Aplicabilidade do Three Delays Model no contexto da mortalidade materna: revisão integrativa

RESUMO

O objetivo desta revisão é sumarizar evidências disponíveis na literatura científica provenientes da aplicabilidade do Three Delays Model no contexto da mortalidade materna quanto aos fatores causais e às medidas interventivas. Trata-se de revisão integrativa da literatura, realizada sem recorte temporal, em sete bases de dados, com os descritores Maternal Mortality, Pregnancy Complications, Maternal Death e a palavra-chave Three Delays Model. 15 estudos foram selecionados para análise. O primeiro atraso destacou-se como determinante para as mortes maternas, sendo a recusa em buscar assistência obstétrica na instituição de saúde uma iniciativa da mulher ou de familiares. No segundo atraso, fatores geográficos e infraestrutura precária das estradas dificultaram o acesso aos serviços de saúde. No terceiro atraso, as condições assistenciais nas instituições de saúde implicaram reduzida qualidade dos cuidados. A aplicabilidade do modelo possibilita demonstrar as barreiras enfrentadas pelas mulheres na busca de cuidados obstétricos e visualizar contextos que necessitam de ações interventivas para enfrentar a problemática.

ABSTRACT

The objective of this review is to summarize evidence available in the scientific literature from the applicability of the Three Delays Model in the context of maternal mortality in terms of causal factors and interventional measures. It is an integrative literature review, carried out with no time frame, in seven databases, with the descriptors Maternal Mortality, Pregnancy Complications, Maternal Death and the keyword Three Delays Model. 15 studies were selected for analysis. The first delay stood out as a determinant of maternal deaths, with the refusal to seek obstetric care in the health institution an initiative of the woman or family members. In the second delay, geographic factors and poor road infrastructure made access to health services difficult. In the third delay, the care conditions in the health institutions implied a reduced quality of care. The applicability of the model makes it possible to demonstrate the barriers faced by women in the search for obstetric care and to visualize contexts that need interventional actions to face the problem.

Quando os sistemas de saúde se mostram insuficientes e/ou limitados para atender às demandas das mulheres ou as desigualdades e barreiras impedem o seu alcance efetivo, ocorre a peregrinação pelos serviços em busca de assistência, que pode resultar em agravamento de quadros clínicos e desfechos negativos, como near miss e morte materna33 Parada CMGL. , sobretudo para elucidar as causas da mortalidade materna e direcionar esforços para a proposição de intervenções.

Nesse sentido, a fim de compreender as mortes maternas, abordagem teórica66 Rosenfield A, Maine D. Maternal mortality-a neglected tragedy: Where is the M in MCH?. The Lancet. 1985; 2(8446):83-5. que correlaciona esse fenômeno à utilização dos serviços de saúde durante a gravidez, parto ou puerpério evidenciou que desfechos negativos se associavam a complicações anteriores, às demoras para chegada da mulher às instituições de referência e à progressão para situações potencialmente graves.

Esse modelo teórico proposto na década de 1990, denominado Three Delays Model66 Rosenfield A, Maine D. Maternal mortality-a neglected tragedy: Where is the M in MCH?. The Lancet. 1985; 2(8446):83-5.,77 Thaddeus S, Maine D. Too far to walk: maternal mortality in context. Sm. Sci. Med. 1994; 38(8):1091-1110., ao envolver conhecimentos de diversas áreas, como antropologia, geografia e ciências sociais, tornou-se referência para análise das causas sociais que envolvem as mortes maternas, pois destacava não apenas sua sequência causal, mas, também, social e comportamental, relacionadas aos indivíduos, às famílias e aos contextos comunitários, correlacionando-os ao acesso aos sistemas de saúde88 Kalter HD, Salgado R, Babille M, et al. O modelo baseia-se na identificação e combinação de fatores e evidências relacionadas e agrupadas em três atrasos capazes de impedir mulheres de receber cuidados de saúde maternos necessários. O primeiro atraso se refere à decisão de procurar atendimento; o segundo relaciona-se ao percurso para alcance e obtenção de cuidados; e, por fim, o terceiro atraso corresponde ao recebimento de cuidados adequados no acesso aos serviços de saúde77 Thaddeus S, Maine D. Síntese de evidências sobre a utilização do Three Delays Model que incluiu estudos realizados na África, Ásia, América Latina e Caribe descreveu seu potencial na identificação de barreiras de acesso à atenção obstétrica em países de baixa e média renda, embora com utilidade limitada no desencadeamento de intervenções preventivas99 Danna VA, Bedwell C, Wakasiaka S, et al. Desse modo, compreende-se que a identificação das condições sociais, econômicas, reprodutivas e assistenciais implicadas no acesso aos serviços obstétricos que resultam em atrasos preditores da mortalidade materna representa uma dimensão importante para subsidiar o planejamento e a proposição de medidas inter-ventivas voltadas à qualificação da assistência ofertada e redução dos óbitos maternos22 Kassara SB, Melo AMC, Coutinho SB, et al. Objetivo

