACT OBJECTIVE: To evaluate double-bundle reconstruction of the medial patellofemoral ligament (MPFL) using a graft from the semitendinosus tendon and fixation with metal anchors over the medium term. METHODS: This was a prospective cross-sectional study. After approval from the research ethics committee, 31 patients with patellofemoral instability who underwent MPFL reconstruction by means of the anatomical double-bundle technique, with fixation using metal anchors, were analyzed between May 2010 and January 2015. To evaluate the effectiveness of the MPFL reconstruction surgery, the Kujala scale and the Tegner-Lysholm score were assessed before the procedure and one year afterwards, along with clinical data such as pain levels, range of motion and J sign. The data were tabulated in the Excel(r) software and were analyzed using the SPSS Statistics(r) software, version 21. The statistical analysis was performed using the Wilcoxon T test and the McNemar test. RESULTS: The mean preoperative score from the Kujala test was 45.64 ± 1.24 and the postoperative score was 94.03 ± 0.79 (p < 0.001). The preoperative Tegner-Lysholm score was 40.51 ± 1.61 and the postoperative score was 91.64 ± 0.79 (p < 0.001). The preoperative range of motion was 125.96 ± 2.11 and the postoperative range was 138.38 ± 1.49 (p < 0.05). CONCLUSION: MPFL reconstruction by means of the anatomical double-bundle technique is easily reproducible, without episodes of recurrence, with satisfactory results regarding restoration of stability and function of the patellofemoral joint.
  • Lesões do manguito rotador e fatores associados à reoperação Original Article

    RESUMO OBJETIVO: Avaliar a prevalência de LMR, descrever o perfil do paciente reoperado, a causa da rerruptura tendínea, a evolução da lesão e a variação de tempo entre os procedimentos cirúrgicos. MÉTODOS: Estudo com delineamento transversal, que envolveu 604 eventos cirúrgicos feitos entre janeiro de 2006 e dezembro de 2012, em dois hospitais de referência regional. Após aprovação pelo comitê de ética, os dados foram coletados em um único momento, descreveu-se o perfil epidemiológico dos pacientes e usou-se a classificação de Cofield para mensurar a extensão das lesões, as quais foram operadas por via artroscópica em todos os pacientes. Os dados foram digitados no programa Epinfo versão 3.5.3(r), e analisados no SPSS versão 18.0(r). RESULTADOS: Entre os 604 procedimentos, o sexo feminino esteve predominantemente acometido com 351 (58,1%) e quando o membro dominante foi o direito ele esteve acometido em 90% dos casos (p < 0,05). O tendão supraespinhal foi acometido em 574 (95%) casos e as lesões foram de tamanho médio em 300 (49,7%) casos. Foram 18 (2,98%) reoperações e o membro superior direito foi o mais acometido (66,6%), com causa não traumática em 12 pacientes. As relesões foram em sua maioria menores (44%), com o tempo entre os dois procedimentos cirúrgicos que variou entre seis e 298 semanas. CONCLUSÃO: Sexo feminino, extensão menor no segundo procedimento e causa não traumática foram encontrados na maior parte dos casos analisados.

    ABSTRACT OBJECTIVE: To evaluate the prevalence of rotator cuff tears and describe the profile of reoperated patients, causes of repeated tendon tears, tear evolution and range of times between surgical procedures. METHOD: This was a cross-sectional study involving 604 surgical procedures performed at two regional referral hospitals between January 2006 and December 2012. After approval by the ethics committee, data describing the patients' epidemiological profile were gathered at a single time, using Cofield's classification to measure the extent of the tears, all of which underwent arthroscopic surgery. The data were entered into Epi Info 3.5.3 and were analyzed using SPSS version 18.0. RESULTS: Among the 604 surgical procedures, females were affected in more cases (351; 58.1%). When the dominant limb was the right limb, it was affected in 90% of the cases (p < 0.05). The supraspinatus tendon was affected in 574 cases (95%) and the tears were of medium size in 300 cases (49.7%). Eighteen reoperations were performed (2.98%) and the upper right limb was the most affected (66.6%). The cause was non-traumatic in 12 patients. The repeated tears were mostly smaller (44%), and the length of time between the two surgical procedures ranged from 6 to 298 weeks. CONCLUSION: Female gender, smaller extent in the second procedure and non-traumatic cause were found in most of the cases analyzed.

