    OBJETIVO: Determinar a frequência das glomerulonefrites nos pacientes espondiloartríticos acompanhados em Serviço de Reumatologia Brasileiro e avaliar variáveis clínicas correlacionadas. PACIENTES E MÉTODOS: Os pacientes foram avaliados quanto às características sociodemográfi cas, tipo de espondiloartrite, tempo e atividade da doença, uso de anti-infl amatórios não esteroides, presença do HLA-B27, níveis de creatinina e ureia séricas, presença de comorbidades e presença de hematúria e/ou proteinúria. Os pacientes com hematúria foram submetidos à pesquisa de dismorfi smo eritrocitário, e aqueles com proteinúria submeteram-se à quantifi cação da proteína na urina de 24 horas. Biópsia renal foi indicada para aqueles com hematúria de origem glomerular e/ou proteinúria maior que 3,5 g. RESULTADOS: Foram avaliados 76 pacientes. A alteração mais frequente no exame de urina de rotina foi a hematúria microscópica (44,7%), geralmente intermitente e em amostra isolada de urina durante o seguimento do paciente. Em oito (10,5%) dos pacientes a hematúria sugeriu origem glomerular. A biópsia renal foi realizada em cinco deles, e mostrou nefropatia por IgA em quatro (5,3%) e doença da membrana fi na em um paciente. CONCLUSÕES: Notou-se alta frequência de alterações no exame de urina desse subgrupo de pacientes, assim como alta prevalência de nefropatia por IgA. Apesar de mais estudos sobre o assunto serem necessários para melhor esclarecimento desses resultados, a realização periódica de exames de urina deveria ser recomendável.

    OBJECTIVE: To determine the frequency of glomerulonephritis in patients with spondyloarthritis followed-up at a Brazilian Rheumatology Service, and to evaluate the clinical variables associated. PATIENTS AND METHODS: Patients were assessed for sociodemographic characteristics, type of spondyloarthritis, time since diagnosis and disease activity, non-steroidal anti-infl ammatory drug use, HLA-B27 positivity, creatinine and urea serum levels, major comorbidities, hematuria and proteinuria. Patients with hematuria were subsequently assessed for the presence of dysmorphic red blood cells in urine, and those with proteinuria underwent 24-hour urine protein measurement. Renal biopsy was performed in patients with glomerular hematuria and/or proteinuria over 3.5 g/24-hour. RESULTS: Seventy-six patients were assessed. Microscopic hematuria was the most frequently found abnormality in urinalysis (44.7%), usually intermittent and in spot urine samples during patients' follow-up. In eight patients (10.5%), glomerular hematuria was suspected. Renal biopsy was performed in fi ve of them, showing IgA nephropathy in four (5.3%) and thin membrane disease in one patient. CONCLUSIONS: A high frequency of urinalysis alterations was observed in that subgroup of patients, as well as a high prevalence of IgA nephropathy. Although further studies on this subject are needed to better clarify these results, periodic urinalysis of patients with spondyloarthritis should be recommended.
  • Prevalência de manifestações clínico-laboratoriais e comorbidades na polimiosite segundo o gênero Artigo Original

    OBJETIVO: Analisar a distribuição e a influência do gênero na polimiosite (PM), quanto às manifestações clínico-laboratoriais, evolução e comorbidades. MÉTODOS: Estudo de coorte retrospectivo, unicêntrico, em que foram avaliados 75 pacientes consecutivos com PM (Bohan e Peter, 1975) entre 1990 e 2010. Os exames complementares referem-se ao início do diagnóstico da PM. RESULTADOS: Este estudo avaliou 52 mulheres e 23 homens (razão 2,3:1), a maioria de cor branca (84,0%), com média de idade de 42,7 ± 13,7 anos (16 a 67 anos), e duração média de doença de 6,9 ± 5,5 anos (0 a 20 anos). Aproximadamente 50% apresentaram recidiva da doença durante o acompanhamento, com 4,0% de óbitos. Apesar disso, dois terços encontravam-se em remissão no desfecho do estudo. Não houve diferença entre os gêneros quanto à distribuição das características demográficas, clínico-laboratoriais, evolução clínica e terapia medicamentosa instituída. Com relação às comorbidades, houve alta prevalência de hipertensão arterial sistêmica (38,7%) e diabetes mellitus (17,3%), igualmente distribuídas entre os gêneros. Verificou-se alta prevalência de depressão e fibromialgia, porém apenas no gênero feminino. CONCLUSÕES: A prevalência de PM entre mulheres foi maior (razão 2,3:1). A prevalência de comorbidades foi alta na casuística estudada, cabendo-nos priorizar seus controles e, assim, oferecer melhor qualidade de vida aos pacientes.

