Por meio de uma revisão das evidências científicas, a Academia Brasileira de Rinologia vem proporcionar sua visão prática e atualizada sobre as medicações tópicas nasais mais utilizadas, excetuando-se as medicações que possuam antibióticos tópicos na sua formulação.

This documents aims at educating those who treat sinonasal diseases - both general practitioners and specialists - about topical nasal treatments. By means of scientific evidence reviews, the Brazilian Academy of Rhinology provides its practical and updated guidelines on the most utilized topical nasal medication, except for the drugs that have topical antibiotics in their formulas.

RESUMO O transplante de pâncreas é o único tratamento capaz de restabelecer os níveis de glicose e hemoglobina glicada em pacientes diabéticos dependentes de insulina, sem o uso de insulina exógena. A evolução do transplante de pâncreas no tratamento de diabetes foi marcada por avanços nos campos da técnica cirúrgica, preservação de órgãos e imunossupressão. A principal complicação da perda do enxerto é a falha técnica, seguida de rejeição aguda ou crônica. Por falha técnica entende-se perda do enxerto dentro dos primeiros três meses seguintes ao transplante devido a: trombose vascular (50%), pancreatite (20%), infecção (18%), fístula (6,5%) e hemorragia (2,4%). Complicações imunológicas ainda afetam 30% dos pacientes, e a rejeição causa perda do enxerto em 10% dos casos. A rejeição crônica é a complicação tardia mais comum. Doenças cardiovasculares são a causa mais frequente de mortalidade tardia no transplante de pâncreas que continua sendo o tratamento mais eficaz para pacientes com diabetes do tipo 1. Há uma importante melhora na qualidade de vida e na sobrevida dos pacientes. O desenvolvimento de ilhotas transplantadas pode eliminar ou minimizar complicações cirúrgicas e a imunossupressão.

ABSTRACT Pancreas transplantation is the only treatment able to reestablish normal glucose and glycated hemoglobin levels in insulin-dependent diabetic patients without the use of exogenous insulin. The evolution of pancreas transplantation in treatment of diabetes was determined by advances in the fields of surgical technique, organ preservation and immunosuppressants. The main complication leading to graft loss is technical failure followed by acute or chronic rejection. Technical failure means graft loss within the first three months following transplantation due to vascular thrombosis (50%), pancreatitis (20%), infection (18%), fistula (6.5%) and bleeding (2.4%). Immunological complications still affect 30% of patients, and rejection is the cause of graft loss in 10% of cases. Chronic rejection is the most common late complication. Cardiovascular diseases are the most common causes of late mortality in pancreas transplantation, so it remains the most effective treatment for type 1 diabetes patients. There is a significant improvement in quality of life and in patient's survival rates. The development of islet transplantation could eliminate or minimize surgical complications and immunosuppression.

A fístula oroantral é a comunicação patológica entre cavidade oral e seio maxilar, quase sempre decorrentes de traumatismos durante procedimentos dentários. OBJETIVO: Apresentar experiência de 25 casos. CASUÍSTICA E MÉTODOS: Realizado estudo retrospectivo de pacientes com FOA no período de 1996 a 2000. O diagnóstico incluiu exame otorrinolaringológico, endoscopia nasal ou da fístula, tomografia computadorizada das cavidades paranasais, pesquisa bacteriológica, fúngica e análise patológica. RESULTADOS: Encontrados 25 casos, sendo 10 de segundo molar, 8 de primeiro molar, 6 de segundo pré-molar e 1 de canino. Todos foram operados pela técnica de Caldwell-Luc, reavivamento das bordas da fístula, meatotomia média e rotação de retalho mucoso geniano. DISCUSSÃO: Nas fístulas de alto débito (n=14), colocou-se enxerto ósseo da própria parede anterior do seio. Todos, exceto um, tiveram resultado cirúrgico bom. A cultura bacteriológica (n=19) demonstrou estreptococos pneumoniae (13), haemophilus influenzae (6), moraxella catarrhalis (2), estafilococos aureus (2). Encontrado aspergilus niger em um caso que apresentava imagem radiológica de bola fúngica. CONCLUSÕES: Após 30 dias, os resultados foram bons em todos, exceto um dos casos. Este foi reoperado com colocação de enxerto ósseo, inicialmente não utilizado, tendo sucesso. Após 6 meses, todos os 23 pacientes localizados não apresentavam problemas.

