EQUAÇÕES PREDITIVAS DO CONSUMO MÁXIMO DE OXIGÊNIO POR MEIO DO TESTE SHUTTLE RUN EM CRIANÇAS E ADOLESCENTES: UMA REVISÃO SISTEMÁTICA

RESUMO

Objetivo:

Revisar sistematicamente na literatura o nível de evidência das equações preditivas do pico de consumo de oxigênio (VO2pico) por meio do teste de shuttle run de 20 metros (SR-20m) em crianças e adolescentes.

Fonte de dados:

As buscas foram conduzidas nas bases de dados eletrônicas Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed, ScienceDirect, Web of Science, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Electronic Library Online (SciELO), de agosto a setembro de 2017, nos idiomas inglês e português. Os critérios de inclusão utilizados foram: estudos originais, com resumo disponível, com equações para predição do VO2pico por meio do SR-20m, adolescentes e/ou crianças, não atletas e com análise correlacional do VO2pico predito e mensurado. O nível de evidência das equações foi caracterizado com base no risco de viés dos estudos, no qual se adotou os seguintes critérios: número da amostra, características da amostra e análise estatística.

Síntese dos dados:

Dezoito estudos foram selecionados, nos quais 12 equações foram encontradas e analisadas. Os estudos foram conduzidos com amostras de ambos os sexos com idades de oito a 19 anos. As equações de Léger e Matsuzaka foram classificadas com forte nível de evidência, com variação de amplitude de estimativa entre r=0,54-0,90 e r=0,65-0,90. Enquanto as equações Ruiz, Barnett e Matsuzaka foram consideradas de evidência moderada, com variação de amplitude de estimativa entre r=0,75-0,96, r=0,66-0,84 e r=0,66-0,89, respectivamente.

Conclusões:

A equação de Matsuzaka apresentou parâmetros satisfatórios para estimar o VO2pico em crianças e adolescentes. Embora não explorados em equações, a adiposidade corporal e o estágio puberal demonstram associações relevantes com a aptidão cardiorrespiratória em crianças e adolescentes.

ABSTRACT

Objective:

To systematically review the literature as for the level of evidence of predictive equations of VO2peak through the 20-meter shuttle run test (20m-SRT) in children and adolescents.

Data sources:

Searches were conducted independently by two researchers, according to the procedures adopted by PRISMA, in the electronic databases MEDLINE via PubMed, ScienceDirect, Web of Science, LILACS and SciELO, for articles published until September 2017 in English and Portuguese. The inclusion criteria were: original studies, abstract available, using predictive equations of VO2peak through 20m-SRT, conducted with adolescents and/or children, non-athletes, and mentioning correlation analysis between predicted and measured VO2peak. The level of evidence of equations was based on the risk of bias of the studies using the following criteria: sample number, sample characteristics, and statistical analysis.

Data synthesis:

Eighteen studies were selected, in which fifteen equations were found and analyzed. The studies had been conducted with samples composed of subjects of both sexes, aged 8 to 19 years. Equations of Léger and Matsuzaka had their level of evidence classified as high, and estimation ranged between r=0.54-0.90 and r=0.65-0.90. Equations by Ruiz, Barnett and Matsuzaka had their level of evidence classified as moderate, and estimation ranged between r=0.75-0.96, r=0.66-0.84 and r=0.66-0.89, respectively.

Conclusions:

Matsuzaka’s equation presented satisfactory parameters for estimates of VO2peak in children and adolescents. Although not explored in equations, body adiposity and pubertal stage are significantly associated with cardiorespiratory fitness in children and adolescents.

