This study aimed to identify and analyze the reasons for tuberculosis treatment default among the Family Health Units' clients in the IV Health District in the city of João Pessoa, PB, Brazil. The data collection was carried out through interviews using the thematic oral history as a methodological reference. Four clients with a history of treatment default were interviewed. Data was analyzed according to the content analysis technique, thematic modality. Results present two thematic axes: factors that make therapeutic success difficult and the involvement of the family health professionals in the development of strategies to improve the client's adherence to their treatment of tuberculosis. The factors associated to abandonment are diverse and complex, with difficulties related to the client, to the treatment, and to the implementation of health care practices. It has been observed that the bond and the reception are essential to assure continuity of the treatment.