    RESUMO OBJETIVO: Apresentar os resultados do reparo direto do tendão com EndoButton e parafuso de interferência nos pacientes com lesão do bíceps distal com evolução maior do que 28 dias. MÉTODOS: Entre janeiro de 2012 e novembro de 2013, 11 pacientes (todos do sexo masculino) com ruptura do bíceps distal com intervalo da lesão e cirurgia maior do que 28 dias. A idade média foi de 46 anos e o mecanismo de trauma mais comum foi uma carga excêntrica com o cotovelo em flexão e supinação. RESULTADOS: Foi feita uma análise subjetiva da dor e função com a Escala Visual Analógica de Dor (EVA) e o Mayo Elbow Performance Score (MEPS) pré e pós-operatório. Houve uma diminuição da EVA de 5 pontos para 0,8 ponto em média. O MEPS melhorou de 69,3 pontos no pré para 97,5 pontos no pós-operatório. A média de flexão foi de 133,1° do lado operado contra 134,3°. A média de extensão foi de -2,5° e 0° (lado operado × não operado). Supinação foi de 88,2° × 89,5° e pronação 82,5° × 84,1° quando comparado o lado operado versus o lado não operado. A força de flexão e supinação foi avaliada com o auxílio de um dinamômetro e verificamos que a força média de flexão e supinação correspondia, respectivamente, a 78,57% e 89,65% a força do membro não operado. CONCLUSÃO: A técnica do reparo direto do tendão com o uso de EndoButton e parafuso de interferência mostra-se como uma opção segura e eficaz para o reparo direto das lesões crônicas do bíceps distal.

    ABSTRACT OBJECTIVE: To present the results from direct tendon repair using EndoButton and interference screws in patients with lesions of the distal biceps that had evolved over a period of more than 28 days. METHODS: Between January 2012 and November 2013, eleven patients (all male) with a torn distal biceps and a time interval between injury and surgery of more than 28 days were evaluated. The patients' mean age was 46 years and the most common mechanism of injury was eccentric loading with the elbow flexed and supinated. RESULTS: A subjective analysis on pain and function was conducted using a visual analog scale of pain (VAS) and the Mayo Elbow Performance Score (MEPS), before and after surgery. The VAS showed a decrease of 5 points to 0.8 points on average. The MEPS improved from 69.3 points before the operation to 97.5 points afterwards. The mean flexion was 133.1° on the operated side, versus 134.3°. The mean extension was -2.5° and 0° (operated side versus non-operated). Supination was 88.2° versus 89.5° and pronation was 82.5° versus 84.1°, comparing the operated side versus the non-operated side. Flexion and supination strengths were evaluated with the aid of a dynamometer, and the mean flexion and supination strengths were found to be respectively 78.57% and 89.65% of the strength of the non-operated limb. CONCLUSION: Use of the technique of direct tendon repair using EndoButton and interference screws was shown to be a safe and effective alternative for repairing chronic lesions of the distal biceps.

    RESUMO OBJETIVO: Analisar quantitativa e qualitativamente os resultados do tratamento do pé torto congênito com seguimento médio de 4,6 anos. MÉTODOS: Foram analisados 26 pacientes que fizeram tratamento pelo método de Ponseti, total de 39 pés. A média da idade do início do tratamento foi 5,65 meses. O tempo de seguimento após a tenotomia do tendão de Aquiles foi em média de 4,6 anos. Foram excluídos pacientes com pé torto secundário. Foram analisados dados epidemiológicos e mensurações radiográficas do ângulo de Kite e aplicados questionário de satisfação e questionário de Laaveg. RESULTADOS: Dos 26 pacientes tratados, um apresentou recidiva da deformidade, foi necessário retornar ao início do tratamento. A pontuação média do questionário e do exame físico foi de 89,76, resultado considerado bom; 99% dos pacientes responderam que os pés nunca doem ou doem somente aos grandes esforços; 88% responderam que o pé não limita as atividades; 96% responderam que estão muito satisfeitos ou satisfeitos com os resultados do tratamento. A média do ângulo de Kite na incidência anteroposterior foi de 28,14° e no perfil 26,11°. CONCLUSÃO: O tratamento para pé torto congênito idiopático pelo método Ponseti é o que traz melhores resultados associado a menor lesão de partes moles, o que confirma a eficácia e a boa reprodutibilidade do método.