Sumarizar evidências disponíveis na literatura científica provenientes da aplicabilidade do Three Delays Model no contexto da mortalidade materna quanto aos fatores causais e às medidas interventivas.

Material e métodos

Tipo de estudo

Revisão integrativa da literatura realizada por meio das etapas: 1) elaboração da pergunta de pesquisa; 2) definição das bases de dados e dos critérios para inclusão e exclusão de estudos; 3) definição das informações a serem extraídas dos estudos selecionados; 4) avaliação dos estudos incluídos na revisão; 5) interpretação dos resultados; 6) apresentação da revisão/ síntese do conhecimento1010 Whittemore R, Knafl K. Elaboração da pergunta de pesquisa

Dessa forma, a pesquisa teve início a partir da construção da questão norteadora: quais fatores causais e medidas interventivas estão disponíveis na literatura científica provenientes da aplicabilidade do Three Delays Model no contexto da mortalidade materna?

Estabelecimento de critérios de inclusão e exclusão

Foram definidos como critérios de inclusão as pesquisas científicas publicadas nos idiomas português, inglês, espanhol ou francês, que utilizaram o Three Delays Model para compreender os fatores diretamente implicados nos ‘atrasos’ que resultam na mortalidade materna. Optou-se por não utilizar recorte temporal de publicação, a fim de abranger uma maior quantidade de estudos. Excluíram-se artigos de revisão de literatura, teses, dissertações e os estudos que não responderam à pergunta de pesquisa.

Fontes de dados e estratégia de busca

A busca pareada foi realizada entre junho e agosto de 2021, por meio do portal de periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Capes), nas bases de dados Medical Literature Analysis and Retrieval System Online (Medline) e Cumulative Index to Nursing And Allied Health (CINAHL) via EBSCO Information Services, Banco de Dados em Enfermagem (BDENF), Web of Science (WoS), Literatura da América Latina e do Caribe em Ciências da Saúde (Lilacs), e Índice Bibliográfico Espanhol de Ciências da Saúde (Ibecs) e biblioteca virtual Scientific Electronic Library Online (SciELO).

Foi utilizado o formulário de busca avançada. E, como estratégia de busca, a combinação de descritores controlados, escolhidos por meio da estratégia PICo – População = gestantes; Fenômeno de interesse = complicações no curso da gestação advindas de algum fator interveniente, demora; e Contexto = mortalidade materna. (Maternal Mortality, Pregnancy Complications, Maternal Death), contidos no vocabulário estruturado do Medical Subject Heading (MeSH), a uma palavra-chave (Three Delays Model). O cruzamento ocorreu por meio do operador booleano AND, resultando em três chaves de busca: 1) Maternal Mortality AND Three Delays Model; 2) Pregnancy Complications AND Three Delays Model; e 3) Maternal Death AND Three Delays Model, de forma padronizada em todas as bases de dados.

A etapa referente à busca nas bases de dados e na biblioteca virtual foi realizada por dois pesquisadores independentes. Inicialmente, foram identificadas 491 referências primárias. Após triagem, realizada por meio da leitura do título e do resumo e análise criteriosa com base nos critérios de inclusão e exclusão, 15 artigos compuseram a amostra. Durante o processo, quando houve divergência na seleção de estudos, os pesquisadores reuniram-se para decidir os artigos incluídos.