    OBJECTIVE: To assess gender distribution in polymyositis (PM) and its influence on disease, regarding clinical and laboratory manifestations, outcome and comorbidities. METHODS: Retrospective single-center cohort study assessing 75 consecutive patients with PM (Bohan and Peter, 1975) from 1990 to 2010. Complementary tests were related to early diagnosis of PM. RESULTS: The study assessed 52 women and 23 men (ratio 2.3:1), most of whom white (84.0%), with a mean age of 42.7 ± 13.7 years (16 to 67 years), and mean disease duration of 6.9 ± 5.5 years (0 to 20 years). Approximately 50% experienced disease relapse during follow-up. Nevertheless, two thirds were in remission at the end of this study, with 4.0% of deaths. There was no difference between genders regarding demographic, clinical and laboratory characteristics, clinical outcome and the drug therapy instituted. Regarding comorbidities, there was a high prevalence of hypertension (38.7%) and diabetes mellitus (17.3%), equally distributed between genders. There was also a high prevalence of depression and fibromyalgia, which were only observed among females. CONCLUSIONS: The prevalence of PM was higher among women than among men (2.3:1). Because the prevalence of comorbidities was high in the case series studied, it is worth emphasizing the need for their control to provide better quality of life for patients with PM.
  • Efeito imediato da utilização da joelheira elástica em indivíduos com osteoartrite Artigo Original

    INTRODUÇÃO: A osteoartrite (OA) de joelho normalmente ocasiona dificuldades na realização de diversas atividades rotineiras, sendo um dos principais motivos de procura por serviços médicos e fisioterapêuticos. Existem diversas modalidades de tratamento, com resultados variados. A utilização da joelheira como recurso adjunto tem-se mostrado controversa na literatura. OBJETIVO: Analisar a eficácia imediata da joelheira elástica na dor e na capacidade funcional em indivíduos com OA de joelho. MÉTODOS: Foram analisados 74 sujeitos sintomáticos (132 joelhos) com OA de joelho por meio dos testes Stair Climb Power Test (SCPT), Timed Up and Go (TUG) e Caminhada de 8 Metros (C8M), além da escala visual analógica (EVA) para dor. Os testes foram realizados com e sem joelheira; a ordem e a presença ou ausência das joelheiras durante os testes foram randomizadas e com avaliador cego. RESULTADOS: Foi encontrada diferença estatisticamente significante entre as duas situações comparadas (com e sem joelheira) para EVA (P < 0,001), mostrando redução da dor com a joelheira. Análises com os três testes funcionais em ambas as condições foram realizadas, resultando diferenças estatisticamente significantes para os testes C8M e TUG (P < 0,05), mas não no SCPT (P > 0,1339). CONCLUSÃO: A joelheira elástica foi eficiente na melhora imediata da capacidade funcional e da dor em indivíduos com OA de joelho, pois melhorou o desempenho durante os testes propostos. Sendo assim, entende-se que se trata de um recurso coadjuvante para o tratamento da OAJ por ser prático, útil, de fácil emprego clínico e que pode auxiliar e/ou facilitar a realização de exercícios terapêuticos.