    ABSTRACT OBJECTIVE: To quantitatively and qualitatively analyze the results from treatment of congenital clubfoot with a mean follow-up of 4.6 years. METHODS: 26 patients who underwent treatment by means of the Ponseti method were analyzed (total of 39 feet). The mean age at the start of the treatment was 5.65 months. The mean length of the follow-up subsequent to tenotomy of the Achilles tendon was 4.6 years. Patients with secondary clubfoot were excluded. Epidemiological data, radiographic measurements on the Kite angle and data from a satisfaction questionnaire and the Laaveg questionnaire were analyzed. RESULTS: Among the 26 patients treated, one presented recurrence of the deformity and had to return to the beginning of the treatment. The mean score from the questionnaire and physical examination was 89.76 points, and this result was considered good. 99% of the patients responded that their feet never hurt or hurt only upon great activity; 88% said that their feet did not limit their activities; and 96% said that they were very satisfied or satisfied with the results from the treatment. The mean Kite angle in anteroposterior view was 28.14° and it was 26.11° in lateral view. CONCLUSION: Treatment for idiopathic congenital clubfoot by means of the Ponseti method brings better results together with less soft-tissue injury, thus confirming the effectiveness and good reproducibility of this method.

    RESUMO OBJETIVOS: Descrição de uma nova técnica cirúrgica para o tratamento de ombro de pacientes com sequela de paralisia obstétrica. Análise preliminar dos resultados obtidos com essa técnica. MÉTODOS: Cinco pacientes consecutivos foram submetidos ao tratamento cirúrgico proposto, que envolve a liberação articular anterior por via artroscópica, seguida da transferência do tendão do músculo grande dorsal (alongado e reforçado com enxerto tendíneo homólogo) para a porção póstero-superior do tubérculo maior, com o uso de uma única via delto-peitoral. Todos foram reavaliados após um período pós-operatório mínimo de 12 meses. A avaliação da função baseou-se na amplitude de movimento e na classificação modificada de Mallet. A pequena casuística não permitiu análises estatísticas. RESULTADOS: De forma geral, as rotações laterais passiva e ativa melhoraram, enquanto a rotação medial piorou. Os outros movimentos (elevação, capacidade de colocação da mão na boca e capacidade de colocação da mão na nuca) tiveram evolução menos consistente. A média do escore de Mallet modificado melhorou 4,2 pontos (de 11,4 para 15,6). CONCLUSÃO: O tendão do músculo grande dorsal pode ser transferido para a porção póstero-superior do tubérculo maior por meio de uma única via delto-peitoral, quando alongado e reforçado com enxerto tendíneo homólogo.

    ABSTRACT OBJECTIVE: Description of a new surgical technique for treating the shoulders of patients with sequelae of obstetric paralysis. Preliminary analysis on the results obtained from this technique. METHODS: Five consecutive patients underwent the proposed surgical procedure, consisting of arthroscopic anterior joint release followed by transfer of the latissimus dorsi tendon (elongated and reinforced with a homologous tendon graft) to the posterosuperior portion of the greater tubercle, using a single deltopectoral approach. All the patients were reevaluated after a minimum postoperative period of twelve months. The functional assessment was based on the range of motion and the modified Mallet classification system. Statistical analyses were not possible because of the small sample. RESULTS: Overall, passive and active lateral rotations increased, while medial rotation decreased. The other movements (elevation, capacity to place a hand in the mouth and capacity to place a hand behind the neck) had less consistent evolution. The mean modified Mallet score improved by 4.2 points (from 11.4 to 15.6). CONCLUSION: The latissimus dorsi tendon can be transferred to the posterosuperior portion of the greater tubercle through a single deltopectoral approach when elongated and reinforced with a homologous tendinous graft.

