This article has the objective of analyzing variants of care for patients with drug-related difficulties and discussing parallels between this area and the Brazilian psychiatric reform. We first present a brief overview of the advances of the psychiatric reform in terms of care in mental health, and raise questions regarding the implementation of this type of care when treating drug abusers. We point out the dangers arising from the current direction the field has taken, involving the contradictory paradigms present in advocating long-term hospitalization.