    BACKGROUND: Knee osteoarthritis (KOA) is one of the major reasons for seeking medical and physical therapy services, because it usually causes difficulties in performing daily life activities. There are several types of treatment, with varied results. The use of knee sleeve as an adjuvant resource has been controversial in the literature. OBJECTIVE: To assess the immediate efficacy of elastic knee sleeve on pain and functional capacity of individuals with KOA. METHODS: Seventy-four patients (132 knees) with symptomatic KOA were assessed by use of the Stair Climb Power Test (SCPT), Timed Up and Go (TUG) and 8-Meter Walk (8MW) tests, in addition to the VAS for pain. The tests were performed with and without knee sleeves, with a cover on the knees to hide knee sleeve. The order and the presence of the knee sleeve were randomized, and the investigator was blind. RESULTS: A statistically significant difference was found between the two compared circumstances (with and without knee sleeve) when using the VAS (P < 0.001), which showed a reduction in pain with the knee sleeve use. Analyses of the three functional tests under both circumstances were performed, resulting in statistically significant differences in 8MW and TUG tests (P < 0.05), but not in SCPT (P > 0.1339). CONCLUSION: The elastic knee sleeve proved to be effective to immediately improve the functional capacity and pain of individuals with KOA, because it enhanced performance during the tests proposed. Thus, the knee sleeve is an adjuvant resource for treating KOA, because it is practical, useful, and of easy clinical use, and can aid in the practice of therapeutic exercises.
  • Correlação de fadiga com dor e incapacidade na artrite reumatoide e na osteoartrite, respectivamente Artigo Original

    OBJETIVO: Investigar a correlação de fadiga com dor em pacientes com artrite reumatoide e de fadiga com incapacidade em pacientes com osteoartrite. MÉTODOS: Foram avaliados 20 pacientes com artrite reumatoide e 20 com osteoartrite. Fadiga foi avaliada com escala visual analógica e questionário de avaliação multidimensional de fadiga. Utilizou-se a escala visual analógica para avaliar dor e para a avaliação global da doença pelo paciente. Incapacidade foi avaliada pelo questionário de avaliação de saúde. Idade, gênero, duração da doença, escolaridade, renda mensal, uso de drogas antirreumáticas e comorbidades foram obtidos. A análise estatística incluiu teste exato de Fisher, Shapiro-Wilk, Kruskal-Wallis e Spearman. O nível de significância foi de 0,05. RESULTADOS: A fadiga mostrou-se significativamente aumentada na osteoartrite em comparação à artrite reumatoide quando se utilizou o questionário de avaliação multidimensional de fadiga (P < 0,05). Dor correlacionou-se com fadiga ao se utilizar a escala visual analógica ou o questionário de avaliação multidimensional de fadiga em artrite reumatoide (r = 0,46; P < 0,05). O questionário de avaliação de saúde foi associado à fadiga por escala visual analógica em osteoartrite (r = 0,54; P < 0,05). Avaliação global da doença pelo paciente correlacionou-se com fadiga pela escala visual analógica (r = 0,44; P < 0,003). Todas as pacientes eram mulheres, predominantemente idosas, com médias de idade semelhantes, longa duração de doença e baixa renda. CONCLUSÃO: Nossos resultados confirmam que em pacientes com artrite reumatoide a fadiga correlaciona-se com dor, enquanto em pacientes com osteoartrite ela associa-se à incapacidade. Portanto, fadiga tem diferentes correlatos em osteoartrite e artrite reumatoide. Sugerimos que incapacidade, e não dor, seja o correlato da fadiga em pacientes com osteoartrite.