Utilizou-se o fluxograma adaptado do Preferred Reporting Items for Systematic Review and Meta-Analyses (Prisma)1111 Page MJ, Moher D, Bossuyt PM, et al. PRISMA 2020 explanation and elaboration: updated guidance and exemplars for reporting systematic reviews. BMJ. 2021; (372):n160. para demonstrar a operacionalização da busca e seleção, conforme figura 1.

Informações extraídas dos estudos selecionados

Elaborou-se um instrumento para extração dos dados, dividido em três seções contendo os itens relativos aos dados de caracterização dos artigos (autores, ano, título do artigo, base de dados, periódico); caracterização metodológica (delineamento, locais, participantes, cenários do estudo e técnica de coleta de dados e, por último, aspectos relacionados ao fenômeno dos óbitos maternos as demoras que norteavam a aplicabilidade do Three Delays Model, as causas, os fatores causais e as medidas inter-ventivas) nos artigos revisados.

Avaliação dos estudos incluídos na revisão

Para avaliação da qualidade metodológica dos estudos incluídos na amostra, utilizou-se a avaliação por nível de evidência, sendo: nível I, a metanálise de múltiplos estudos controlados; nível II, o estudo individual com delineamento experimental; nível III, o estudo com delineamento quase-experimental, tendo-se como exemplo estudo sem randomização com grupo único pré-teste e pós-teste, séries temporais ou caso-controle; nível IV, o estudo com delineamento não experimental, como pesquisa descritiva correlacional e qualitativa ou estudos de caso; nível V, relatório de casos ou dado obtido de forma sistemática, de qualidade verificável ou dados de avaliação de programas; e, no nível VI, opinião de autoridades respeitáveis baseada na competência clínica ou opinião de comitês de especialistas, incluindo interpretações de informações não baseadas em pesquisas1212 Galvão CM. Interpretação dos resultados e apresentação da síntese do conhecimento

Para análise, utilizou-se o método de redução de dados1010 Whittemore R, Knafl K. The integrative review: updated methodology. J Adv Nurs. 2005; 52(5):546-53. que envolveu a divisão e a classificação das fontes primárias em subgrupos por meio da organização, codificação e do agrupamento dos dados por semelhanças e divergências, a fim de identificar padrões, temas ou relacionamentos capazes de permitir em uma estrutura lógica a comparação dos dados, facilitando, assim, o processo de análise. Os resultados foram analisados interpretativamente a partir dos padrões recorrentes que emergiram e apresentados por meio de síntese descritiva e figuras. E, por fim, discutidos em conformidade com a literatura sobre o assunto.

Resultados

Nos dados compilados no quadro 1, encontra-se a caracterização dos estudos que aplicaram o Three Delays Model para compreender aspectos determinantes do atraso para receber e/ou ofertar assistência no contexto da mortalidade materna.

Discussão

A ocorrência das perdas maternas é resultado de interações biológicas, sociais, econômicas, culturais e comportamentais. Essas mostram-se acentuadas nos países em desenvolvimento, em especial, nas classes sociais de média e baixa renda, em decorrência da associação de fatores contribuintes para a sua ocorrência2828 Moyer CA, Johnson C, Kaselitz E, et al. A presença de atrasos na assistência obstétrica encontra-se diretamente ligada a desfechos maternos desfavoráveis. A ocorrência do óbito de uma mãe resulta em desestruturação para o núcleo familiar, sobretudo quando ela é a provedora, e evidencia a necessidade de se compreenderem as limitações existentes nos sistemas de saúde, a fim de que sejam eliminadas as barreiras que contribuem para esses atrasos.