    RESUMO OBJETIVO: Avaliar a evolução do quadril contralateral após a reconstrução unilateral de luxação de quadril em pacientes classificados como GMFCS IV-V e identificar possíveis fatores prognósticos da evolução. MÉTODOS: Estudo retrospectivo de 17 pacientes portadores de paralisia cerebral espástica, classificados pela escala GMFCS (Gross Motor Functional Classification System) em graus IV e V, submetidos a cirurgia de reconstrução unilateral de luxação de quadril (liberação de adutores, osteotomia varizante femoral e acetabuloplastia). O seguimento pós-operatório mínimo foi de 30 meses. Foram avaliados parâmetros clínicos (sexo, idade na ocasião do procedimento cirúrgico, tempo de seguimento após a cirurgia e amplitude de abdução), de tratamento (a feitura ou não de encurtamento femoral, aplicação de toxina botulínica e se houve procedimentos musculares prévios) e radiográficos (índice de extrusão de Reimers [IR], ângulo acetabular [AC] e continuidade do arco de Shenton [AS]). RESULTADOS: Dos 17 pacientes avaliados, oito deslocaram (grupo I) e nove não (grupo II). O grupo I contava com três pacientes do sexo masculino e cinco do feminino; grupo II apresentou um paciente do sexo masculino e oito do feminino. A média de idade no momento da cirurgia dos pacientes do grupo I foi de 62 meses e o tempo de seguimento médio foi de 62 meses. No grupo II foram de 98 e 83 meses, respectivamente. Houve tendência dos pacientes operados com maior idade não evoluírem com luxação contralateral. Dos nove pacientes que apresentavam a combinação de IR < 30% e AC< 25°, apenas um apresentou luxação no seguimento. A subluxação contralateral ocorre nos dois primeiros anos de pós-operatório. CONCLUSÃO: Quadris que apresentam um IR < 30° e AC < 25° não tendem a evoluir para subluxação e podem ser mantidos em observação. Medidas clínicas e radiográficas isoladas no pré-operatório não foram úteis para indicar a evolução natural do quadril não operado. O período crítico para subluxação são os dois primeiros anos do pós-operatório.

    ABSTRACT OBJECTIVE: To evaluate the progression of the contralateral hip after unilateral reconstruction of hip dislocation in patients classified as GMFCS IV-V; and to identify potential prognostic factors for their evolution. METHODS: This was a retrospective study on 17 patients with spastic cerebral palsy, who were classified on the GMFCS scale (Gross Motor Functional Classification System) as degrees IV and V, and who underwent unilateral reconstruction surgery to treat hip dislocation (adductor release, femoral varus osteotomy and acetabuloplasty). The minimum postoperative follow-up was 30 months. The clinical parameters evaluated were sex, age at time of surgery, length of follow-up after surgery and range of abduction. The treatment parameters were use/nonuse of femoral shortening, application of botulinum toxin and any previous muscle releases. The radiographic parameters were Reimer's extrusion index (REI), acetabular angle (AA) and the continuity of Shenton's line. RESULTS: Among the 17 patients evaluated, eight presented dislocation (group I) and nine did not (group II). Group I comprised three males and five females; group II comprised one male and eight females. The mean age at the time of surgery among the group I patients was 62 months and the mean follow-up was 62 months. In group II, these were 98 and 83 months, respectively. There was a trend in which patients of greater age did not evolve with contralateral dislocation. Among the nine patients with the combination of REI < 30% and AA < 25°, only one presented dislocation during the follow-up. Contralateral subluxation occurred within the first two years after the surgery. CONCLUSION: Hips presenting REI < 30° and AA < 25° do not tend to evolve to subluxation and can be kept under observation. Preoperative clinical and radiographic measurements alone are not useful for indicating the natural evolution of non- operated hips. The critical period for subluxation is the first two years after surgery.