    OBJECTIVE: To investigate the correlation of fatigue with pain in rheumatoid arthritis patients and with disability in osteoarthritis patients. METHODS: Twenty patients with rheumatoid arthritis and 20 patients with osteoarthritis were evaluated. The degree of fatigue was evaluated with a visual analogue scale and the Multidimensional Assessment of Fatigue. Pain was evaluated with a visual analogue scale as well as Patient Global Assessment. For disability evaluation, the Health Assessment Questionnaire was performed. Age, gender, disease duration, education, income, antirheumatic drugs used and comorbidity were also obtained. Statistical analysis included Fisher exact, Shapiro-Wilk, Kruskal-Wallis and Spearman tests. The significance level was 0.05. RESULTS: Fatigue was more significantly increased in patients with osteoarthritis than in patients with rheumatoid arthritis when evaluated with Multidimensional Assessment of Fatigue (P < 0.05). Pain was found to correlate with fatigue evaluated with visual analogue scale or Multidimensional Assessment of Fatigue in patients with rheumatoid arthritis (r = 0.46; P < 0.05). Health Assessment Questionnaire was associated with fatigue visual analogue scale in patients with osteoarthritis (r = 0.54; P < 0.05). Patient Global Assessment correlates with fatigue visual analogue scale (r = 0.44; P < 0.003). Patients were similar in both groups: all females, similar mean age, with long disease duration and low income. CONCLUSION: Our results corroborate that fatigue in rheumatoid arthritis patients correlates with the degree of pain, while in osteoarthritis patients it is associated with disability. Therefore, we found that fatigue has different correlates in osteoarthritis and rheumatoid arthritis, and we suggest that disability, not pain, is a correlate of fatigue in osteoarthritis patients.
  • Prolactina, estradiol e anticorpos anticardiolipina em amostra de mulheres pré-menopáusicas com lúpus eritematoso sistêmico: estudo-piloto Artigo Original

    INTRODUÇÃO: O lúpus eritematoso sistêmico (LES) é uma doença autoimune com maior prevalência em mulheres. A maior incidência ocorre durante os anos reprodutivos, sugerindo que o estradiol tenha influência na apresentação clínica do LES. Anticorpos anticardiolipina (ac-ACL) estão relacionados com a síndrome do anticorpo antifosfolipídeo (SAF), mas podem estar presentes em pacientes com LES sem SAF, sendo relacionados com risco cardiovascular e nefrite. OBJETIVO: Determinar se a presença de ac-ACL está associada a alterações hormonais em uma amostra de mulheres com LES. MÉTODOS: Foram avaliadas 47 mulheres com LES de acordo com os critérios do American College of Rheumatology, com idade média de 30,8 ± 8,12 anos. Nenhuma fazia uso de anticoncepcional hormonal, e a atividade do LES foi estimada pelo índice de atividade da doença (SLEDAI). As pacientes foram estratificadas de acordo com a presença ou não de ac-ACL, e os níveis séricos de estradiol e prolactina foram determinados. RESULTADOS: Nove (19,1%) das 47 pacientes tiveram ac-ACL positivos. Idade, tempo de doença e o SLEDAI foram similares entre os grupos. No entanto, a mediana do estradiol foi menor no grupo com ac-ACL positivo [46,8 (21,0-72,1) pg/mL] com relação ao grupo com ac-ACL negativo [122,3 (64,8-172,7) pg/mL, P = 0,004]. CONCLUSÃO: Estes resultados sugerem, pela primeira vez, uma associação inversa entre ac-ACL e níveis de estradiol em pacientes pré-menopáusicas com LES. Considerando que tanto níveis reduzidos de estradiol endógeno quanto presença de ac-ACL estão associados a aterosclerose, este achado pode ser clinicamente relevante em predizer risco cardiovascular e/ou desenvolvimento de SAF no LES.

    INTRODUCTION: Systemic lupus erythematosus (SLE) is an autoimmune disease, with higher prevalence in women. An incidence peak occurs during the reproductive years, suggesting that estradiol may play a role in the clinical presentation of SLE. Anticardiolipin antibodies (ACA) are associated with antiphospholipid antibody syndrome (APLS), but can be found in patients with SLE without APLS, and relate to cardiovascular risk and nephrite. OBJECTIVE: This study aimed at assessing whether the presence of ACA is associated with hormonal changes in a sample of women with SLE. METHODS: Forty-seven women diagnosed with SLE according to the American College of Rheumatology criteria, aged 30.8 ± 8.12 years, were evaluated. None was on hormonal contraception, and their SLE activity was estimated using the SLE Disease Activity Index (SLEDAI). Patients were stratified, according to the presence or absence of ACA, and estradiol and prolactin levels were measured. RESULTS: Nine (19.1%) of 47 patients were positive for ACA. No differences were found between groups concerning age, duration of disease, and SLEDAI. In contrast, the median estradiol level was lower in the ACA-positive group [46.8 (21.0-72.1) pg/mL] than in the ACA-negative group [122.3 (64.8-172.7) pg/mL, P = 0.004]. CONCLUSION: These results suggest, for the first time, an inverse association between ACA and estradiol levels in premenopausal SLE patients. Considering that both lower endogenous estradiol levels and ACA positivity are related to atherosclerosis, our finding may be clinically relevant in predicting cardiovascular risk and/or APLS development in SLE.
  • Estudo da frequência dos alelos de HLA-DRB1 em pacientes brasileiros com artrite reumatoide Artigo Original

