  • A genetic study of a Staphylococus aureus plasmid involving cure and transference Special Article

    A transferência e a eliminação de resistência a drogas, em altas freqüências, podem evidenciar herança extracromossômica de determinantes genéticos. Este trabalho mostra a cura e transferência de resistência à tetraciclina e de um plasmidio pequeno, na linhagem S. aureus 1030 (55)TetR. São várias as metodologias disponíveis para a eliminação de plasmídios. Empregou-se a técnica do agente curagênico brometo de etídio, que se intercala na fita de DNA, inibindo o DNA polimerase. O resultado foi perda do plasmídio e da resistência à tetraciclina em alta freqüência. O experimento de transferência foi realizado em meto de cultura líquido, e a freqüência foi da ordem de 10-6. Esses estudos têm grande importância em estudos de infecções. Dando aos pesquisadores e clínicos esclarecimento de possíveis mecanismos de aquisição de resistência a antibióticos que podem ocorrer também in vivo, principalmente no ambiente hospitalar

    High frequency transfer and elimination of drug resistance may indicate an extrachromosomal inheritance of genetic determinants. This study shows the cure and transfer of a small plasmid and tetracycline resistance in Staphylococcus aureus 1030 (55)TetR strains. Several methods are available for plasmid elimination. We used ethidium bromide, an agent that binds to DNA, and thus inhibits DNA polymerase. This caused a high frequency of loss of the small plasmid and resistance to tetracycline. Transfer of tetracycline resistance was done in a mixed culture at a frequency of 10-6. This type of study is very important to physicians and epidemiology investigators and provides better knowledge on antibiotic-resistance mechanisms that may occur in vivo in a hospital environment.

    A Paulínia cupana (o guaraná) é uma planta da flora brasileira que goza de grande prestígio na medicina popular, entre eles de ser potente estimulante das funções cerebrais. Os autores avaliaram os efeitos da administração crônica da guaraná sobre a cognição de voluntários idosos normais. Foram estudados 45 voluntários, sendo distribuídos aleatoriamente para um dos três grupos experimentais. Placebo (n=15), Cafeína (n=15) e Guaraná (n=15), em estado duplo-cego. Não obteve-se nenhuma mudança significativa na cognição desses voluntários.

    Paulinia cupana (guarana) is a Brazilian plant given great prestige in popular medicine, for example as being a potent stimulator of brain functions. The authors assessed the effects of the long-term administration of guarana on the cognition of normal, elderly volunteers. Forty-five volunteers were studied, with a random distribution in three experimental groups: placebo (n=15), caffeine (n=15), and guarana (n=15), in a double-blind study. There were no significant cognitive alterations in these volunteers.
  • Surgical treatment of noniatrogenic trauma of the femoral arteries Original Article

    O trauma de artérias femorais corresponde a 30 percent do total de traumas arteriais. Os autores analisaram 74 pacientes com trauma não-iatrogénico de artérias femorais atendidos de janeiro de 1991 a dezembro de 1993. A idade variou dentre 11 e 50 anos, com media de 24 anos, 71 pacientes eram do sexo masculino e três do feminino. Cinquenta e dois pacientes (70,2 percent) eram da raça branca, vinte da negra (27 percent) e dois da amarela(2,8 percent). O trauma por ferimento por arma de fogo foi o mais frequente com. 61 casos (82,4 percent). A ausência de pulsos foi a manifestação clínica mais freqüente (62,5 percent). A isquemia grave, com. risco de perda de membro foi constatada e 66,2 percent dos casos. A artéria femoral superficial foi a lesada em 77 percent dos casos. Foi realizada arteriografia pós-operatória em apenas cinco pacientes, vítimas de múltiplos traumas penetrantes ou ferimento penetrante assintomático tico em trajeto vascular. Em seis deles, optou-se pela ligadura arterial e venosa. Em três casos foi realizada anastomose arterial primária. Em um paciente foi realizada arteriorrafia simples. Em sessenta e quatro pacientes, foi realizado enxerto venoso com. segmento de veia safena magna invertida, retirada do outro membro inferior. Empregou-se a fasciotomia em 32 pacientes (43,2 percent), todos eles com empastamento de musculatura de membro inferior desde a admissão. Um paciente faleceu no pós-operatório imediato devido a falência de múltiplos órgãos por politraumatismo. A preservação do membro foi obtida em 72 doentes (97,3 percent). As únicas duas amputações se deveram a isquemia, previamente muito grave, com longo tempo decorrido entre o momento do trauma e o procedimento cirúrgico. Um deste doentes, além de isquemia grave, apresentava lesões extensas de partes moles. Concluímos que o trauma das artérias femorais tratado quando o membro ainda mantém a viabilidade, tem um bom prognóstico clínico, com alto índice de preservação de membro. A mortalidade é geralmente causada pelo trauma a outros órgãos.

