The article analyzes if the accountability by managed competition has materialized itself in the brazilian sanitary system since 1990. For that, it approaches the accountability by managed competition; the reforms in the sanitary system; the accountability by managed competition based on the analysis categories – contractualization, plurality of providers, bureaucracy regulation structure, and user participation in the service rendering evaluation. Finally, it concludes that the accountability is still an ongoing process at the National Health System. There is a preference for the aspects that allow an opening to the market in detriment to the aspects that refer to the citizen and to the regulation structure.