INTRODUÇÃO

A aptidão cardiorrespiratória (APCR) é importante marcador de saúde em crianças e adolescentes11. Ruiz JR, Huybrechts I, Cuenca-García M, Artero EG, Labayen I, Meirhaeghe A, et al. O consumo de oxigênio (VO2) é considerado o principal índice para determinação da APCR.99. Arena R, Cahalin LP. Em crianças e adolescentes, geralmente é utilizado o pico de consumo de oxigênio (VO2pico), definido como o pico de VO2 atingido no final do período de esforço máximo.99. Arena R, Cahalin LP. O VO2pico pode ser mensurado por métodos diretos com a utilização de análises ergoespirométricas em testes máximos conduzidos em laboratório em diferentes ergômetros ou em campo com a simulação da atividade desportiva. 1010. A partir dos testes diretos, autores propõem equações que permitem a análise do VO2pico por métodos denominados como indiretos, que podem ser executados em testes máximos ou submáximos, aumentando a praticidade e reduzindo o custo das avaliações.1111. Em estudos epidemiológicos, os testes indiretos de campo são os mais indicados, pois geralmente demandam baixo custo, menor tempo de execução e facilidade de aplicação simultânea em maior número de indivíduos. 1212. O SR-20m é considerado método simples, necessita de poucos equipamentos, pode ser executado em ambientes com pouco espaço e, ainda, possibilita a avaliação de vários indivíduos ao mesmo tempo, o que pode aumentar a motivação dos participantes.22. Nas últimas décadas, o SR-20m foi incluído em várias baterias de testes de aptidão física como EUROFIT e FITNESSGRAM,66. Tomkinson GR, Lang JJ, Tremblay MS, Dale M, LeBlanc AG, Belanger K, et al. Para isso, equações foram desenvolvidas com a utilização de modelos matemáticos de regressão ou redes neurais artificiais, incluindo características biológicas como idade, sexo, massa corporal e informações do desempenho alcançado no teste.1717. Por outro lado, a predição do VO2pico por meio de equações pode apresentar variação de medidas ao depender das características da amostra, principalmente a faixa etária, o estágio de maturação sexual, o sexo e a composição corporal.1616. Nesse sentido, para que uma equação seja considerada apropriada, deve apresentar adequada validade, ou seja, produzir pouca amplitude de variação entre os valores de estimativa.1919. Rev Paul Pediatr. 2017;35:222-33. apontam a relevância da análise do nível de evidência de equações desenvolvidas para estimar o VO2pico em crianças e adolescentes, que contribuiu para esta revisão de forma mais criteriosa e ordenada.

Portanto, não está clara qual equação estabelece melhor precisão de estimativa com base nas diferentes características da população infantojuvenil, bem como quais variáveis são importantes na predição do VO2pico, pois até o momento não foram analisados de forma sistemática os resultados dos diferentes estudos. Sendo assim, o objetivo deste estudo foi revisar sistematicamente a literatura para analisar o nível de evidência das equações preditivas do VO2pico por meio do SR-20m em crianças e adolescentes.

MÉTODO

Este trabalho foi conduzido com base nas recomendações da Preferred Reporting Items for Systematic Review and Meta-analyses: the PRISMA statement,2020. Moher D, Liberati A, Tetzlaff J, Altman DG, PRISMA Group. Preferred reporting items for systematic reviews and meta-analyses: the PRISMA statement. PLoS Med. 2009;6:e1000097. no período de agosto a setembro de 2017.

Foram selecionadas cinco bases de dados eletrônicas de acordo com a área de conhecimento e relevância científica mundial: Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed, ScienceDirect, Web of Science, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Eletronic Library Online (SciELO). Além disso, buscas foram realizadas nas listas de referências dos artigos selecionados e relacionados ao tema.

As estratégias de pesquisa foram definidas após a identificação e a seleção dos descritores de busca, com base nos termos DECS (descritores em ciências da saúde da BIREME) e MESH (Medical Subject Headings - vocabulário controlado utilizado para indexação de artigos para PubMed). Desse modo, optou-se pelas seguintes palavras-chave em língua inglesa e portuguesa: Cardiopulmonary Exercise Test, Oxygenconsumption, Children e Adolescents. As palavras-chave foram combinadas por meio dos termos booleanos “AND” e/ou “OR” e o período da publicação dos estudos foi fixado até o mês de setembro de 2017.

As etapas do processo de busca, seleção, análise dos artigos, aplicação dos parâmetros de risco de viés e extração de dados foram realizadas de forma independente por dois pesquisadores - (F.J.M.J.) e (I.C.J.) - , e nos casos de discordância, um terceiro pesquisador (N.L.) foi convidado a decidir sobre os pontos divergentes.