    RESUMO OBJETIVO: Comparar os resultados pós-operatórios do uso da antibioticoprofilaxia por um e cinco dias nos pacientes submetidos à artrodese lombar de até três níveis. MÉTODOS: Foram avaliados 43 pacientes, todos submetidos à artrodese lombar devido à doença degenerativa discal com um, dois ou três níveis. Divididos de forma randomizada em dois grupos, um grupo recebeu antibioticoprofilaxia com cefalotina 1 g e o outro grupo recebeu o mesmo antibiótico por cinco dias. Após a intervenção cirúrgica, fez-se a avaliação dos pacientes na data da alta, no primeiro retorno ao ambulatório, após duas semanas, e após 90 dias da data do procedimento cirúrgico, com exame clínico da ferida operatória e exames laboratoriais de ambos os grupos. RESULTADOS: Observou-se que os pacientes do grupo com um dia de antibioticoplofilaxia, 28,6% apresentaram complicações na ferida operatória e o grupo de cinco dias, 27,9%. CONCLUSÃO: Este estudo demonstra que uma única dose de antibioticoprofilaxia é tão eficaz quanto o regime de múltiplas doses em cirurgias de artrodese lombar até três níveis. Não justifica os custos e riscos de submeter o paciente a internação sob regime medicamentoso prolongado.

    ABSTRACT OBJECTIVE: To compare the postoperative results from use of antibiotic prophylaxis for one and five days among patients undergoing lumbar arthrodesis at up to three levels. METHODS: Forty-three patients who all underwent lumbar arthrodesis due to degenerative disc disease at one, two or three levels were evaluated. They were divided randomly into two groups: one received antibiotic prophylaxis with cefalotin (1 g) and the other received the same antibiotic for five days. After the surgical intervention, the patients were evaluated at the time of hospital discharge, at the first return to the clinic (two weeks later) and 90 days after the date of the surgery with the surgical wound, with clinical examination of the surgical wound and laboratory tests on both groups. RESULTS: It was observed that among the patients in the group with one day of antibiotic prophylaxis, 28.6% presented complications in the surgical wound, while in the group with five days, 27.9% presented complications. CONCLUSION: This study demonstrates that a single dose of antibiotic prophylaxis is as effective as a regimen of multiple doses in lumbar arthrodesis surgery at up to three levels. Thus, the costs and risks of subjecting patients to hospitalization under a prolonged drug regimen are unjustifiable.

    RESUMO OBJETIVO: Apresentar os resultados preliminares do tratamento da DLCP com o uso de artrodiástase com fixador externo monolateral aplicado ao quadril e descrever sucintamente a técnica operatória usada em um estudo prospectivo. MÉTODOS: Estudo prospectivo de 18 pacientes com DLCP submetidos ao tratamento operatório com a técnica de artrodiástase do quadril por meio de fixador externo unilateral. São 13 pacientes do gênero masculino e cinco do feminino com idade média de 8,5 anos com variação de cinco a 13 anos. Todos os pacientes com acometimento unilateral do quadril, nove à direita e nove à esquerda. A avaliação dos resultados foi feita na maturidade e considerou critérios clínicos e radiográficos. RESULTADOS: Todos os pacientes evoluíram com melhoria da mobilidade articular com alívio da dor obtido em 88,9% dos pacientes. A reossificação da epífise femoral ocorreu nos primeiros três meses do tratamento. Os quadris operados na fase de necrose não passaram pela fase de fragmentação e abreviaram o tempo de evolução da doença. Os resultados foram 77,8% satisfatórios e 22,2% insatisfatórios. CONCLUSÕES: A artrodiástase do quadril com fixador externo monolateral na fase ativa da DLCP melhora o grau de mobilidade articular. O emprego da técnica de artrodiástase nas fases de necrose e fragmentação (fase ativa da doença) apresenta resultados satisfatórios no tratamento da DLCP.

    ABSTRACT OBJECTIVE: To present the preliminary results from treating patients with Legg-Calvé-Perthes Disease (LCPD) by means of hip arthrodiastasis using a monolateral external fixator applied to the hip and to succinctly describe the surgical technique used, in a prospective study. METHODS: Prospective study on 18 patients with LCPD who underwent surgical treatment by means of the hip arthrodiastasis technique using a monolateral external fixator. There were 13 male and five female patients of mean age 8.5 years, ranging from five to 13 years. All the patients presented unilateral hip impairment: nine on the right side and nine on the left. The results were evaluated at maturity using clinical and radiological criteria. RESULTS: All the patients evolved with improvement of joint mobility, and pain relief was achieved in 88.9% of them. Reossification of the femoral epiphysis occurred within the first three months of the treatment. The hips operated at the necrosis stage of the disease did not passed through the fragmentation stage, thus shortening the evolution of the disease. The results were 77.8% satisfactory and 22.2% unsatisfactory. CONCLUSION: Hip arthrodiastasis with a monolateral external fixator during the active phase of LCPD improved the degree of joint mobility. Use of the arthrodiastasis technique at the necrosis stage or at the fragmentation stage (active phase of the disease) presented satisfactory results from treatment of LCPD.