    Trauma to the femoral arteries corresponds to 30 percent of all arterial traumas. The authors reviewed 74 patients with noniatrogenic trauma of the femoral arteries treated from January 1991 to December 1993. Ages ranged from 11 to 50 years, with a mean of 24. Seventy-one patients were male and three female. Fifty-two patients (70.2 percent) were white, 20 (27 percent) were black and two (2.8 percent) were Asian. Trauma due to firearms had the highest incidence, with 61 cases (82.4 percent). Absence of pulse was the most frequent clinical symptom (62.5 percent). Severe ischemia, with risk of loss of limb, was found in 66.2 percent of the cases. The superficial femoral artery was impaired in 77 percent of the cases. A preoperative arteriography was performed on only five patients, victims of multiple penetrating trauma or an asymptomatic penetrating wound along a vessel passage. In six cases, arterial and venous ligature was the chosen procedure. In three cases, a primary arterial anantomosis was performed. Simple arterriorraphy was feasible in one patient. In 64 of the patients, a venous graft was undertaken using a segment of the inverted great saphenous vein withdrawn from the other lower limb. Fasciotomoy was used in 32 patients (43.2 percent), all of whom exhibited pasting of the lower limb muscles at admission. One patient died during the immediate postoperative period as the result of multiple organ failure caused by polytraumatism. Preservation of the limb was attained in 72 patients (97.3 percent) Severe, previously-incurred ischemia was responsible for the only two amputations, aggravated by an exceedingly long delay between the time of injury and surgery. One of these patients, in addition to severe ischemia, had extensive injuries to the soft tissues. We conclude that trauma of the femoral arteries, attended while the limb still maintains its vitality, has a positive clinical outcome with a high rate of limb preservation. Mortality usually results from injury to other organs.
  • Chronic myeloid leukemia (CML): prognostic factors and survival analysis Review & Updating

    Avaliamos o valor prognóstico de diferentes fatores, ao diagnóstico, em 45 pacientes com LMC Ph1-positivos. A sobrevida mediana foi de 48 meses. A análise univariada identificou 5 fatores associados a pior prognóstico (sexo masculino, idade inferior a 45 anos, blastos na medula óssea maior ou igual a 10 percent, basófilos no sangue periférico maior ou igual a 6 percent e eosinófilos no sangue periférico maior ou igual a 6 percent), originando um sistema de estadiamento: estágio I com zero ou um fator e sobrevida de 100 percent em dois anos; estágio II com dois ou três fatores e sobrevida de 72,2 percent em dois anos; estágio III com 4 ou 5 fatores e sobrevida de 0 percent em dois anos (p = 0.00016). A análise multivariada demonstrou que a basofilia no sangue periférico e os blastos na medula óssea foram os fatores que melhor se correlacionaram com o tempo de sobrevida. Concluímos que a combinaçóo de fatores presentes no diagnóstico permite a identificação de diferentes grupos de risco na LMC, podendo ser útil na determinaçóo do prognóstico e na abordagem terapêutica.