Evidencia-se que camadas sociais mais pobres, além de disporem de precárias condições de vida, têm menor acessibilidade aos serviços de saúde, seja por falta de conhecimentos, precariedade nas estradas, transportes ou mesmo condições financeiras limitadas. Dessa forma, alguns dos fatores contribuintes para a ocorrência dos atrasos que levam à morte materna incluem as barreiras educacionais, financeiras, geográficas, de qualidade e disponibilidade dos transportes, de estradas e serviços de saúde2929 Serbanescu F, Goodwin MM, Binzen S, et al. O acesso limitado à educação, ao mercado de trabalho ou a recursos adequados impede que as mulheres obtenham os cuidados obstétricos necessários3030 Kumari K, Srivastava RK, Srivastava M, et al. Para enfrentar as elevadas taxas de mortalidade materna, as ações devem ser coordenadas e integradas entre a mulher, o núcleo familiar/comunitário, o Estado, os serviços de saúde e a comunidade. Crenças, cultura, práticas e costumes de um determinado local são influenciadores diretos na busca de cuidados profissionais de saúde. A utilização dos serviços de saúde materna é fundamental para que haja prevenção da mortalidade materna, um fenômeno que se constitui em um problema de saúde pública. Desse modo, a caracterização quantitativa e qualitativa da mortalidade materna torna-se relevante, tendo em vista a necessidade de se reconhecerem as diversas particularidades que envolvem esse fenômeno nas diversas regiões do mundo3434 Oliveira AS, Lima DJJ, Menezes NC. Destarte, medidas que visem ao aprimoramento da formação de profissionais de saúde que prestam serviços às gestantes, parturientes e puérperas, de forma a prevenir os óbitos maternos por meio da garantia de uma assistência segura, são necessárias. São imprescindíveis ações amplas e articuladas que visem a uma mudança efetiva do atual modelo assistencial na atenção obstétrica, haja vista as dificuldades evidenciadas na prestação de cuidados obstétricos e o elevado número de óbitos evitáveis3535 Souza JP. Assim, a utilização de parâmetros científicos, tais como a aplicabilidade de um modelo, permite estimar quais são os fatores envolvidos e como eles interagem para o resultado adverso àquela vida, pois a redução dos óbitos apenas será alcançada quando as ações visualizarem as particularidades dos problemas sociais, as diversificadas culturas, os sistemas de saúde e as diferentes realidades vivenciadas pelas mulheres, tendo em vista que a análise dos atrasos fornece dados sobre os determinantes implicados nas mortes maternas.

Limitações do estudo

As limitações do estudo associam-se aos termos empregados na estratégia de busca, considerando que a não existência de descritores controlados específicos que representassem o modelo em estudo e a adoção de palavra-chave para as buscas podem ter interferido nos resultados. Além disso, a adoção de critérios de elegibilidade relacionados ao idioma e o conjunto de bases de dados específicas da área da saúde podem ter limitado o escopo da busca.

Contribuições para a área da saúde ou política pública

A partir do modelo Three Delays Model apresentado, este estudo fornece subsídios para discussão e planejamento de propostas, estratégias e/ou medidas interventivas, que visem à reorientação de políticas e à implementação de práticas eficazes capazes de estreitar relações entre legislações, políticas públicas e a assistência prestada, voltadas ao enfrentamento das problemáticas associadas ao acesso aos serviços de saúde que resultam em desfechos materno-infantis desfavoráveis, sobretudo no contexto dos países em desenvolvimento.

Considerações finais

Esta revisão permitiu sintetizar contextos, barreiras e consequências associadas às demoras para acesso aos serviços, oferta e obtenção de cuidados em saúde no contexto da mortalidade materna. Evidenciou-se que a aplicação do Three Delays Model ocorre em maior concentração no primeiro atraso, que engloba a decisão de procurar atendimento. Os fatores causais do primeiro atraso encontram-se mais elucidados; em contrapartida, as medidas interventivas descritas associadas ao terceiro atraso, que engloba o recebimento de cuidados de saúde adequados, são escassas.

Este modelo mostra-se valioso instrumento para análise do fenômeno da mortalidade materna em diferentes realidades de saúde, na medida em que auxilia na compreensão dos fatores associados aos óbitos cuja origem advenha não somente de complicações obstétricas, mas de uma complexa rede de atrasos provenientes da comunidade e da realidade de saúde na qual aquela mulher está inserida.

Dessa forma, a compreensão aprofundada desses fatores, através de uma abordagem integrada, permite conhecer melhor o fenômeno e, por meio de estratégias e políticas públicas, direcionar esforços para enfrentamento das barreiras impostas às mulheres no acesso aos serviços obstétricos, tornando possível, assim, a oferta de uma assistência qualificada com segurança no ciclo gravídico-puerperal.