Após a aplicação dos descritores selecionados, os estudos identificados com duplicidades foram descartados e realizou-se a aplicação dos critérios de inclusão (1, 2 e 3) e não inclusão (4, 5 e 6) aos estudos rastreados, diante da leitura do título e do resumo:

  • Estudos originais com desenho transversal, longitudinal ou de ensaio clínico.

  • Acesso ao resumo nas bases indexadoras pesquisadas.

  • Estudos com equações para VO2pico por meio do teste SR-20m.

  • Amostras com adultos e/ou idosos.

  • Estudos em atletas.

  • Ausência de análise de correlação entre VO2pico predito por método indireto e VO2pico mensurado por método direto.

Após essa fase, foram realizadas a leitura e a análise dos artigos completos classificados como elegíveis; os estudos foram excluídos por várias razões: amostra com patologia diagnosticada, amostra de adultos e adolescentes analisados juntos, protocolo shuttle run adaptado, estudos sem mensuração direta do VO2pico, sem análise de correlação e/ou sem predição do VO2pico.

Os critérios de avaliação do risco de viés aplicados aos estudos selecionados foram adaptados por Batista et al.,1919. Batista MB, Romanzini CL, Castro-Piñero J, Ronque ER. Rev Paul Pediatr. 2017;35:222-33. em que três parâmetros foram observados: número de participantes, descrição da amostra e análise estatística. A fim de mensuração, a cada parâmetro foi atribuído uma pontuação de zero a dois pontos.

Nesse sentido, para o número de participantes, os estudos foram classificados como “0”, quando apresentava menos de 10 participantes; “1”, entre 11 e 50 participantes; ou “2”, acima de 51 participantes. Para o parâmetro da descrição da amostra foram consideradas as características de idade, sexo, estado de saúde, nível de aptidão física, estado púbere, composição corporal, nível de atividade física e etnia. Com base nesses aspectos, os estudos foram classificados com “0”, quando descrito menos de quatro características; “1”, com descrição de quatro características; ou “2”, com descrição maior que quatro. Com relação à análise estatística, os estudos foram classificados com “0”, sem análise de regressão ou medidas de erro; “1”, quando aplicado análise de regressão e/ou medidas de erro; e “2”, presença de mais de três análises estatísticas ou gráfico Bland-Altman plot e/ou análise de variância (ANOVA) de medidas repetidas. Os estudos foram categorizados de acordo com a pontuação recebida: alto risco de viés (0-2 pontos), moderado risco de viés (3 e 4 pontos) e baixo risco de viés (5 e 6 pontos).

Posteriormente, uma análise da validade das equações identificadas foi conduzida com base nos critérios de nível de evidência expressos por Castro-Piñero et al.:2121.   • Evidência forte: equações validadas por três ou mais estudos com baixo risco de viés.

  • Evidência moderada: equações validadas por dois estudos com baixo risco de viés.

  • Evidência limitada: equações validadas por estudos com alto risco de viés, resultados inconsistentes de diversos estudos, independentemente do risco de viés, ou os resultados de um único estudo.

Nos artigos selecionados, foram destacadas características das amostras, como idade, sexo e quantidade dos sujeitos. Foram extraídos os valores de coeficiente de correlação (r) e erro padrão de estimativa (EPE), em mL/kg.min, quando disponível. A variação da amplitude de estimativa (∆AE) de cada equação foi determinada por meio da descrição do menor e do maior valor do coeficiente de correlação obtido pela equação entre os estudos. Para facilitar a identificação, optou-se pela nomeação das equações com o nome do primeiro autor do respectivo estudo em que foi validada. Além disso, quando identificado que um autor desenvolveu duas ou mais equações em um único estudo, foi aderida uma letra a cada equação - (a), (b) ou (c).

As etapas do processo de busca, seleção, análise dos artigos, aplicação dos parâmetros de risco de viés e extração de dados foram realizadas de forma independente por dois pesquisadores - (F.J.M.J.) e (I.C.J.) - , e nos casos de discordância, um terceiro pesquisador (N.L.) foi convidado a decidir sobre os pontos divergentes.