The oroantral fistula is a pathological connection between the maxillary sinus and with the oral cavity. The condition mostly follows dental extraction. AIM: To present the experience of 25 cases. MATERIAL AND METHODS: Retrospective cases between 1996-2000. The ORL examination included nasal or sinusal endoscopy, a CT scan and histopathological analysis. RESULTS: Twenty-five cases were found: ten 2nd molar cases, eight 1st molar cases, six 2nd premolar cases, and one canine case. All patients underwent a Caldwell-Luc operation plus excision of the epithelium lining the fistula, that was then completely covered by a flap of mucosa rotated from the genian region. DISCUSSION: In cases of major fistulae a bone autograft taken from the anterior sinus wall was used. Bacterial cultures (n=19) revealed streptococus pneumoniae (13), haemophillus influenza (6), Moraxella catharralis (2) and staphylococus aureus (2). Aspergillus niger was found in one case presenting as a "fungic ball". CONCLUSIONS: The only case of surgical failure, after 30 days postoperatively, was reoperated, using a bone graft. After a 6-month follow up all of the patients progressed satisfactorily, including the reoperated patient.

A apresentação faringolaríngea da tuberculose como forma isolada da doença é rara, entretanto, nos últimos anos, com o aumento progressivo do número de casos de tuberculose em geral, a possibilidade dessas localizações aumentou. OBJETIVO: Descrever caso de tuberculose faringolaríngea primária em paciente grávida e imunocompetente. RELATORIO DE CASO: Paciente do sexo feminino, 30 anos, com história de odinofagia há 10 meses, sem qualquer sintoma sistêmico ou outras queixas referentes às vias aéreas superiores. Apresentava lesão granulomatosa em orofaringe e laringe, e o resultado da biópsia revelou bacilos álcool-ácido resistentes. Exames clínico e radiológico do aparelho respiratório sem alterações. Realizado tratamento anti-tuberculose por nove meses com resolução completa do quadro. CONCLUSÃO: Autores realçam a importância epidemiológica da tuberculose e a necessidade de um alto grau de suspeição de lesões das vias aéreas superiores para o diagnóstico precoce da doença.

Tuberculosis of the Larynx and pharynx only is rare. However, in the last few years, the number of tuberculosis cases in general have had a dramatic increase, thus increasing the possibility of isolated laryngo-pharyngeal lesions. AIM: To report a case of isolated laryngo-pharyngeal tuberculosis in a pregnant, immunocompetent host. CASE REPORT: A 30- year-old pregnant female had complained of odynophagia for the last ten months. There were no other respiratory or systemic symptoms. An oro-pharyngeal granulomatous lesion was found, and the biopsy revealed acid-fast bacilli. There were no clinical or radiologic pulmonary findings. She was submitted successfully to an tuberculosis treatment protocol for nine months. CONCLUSIONS: The authors point out the epidemiological importance of tuberculosis and the need for a higher degree of suspicion when dealing with uncommon upper airway lesions to make an early diagnosis.

The authors report a case of laryngeal chondroma , a rare benign larynx tumor in the cricoid and arytenoid cartilages. There are aproximately 250 cases previously described in literature. A male, 41 year-old patient, presented a severe and progressive dyspnea leading to tracheostomy. Direct microlaryngoscopy revealed arytenoid enlargement and subglottic stenosis and the endoscopical biopsy was inconclusive, since the tumor present a hard gnistency. We performed surgical excision by laringofissura and total resection of the tumor, with good result. The histopathological examination showed a chondroma with no malignance.

Os autores apresentam uma revisão da literatura referente às alternativas de tratamento cirúrgicas e não-cirúrgicas para a Síndrome da Apnéia e Hipopnéia Obstrutiva do Sono (SAHOS) e para a Síndrome da Resistência das Vias Aéreas superiores (SRVAS) com enfoque no princípio de ação e evolução dos aparelhos de avanço mandibular e, em particular, no mecanismo dinâmico de ação do Aparelho Anti-Ronco® (AAR-ITO).

The authors present a literature review of the surgical and non-surgical treatment alternatives to the Obstructive Sleep Apnea and Hypopnea Syndrome (OSAHS) and snoring with focus in the mechanism of action and progress of mandibular advancement devices mainly in the dynamic action mechanism of the Mandibular Advancement Device® (MAD-ITO).

The authors present a case of a female. 43-year-old patient. with a neck mass suggestive of a thyroglossal duct cyst. During the operation. a hard and adherent mass was found and the pathological findings demonstrated a metastatic follicular carcinoma. Despite of the negative image, total thyroidectomy was performed and a small primary tumor of 0,2cm was found in the left inferior lobe. There were only three of these cases reported.

The authors present five cases of maxillary sinus foreign bodies, four of them (80%) originated from surgical problems. They discuss their findings, enphasizing the difficulty and the importance of early diagnosis and treatment. They recommend the use of tied gauze in the surgical procedures in the maxillary sinus.

The authors present a case of chronic recurrent sinusitis of rare etiology in a man submitted previously to a facial peeling. The computed tomography and the sinusal endoscopy revealed periorbital fat in the maxilary sinus. There was a history of old orbital floor fracture.