    Os alelos HLA-DRB1, que codificam uma sequência de aminoácidos (QKRAA/QRRAA/RRRAA) nas posições 70 a 74 da terceira região hipervariável da cadeia β1 do gene DRB1, denominada epítopo compartilhado (EC), estão associados a maior suscetibilidade e gravidade para artrite reumatoide (AR) em diversas populações. OBJETIVO: Determinar a frequência dos alelos HLA-DRB1 em pacientes brasileiros com AR, e sua associação a fator reumatoide (FR) e anticorpos antipeptídeos citrulinados (ACPA). MATERIAL E MÉTODOS: Foram incluídos 412 pacientes com AR e 215 controles. A tipificação HLA-DRB1 foi realizada pela reação em cadeia da polimerase (PCR) usando primers específicos e hibridização com oligonucleotídeos de sequência específica (SSOP). A pesquisa de ACPA foi determinada pela técnica de ELISA, e a do FR por nefelometria. Para análises estatísticas foram utilizados os testes do qui-quadrado e t de Student e a regressão logística. RESULTADOS: Alelos HLA-DRB1*04:01, *04:04 e *04:05 associaram-se à AR (P < 0,05)); a despeito do amplo intervalo de confiança, vale a pena ressaltar a associação observada entre o alelo DRB1*09:01 e a doença (P < 0,05). Alelos HLA-DRB1 EC+ foram observados em 62,8% dos pacientes e em 31,1% do grupo-controle (OR 3,62; P < 0,001) e estiveram associados a ACPA (OR 2,03; P < 0,001). Alelos DRB1-DERAA mostraram efeito protetor para AR (OR 0,42; P < 0,001). CONCLUSÃO: Em uma amostra de pacientes brasileiros com AR de etnia majoritariamente mestiça, alelos HLA-DRB1 EC+ estiveram associados à suscetibilidade à doença e à presença de ACPA.

    The HLA-DRB1 alleles encoding an amino acid sequence (QKRAA/QRRAA/RRRAA) at position 70 74 of the third hypervariable region of the β1 chain of the HLA-DRB1 gene, called shared epitope (SE), are associated with increased susceptibility to and severity of rheumatoid arthritis (RA) in different populations. OBJECTIVE: To determine the frequency of HLA-DRB1 alleles in Brazilian patients with RA and their association with rheumatoid factor (RF) and anti-citrullinated peptide antibodies (ACPA). METHODS: Four hundred and twelve patients with RA (ACR 1987) and 215 controls were included. HLA-DRB1 typing was performed by use of polymerase chain reaction (PCR) with specific primers and hybridization with sequence-specific oligonucleotide probe (SSOP). ACPA was measured by use of the ELISA technique and RF by nephelometry. The statistical analysis comprised the chi-square and Student t tests and logistic regression. RESULTS: HLA-DRB1*04:01, *04:04, *04:05 alleles were associated with RA (P < 0.05); despite the wide confidence interval, it is worth noting the association between the DRB1*09:01 allele and RA (P < 0.05). HLA-DRB1 SE+ alleles were observed in 62.8% of the patients and in 31.1% of controls (OR 3.62; P < 0.001) and were associated with ACPA (OR 2.03; P < 0.001). DRB1-DERAA alleles showed a protective effect against RA (OR 0.42; P < 0.001). CONCLUSION: In a sample of Brazilian patients with RA, most of whom of mixed heritage, HLA-DRB1 SE+ alleles were associated with susceptibility to disease and presence of ACPA.
  • Efeitos terapêuticos do treinamento físico em pacientes com doenças reumatológicas pediátricas Artigo De Revisão