    RESUMO OBJETIVO: Avaliar se um novo cimento bifásico composto por sulfato de cálcio e beta fosfato tricálcico com controle de potencial zeta poderia induzir ou conduzir a neoformação óssea em defeitos críticos. MÉTODOS: Foi feito um defeito crítico de 8 mm de diâmetro na calvária de 40 ratos Wistar machos. No grupo teste (n = 20) os defeitos foram preenchidos pelo cimento. No grupo controle (n = 20) os defeitos não foram preenchidos, permaneceu apenas o coágulo. Os animais sofreram eutanásia em 7, 14, 21 e 42 dias do pós-operatório. Espécimes da calvária foram microtomografados e posteriormente preparados para análise histológica. As análises incluíram a avaliação morfológica da histopatologia do reparo e a avaliação morfométrica da área de formação das trabéculas ósseas comparativamente entre os grupos e coloração histoquímica por meio da fosfatase tartrato-resistente (TRAP) para identificação de osteoclastos. RESULTADOS: As imagens microtomográficas dos defeitos preenchidos pelo cimento não apresentaram diminuição da área de acordo com a progressão dos períodos pós-operatórios. No grupo teste houve permanência do material e resposta corpo estranho até os últimos períodos de observação. A histomorfologia mostrou agrupamentos mais expressivos de células gigantes no grupo teste e osso neoformado mais maduro no grupo controle e comprovou a presença de material exógeno. Na histomorfometria, a área total de neoformação óssea foi significativamente maior (p = 0,009) e crescente no grupo controle. As células gigantes apresentaram expressão histoquímica positiva para TRAP e não foram observados osteoclatos. CONCLUSÃO: O cimento cerâmico não induziu ou conduziu a neoformação óssea sob o ponto de vista microtomográfico e histológico.

    ABSTRACT OBJECTIVE: To evaluate whether a new biphasic cement composed of calcium sulfate and beta tricalcium phosphate with zeta potential control could induce or lead to bone neoformation in critical defects. METHODS: A critical defect of diameter 8 mm was made in the calvaria of forty male Wistar rats. In the Test Group (n = 20), the defects were filled with cement. In the Control Group (n = 20), the defect was not filled and only coagulum was present. The animals were sacrificed 7, 14, 21 and 42 days after the operation. Calvaria specimens were subjected to microtomography and were then prepared for histological analysis. The analyses included morphological assessment on the histopathology of the repair; comparative morphometric evaluation of the area of formation of bone trabeculae between the groups; and histochemical staining by means of tartrate-resistant phosphatase (TRAP) in order to identify osteoclasts. RESULTS: Microtomographic images of the defects filled by the cement did not show any decrease in area over the course of postoperative evolution. In the Test Group, the material continued to present a foreign-body response until the last observational periods. Histomorphological analysis showed that there were more significant groupings of giant cells in the Test Group and greater maturity of neoformed bone in the Control Group. Exogenous material was also present. Histomorphometric analysis showed that in the Control Group, the total area of bone neoformation was significantly greater (p = 0.009) and grew progressively. The giant cells presented a positive reaction to TRAP but no osteoclasts were observed. CONCLUSION: The ceramic cement did not induce or lead to bone neoformation from the microtomographic or histological point of view.

