Dear Editor,

The recent report on “Achilles tendon xanthoma” is very interesting.11 Fernandes EA, Santos EH, Tucunduva TC, Ferrari AJ, Fernandes AD. Achilles tendon xanthoma imaging on ultrasound and magnetic resonance imaging. Rev Bras Reumatol. 2015;55(3):13-6. Fernandes et al. concluded that imaging studies were useful for diagnosis of Achilles tendon xanthoma.11 Fernandes EA, Santos EH, Tucunduva TC, Ferrari AJ, Fernandes AD. Achilles tendon xanthoma imaging on ultrasound and magnetic resonance imaging. Rev Bras Reumatol. 2015;55(3):13-6. In fact, the Achilles tendon xanthoma is rare. The differential diagnosis should include gouty nodule at Achilles tendon. An important concern is the cost-effectiveness of MRI imaging use. It should be specifically selected in case of underlying hyperlipidemia. Besides, it is reported that the diagnostic property of the MRI is not good. Dussault and Kaplan concluded that “xanthoma appearance in MRI is not pathognomonic”.22 Dussault RG, Kaplan PA, Roederer G. MR imaging of Achilles tendon in patients with familial hyperlipidemia: comparison with plain films, physical examination, and patients with traumatic tendon lesions. AJR Am J Roentgenol. 1995;164(2):403-7.