RESULTADOS

Foram encontrados 2.125 estudos com a combinação dos descritores selecionados, sendo que 194 foram descartados após verificação de duplicidade. Logo após, aplicaram-se os critérios de inclusão e não inclusão, o que resultou em 64 estudos classificados como elegíveis com texto completo, finalizando a seleção em 14 artigos para síntese qualitativa. Adicionalmente, quatro estudos identificados em outras listas de referências dos artigos selecionados e relacionados ao tema foram incluídos, totalizando 18 estudos na seleção final. A seleção dos estudos está esquematizada na Figura 1.

A presente revisão sistemática identificou 18 estudos, nos quais foram identificadas 12 equações. Entre estas, diferentes variáveis foram empregadas, incluindo características da amostra e dados de desempenho no teste SR-20m. Duas equações apresentaram nível de evidência forte; 2323. Apesar de a idade cronológica ter sido frequentemente utilizada para caracterizar o perfil da aptidão física, os diferentes estágios de maturação sexual tendem a mostrar características de aptidão física diferentes em crianças e adolescentes.4242. Em específico, meninas demonstram diferenças significativas na APCR em diferentes estágios de maturação sexual, geralmente demonstrando decréscimo do VO2pico ao passo em que o estado de maturação sexual avança.4444. Rev Paul Pediatr. 2013;31:189-97. No entanto, essa variável ainda não foi evidenciada em equações de predição.

Além disso, observa-se que crianças e adolescentes de diferentes classes econômicas, perfil de comportamento sedentário e nível de atividade física habitual podem apresentar diferenças relacionadas diretamente a parâmetros relacionados à saúde.4040. No entanto, informações sobre a aplicação dessas variáveis para a predição do VO2pico em crianças e adolescentes ainda são limitadas, havendo a necessidade de que novos estudos relacionados ao tema sejam desenvolvidos para melhor entendimento sobre a influência dessas variáveis na predição do VO2pico.

Este estudo apresenta algumas limitações para análise que devem ser pontuadas, como a falta de informações de características da amostra, o nível de adiposidade, o nível de atividade física e o comportamento sedentário. Principalmente relacionado a análises de correlação para as amostras isoladas por sexo. Essas limitações tornaram a identificação da melhor equação preditiva para diferentes grupos de crianças e adolescentes um desafio.

Portanto, pesquisas futuras devem disponibilizar mais informações da amostra, como etnia, tempo em comportamento sedentário, nível de atividade física, aspectos da composição corporal e estágio de maturação sexual, bem como promover análises correlacionais com a APCR. Dessa forma, dúvidas relacionadas à associação entre essas variáveis e a predição do VO2pico podem ser mais bem entendidas, possibilitando que equações mais acuradas sejam elaboradas. Além disso, torna-se importante que pesquisas futuras sejam conduzidas para verificar a reprodutibilidade das equações Ruiz,1717. Essas equações devem ser avaliadas em crianças e adolescentes que apresentam diferentes estados nutricionais, bem como análises específicas por sexo devem ser disponibilizadas.

No presente estudo, foi possível apontar as equações com melhor validade de predição do VO2pico em crianças e adolescentes, bem como identificar aspectos que dificultaram análise mais satisfatória para eleição de uma equação definitiva. As sugestões presentes nesta revisão contribuem para que estudos futuros sejam mais bem elaborados e descritos, o que colabora para a ampliação do conhecimento científico e prático das análises de equações preditivas do VO2pico no público infantojuvenil.

Em conclusão, nossos achados sugerem que a utilização de mais de uma característica da amostra nas equações tende a exercer maiores valores de associação entre o VO2pico predito e mensurado. Nesse sentido, a equação Matsuzaka (a) tende a ser a equação de forte nível de evidência com maior precisão de estimativa em crianças e adolescentes. Embora não explorados em equações de predição, a porcentagem de gordura corporal e o estágio de maturação sexual demonstram associações relevantes com a APCR em crianças e adolescentes e motiva-se a analisá-las em futuras equações. Contudo, novas pesquisas devem ser conduzidas para avaliar a reprodutibilidade de equações identificadas nesta revisão como promissoras, bem como ampliar o entendimento sobre a relação entre variáveis antropométricas, componentes da composição corporal e estágio de maturação sexual na predição do VO2pico em crianças e adolescentes.