    RESUMO OBJETIVO: O questionário Hip Function Recovery Score consiste em um instrumento desenvolvido para avaliação da recuperação funcional de pacientes acima de 60 anos submetidos a tratamento cirúrgico devido a fraturas do quadril. O objetivo deste estudo foi fazer a tradução transcultural de forma criteriosa do questionário e adaptá-lo às características individuais e culturais da população brasileira. MÉTODO: Este método consiste em tradução inicial, retrotradução, elaboração de uma versão de consenso e pré-teste comentado com posterior elaboração de uma versão final após as alterações necessárias sem modificar a semântica das perguntas do texto original. RESULTADOS: A versão de consenso foi aplicada em 30 pacientes, acima de 60 anos, que foram submetidos a tratamento cirúrgico devido a fraturas do quadril. Foram observadas, entretanto, algumas dificuldades no entendimento de algumas palavras e expressões, as quais foram substituídas por termos de mais fácil entendimento. Após a elaboração da versão final, reaplicou-se essa versão aos mesmos pacientes e obteve-se um entendimento total. CONCLUSÃO: A tradução transcultural do questionário terá impacto imediato na avaliação funcional dos pacientes com mais de 60 anos operados devido a fraturas do quadril e, posteriormente, outros trabalhos nacionais podem usar esse questionário padronizado e adaptado à nossa cultura para comparação de resultados e enriquecer a produção científica do Brasil.

    ABSTRACT OBJECTIVE: The Hip Function Recovery Score questionnaire is an instrument that was developed for evaluating the functional recovery of patients over 60 years of age who undergo surgical treatment due to hip fractures. The objective of this study was to make a thorough transcultural translation of this questionnaire, with adaptation to the individual and cultural traits of the Brazilian population. METHOD: This translation method consisted of initial translation, back translation, drafting of a consensual version and pretesting with comments invited. Subsequently, a final version was drawn up after making the necessary adjustments, without altering the semantics of the questions in the original text. RESULTS: The consensual version was applied to thirty patients over the age of 60 years who had undergone surgical treatment due to hip fractures. However, some difficulties in understanding some words and expressions were observed, and these were then replaced with terms that were more easily understood. After the final version had been drawn up, this was applied to the same patients and full understanding was achieved among some of them, without altering the semantics of the questions of the original text. CONCLUSION: The transcultural translation of the Hip Function Recovery Score will have an immediate impact on functional evaluations on patients over 60 years of age who underwent surgery due to hip fracture. It will subsequently be possible for other Brazilian scientific studies to use this questionnaire, which has been standardized and adapted to Brazilian culture, in order to make comparisons between results, thereby enriching Brazilian scientific production.
  • Rara variação anatômica do nervo musculocutâneo - Relato de caso Case Report

    RESUMO A importância clínica e cirúrgica do conhecimento anatômico do nervo musculocutâneo e de suas variações deve-se ao fato de que uma das complicações em diversos procedimentos cirúrgicos do membro superior envolve sua lesão. Em uma dissecação de rotina do membro superior direito de um cadáver masculino observamos uma variação anatômica desse nervo. O nervo musculocutâneo originou-se no fascículo lateral, seguiu lateralmente, passou sob o músculo coracobraquial e seguiu até seu primeiro ramo para o músculo bíceps braquial. Logo após forneceu mais dois ramos, o nervo cutâneo lateral do antebraço e um ramo para o músculo braquial, e então uniu-se ao nervo mediano. O nervo mediano seguiu medialmente no braço até a região da fossa cubital e deu então origem ao nervo intermédio anterior do antebraço. A união do nervo musculocutâneo com o nervo mediano aconteceu aproximadamente no ponto médio do braço e do nervo mediano. Tendo em vista que as classificações encontradas na literatura ou não abrangem nosso exemplo ou o mesmo se adequa a mais do que uma variação proposta, sem que tenhamos encontrado alguma fielmente semelhante, consideramos essa variação como rara.

    ABSTRACT The clinical and surgical importance of anatomical knowledge of the musculocutaneous nerve and its variations is due to the fact that one of the complications in many upper-limb surgical procedures involves injury to this nerve. During routine dissection of the right upper limb of a male cadaver, we observed an anatomical variation of this nerve. The musculocutaneous nerve originated in the lateral cord and continued laterally, passing under the coracobrachialis muscle and then continuing until its first branch to the biceps brachialis muscle. Just after this, it supplied another two branches, i.e. the lateral cutaneous nerve of the forearm and a branch to the brachialis muscle, and then it joined the median nerve. The median nerve followed the arm medially to the region of the cubital fossa and then gave rise to the anterior intermediate nerve of the forearm. The union between the musculocutaneous nerve and the median nerve occurred approximately at the midpoint of the arm and the median nerve. Given that either our example is not covered by the classifications found in the literature or that it fits into more than one variation proposed, without us finding something truly similar, we consider this variation to be rare.

