Open-access The ABCDEFG of Eclampsia: Advanced Life Support in Eclampsia (ALSE)

Abstract

Eclampsia remains a time-dependent obstetric emergency, with a high risk of death and sequelae when there are delays, omissions, or hasty decisions. This article operationalizes the ABCDEFG mnemonic [airway and oxygenation; ventilation; circulation and fluid management; prevention of neurological damage with magnesium sulfate (MgSO₄); immediate recognition and treatment of severe hypertension and targeted tests; fetal assessment; and obstetric decision] as an integrated protocol, aligned with the grammar of emergency care and the specificities of the pregnant woman. The structure prioritizes A-E, only then advancing to F and G. Objective criteria for advanced airway management, operational targets for peripheral oxygen saturation (SpO₂), conduct and therapeutic targets for blood pressure levels, preparation/monitoring of MgSO₄, minimum laboratory panel, and neuroimaging indications are described. The obstetric decision regarding the preferred route of delivery is vaginal, with cesarean section reserved for specific indications. The manuscript also proposes elements for institutional implementation: closed-loop communication, brief simulation, critical time auditing, and indicators (magnesium port, blood pressure control port, and cesarean section for transient tracing). By translating evidence into feasible routines, ABCDEFG seeks to reduce variability in care, shorten decision times, and favor obstetric choices proportional to risk.