    The prognostic value of different factors upon diagnosis of CML was analysed in 45 Philadelphia (Ph1)-positive patients. The median survival was 48 months. Univariate analysis showed 5 poor prognostic factors (male sex, under 45 years-old, bone marrow blasts greater than or equal to 10 percent, blood basophils greater than or equal to 6 percent and blood eosinophils greater than or equal to 6 percent) which provided for the development of a clinical staging system: Stage I with none or one factor and a two-year survival rate of 100 percent; Stage II with two or three factors and two-year survival of 72.2 percent; and Stage III with four or five factors and two-year survival of 0 percent (p = 0.00016). Multivariate survival analysis showed that combination of blood basophilia and bone marrow blasts had the strongest predictive relationship to survival time. We conclude that a combination of pretreatment factors identifies different risk subcategories in CML patients and is helpful in assessing the overall prognosis and the treatment approach.
  • Tomographic aspects of xanthogranulomatous pyelonephritis and related complications Case Report

    Os autores apresentam a sua experiência em 7 pacientes com diagnóstico anátomopatológico de Pielonefrite Xantogranulomatosa (PXG) submetidos a tomografia computadorizada (TC) pré-operatória. Os parâmetros estudados e seus resultados foram: a) presença de cálculos em 86% dos casos, b) volume renal frequentemente aumentado, c) hidronefrose, d) medidas de densidade variando entre 14 e 29 HU, e) presença de realce em todos os casos, f) comprometimento extra-renal, também presente em todos os casos. A TC demonstrou ser um método bastante fidedigno na caracterização da PXG e sua extensão extra-renal.

    The authors present their experience involving seven patients with histopathologic diagnosis of xanthogranulomatous pyelonephritis who were submitted to preoperative computed tomography (CT). The results are the following: a) stones (86 percent of the cases), b) increase in renal volume, c) hydronephrosis, d) density measurements (from 14 to 29 HU), e) enhancement found in all cases, f) extrarenal involvement (all cases). CT has shown to be a reliable method in characterizing xanthogranulomatous xyelonephritis and extrarenal involvement.
  • Inguinal hernia incarceration as a form of intestinal tuberculosis Case Report

    Relatamos um caso de paciente do sexo masculino, 41 anos, que deu entrada em Serviço de imergência com queixa de dor abdominal, sendo feito diagnóstico de abdome agudo obstrutivo por encarceramento de hérnia inguinal à direita. A cirurgia, evidenciou-se alça intestinal com acometimento inflamatório granulomatoso, aderida ao. saco herniário. O estudo anátomopatológico do material cirúrgico confirmou o diagnóstico de tuberculose. Apresentamos uma revisão sobre as diferentes formas clínicas da tuberculose intestinal e dificuldades encontradas para seu diagnóstico diferencial, enfatizando a inusitada forma de apresentação descrita em nosso paciente.

    We report the case of a 41 year-old male who came to the emergency room with a complaint of abdominal pain, and was diagnosed to have an acute obstructive abdomen due to a right inguinal hernia incarceration. During surgery, an intestinal granulomatous inflammation was observed adhered to the hernial sac.The histopathologic study confirmed the diagnosis of tuberculosis. We present a review of the different clinical forms of intestinal tuberculosis and the difficulties encounted in the differential diagnosis of such, emphasizing the uncommon presentation described in our patient.
  • Determination of glucose levels using dried filter paper blood spots: new perspective in home monitoring Short Communication

    Apresentamos um método de dosagem de glicemia em papel de filtro. Para comprovar seu valor, comparamos os resultados obtidos no papel de nitro e no sangue venoso colhidos simultaneamente. Demonstramos que existe uma relação linear entre a glicemia dosada no papel de filtro e a do sangue periférico. Não houve diferença estatística entre os dols métodos (p>0,05). 0 método se constitui em uma alternativa barata no controle do diabetes melittus e pode ser usado também no diagnóstico da hipoglicemta reativa e em outras situações especiais.

    We present a method for the determination of blood glucose using dried filter paper blood spots.To validate this method, we compared our results using filter paper and simultaneously collected venous blood. We demonstrated that there is a linear relationship between the filter paper glucose levels and those determined in whole blood (r=0.98). There was no significant difference between the results of the two methods (p>0.05).This method is a cheap alternative which may improve the control of diabetes mellitus, and may also be very useful in the diagnosis of postprandial hypoglycemia and other special situations.