    RESUMO A variante sólida do cisto ósseo aneurismático (COA) é considerada lesão rara, ocorre com maior frequência nos pacientes pediátricos e nos ossos da tíbia, fêmur, pelve e úmero. Apresentamos o caso de uma lesão lítica metafisária na extremidade distal do rádio de uma criança em que, ao exame radiográfico feito devido a um trauma de baixa energia, foi aventada a hipótese de fratura em um osso patológico secundária a um cisto ósseo aneurismático. Após a biópsia, a criança foi submetida a ressecção intralesional sem interposição de enxerto e o exame histopatológico foi condizente com a variante sólida do cisto ósseo aneurismático.

    ABSTRACT The solid variant of aneurismal bone cysts (ABC) is considered rare. It occurs with greater frequency in pediatric patients and in the tibia, femur, pelvis and humerus. We present a case of a metaphyseal lytic lesion on the distal extremity of the radius in a child whose radiograph was requested after low-energy trauma. The hypothesis of a pathological bone fracture secondary to an aneurysmal bone cyst was suggested. After biopsy, the child underwent intralesional excision without bone grafting and the histopathological findings were compatible with the solid variant of aneurysmal bone cyst.
  • Fratura de estresse no teto acetabular por motocross: relato de caso Case Report

    RESUMO Um dos primeiros passos para se reduzirem lesões, como a fratura de estresse no esporte, é conhecermos e nos aprofundarmos no estudo da natureza e extensão dessa patologia. A seguir apresentamos um relato de caso de fratura de estresse no teto acetabular por motocross. Caso considerado raro na literatura consultada. Descrição do quadro clínico: paciente de 27 anos; masculino, iniciou seguimento médico por dores incaracterísticas no quadril esquerdo, concentradas principalmente na região inguinal do quadril esquerdo durante a prática de motocross. Após investigação clínica e por exames complementares, diagnosticou-se fratura de estresse no teto acetabular.

    ABSTRACT One of the first steps to be taken in order to reduce sports injuries such as stress fractures is to have in-depth knowledge of the nature and extent of these pathological conditions. We present a case report of a stress fracture of the acetabular roof caused through motocross. This type of case is considered rare in the literature. The description of the clinical case is as follows. The patient was a 27-year-old male who started to have medical follow-up because of uncharacteristic pain in his left hip, which was concentrated mainly in the inguinal region of the left hip during motocross practice. After clinical investigation and complementary tests, he was diagnosed with a stress fracture of the acetabular roof.
  • Reconstrução de ruptura crônica do tendão patelar com enxerto patelar contralateral associado a reforço com tendões flexores. Relato de caso Case Report

    RESUMO A ruptura crônica do tendão patelar é lesão rara e incapacitante e ainda tecnicamente difícil de abordar. A verdadeira prevalência dessa lesão é desconhecida. A reconstrução tardia das rupturas crônicas do tendão patelar apresenta resultados clínicos e funcionais abaixo do ideal. Muitos métodos cirúrgicos diferentes foram relatados para a reconstrução do tendão patelar em lesões crônicas. Neste relato apresentamos um caso com lesão crônica do tendão patelar abordado com técnica até então não descrita na literatura, que combina, todavia, técnicas previamente relatadas. O método de reconstrução presente neste artigo tem a vantagem de ser fácil e reprodutível, sem a necessidade de aloenxertos.

    ABSTRACT Chronic patellar tendon rupture is a rare disabling injury that is technically difficult to repair. The true prevalence of this injury is unknown. Delayed reconstruction of chronic patellar tendon rupture has yielded suboptimal clinical and functional results. Many different surgical methods for reconstruction of chronic patellar tendon injury have been reported. In this report, we present a case with chronic patellar tendon injury that was addressed using a technique that had not previously been described in the literature, through combining procedures that had been described separately. The reconstruction method presented in this article has the advantages of being easy and reproducible, without a requirement of allografts.