    Ao longo das últimas décadas, o papel do exercício em doenças reumatológicas tem sido bastante explorado. Sabe-se, atualmente, que a prática de atividade física promove inúmeros benefícios ao paciente com osteoporose, osteoartrite, lúpus eritematoso sistêmico, esclerose sistêmica, miopatias idiopáticas inflamatórias, fibromialgia e artrite reumatoide. Dessa forma, o exercício físico tem sido considerado ferramenta valiosa no tratamento do paciente reumático. Os efeitos terapêuticos do treinamento físico em doenças reumatológicas pediátricas também têm sido alvos recentes de investigação. Em conjunto, os estudos têm revelado grande potencial terapêutico do exercício para pacientes com artrite idiopática juvenil, lúpus eritematoso sistêmico juvenil, dermatomiosite juvenil, fibromialgia juvenil e outras causas de dor crônica. Esta revisão narrativa tem como objetivos familiarizar o reumatologista pediátrico ao campo da ciência do exercício, discutir os potenciais benefícios do exercício físico na reumatologia pediátrica, com ênfase nas perspectivas desse promissor campo de atuação clínica e científica, e apresentar modelos práticos de exame de pré-participação e contraindicações ao exercício físico.

    Over the past decades, the role of exercise training in rheumatic diseases has been largely explored. Currently, physical activity is well known to benefit patients with osteoporosis, osteoarthritis, systemic lupus erythematosus, systemic sclerosis, idiopathic inflammatory myopathy, fibromyalgia and rheumatoid arthritis. Therefore, exercise training has been considered a valuable tool for treating rheumatic patients. The therapeutic effects of exercise training have also been investigated in pediatric rheumatic diseases. Collectively, studies have revealed the therapeutic potential of exercise in juvenile idiopathic arthritis, juvenile systemic lupus erythematosus, juvenile dermatomyositis, juvenile fibromyalgia and other causes of chronic pain. The aim of this review is to familiarize the pediatric rheumatologist with the exercise science field; discuss the potential benefits of exercise training in pediatric rheumatic diseases, emphasize both research and clinical perspectives of this promising field; and propose practical models of pre-participation examinations and contraindications to exercise.
  • Expressão de proteínas reguladoras do complemento CD55, CD59, CD35 e CD46 na artrite reumatoide Artigo De Revisão

    A artrite reumatoide (AR) é uma doença autoimune, associada à sinovite poliarticular inflamatória, que acomete principalmente as articulações periféricas. Cerca de 1% da população mundial é afetada, sendo duas a três vezes mais prevalente em mulheres. Apresenta patogênese complexa e multifatorial. A sinóvia das articulações afetadas é infiltrada por linfócitos T e B, macrófagos e granulócitos. A sinóvia reumatoide adquire características proliferativas, formando o pannus, e invade a cartilagem articular e o osso, levando à destruição da arquitetura normal da articulação e à perda de função. A diminuição da expressão de proteínas reguladoras do complemento (PCR) parece desempenhar papel importante na atividade da AR, associada ao agravamento dos sintomas clínicos. A superativação do sistema complemento (SC) é a causa da exacerbação da doença em vários modelos de doenças autoimunes. O presente artigo tem por objetivo revisar os principais aspectos relacionados à regulação do SC na AR, a fim de propiciar melhor compreensão do potencial papel desse sistema na fisiopatologia e na atividade da doença.

    Rheumatoid arthritis (RA) is an autoimmune disease associated with polyarticular inflammatory synovitis affecting mainly peripheral joints. It affects approximately 1% of the world population, being two to three times more prevalent in women. Rheumatoid arthritis has a complex and multifactorial pathogenesis. The synovium of the affected joints is infiltrated by T and B lymphocytes, macrophages, and granulocytes. The rheumatoid synovium has proliferative characteristics, forming the pannus, which invades cartilage and bone, leading to normal architecture destruction and function loss. The decreased expression of complement regulatory proteins (CRP) seems to play an important role in RA activity, and is associated with worsening of the clinical symptoms. In several models of autoimmune diseases, the overactivation of the complement system (CS) is the cause of disease exacerbation. This article aimed at reviewing the main aspects related to CS regulation in RA in order to provide a better understanding of the potential role of this system in the pathophysiology and activity of the disease.
  • Flegmasia cerúlea dolens em paciente com lúpus eritematoso sistêmico no puerpério remoto Relato De Caso

    As manifestações vasculares no lúpus eritematoso sistêmico não são raras, e grande parte dos casos está associada à presença de anticorpos antifosfolípides. A flegmasia cerúlea dolens é uma incomum e grave complicação de trombose venosa profunda de membros inferiores, com altas taxas de morbimortalidade. Encontramos na literatura apenas dois casos de flegmasia cerúlea dolens associados à síndrome antifosfolípide, e não encontramos relatos associados ao lúpus eritematoso sistêmico. Relatamos um caso de flegmasia cerúlea dolens, com rápida evolução para óbito, em paciente com lúpus eritematoso sistêmico no puerpério remoto.

    Vascular manifestations are not rare in systemic lupus erythematosus and, in most cases, are associated with antiphospholipid antibodies. Phlegmasia cerulea dolens is an unusual and severe complication of deep venous thrombosis of the lower limbs, which has a high mortality rate. In the literature, only two cases of phlegmasia cerulea dolens associated with primary antiphospholipid syndrome have been reported, but none associated with systemic lupus erythematosus. We report one case of phlegmasia cerulea dolens with rapid evolution to death in a patient with systemic lupus erythematosus in the remote postpartum period.
  • Artrite séptica por Streptococcus bovis em paciente com cirrose hepática devido ao vírus da hepatite C: relato de caso e revisão de literatura Relato De Caso

    A monoartrite continua sendo diagnóstico desafiador para reumatologistas e ortopedistas. O autor descreve um caso de artrite séptica por Streptococcus bovis após vários episódios de derrame articular tratado com ácido hialurônico (hilano G-F 20) e acetato de metilprednisolona em paciente de 69 anos de idade portador de cirrose hepática devido ao vírus da hepatite C, sem presença de adenoma de cólon ou endocardite. Discutem-se as possibilidades diagnósticas para este caso de monoartrite, bem como as patologias associadas às infecções causadas por esse microrganismo de interesse para o reumatologista, e a possível implicação da reação da articulação ao ácido hialurônico como possível fator predisponente do derrame articular.

    Monoarthritis remains a diagnostic challenge in Rheumatology and Orthopedics. The author reports a case of septic arthritis due to Streptococcus bovis after several episodes of joint effusion treated with hyaluronic acid (Hylan G-F 20) and methylprednisolone acetate in a 69-year-old patient with liver cirrhosis due to hepatitis C virus. Neither adenoma of the colon nor endocarditis was present. The diagnostic possibilities for this case of monoarthritis, the pathologies related to the microorganism of interest to the rheumatologist, and the possible involvement of joint reaction to hyaluronic acid as a predisposing factor to joint effusion are discussed.

    A arterite de Takayasu (AT) é uma vasculite sistêmica que acomete principalmente a aorta e seus ramos. Apesar de ser a terceira vasculite mais frequente na infância, sua ocorrência na faixa etária pediátrica é pouco descrita. Relatamos três casos de AT na faixa etária pediátrica com ênfase nas manifestações clínicas, alterações angiográficas e abordagem terapêutica.

    Takayasu's arteritis (TA) is a systemic vasculitis that affects mainly the aorta and its major branches. Despite being the third most frequent vasculitis in childhood, the occurrence of TA in the pediatric age group is scarce. We report three cases of TA in children, emphasizing signs and symptoms, angiographic alterations and therapeutics.
